ABSTRACT

Objective:To build the standardized knowledge base for hierarchical prevention care of neonatal hypoglycemia based on the risk prediction model of neonatal hypoglycemia, and to provide a decision-making basis for risk management to achieve predicitive neonatal hypoglycemia.Methods:Based on the best evidence summarized in strategies for the prevention and management of neonatal hypoglycemia published in 2020, evidence on the prevention and management of neonatal hypoglycemia was searched from BMJ Best Practice, UpToDate, Registered Nurses Association of Ontario, CNKI and other domestic and foreign databases and professional association websites. The retrieval period was from September 1, 2019 to August 31, 2022. The quality of newly included literature was evaluated, new evidence was extracted, and the best evidence in the prevention and management strategy of neonatal hypoglycemia published in 2020 was summarized and combined to form the first draft of the knowledge base. Experts in the field of neonatal nursing were invited to revise and discuss each item of the knowledge base, and the final draft of the knowledge base was formed. The final draft of the knowledge base was coded using the 2.5 version of the Clinical care classification system as the standardized language.Results:The risk prediction model of neonatal hypoglycemia was used as a grading tool, the final draft of the knowledge included 1 nursing diagnosis, 6 modules and 18 specific preventive nursing measures.Conclusions:The knowledge base for hierarchical prevention care of neonatal hypoglycemia based on risk prediction model can realize the prospective hierarchical nursing of neonatal hypoglycemia, which is scientific and practical, and is the basis to assist nurses to make clinical decisions.

ABSTRACT

Objective:To construct the knowledge base of Tibetan medicine prescriptions and explore to standardize the names of Tibetan medicine prescriptions. Method:By using the concept of "man-machine combination",through the construction of Tibetan medicine terminology glossary (data sources: national drug standards,local drug standards,text classics on Tibetan medicines,etc.),the terminology glossary of Tibetan medicine prescriptions was mined. Upon its combination with expert review,the text association between Tibetan medicine prescriptions and various drug standards and dictionaries was constructed,and the standardization methods and techniques of prescription drug names were explored. Result:In this paper,the Tibetan medicine prescriptions approved for marketing in China were taken as the research object,and various inconveniences caused by the inconsistency between the names of prescriptions and the names of medicinal herbs were revealed. This paper also discussed the design ideas on name standardization of Tibetan medicines from three levels: text association,optimization of evaluation methods,and formation of expert decision-making system. We put forward a five-in-one (screening, evaluation, reviewing, fixing, and renewing) research model of Tibetan medicine name standardization. The construction,functions and advantages of the database and thesaurus of Tibetan medicine prescriptions were described in detail, and in combination with the text notes, association between the standard medicinal materials and the prepared prescriptions was then established. Conclusion:The text association method in this paper can accurately reflect the nonstandard names of Tibetan medicine prescriptions. Combined with expert review,it can be, to a certain extent, extended to the standardization of herb names in prescriptions with large scale of or more complex network structures.

ABSTRACT

The construction of medical knowledge platform is a core value of the intelligent construction of electronic medical records. The hospital-wide knowledge base construction covers a wide range of content, including multiple healthcare scenarios such as medicine, testing, inspection, surgery, blood transfusion and nursing. This article introduced how Jiangsu Province People′s Hospital used knowledge graphs and rule engine to construct a hospital knowledge management platform, realize the integration of knowledge-based knowledge base and a non-knowledge-based knowledge base, and embed clinical diagnosis and treatment rules into the information system for different flexible application scenarios.Finally, a multi-dimensional knowledge base was formed to realize the unified knowledge information integration of various clinical expert knowledge, and to provide integrated display and decision support for all departments, as well as realizing real-time data verification, prompting and control in each link.

ABSTRACT

ABSTRACT From 1980s to 1990s, the Brazilian Physical Education (BPE) was strongly influenced by the discussion concerning its identity as an academic discipline; the final period of the Brazilian military dictatorship; the regulation of the BPE as a profession. Hence, this study analyzed the influence of that epistemological and political setting on the curricula of three BPE undergraduate courses. The Political Pedagogic Project and the Curriculum Matrix of the three undergraduate courses, from public universities, were analyzed using two qualitative research techniques: body of knowledge analysis, and content analysis. The results showed that the curricula analyzed did not cover a definition of professional intervention procedures based on ethical conduct specific to the area. Furthermore, the knowledge basis of the curricula is mainly constituted by discipline knowledge, which contributes to distancing the student's training from their field of professional practice . These findings may be associated with the insufficient epistemological discussion early initiated by Henry's works and unclear definition if the BPE is an academic or professional area. This investigation suggests that political disputes related to professionalization of BPE overlapped the academic discussions, which might have contributed to the problems identified on the preparation through bachelor courses of physical education professionals.


