ABSTRACT

OBJECTIVE@#To explore the effectiveness of Nice knot technique for wound closure in Gustilo type ⅢA and ⅢB open tibial fractures.@*METHODS@#A retrospective study was performed on 22 patients with Gustilo type ⅢA and ⅢB open tibial fractures, who underwent wound closure using the Nice knot technique and were admitted between June 2021 and June 2022. There were 15 males and 7 females. The age ranged from 18 to 67 years, with an average of 41.9 years. The causes of injury included traffic accident in 11 cases, falling from height in 7 cases, and heavy object injuries in 4 cases. Fractures were located on the left side in 9 cases and on the right side in 13 cases. And 9 cases were type ⅢA fractures and 13 were type ⅢB fractures according to Gustilo classification. All patients had extensive soft tissue injuries, and no vascular or neurological damage was observed. The time from injury to debridement was 3-8 hours (mean, 6.5 hours). The sizes of wounds before operation and at 2 weeks after operation were measured and wound healing rate at 2 weeks after operation were calculated. The wound healing time and wound healing grading were recorded. The Vancouver Scar Scale (VSS) score was used to assess the wound scar after wound healed and the excellent and good rate was calculated.@*RESULTS@#The wound area was 21.0-180.0 cm 2 (mean, 57.82 cm 2) before operation, and it was 1.2-27.0 cm 2 (mean, 6.57 cm 2) at 2 weeks after operation. The wound healing rate at 2 weeks after operation was 76%-98% (mean, 88.6%). After operation, 2 cases needed to adjust Nice knot due to skin cutting and 1 case occurred soft tissue infection on the wound. The other patient's wounds healed. The average wound healing time was 27.8 days (range, 18-44 days). And the wound healing were grade A in 13 cases and grade B in 9 cases. VSS score was 2-9, with an average of 4.1; 10 cases were rated as excellent, 10 as good, and 2 as poor, with an excellent and good rate of 90.9%. All patients were followed up 9-24 months (mean, 14.6 months). During follow-up, no deep infection or osteomyelitis occurred. Two cases experienced fracture non-union, and were treated with compression fixation and bone grafting. The fractures of the other patients all healed, with a healing time of 85-190 days (mean, 148.2 days).@*CONCLUSION@#Nice knot technique can be used in wound closure of Gustilo type ⅢA and ⅢB open tibial fractures effectively, which is easy to operate.

ABSTRACT

Despite the introduction of novel sutureless posterior chamber intraocular lens (IOL) fixation techniques, some conditions still require suture-assisted scleral fixation. If the scleral fixation suture knot is left directly under the conjunctiva, it may become exposed, resulting in an increased risk of endophthalmitis. To avoid this problem, we offer a new alternative, simple, and safe way for burying the end of the suture using knots in this report.

ABSTRACT

OBJECTIVE@#To investigate effectiveness of suture anchor fixation combined with Nice knot strapping via longitudinal patellar drilling in the treatment of patellar inferior pole fractures.@*METHODS@#A clinical data of 37 patients with unilateral patellar inferior pole fracture who met the selection criteria between June 2017 and June 2021 was retrospectively analyzed. Among them, 17 cases were treated with the suture anchor fixation combined with Nice knot strapping via longitudinal patellar drilling (group A), and 20 cases were treated with the traditional Kirschner wire tension band technique (group B). There was no significant difference in terms of gender, age, body mass index, fracture side, combined medical disease, and preoperative hemoglobin between the two groups ( P>0.05). Operation time, intraoperative blood loss, postoperative complications, fracture healing time, knee range of motion, and knee function Bostman score (range of motion, pain, daily work, muscle atrophy, walking aids, knee effusion, soft leg, and stair climbing) and grading were recorded in both groups at last follow-up.@*RESULTS@#There was no significant difference in operation time and intraoperative blood loss between the two groups ( P>0.05). All incisions healed by first intention. All patients were followed up 1-2 years, with an average of 1.7 years. X-ray films reexamination showed that all fractures in group A healed, while 2 cases in group B did not heal. There was no significant difference in bone healing time between the two groups ( P>0.05). At last follow-up, the knee range of motion, the range of motion score of Bostman score, total score and effectiveness grading in group A were significantly better than those in group B ( P<0.05). There was no significant difference in the other items of Bostman scores between the two groups ( P>0.05). During follow-up, 2 cases of internal fixation failure and 1 case of internal fixator irritation occurred in group B, and no complication related to internal fixation occurred in group A. The occurrence of complications was significantly lower in group A than in group B ( P<0.05).@*CONCLUSION@#Compared with the traditional Kirschner wire tension band technique, the suture anchor combined with Nice knot strapping via longitudinal patellar drilling for the patellar inferior pole fractures has the advantages of simple operation, reliable fixation, early flexion and extension activity, and better functional recovery of knee joint.

