ABSTRACT
Lichen spinulosus (LS), a disorder of keratinization, is characterized by 1 to 3 mm sized follicular keratotic papules with a central horny spine that are grouped into large patches. Involved lesions were symmetrically distributed over extensor surfaces of the arms and legs, back, chest, face and neck. Although the exact etiology is unknown, several factors such as atopic dermatitis, infections and genetic components have been proposed as playing a role in LS. Additional associations reported in the literature include Crohn's disease, Hodgkin's disease, HIV infection, administration of omeprazole, and seborrheic dermatitis. Herein, we report a case of wide spread lichen spinulosus occurring on a preexisting seborrheic dermatitis lesion in a 50-year-old woman.

Lichen spinulosus is an uncommon idiopathic disorder characterized by scattered 2 cm to 6 cm patches of keratotic follicular papules. Individual papules are 1 to 3 mm in diameter and consist of a hairlike horny spine. Lesions are typically skin colored and asymptomatic or mildly pruritic. It occurs mainly in children and adolescents. Although several factors, such as atopy, infection, drug reaction, id reaction to fungal infection, and heredity, have been proposed as playing a role in lichen spinulosus, most cases are not associated with any systemic abnormalities. Lichen spinulosus is usually a symmetrical lesion that occurs on the trunk or extremities. Herein, we report a case of lichen spinulosus in a 7-year-old child in the submental area, an uncommon site.

Lichen spinulosus is a benign follicular eruption seen primarily in younger persons and is frequently classified a disorder of keratinization. The lesion is characterized by the development of patches of small perifollicular papules that have a central horny spine. We report a case of lichen spinulosus developed in an 8-year-old male patient who had atopic dermatitis.
