ABSTRACT

Objective:To evaluate the application value of patient-based real-time quality control (PBRTQC) algorithms in intralaboratory comparison between various hematology analyzers.Method:From April 1 st 2020 to March 31 th 2021, data of white blood cell (WBC) counts and daily comparison results of fresh venous blood, measured by five hematology analyzers, were collected at the Department of Laboratory Medicine in Hebei Children′s Hospital. First, the professional intelligent PBRTQC software system was applied to conduct the parameter setting, program establishment, and performance verification. Three concentration ranges of WBC were selected, low concentration (2.5-4.5)×10 9/L, medium concentration (6.0-8.0)×10 9/L and high concentration (12.0-14.0)×10 9/L for the comparison. Next, WBC counts were calculated with both of the EWMA and median methods, the results were then analyzed by PBRTQC using the module of"intralaboratory comparison of hematology analyzers". Finally, bias of intralaboratory comparison among various hematology analyzers analyzed by means of EWMA and daily comparison results of fresh venous blood were compared. Based on the standard of WS/T 406-2012,allowable error ±7.50% in WBC counts was set as the relative bias standard among different instruments. Results:(1) A total of 38 313 sample results were included, there were 70 warning results out of these samples based on the EWMA quality control method established on the data of patients with white blood cell count in our laboratory, with an early warning rate of 0.183‰, a probability of error detection of 100%, and a probability of false loss of control of 0. EWMA quality control efficiency met the quality objectives. (2) In the comparison monitoring of the results of 5 blood cell analyzers at high concentrations, the coincidence rate between EWMA and median method were both 100% (46/46) in weekly and monthly comparison, and EWMA could maintain a relatively stable monitoring efficiency in daily comparison. (3) In the selected natural month, the consistency rate between EWMA method and fresh blood comparison method was 95.24% (20/21).Conclusion:PBRTQC can be used as a valuable supplementary tool of IQC to continuously and effectively monitor the consistency of data derived from intralaboratory hematology analyzers with different bands and types, which can not only reduce the risk of quality and operating costs, but also improve the efficiency of laboratory management.

ABSTRACT

The Laboratory of Pulmonary Diseases at West China Hospital in Sichuan University starts with strengthening the laboratory safety education and research integrity, develops a special individualized experimental skill training, improves communication between project team and laboratory, and promotes the cooperation between the laboratory and university academic community. Combined with the new model of project hierarchical management, the "Innovation and Enterpreneurship Training Programs for Undergraduates" are dynamically evaluated and effectively supervised. With the implementation of the new management mode, the number of projects successfully obtained has increased by 16.7%, the number of published academic papers has increased by 6.5 times, and the quality of published papers has been also significantly improved.

ABSTRACT

BACKGROUND: Clinical microbiological tests are conducted at night, on weekends, and during public holidays in various manners due to the fact that both manual processes and the form of working type, are not well known. Therefore, we surveyed the current running condition of these laboratories and made some suggestions for better-quality clinical microbiology tests. METHODS: We conducted a survey, both online and offline, focusing on the operating styles of clinical microbiological tests within laboratories that had participated in an external quality assessment program, conducted by the Korean Association of External Quality Assessment Service (KEQAS). RESULTS: Out of 341 laboratories that participated in the microbiology program of KEQAS, 128 replied to our questionnaires. In Korea, various types of operations occur within clinical microbiology laboratories. Those in night duty operate in either shifts or dedicated duties. In the case of weekend shifts, they either operated on single-day schedules (either on a Saturday or a Sunday), or over the entire weekend. For public holidays, the laboratories operated in various manners, depending on the number of days off. Among the clinical microbiological tests conducted at night, on weekends, and during public holidays, Gram staining and inoculations are the most common tasks carried out, with some laboratories conducting antibiotic susceptibility tests as well. CONCLUSIONS: Rapid reporting of clinical microbiological test results is currently inadequate due to both cost and labor constraints, despite its many advantages. It would be ultimately beneficial for both the patient and the hospital to switch to a 24/7 operating schedule through the utilization of a variety of methods, including cost control, coordination of a fine workforce, and prioritization of tests needing to be reported.

