ABSTRACT

OBJECTIVE@#To observe the analgesic effect of Tuina by pressing and kneading the Huantiao (GB30) acupoint on rats with chronic constriction injury (CCI) and to explore the analgesic mechanism of Tuina on sciatica rats.@*METHODS@#Thirty-two SPF male SD rats weighing 180 to 220 g were randomly divided into fore groups:blank group (without any treatment), sham group (only exposed without sciatic nerve ligating), model group (sciatic nerve ligating) and Tuina group (manual intervention after lsciatic nerve ligating). The CCI model was prepared by ligating the right sciatic nerve of the rats, on the third day of modeling, the rats in the Tuina group were given pressing and kneading the Huantiao (GB30) point for 14 days, and the changes of paw withdrawal threshold(PWT), paw withdrawal latency(PWL) were measured before and on the 1st, 3rd, 7th, 10th, 14th and 17th days after modeling. The changes of sciatic functional index(SFI) were measured before and on the 1st and 17th day after modeling. The morphological changes of the sciatic nerve were observed by hematoxylin-eosin(HE) staining;and the differences in NF-κB protein expression in the right dorsal horn of the spinal cord of rats were detected.@*RESULTS@#Following modeling, there was no significant difference in PWT, PWL and SFI between the blank group and the sham group (P>0.05), but the PWT, PWL and SFI of the model group and the Tuina group decreased significantly (P<0.01). After manual intervention, the pain threshold of rats in Tuina group increased. On the 8th day of manual intervention (the 10th day after modeling), PWT in Tuina group increased significantly compared with that in model group (P<0.01). On the 5th day of manual intervention (the 7th day after modeling), the PWL of the massage group was significantly higher than that of the model group (P<0.01). The pain threshold of rats in Tuina group continued to rise with the continuous manipulation intervention. After 14 days of manipulative intervention, the sciatic nerve function index of rats in the Tuina group increased significantly(P<0.01). Compared with the blank group and sham group, the myelinated nerve fibers of sciatic nerve in the model group were disordered and the density of axons and myelin sheath was uneven. Compared with the model group, the nerve fibers of rats in the Tuina group were gradually continuous and the axons and myelin sheath were more uniform than those in the model group. Compared with the blank group and sham group, the expression of NF-κB protein in the right spinal dorsal horn of the model group was significantly increased(P<0.01). Compared with the model group, the expression of NF-κB protein in the right spinal dorsal horn of rats in Tuina group decreased significantly(P<0.01).@*CONCLUSION@#Pressing and kneading the Huantiao (GB30) point restores nerve fiber alignment;and improves the PWT、PWL and SFI in the CCI model by decreasing NF-κB p65 protein expression in the spinal dorsal horn. There fore, Tuina demmstrates an analgesic effect and improves the gait of rats with sciatica.

ABSTRACT

Objective: To collect, compare and analyze the changes in the strength and stability of the forearm Rou-Kneading manipulation before and after the training in Shaolin Neigong (internal Qigong). Methods: Ninety first-year undergraduates were randomized into three groups using the random number table method, with 30 people in each group. The Gongfa (Qigong method) group received training in both Shaolin Neigong and forearm Rou-Kneading manipulation. The manipulation group only received training in forearm Rou-Kneading manipulation. The control group only received training in forearm Rou-Kneading manipulation for one week. The ZTC-1 intelligent Tuina (Chinese therapeutic massage) manipulation parameter detection system was used to collect the wave crest, wave trough, and crest-trough difference of the strength and frequency of the forearm Rou-Kneading manipulation on the Z-axis (up and down), X-axis (left and right) and Y-axis (backward and forward) at weeks one, five and ten from the three groups. The collected data were then processed and analyzed. Results: The intra-group comparisons showed statistical significance in the Gongfa group and manipulation group (P<0.05). The strength and stability shown on the axes Z, X, and Y constantly grew with the increase of training time in the Gongfa group. The wave crest on the axes Z and Y steadily rose in the manipulation group, as well as the frequency on the axes Z, X and Y. The control group failed to show statistical significance in any of the three times of intra-group comparisons (P>0.05). The between-group comparisons showed statistical significance among the three groups at weeks five and ten (P<0.05). At week five, the wave crest on the axes Z, X, and Y, and the crest-trough difference on the axes X and Y were more prominent in the Gongfa group than in the manipulation group, showing statistical significance (P<0.05). At week ten, the wave crest, wave trough, and crest-trough differences on the axes Z, X, and Y were more prominent in the Gongfa group than in the manipulation group, presenting statistical significance (P<0.05). Conclusion: Practicing Shaolin Neigong can help the trainees reach the level of strength and frequency of Tuina clinicians in a shorter time in the forearm Rou-Kneading manipulation training. It can advance the efficiency in studying the forearm Rou-Kneading manipulation and promote the quality of the manipulation.