RESUMO De 1980 a 1990, a Educação Física Brasileira (EFB) foi fortemente influenciada pela discussão sobre sua identidade como disciplina acadêmica; pelo período final da ditadura militar; e pela regulamentação da EFB como profissão. Assim, este estudo analisou a influência dessa configuração epistemológica e política sobre o currículo de três cursos de graduação em Educação Física (EF) no Brasil. O Projeto Político Pedagógico e a Matriz Curricular dos três cursos, de universidades públicas, foram analisados utilizando duas técnicas de pesquisa qualitativa: análise do corpo de conhecimento e análise de conteúdo. Os resultados mostraram que todos os currículos analisados não apresentaram uma definição clara dos procedimentos de intervenção profissional baseados em condutas éticas específicas da área. Além disso, a base de conhecimento dos currículos é principalmente constituída pelo conhecimento acadêmico, contribuindo para o distanciamento da formação do graduando com o campo de atuação profissional. Essas descobertas podem estar associadas à insuficiente discussão epistemológica que foi iniciada pelos trabalhos de Henry e a não definição se a EFB é uma área acadêmica ou profissional. Esta investigação sugere que as disputas políticas relacionadas à profissionalização da EFB se sobrepuseram às discussões acadêmicas, o que pode ter contribuído para os problemas identificados na formação do tipo bacharelado dos profissionais de EF.

ABSTRACT

OBJECTIVE: To provide technical and practical basis for data-driven pharmaceutical management model. METHODS: The information of drug attribute classification and the way of network acquisition were studied, and the construction of drug attribute classification knowledge base was realized by using network crawler technology. On this basis, by using NLP technology and regular expression matching technology, the information of the dictionary table of medicines in HIS and the drug name, specifications and manufacturers in the drug attribute classification knowledge base were compared one by one. The correspondence of drug codes between “HIS database” and “drug attribute classification knowledge base” was realized. Finally, drug attribute classification knowledge base can be used to practice drug use analysis in medical institutions. RESULTS: Established drug attribute classification knowledge base can rapidly and accurately analyze the use of antibiotics, evaluate the reasonability of drug procurement and screen unreasonable drug use case rapidly. “Medical Institution Pharmaceutical Management Analysis and Decision-Making System” was established by using knowledge base of drug attribute classification and control technology. All prescription reviews had been realized with this system, and the main problems of irrational drug use in clinic, such as over-dose, over-course of treatment, combined use of main drugs with the same function and over-indication drug use could be solved. Through the practice and application of the prescription comment model, the above problems in our hospital’s prescription comment were almost nonexistent. Antibacterial use indicators were also in line with national requirements. CONCLUSIONS: It is beneficial to the information construction of pharmacy administration by linking actual use of drugs in medical institutions with drug-related knowledge base through modern information technology and taking the actual use of drugs in medical institutions as the object of analysis. In the future, it is necessary to build knowledge bases for different purposes and types based on the requirements of pharmaceutical administration. It lays a foundation for the practice of data-driven pharmaceutical administration.

ABSTRACT

OBJECTIVE: To promote the implementation of the pre-prescription review work, and to ensure the rational drug use of patients. METHODS: With the idea of PDCA (Plan, Do, Check, Action) cycle management, the phased improvement of three PDCA cycles was gradually implemented in the operation of the pre-prescription review system, aiming at the establishment of the working mode of the prescription review work, the improvement of the rules of knowledge base in the review system and the improvement of the ability of pharmacists to review prescriptions. The operation results of the system were evaluated by comparing the irrational prescription rate of outpatient pharmacies before and after the operation of pre-prescription review system. RESULTS: Through adopting the prescription review mode of “rigid” and “flexible” interception, regular revision of knowledge base rules, regular training and examination of prescription pharmacists, pre-prescription review system operated smoothly, and the pre-prescription review work was carried out in the process of continuous improvement. In the three PDCA cycles, the irrational rate of prescriptions decreased significantly, such as after the first PDCA cycle, the irrational rate of TCM outpatient prescriptions decreased from 22.0% (1 393/6 332) in Jan. 2017 to 7.4% (416/5 627) in Jun. 2017; after the second PDCA cycle, the irrational rate of outpatient prescriptions in hospital decreased from 4.87% (5 244/107 691) in Mar. 2018 to 2.21% (2 219/100 412) in Aug. 2018. After the third PDCA cycle, the percentage of over-treatment course prescriptions in total prescriptions decreased from 16.97% (15 728/92 684) in Jun. 2018 to 5.55% (5 394/97 275) in Sept. 2018. CONCLUSIONS: The pre-prescription review system can effectively intercept and interfere with irrational prescriptions, and PDCA cycle management can effectively promote the operation of the pre-prescription review work.