Because of the particularity and the complexity of integrated management of food, government regulators must perform the whole process of quality control and trace to evaluate and judge the merits of food instead of only depending on product standards.Quality metrics is a new concept for the food industry, and can be regarded as the comprehensive evaluation and measurement for product quality and the standard for judging the quality of food products.Request for Quality Metrics Guidance for Industry was issued, which not only established a new model for the quality control of pharmaceutical industry, but also provided a new way of quality management for the inspection organization.By introducing the contents of Request for Quality Metrics Guidance for Industry issued by the US FDA, on the basis of summarization and thought, the quality management and evaluation of pharmaceutical industry was compared, and the feasibility and applicability of quality metrics management in inspection and testing field, especially in food testing field was evaluated.In this paper, the concept of quality metrics was extended to the level of food inspection laboratory management with hope to provide ideas for the future development of food quality management model.

ABSTRACT

Based on the construction and management practice of the morphologic experimental center in Xi'an Medical University, the achievements in laboratory daily operation and institutional man-agement were summarized in the area of lab rules and regulations, instrument and equipment, experiment teaching, lab environment and safety, lab staff administration and so on. The management work has been refined using the practice model of resource sharing, system administration, individual responsibility, and unified staff supervision. The lab rules and responsibilities were also effectively implemented on specific person. Taking the opportunity in teaching evaluation at the experimental center, the lab connotation con-struction was further strengthened. The evaluation system was thoroughly examined in order to look for gaps and promote the lab construction. Further work could be carried out on the laboratory software and hard-ware, such as instrument and equipment update, experimental teaching system reformation in morphology, promotion on multidisciplinary integration and unified management of lab staff.

The current situation in basic biomedicine research laboratory is limited space,crowded instruments and equipment,and with heavy duty on lab management.Over the past decade we have applied the effective implementation of the laboratory equipment and space sharing,with a democratic centralism simplify lab management.The pattern of professors under the leadership of director (or principal investigator) collective leadership,and combined centralized management mode with the laboratory equipment and space sharing,and simplified mode that the professors or principal investigators are responsible,to make the use of lab space and equipment more rational,which also improve the effectiveness of lab management,more cohesive of lab staff,lab work running stable and smooth.

O uso dos indicadores da qualidade vem sendo valorizado na gestão dos laboratórios clínicos para otimizar a qualificação e a quantificação das falhas nos diferentes processos laboratoriais, bem como para auxiliar a implantação de medidas corretivas e preventivas e apontar a eficácia das ações tomadas. O objetivo deste trabalho é discorrer sobre a evolução da qualidade na área da saúde, com ênfase na área laboratorial. Alguns indicadores laboratoriais citados na literatura nas fases pré-analítica, analítica e pós-analítica também são apresentados e discutidos neste artigo. Por fim, destaca-se a experiência brasileira do Programa de Indicadores Laboratoriais desenvolvido pela Sociedade Brasileira de Patologia Clínica/Medicina Laboratorial (SBPC/ML) em parceria com a Control-Lab e o projeto Model of Quality Indicator, em fase de desenvolvimento pela International Federation of Clinical Chemistry and Laboratory Medicine (IFCC).


The use of quality indicators has been appreciated in laboratory management so as to optimize quality and error quantification in several laboratory processes. Furthermore, it assists in the implementation of preventive and corrective measures and it shows their corresponding efficiency. The objective of the present study is to discuss the evolution of quality, mainly in the laboratory area, focusing on the importance of quality indicators in laboratory management. Some pre-analytical, analytical and post-analytical laboratory indicators are also presented and discussed in this work. Finally, we highlight the Brazilian initiative in the Laboratory Indicator Program developed by the Brazilian Society of Clinical Pathology and Laboratory Medicine (SBPC/ML) in partnership with Control-Lab and the Model of Quality Indicator project, which has been developed by the International Federation of Clinical Chemistry and Laboratory Medicine (IFCC).