ABSTRACT

Abstract Umbilical cord abnormalities are associated with adverse effects during pregnancy. The true umbilical cord knot is present in 0.3-2.1% of pregnancies and has been associated with fetal death. Risk factors include maternal age, high BMI, multiparity, hypertensive disorders, smoking, polyhydramnios, gestational diabetes, male sex of the fetus, and other malformations. Prenatal diagnosis of the true knot is rare and challenging. Most are diagnosed at birth and only 0.08% of true knots are diagnosed prenatally. Non-reassuring cardiotocography traces may be present, reflecting hypoxic or mechanical stress disrupting fetal oxygenation. The aim of this case report is to present a case of true umbilical cord knot presenting with persistent fetal tachycardia during labor, resolved by vaginal delivery.

ABSTRACT

Abstract Biocontrol of the nematode Meloidogyne javanica was studied using the Argentinean strains Pseudomonas fluorescens MME3, TAE4, TAR5 and ZME4 and Bacillus sp. B7S, B9T and B19S. Pseudomonas protegens CHA0 was used as a positive control. Egg hatching and juvenile mortality were evaluated in vitro by exposure of nematodes to bacterial suspensions or their cell-free supernatants (CFS). The effect of bacteria on nematode infestation of lettuce was also studied. results showed that most of the tested strains and CFS reduced egg hatching and juvenile survival in vitro. The bacterial suspension of Bacillus sp. B9T produced the lowest hatching of eggs. Juvenile mortality was higher when M. javanica was exposed to Bacillus sp. than to Pseudomonas spp. suspensions. Except for CFS of B9T, all filtrates inhibited hatching at levels similar to or higher than the biocontrol strain P. protegens CHA0. The CFS of CHA0 showed the highest level of juvenile mortality followed by Bacillus sp. strains and P. fluorescens TAE4. None of the inoculated rhizobacteria reverted the negative effect of infestation on the aerial dry weight of lettuce plants. However, inoculation impacted on reproduction of M. javanica by reducing the development of galls and egg masses on roots and diminishing the number of individuals both on roots and in the substrate, as well as the reproduction factor. These results show that most of the analyzed native strains can control the nematode M. javanica. Among them, P. fluorescens TAE4 and Bacillus sp. B9T showed the most promising performances for the biocontrol of this pathogen and have a potential use in the formulation of commercial products.

ABSTRACT

Abstract The true umbilical cord knot is a pathology with an incidence relatively low. It is associated with the risk of loss of fetal well-being and stillbirth. There are both maternal and fetal risk factors, with different associated perinatal outcomes. Prenatal diagnosis of a true knot is a diagnostic challenge for the obstetrician, however, nowadays it is possible by ultrasound for incidental finding or by intentional search, mainly in patients with risk factors. In the same way it should be suspected in the presence of a non-reassuring cardiotocography, especially when there is no response to the usual intrauterine resuscitation maneuvers, such as left lateral decubitus position, supplemental oxygen and maternal hydration. The purpose of this report is to describe 4 cases of true knot in term pregnancies, in a third level hospital, where an incidence of 0.03% was estimated,, with a mean gestational age of 39.3 weeks and a mean maternal age of 32.2 years. The main intrapartum findings were alterations in the cardiotocography, such as variable and late decelerations. The route of pregnancy resolution was equitable among the patients, 2 were resolved by vaginal delivery and the other 2 by cesarean section. Pregnancy was resolved by vaginal delivery in 2 of the patients, while the other 2 were resolved by cesarean section. One case of stillbirth was registered

