ABSTRACT

@#Introduction: Atrial fibrillation (AF) is known to lead to stroke and thromboembolism, causing a five-fold increase in the risk of stroke and almost doubling the mortality rate. Optimal anticoagulant therapy is effective in reducing AF-related death. However, prescription of anticoagulants in AF in East Asian countries has been low, ranging from 0.5% to 28%. This study aimed to determine whether vocational training in family medicine improves primary care physicians’ knowledge, attitude, and practice in the management of AF. Method: This investigation was a cross-sectional study carried out during centralized workshops for two groups of trainees using a validated questionnaire: (i) junior trainees were newly enrolled postgraduate trainees in the Graduate Certificate in Family Medicine (GCFM) program, and (ii) senior trainees were postgraduate trainees in Advance Training in Family Medicine (ATFM) programs of the Academy of Family Physicians of Malaysia (AFPM). Results: A total of 223 trainees (127 junior and 96 senior) participated in this study. Only 55.2% of the trainees passed the knowledge test; senior trainees were more likely to pass the knowledge test compared to junior trainees (69.8% vs. 44.1%, p <0.001). Female trainees were significantly more likely to pass the knowledge test than male trainees. While the attitude of senior and junior trainees was similar, more of the latter group worked in public clinic that provide better support where there is better support for outpatient anticoagulation treatment (e.g., same-day INR test, direct access echocardiogram, and warfarin in in-house pharmacy). Conclusion: Vocational training in family medicine appears to improve primary care physicians’ knowledge regarding the management of AF. Better knowledge will help vocationally trained primary care physicians to provide anticoagulation treatment for AF within primary care clinics. More optimal AF management within primary care can take place if the identified barriers are addressed and a shared care plan can be implemented.

ABSTRACT

ABSTRACT Objective: To determine the knowledge attitude and practice (KAP) and prevalence of syphilis and to investigate the sexual health practices and constraints among commercial sex workers (CSWs) in Guyana. Methods: The participants were randomly selected from CSWs participating in support group meetings held in Georgetown and Berbice. The survey was cross-sectional, and SPSS 20.0 was used to perform the data analysis. Results: The majority (92.9%) of CSWs tested negative for syphilis, whereas two (2.9%) of the five (7.1%) reactive Venereal Disease Research Laboratory cases indicated prior history of syphilis infection. In the study, 54.3% of participants always used condoms, whereas 74.3% did not use drugs and only 2.9% consumed alcohol every day. The participants had a fair KAP towards syphilis but few misconceptions exist. Stigma and discrimination was identified as the major constraint faced by male sex workers, and 97.1% of CSWs indicated that they preferred giving up sex work. Conclusion: Syphilis was not prevalent among the selected CSWs in Georgetown and Berbice. Prevention programmes should be continued through the support groups to maintain and increase safe sexual practices among female sex workers. Strategies should also be tailored to provide rehabilitation to the CSWs, especially for those willing to give up sex work.

ABSTRACT

Background: Stressful puerperal period and its complications can be prevented by adequate care, proper diet and exercises. Though studies have indicated post-natal exercises (PNE) is helpful in reducing the effect of perineal muscle weakness, many women are unaware of its full benefits. This study is planned to know the knowledge, attitude and practice of PNE in post-natal mothers. Methods: Descriptive cross-sectional study was conducted in department of obstetrics and gynecology for 3 months (November 2019 to January 2020). 160 post-natal women were enrolled with convenient sampling. Structured questionnaire was used to collect socio-demographic and obstetric details, source of information, knowledge, attitude and practice of PNE.Results: A total 58.8% of this study participants revealed health care professions were the commonest source of information. More than half of them had adequate knowledge but still many were not aware of full benefits of PNE as only 3-5% aware of other benefits like prevention of urinary incontinence. 98.8% of the women’s felt PNE is essential and 62.5% felt household work hinder them doing. 92.5% said they will emphasize the importance of PNE to others. No significant correlation found between socio-demographic characters and knowledge, attitude and practice of PNE.Conclusions: Though most of the study population had adequate knowledge and positive attitude, household activities were the commonest barrier in doing PNE. Educating and creating awareness among family members is essential to improve the adherence. Continuing influence by health care workers during antenatal and postnatal period will be the key for improving adherence.

ABSTRACT

Objective : To investigate the correlation between knowledge, self-efficacy and prevention management behaviour of osteoporosis among middle-aged and elderly community residents in Shanghai, based on the knowledge, attitude, and practice (KAP) theory. Methods ¡¤ By using convenient sampling, a cross-sectional study was conducted to obtain information of 2 058 residents from 20 communities in Shanghai, concerning socio-demographics, osteoporosis knowledge scale, osteoporosis self-efficacy scale and osteoporosis prevention management be-haviour scale questionnaires. The structural equation model was applied to correlation analysis and path analysis. Results ¡¤ A total of 2 001 valid questionnaires were collected, the average age being (62.12±12.08) years; female osteoporosis prevention self-management behaviour (17.25±5.35) was shown to be better men's (t=3.78, P<0.001). A higher score of prevention management behaviour in osteoporosis was obtained from residents of college education or higher (18.25±5.18), compared with those of primary school or lower (15.34±4.74), and high school (17.09±5.31) (F=26.93, P<0.001), respectively. Higher scores were revealed in groups with family history (18.13±4.73) compared with those without (16.89±5.33) (t=3.89, P<0.001). Higher income groups achieved higher scores (17.90± 4.52), compared with relatively lower income groups (F=8.66, P<0.001). Multiple factor analysis demonstrated that knowledge and self-efficacy were significantly correlated with prevention management behaviour (β=0.27, 95% CI 0.22-0.32; β=1.01, 95% CI 0.92-1.11). Path analysis showed that relevant knowledge (β=0.169, P<0.001) had direct effect on the prevention management behaviour, and could also be mediated by self-efficacy (β=0.410, P<0.001). Conclu-sion ¡¤ Male, low education, no family history of osteoporosis and low income are the risk factors for the decrease of osteoporosis prevention and management behaviour.

ABSTRACT

@#Introduction: Hypertension is a public health concern that is associated with heart disease. This study aims to investigate the prevalence of undiagnosed hypertension and its associated factors among the staff in a private university in Selangor, Malaysia. Methods: 100 respondents participated in this cross-sectional study. Blood pressure, Body Mass Index (BMI), percentage body fat (%BF), visceral fat rating (VFR), and waist circumference (WC) were measured. Questionnaires regarding knowledge, attitude, and practice (KAP) of salt diet validated from WHO/PAHO and Food Frequency Questionnaires regarding sodium intake validated from IPH Malaysia were administered. Statistical data was analysed using SPSS. Results: The prevalence of undiagnosed hypertension was 24%. About 66% of the participants had excessive sodium intake. The mean sodium intake was 2869.43±930.75mg/d. Simple linear regression showed that BMI. %BF, VFR, and WC were significantly correlated with systolic blood pressure (SBP). Age and sodium intake were not correlated with SBP. After controlling all the variables in multiple linear regression, VFR remained as a significant contributor to SBP (adjusted R2=0.419, F=18.833, p=<0.001). Soy sauce, omelette, fried rice, and nasi lemak were the main contributors of sodium intake. A lack of knowledge on the negative impacts of high salt diet was significantly associated with high dietary sodium intake (>2400mg/d). Conclusion: Our study found a high prevalence of undiagnosed hypertension among the private university staff. The association between VFR and SBP is an important finding for community study. Increased awareness on the excessive salt consumed and its association with health is needed to reduce the sodium intake.