Objective Analyze the historical data of critical values lists,providing scientific evidence for continuous improvement of critical value systems.Methods Screen out critical value lists data of 2006 from laboratory information system,after pretreatment and transformation of data,calculate the percentage of critical value,and its daily distribution,weekly distribution and department distribution, evaluate the range and turnaround time for critical value.Results The rate of critical value was 1.67%.It was mainly concentrated from 8 to 13 O'clock.Monday and Thursday have more critical value than other days.From the perspective of department,the majority critical value was from hematology department and transplantation department.After the evaluation of distribution diagram of critical value range,the lower critical value limit of blood potassium was adjusted from 3.0 mmol/L to 2.8 mmol/L,the blood platelet and leukocyte counts for parlents with hematology disease were a(Ijusted from 20×109/L,1.5×109/L to 10×1O9/L,1.0×109/L respectively.The laboratory turnaround time for 76.2% critical value was less than 1 hour.Conclusion Review and analyze critical value lists data regularly can improve the work efficiency and quality for the laboratory and clinic department and better meet patients' safety needs.

ABSTRACT

Se estudió la estabilidad de veinticinco analitos conservados durante siete días en dos tipos de tubos primarios: tubos de polimetilmetaacrilato (PMMA) con gránulos y tubos con gel separador, ambos con acelerador de la coagulación. El objetivo fue determinar en qué tubo los analitos tenían mayor estabilidad y como consecuencia, cuánto tiempo podían conservarse las muestras en ambos tubos. Las muestras fueron conservadas en el tubo primario a 4-8 ºC y se analizaron por duplicado diariamente durante siete días. La estabilidad de los analitos se determinó comparando los resultados de los siete días con los valores obtenidos el primer día y los límites de aceptabilidad derivaron del coeficiente de variación (CV) de cada determinación, de la variabilidad biológica (VB) intraindividual y de un análisis multivariado de varianza MANOVA. Los promedios de aldolasa, fósforo, glucosa, lactatodehidrogenasa, potasio y sodio cuando la muestra se conservó con gránulos, tuvieron diferencias estadísticamente significativas al compararlos con los promedios de los mismos analitos conservados en gel; resultados similares se obtuvieron cuando se tomó como límite de aceptabilidad el CV y la VB intraindividual. Cada laboratorio debería establecer la estabilidad de sus analitos según el tubo primario utilizado, con el objetivo de responder a no conformidades que requieran el procesamiento de analitos sobre muestras previamente extraídas en el caso de que éstas se conserven en tubo primario.


The stability of 25 analytes during a seven-day storage in two different kinds of tubes was studied. PMMA (polimetilmetaacrilate) tubes contain a clotting activator (glass particles) and the other tubes contain a gel barrier. The aim was to determine the analytes' stability in both kinds of tubes and, as a consequence, to determine the optimal time for storage of the samples in the primary tube. The specimens were maintained at 4-8 ºC and analyzed in duplicate during seven days. The analytes' stability was determined by comparing the results obtained each day with those obtained on the first day. The significant change limits were based on the determination of the variation coefficient, on the biological variation and on statistically significant changes using a MANOVA test. Mean values of aldolasa, phosphate, glucose, lactic dehydrogenase, potassium and sodium stored in tubes with a clotting activator had statistical differences in comparison with the mean values of the same analytes stored with a gel barrier. Each laboratory should investigate the specific analytes' stability according to the collection device used.

ABSTRACT

Objective To implement a remote monitor system in a core medical laboratory.Methods Firstly,the Hub of StreamLab laboratory automation system was connected to a wireless router to build a wireless network,and several equipments were also integrated into the wireless network using wireless network cards.Secondly,VNC Server was installed into the control PCs of StreamLab LAS,CA7000 Coagulation Analyzer,DPC IMMULITE 2000 SMS and its chemiluminescence photometer,and remote control function of Dimension RxL Max was activated,then some control software such as VNC Viewer,Q/Remote and PC Anywhere were setup in two agency control computers.Finally,PCAnywhere and VNC Viewer were setup in the remote control computers in the Internet and Intranet,respectively.Results A remote control system with three functions was built successfully in a core medical laboratory.Conclusion The remote control system makes the daily operation and remote maintenance of the equipments easy.