ABSTRACT

Root-knot nematode, Meloidogyne incognita (Kofoid & White) Chitwood, is a major threat to mungbean cultivation. The pest causes a significant reduction in plant growth parameters that ultimately results in loss of grain yield. The present study was carried out under glass house condition to study the effect of different inoculum load of root-knot nematode M. incognita on plant growth, nodulation and nematode development and nutrients status of Mungbean. The results revealed a progressive decline in plant growth parameters viz., fresh and dry shoot weight and shoot length with respect to increase in inoculum level. However, fresh and dry root weight showed the opposite trend. The fresh and dry shoot weight was decreased by 44% and 66%, respectively at 4 J2s/g soil. The chlorophyll content in the leaves also decreased with the increase of inoculum level from 100-6000 J2s/pot. Nutrients contents of the plant viz. N, P, K, Ca and Mg were significantly reduced in shoots while in roots these was increased with an increase of inoculum levels. Nodulation was affected by 80% at the highest inoculum level i.e. 6000 J2s/pot. Also leghaemoglobin, bacteroid content and nitrogenase activity was reduced progressively with increased levels of nematode inoculum. Thus, the root-knot nematode, M. incognita interferes with the process of symbiotic nitrogen fixation between mungbean host and rhizobium and that can affect the quality of produce.

ABSTRACT

Nematicidal substances have been identified from plants and are potentially useful for the management of plant-parasitic nematodes. Cabralea canjerana, (Meliaceae) and Schinus terebinthifolius (Anacardiaceae) produce bioactive compounds during their secondary metabolism and little is known about the effect of such substances on plant-parasitic nematodes. In the present study, we assessed the effect of aqueous and ethanolic extracts of C. canjerana and S. terebinthifolius at 1% (m:v) and purified substances from C. canjerana (gedunin, ocotillone, cabraleadiol, a mixture of ocotillone + cabraleadiol and a mixture of shoreic acid + eichlerianic acid) on hatching and mortality of Meloidogyne incognita juveniles. Aqueous extracts of C. canjerana fruits and seeds reduced hatching by 70.3 to 95.7%. Aqueous extracts of S. terebinthifolius fruits killed 42.8 to 77.1% of juveniles. The purified substances of C. canjerana inhibited the hatching of M. incognita from 57 to 90% and did not increase the mortality of juveniles. Therefore, C. canjerana extracts and its purified substances reduce M. incognita hatching and aqueous extracts of S. terebinthifolius kill J2 of this nematode.

ABSTRACT

Objective:To explore the curative effects of Nice knot fixation on tuberosity healing in hemiarthroplasty for complex proximal humeral fractures. Methods:A retrospective analysis was conducted of the eligible 32 complex proximal humeral fractures which had been treated at Department of Trauma and Orthopedics, Peking University People's Hospital between May 1, 2016 and May 1, 2019. Nice knot fixation was used to repair greater and lesser tuberosities in hemiarthroplasty for all the patients. There were 6 males and 26 females, aged from 60 to 90 years (mean, 74.9 years). By the Neer classification, there were 4 three-part fractures combined with dislocation, 20 four-part fractures, and 8 four-part fractures combined with dislocation. Shoulder joint X-rays were taken at postoperative 1, 2, 3, 6, and 12 months at the outpatient clinic to evaluate the patients' shoulder joint mobility, visual analog scale (VAS) pain score and Constant-Murley shoulder score. Tuberosity healing was assessed based on the X-rays and related complications were recorded. Results:The 32 patients received complete follow-up for 12 to 25 months (average, 17.82 months). At the 12-month follow-up, their shoulder flexion averaged 131.3° (from 80° to 155°), abduction 126.9° (from 80° to 155°), external rotation 48.4° (from 30° to 60°), internal rotation the L2 level, VAS pain score 0.9 (from 0 to 5), and Constant-Murley score 83.4 (from 58 to 96). The rate of patient satisfaction was 87.5%(28/32). Tuberosity-related complications were observed in 6 cases with an incidence of 18.8%. Complications like infection and prosthetic loosening were found in none of the patients. Conclusion:In hemiarthroplasty for complex proximal humeral fractures, application of Nice knot to fixate greater and lesser tuberosities can lead to rigid fixation, definite curative effects and a low incidence of tuberosity-related complications.