Objective: To fulfill the TCM (traditional Chinese medicine) health management needs of female through constructing the intervention knowledge base, explore the long-term development mechanism of intervention knowledge base. Methods: The structure of female TCM Health Intervention Knowledge was designed base according to the characteristics of TCM health status of females. The inferential mechanism by studying the relation between the female health status and the diet therapy scheme was explored based on syndrome differentiation. A Long-term development mechanism of TCM health intervention knowledge database was designed based on big data and effect feedback mechanism. Results: The classification standard of the TCM Health Intervention Knowledge base of female includes the state of the viscera functions, qi and blood, yin and yang, and pathogenic properties of the body under the illness and health condition. The dietary therapy formula inference engine can provide food material according to the region, natures, function of tonifying, evil eliminating, to match the female health status, and constantly adjusting and improving the Intervention plan according to the feedback. Conclusion: The study proves the construction plan of TCM Health Intervention Knowledge base of female is able to meet the needs of female TCM health management intervention, and the method of constantly adjusting and improving the intervention plan according to the feedback helps to realize the precise recommendation and long-term development of intervention knowledge base.

A laboratory animal expert consultation system provides an interactive platform for industry experts and users. Based on intelligent mobile terminal used as the application supporter, the platform combines the internet with laboratory animal to share the experts' knowledge, breaks through temporal and spatial restrictions, and resolves other problems such as poor timeliness of inquire service, knowledge data staticization, and irrational exploitation of internet to gain prompt inquire service. The platform would meet the needs of industry users for real-time aids and bridge the gap between industry experts and users. Thus, the transfer of expert' knowledge is realized in pace with the increasing knowledge resource of this platform to accelerate the transformation of theoretical knowledge into the industry. Our study focuses on the system characteristics, the main frame structure, the design of function module and the system implementation.

ABSTRACT

Objective To investigate the knowledge base and research focuses of stroke rehabilitation via a bibliometrical approach. Methods We searched the Web of Science and identified a total of 5 253 entries on stroke rehabilitation which were published from 2010 to 2015, The literature amount, country distribution, highly cited literature, and high frequency keywords of the 6 years literatures were recorded, and the retrieved literatures were then subjected Cite Space visualization software analysis to draw the co-citation network clustering map and keyword co-occurrence graph. The basics and research focuses of stroke rehabilitation were analyzed. Results The publications on stroke rehabilitation increased year by year during 2010-2015. The United States, South Korea and Australia were the major countries for stroke rehabilitation research; and the literatures of China had a great increase. The highly cited articles of stroke rehabilitation mainly involved researches about Fug-Meyer scale, MMSE scale, modified Ashworth scale and Barthel index rating scale and dysfunction recovery. High frequency keyword analysis showed that the researches mainly focused hemiplegia dysfunction in stroke rehabilitation and new techniques and ideas represented by optimized constraint-induced movement therapy and transcranial magnetic stimulation. Conclusion Researches on stroke rehabilitation is in a period of vigorous development, and increasingly more emphasis has been put on stroke rehabilitation research. The highly cited literatures and high frequency keywords can reflect the knowledge base and research focus of stroke rehabilitation, which can be used for further related research.

With the development of hospital information construction,the focus has gradually changed from functions of the information system to data utilization.The utilization of medical data has great significance in health management,disease analysis,summary of treatment experience,and development of new technologies.By data mining technology,the paper extracts data related to the author's hospital,starts from intelligent diagnosis and simulates the establishment of the knowledge base for hospital clinical diagnosis.Upon verification,the relevance judged by the system is basically consistent with the clinical practice.Thus,it proposes the possibility to extend the application to hospital clinical pathways.

