ABSTRACT

ObjectiveTo explore the effect of low frequency or high frequency repetitive transcranial magnetic stimulation (rTMS) on right Broca's homologue in stroke patients with nonfluent aphasia. MethodsFrom January, 2019 to August, 2022, 80 inpatients in Beijing Bo'ai Hospital were randomly divided into control group (n = 20), sham stimulation group (n = 20), low-frequency (1 Hz) rTMS (LF-rTMS) group (n = 20) and high-frequency (10 Hz) rTMS (HF-rTMS) group (n = 20). All the patients received routine language therapy. LF-rTMS group and HF-rTMS group received ten days of rTMS (1 Hz or 10 Hz), and the sham group received ten days of sham rTMS. The Western Aphasia Battery (WAB) was used to evaluate the language function before, after treatment, and two months after treatment. ResultsBefore treatment, there was no significant difference in the scores of WAB among four groups (P > 0.05). All the scores improved in the four groups immediately after treatment and two months after treatment (P < 0.05). Compared with immediately after treatment, all the scores of WAB improved in LF-rTMS group (P < 0.05), and the scores of recall, name and aphasia quotient (AQ) improved in HF-rTMS group (P < 0.05) two months after treatment. Immediately after treatment, the scores of content and fluency, auditory comprehension and AQ were higher in LF-rTMS group than in HF-rTMS group (P < 0.05). Two months after treatment, the scores of content and fluency were higher in LF-rTMS group than in HF-rTMS group (P < 0.05). ConclusionBoth 1 Hz and 10 Hz rTMS could improve the language function of stroke patients with nonfluent aphasia, especially 1 Hz.

ABSTRACT

ObjectiveTo observe the effect of acupuncture plus language function training on the recovery of language function in patients with post-stroke aphasia.MethodNinety patients were divided into an acupuncture-rehabilitation group (intervened by acupuncture plus language function training) of 30 cases, an acupuncture group (intervened by acupuncture alone) of 30 cases, and a control group (intervened by language function training alone) of 30 cases. The Western Aphasia Battery (WAB) was adopted to evaluate the language function, and also taken as criteria for diagnosis and therapeutic efficacy. The aphasia quotient (AQ), and the 4 basic language functions including spontaneous speech, auditory comprehension, repetition and nomination were observed. ResultAfter intervention, AQ, and scores of spontaneous speech, auditory comprehension, repetition and nomination increased significantly in the three groups (P0.05).ConclusionAcupuncture plus language function training can produce a positive effect on patients with post-stroke aphasia, showing therapeutic advantage in improvingspontaneous speech, repetition, and nomination.

ABSTRACT

Objective To revise the standards of the Language Function Evaluation for Hearing-impaired Children and to study the characteristics of language functions in hearing-impaired children. Methods A total of 741 normal hearing children at the age of 3~7 year old from 15 provinces in China were assessed using the Language Function Evaluation for Hearing-impaired Children.A total of 90 hearing-impaired children also completed the e-valuation.The scores of Representation Function,Tool Function,Coordination Function,Expression Function, Entertainment Function and Comprehensive Function of all the children were collected.ResuIts The regression e-quations of the language function were established:Representation Function score=3.34 +0.81 ×children's age, Tool Function score =6.00+0.51×children's age,Coordination Function score =5.72 +0.52 ×children's age,Ex-pression Function score =4.44+0.67×children's age,Entertainment Function score =6.34+0.47×children's age, Comprehensive Function score =4.96+0.62×children's age;Through the regression analysis, the reference stand-ards of the tool for children at the age of 3~6 year old were set down.Five-year-old hearing-impaired children's Tool Function and Expression Function were better than those of three or four-year-old ones(P<0.05).Five-year-old hearing-impaired children's Comprehensive Function were better than those of three-year-old ones(P<0.05).The language function scores had a big gap between the hearing-impaired children and the hearing-nor-mal children at the same ages.ConcIusion The new reference standards of the Language Function Evaluation for Hearing-impaired Children provides the Expectation values and Minimum values of language function from children at the age of 3 -6.They are rehabilitation criterions for hearing-impaired children. The older the hearing-im-paired children are,the higher their scores of language function evaluation. But their scores are much lower than those of the normal hearing children at the same age.

ABSTRACT

Dominant hemisphere for language function is the left hemisphere, however patients experiencing aphasia followed by damage to language areas often shows good recovery in the days to weeks to even years after brain injury. According to the duration from brain injury, recovery period of language function in aphasia can be divided into three overlapping periods (acute, subacute, and chronic phase) with different underlying neural mechanisms. During subacute period of weeks to months following brain injury, reorganization of neural network through brain plasticity occurring in the both hemisphere plays a crucial role in recovery of language function. Recently, the potential use of functional neuroimaging has been raised to explain the underlying neural mechanism for language recovery, however, the brain areas and various factors affecting brain reorganization are still controversial. This article reviews the clinical evidence for recovery of language function through brain plasticity and reorganization and the factors affecting the recovery of language function in aphasic patients following brain injury.