ABSTRACT

In this article we will investigate the childhood psychosis based on the psychoanalyst Laznik proposal to address childhood psychopathologies through the topology of the borromean knot. In the case of childhood psychosis, the impasses in the formation of the subject correspond to the failures in movements 2, 3 and 6 of the braid that would give the Borromean knot its consistency in three links representing the Real, Symbolic and Imaginary. Bearing in mind that the constituent movements concern the interactions of the primordial Other incarnated by the maternal agent and the subject in training, the questions raised to the child who is heading for a possible psychosis and the specificity of his position before the Other will be highlighted

ABSTRACT

Objective: Wisdom teeth were the most common teeth affected among the rest of the teeth, these teeth need accurate evaluation and diagnosis, surgical removal of impacted wisdom needs time for incision, bone removal and suture, so the use of any instrument helps to reduce this time is essential. The objectives of this study were to estimate the time needed for the suture of oral flaps post-surgical removal of the lower wisdom teeth by using Negus knot pusher and compare it with the time used for stitching in conventional manual holding single stitching. Material and Methods: Data had been collected from thirty patients through history, clinical examination and radiographic assessment, followed by classical surgical removal performed on the same principles for all patients, at the suture level patients divided arbitrarily into two groups, control group 15 patients and 15 patients Negus pusher group, using Negus knot pusher, which is usually used to hold the stitch and stop bleeding post tonsillectomy operations by ligation, time elapsed for suturing with knot tying is calculated for both groups. Results: Among the 30 patients incorporated in the clinical study, control group mean was (1 minutes and 11 seconds and 70 milliseconds) while for the pusher group was (1 minutes and 32 seconds and 57 milliseconds), the comparison by means of T-test was not significant with the (0.424) value. Conclusion: the Negus pusher instrument can be used for the knotting stiches post wisdom teeth removal as an auxiliary tool and the time can be shortened by the use of a modified handling technique and more satisfaction for patients. (AU)

ABSTRACT

ABSTRACT: To investigate the degree of parasitism of two populations of Meloidogyne exigua, the gall index (GI) and the reproduction factor (RF) of M. exigua races 1 (Est E2) and 2 (Est E1) were analyzed in 47 progenies on F3:4 or F4:5 generation derived from the crossing between Coffea arabica cv. Catuaí Amarelo and Timor Hybrid. C. canephora cv. Apoatã IAC 2258 and C. arabica cv. Catuaí Vermelho IAC 144 were used as resistance and susceptibility checks, respectively. The genotypes that were classified as resistant or susceptible by RF were similarly classified by GI, showing a close relationship between both methodologies. The data also indicated no differences in virulence between the nematode populations, since the progenies showed similar resistance reactions to the M. exigua races 1 and 2. According to GI from the 47 mother plants evaluated, 27 progenies (57.4%) were classified as resistant to M. exigua races 1 and 2, with GI ranging from 0.0 to 1.4 and 20 progenies (42.6%) were susceptible with GI from 2.6 to 4.4. These results showed that most of the evaluated germplasm was very promising in relation to the development of new Arabica coffee cultivars with resistance to M. exigua.

ABSTRACT

Plant parasitic nematodes are major threats to Brazilian and world agriculture. Among them, Pratylenchus brachyurus and Meloidogyne incognita stand out as major pests for several crops, including corn, cotton, soybean among others, which can be components of integrated crop-livestock-forestry systems (ICLFs). In this context, information about the host status of eucalypts (Corymbia spp. and Eucalyptus spp.) to plant parasitic nematodes becomes more relevant in Brazil, due to the use of eucalypts in ICLFs. If tree components used on this system increase the population density of P. brachyurus and M. incognita, it is possible that these pathogens could damage shorter-cycle crops. Since information about the host status of eucalypts to plant parasitic nematodes is scarce, this study evaluated the host status of some eucalypt species to P. brachyurus and M. incognita. Two greenhouse trials were done to evaluate the reproduction of P. brachyurus and one to M. incognita, using some of the most cultivated species and hybrids of eucalypts in Brazil. The population growth of P. brachyurus increased on Corymbia citriodora, Eucalyptus grandis, Eucalyptus dunnii × Eucalyptus urophylla, and E. grandis × E. urophylla after ~90 days of inoculation. Conversely, despite M. incognita reproducing well in the control plants, no individuals were recovered from C. citriodora, E. urophylla and E. grandis × E. urophylla, which were classified as resistant plants. Based on both obtained and available data, M. incognita poses no threat to eucalypt species today. However, P. brachyurus is suggested to be a threat to eucalypts.