OBJECTIVE:To improve the level of rational drug use,and to reduce the workload of clinical pharmacists. METH-ODS:Established“Intelligent Management and Solution for Clinical Rational Drug Use”of our hospital was introduced,including post-prescription review(all prescriptions review)system, self-maintenance system for rational drug use knowledge base,pre-pre-scription review (rational drug use real-time review) early warning system. The effects of intelligent system were evaluated. RE-SULTS & CONCLUSIONS:After the application of the system,the following aspects could be achieved,including“all prescrip-tions review”,“multi-prescriptions/longitudinal review”,system knowledge base real-time update and autonomous definition of re-view rules,effective communication and intervention between physicians and pharmacists before printing prescription by Yaozhidao instant messaging system. Through related trial evaluation,the proportion of false positive prescriptions decreased by 17.65% after 4 months of the application of the system;drug consultation prescriptions review average score of outpatient pharmacists increased from 81 to 94.85;the time of prescription review decreased by 4.72-5.46 times. Through mandatory interception of unqualified pre-scriptions,severe warning,timely prompt and pharmacists’intervention,the rate of unqualified prescriptions pre-reviewed by the system in respiration department decreased from 32% in Jan. 2015 and 21% in May to 4.62% in Dec. So,the application of the system promotes rational drug use in outpatient department,reduces the workload of pharmacists and ensures the safety of drug use comprehensively.

The combination of biochemistry analyzer and medical expert system was proposed in this report. Biochemistry analyzer is one of the most important analytical instrument used in clinical detection. It could take immunological examination and biochemical analysis for blood, urine, pleural effusion and cerebrospinal fluid and other body fluids. Medical expert system is an intelligent program system with knowledge and experience of a large number of medical specialists. It could use the knowledge and method of medical experts to solve and deal with problems in the field. This system mainly includes human interface, inference engine, interpreter, knowledge acquisition procedures, integrated database and knowledge base. Some parts of system design, such as the expression and design, and interpretation mechanism of the knowledge base, have been interpreted in details. It adopts production as an expression of knowledge. Generally, knowledge was expressed as if the conditions, then the conclusion form. Interpretation mechanism use the error counter propagation of neural network to train the algorithm (BP algorithm for short).The combination could automatically conduct comprehensive analysis of various data generated by the instruments, and then obtain the science theoretical foundation and the most reasonable specialist conclusions. This report provides an overview of the system design of medical expert system.

ABSTRACT

La opacidad de la cápsula posterior (OCP) es actualmente uno de los aspectos más importantes en la cirugía de catarata de los tiempos modernos. Sigue siendo la complicación posoperatoria tardía más frecuente tras la cirugía de catarata asociada con disminución de la agudeza visual, deterioro de la sensibilidad al contraste y problemas de deslumbramiento que conllevan importantes repercusiones sociales, médicas y económicas. El software PANDOC provee al oftalmólogo de una herramienta por medio de la cual este es capaz de cuantificar numéricamente y detectar diferencias de opacidad (a veces imperceptibles para el ojo humano), logrando así una evaluación objetiva del grado de opacidad y de esta forma minimizar el sesgo de observación entre un médico y otro. Por ello surge la necesidad de identificar automáticamente la OCP en estas imágenes, para lo cual se diseña un sistema basado en casos. La presente investigación se enmarca en la aplicación de un sistema basado en casos integrado al software PANDOC, para identificar y cuantificar objetivamente OCP mediante el uso de las imágenes resultantes de la cámara Scheimpflug del Pentacam(AU)


The posterior capsule opacity (PCO) is nowadays one of the most important aspects in cataract surgery of modern times. It´s still the most frequent post operatory complication associated with visual acuity diminution, deterioration of contrast sensitivity and blinding problems that lead to social, medical and economic repercussions. PANDOC software is a tool for numerical quantification and detection of opacity differences (sometimes imperceptible for human eyes), reaching an objective evaluation of opacity degree and minimizing the observation bias between one doctor and another. For that reason is necessary to identify automatically PCO in these images, for which is designed a case-based system. The present research is oriented to the application of a case-based system for the PANDOC software, to identify and quantify objectively PCO using the resulting images of the Pentacam´sScheimpflug camera(AU)