ABSTRACT

Objective To observe the effect of traditional Chinese medicine hot-ironing therapy at Shenque point on cancerous abdominal pain in advanced malignant tumors. Methods Forty patients with cancerous abdominal pain who met the inclusion criteria were randomly divided into two groups, 20 in each group.The control group was treated with traditional Chinese medicine hot-ironing analgesic package at Shenque point, and the treatment group was treated with abdominal massage therapy on the basis of the control group. Both groups were treated continuously for 7 days. The degree of abdominal pain was assessed using a digital assessment scale, and the quality of life was assessed using a quality of life assessment scale. The time to remission of cancerous abdominal pain was observed during the treatment period. Results After treatment, the NRS scores on the 3rd day (1.9 ± 0.6 vs. 2.4 ± 0.3, t=3.333), 5th day (1.0 ± 0.3 vs. 1.6 ± 0.5, t=4.602), 7th day (1.0 ± 0.6 vs. 2.4 ± 0.7, t=6.791), in the treatment group were significantly lower than those in the control group (P<0.01). The remission time of the treatment group was longer than that of the control group on the 1st, 3rd, 5th and 7th day after treatment, but there was no significant difference between the groups (t values were 0.243, 1.374, 0.185, 0.7284, P>0.05). The body function score in the treatment group (88.6 ± 13.7 vs. 77.0 ± 19.4, t=2.184) was significantly higher than that in the control group (P<0.05). Conclusions The combination of traditional Chinese medicine hot-ironing package combined with pushing therapy can alleviate the pain level and improve the quality of life of patients with cancerous abdominal pain.

ABSTRACT

Objective] To observe the clinical effect of“eliminating the accumulative stagnation and heat with pressing and kneading manipulation”treatment efficacy of children with functional constipation. [Method] Randomly divide 60 cases into control group and treatment group, 30 cases in each;the control one took conventional pediatric tuina method; the treatment one took “eliminating the accumulative stagnation and heat with pressing and kneading manipulation”method. Compare their effects after three courses. [Result] After three courses treatment, in treatment group, 20 cases had marked effect, 8 effective, 2 void, total effective rate 93.33%;in control group, they were respectively 10,12,8 and 73.33%. The comparison of total effective rates had difference of statistical significance(P<0.05). [Conclusion] The method of “eliminating the accumulative stagnation and heat with pressing and kneading manipulation”has better curative effect than the conventional pediatric tuina method. And it is very simple to manipulate, and worthy of being popularized.

ABSTRACT

Objective To observe the therapeutic effect of abdominal kneading massage in treating chronic superficial gastritis. Methods Forty-six cases of chronic superficial gastritis were randomly divided into test group and control group, 23 cases in each group. The patients of the test group were treated by the abdominal kneading massage, once a day, 7 times for a course of treatment, and the treatment lasted 2 courses. The patients of the control group took the western medicine Domperidone tablets orally, 10 mg each time, 3 times a day, for 2 continuous weeks. After treatment, we compared the clinical efficacy on syndromes and gastroscopy detection results in both groups. Results ( 1) After treatment, the scores of syndromes were decreased in both groups ( P<0.01 compared with those before treatment) , and the decrease was more obvious in the test group ( P<0.05 compared with the control group). ( 2) The total efficiency of test group was 91.30%, and that of the control group was 60.87%, the difference being significant ( P<0.01). ( 3) The results of gastroscopy showed that the relief of the gastritis lesions of the test group was significantly better than that in the control group, and the difference was significant ( P < 0.01). Conclusion Abdominal kneading massage can effectively relieve the symptom and gastric mucosa inflammatory manifestations of the chronic superficial gastritis patients, and this treatment can be recommended in clinical application.