ABSTRACT

ABSTRACT: Meloidogyne enterolobii is known as the most aggressive root-knot nematode (RKN) in Brazil. Common bean (Phaseolus vulgaris) cultivated in the experimental field in Morrinhos, Southern Goiás State, exhibited symptoms typical to those caused by RKN. In order to clarify the etiology of the disease, root systems were analyzed. Typical galls of varying thickness were observed, with thickening larger than 3 cm in diameter containing RKN females inside roots and egg masses on the root surface. The identification of the nematode species was confirmed by isoenzyme electrophoresis (esterase and malate dehydrogenase phenotypes). This is the first report of M. enterolobii in Goiás and in Brazil parasitizing common bean under field conditions. This result is an alert to producers and to the State phytosanitary defense authorities, in order to prevent the further dissemination of this pest, especially in fields of common bean and industrial tomato in the State.

ABSTRACT

Brazil is currently the world's largest producer and exporter of sugarcane, and the crop has high socioeconomic importance in the country. Root-knot nematodes (Meloidogyne spp.) are one of the major limiting factors in sugarcane production. These plant parasites have wide geographic distribution, high damage potential, and are difficult to control. Recently, the species Meloidogyne enterolobii was identified in sugarcane crops in the state of Pernambuco, Brazil. Given the importance of genetic resistance for integrated nematode management and the lack of research on the M. enterolobiisugarcane pathosystem, this study aimed to assess the response of sugarcane cultivars to M. enterolobii. Thirteen cultivars were evaluated for their resistance to M. enterolobii based on the nematode reproduction factor. The experiment was conducted in a greenhouse, in a completely randomized design, with 14 replicates. Pre-sprouted sugarcane seedlings were transplanted to 5 L pots, and each pot was considered an experimental unit. At 15 days after transplanting, the seedlings were inoculated with 5,000 eggs and second-stage juveniles of M. enterolobii. Tomato and okra plants were also inoculated to test the viability of the inoculum. At 240 days after inoculation, plant roots were processed and evaluated for nematode number. This parameter was used to calculate the nematode reproduction factor on each cultivar. All sugarcane cultivars were found to be immune to M. enterolobii, with a reproduction factor of 0.

ABSTRACT

Introduction: Laparoscopic cholecystectomy today hasbeen rapidly embraced worldwide as the procedure of choicefor cholecystectomy. Conventionally, titanium clips areused to ligate the cystic duct. Recently, various methods ofsuture ligation of the cystic duct both intra corporeal andextra corporeal have been described during laparoscopiccholecystectomy. Only limited number of studies have beencarried out prospectively to compare the various methods. Thepresent study was thus designed to compare clip occlusionversus extra corporeal suture ligation (Roeder knot) of thecystic duct in laparoscopic cholecystectomy.Material and methods: This study was a single blindedrandomized controlled trial conducted on 150 adult patientsundergoing laparoscopic cholecystectomy. They wererandomized into two groups of 75 each to compare theprocedures of clip occlusion versus extracorporeal sutureligation (Roeder knot) of the cystic duct with respect to theoperative time, complications, associated morbidity, operativecost and hospital stay. Results: There was no significant (p>0.05) differencein duration of surgery between the groups (Clipligation=40.44±4.63 minutes, suture ligation=43.32±4.44minutes). Bile leakage due to slippage of ligature was presentin 2 (2.7%) patients with clip ligation as compared to 0% insuture ligation. This difference was found to be statisticallysignificant (p=0.03). Obstructive jaundice due to accidentalligation of CBD was present in 2 (2.7%) patients with clipligation as compared to 0% in suture ligation. This differencewas also found to be statistically significant. (p=0.03). Thecost of Clip ligation (Rs.369.07±8.08) was higher than sutureligation (Rs.300.00±0.00).Conclusion: The results of this study indicate thatextracorporeal (Roeder’s knot) is a safe, cost effectivealternative with a low complication rate as compared totitanium clip ligation of the cystic duct in laparoscopiccholecystectomy