ABSTRACT

Objective] To study the effect of kneading pull-turning methods together with the family care and treatment on children congenital muscular torticollis. [Methods] 60 cases of congenital muscular torticollis were randomly divided into kneading pull-turning methods together with the family care group(treatment group) and conventional massage group(control group) with symptoms, signs and evaluation of clinical curative effect between groups of children. [Results] The treatment group cure rate was 76.67%, control group cure rate was 43.33%, the curative effect of treatment group was obviously better than the control group(P<0.05). [Conclusion] Kneading pull-turning methods together with the family care in treatment of children with congenital muscular torticollis has the characteristics of high cure rate which is conducive to recovery of affected children.

ABSTRACT

Clonazepam (CLZ) is an anticonvulsant benzodiazepine widely used in the treatment of epilepsy. CLZ is a BCS Class II drug and its bioavailability is thus dissolution limited. The objective of the present study was to prepare solid dispersions (SDs) of CLZ by various techniques, using the amphiphilic carrier Gelucire 50/13 in various proportions, to increase its water solubility. Drug-polymer interactions were investigated by Fourier-transform infrared (FTIR) and Ultra-Violet (UV) spectroscopy. The SDs were characterized physically by differential scanning calorimetry (DSC) and X-ray diffraction (XRD). A phase solubility study was performed and the stability constant (Ks) was found to be 275.27, while the negative Gibbs free energy (?Gotr) indicated spontaneous solubilization of the drug. The dissolution study showed that the SDs considerably enhanced the dissolution rate of the drug. The FTIR and UV spectra revealed no chemical incompatibility between the drug and Gelucire 50/13. XRD patterns and the DSC profiles indicated the CLZ was in the amorphous form, which explains the improved dissolution rate of the drug from its SDs. Finally, mouth dissolving tablets (MDTs) were prepared from the optimized batches (kneading method) of solid dispersion, using crospovidone and Doshion P544 resin as superdisintegrants. The tablets were characterized by in-vitro disintegration and dissolution tests. The study of the MDTs showed disintegration times in the range 32.0±0.85 to 20.0±1.30 sec and dissolution was faster than for the commercial preparation. In conclusion, this investigation demonstrated the potential of solid dispersions of a drug with Gelucire 50/13 for promoting the dissolution of the drug and contributed to the understanding of the effect of a superdisintegrant on mouth dissolving tablets containing a solid dispersion of a hydrophobic drug.

ABSTRACT

Etoricoxib, a non-steroidal anti-inflammatory drug, is used to Osteoarthritis, Rheumatoid arthritis and Acute Gouty arthritis. Etoricoxib is practically insoluble in water; hence present study was carried out to enhance dissolution properties of Etoricoxib through the preparation of Solid Dispersions using PEG 6000 as carrier at various proportions by using different techniques like Physical mixtures, Kneading Method and Solvent Evaporation Method. The drug release profile was studied in 0.1N HCl containing 1 % SLS. U.V. Spectrophotometric method was selected for assay as well as in-vitro dissolution studies at 234nm.All the solid dispersions exhibited superior dissolution than pure drug. The drug dissolution studies followed first order kinetics. Solvent evaporation method was found to be superior to other methods.

ABSTRACT

The present study was aimed to increase the solubility of the poorly water soluble drug (Gliclazide) by using hydrophilic polymers (PVP K-30 and HPMC E4). Solid dispersions were prepared by kneading method. Phase solubility study, in-vitro dissolution of pure drug, physical mixtures and solid dispersions were carried out. PVP and HPMC were found to be effective in increasing the dissolution of Gliclazide in solid dispersions when compared to pure drug. FT-IR spectroscopy, differential scanning calorimetry and X-ray diffractometry studies were carried out in order to characterize the drug and solid dispersion.
