ABSTRACT

ABSTRACT Purpose Compare infant suction in babies with and without ankyloglossia using a microprocessor-controlled pressure sensor coupled to a pacifier. Methods Fifty-five infants from 0 to 2 months of age underwent clinical examination for ankyloglossia, after which they were offered a silicone pacifier connected to the pressure acquisition device and suction activity was recorded. Thus, we extracted the frequency of sucks within a burst, the average suck duration, the burst duration, the number of sucks per burst, the maximum amplitude of sucks per burst and the inter-burst interval. Results The key difference in newborns with ankyloglossia in relation to control was that they perform longer bursts of suction activity. Conclusion The longer burst durations are likely a compensatory strategy and may underlie the pain reported by mothers during breastfeeding. We therefore propose a method for objectively quantifying some parameters of infant suction capacity and demonstrate its use in assisting the evaluation of ankyloglossia.


RESUMO Objetivo Comparar a sucção infantil em bebês com e sem anquiloglossia usando um sensor de pressão controlado por microprocessador acoplado a uma chupeta. Método Cinquenta e cinco lactentes de 0 a 2 meses de idade foram submetidos ao exame clínico de anquiloglossia, em seguida foi oferecido uma chupeta de silicone conectada ao dispositivo de aquisição de pressão e a atividade de sucção foi registrada. Assim, obtivemos dados sobre a frequência de sucções dentro de um período de sucções, a duração média da sucção, a duração da rajada, o número de sucções por rajada, a amplitude máxima das sucções por rajada e o intervalo entre rajadas. O teste t não pareado foi utilizado para comparações entre os grupos. Resultados A principal diferença dos recém-nascidos com anquiloglossia em relação aos do grupo controle é que eles realizam rajadas mais longas durante a atividade de sucção. Conclusão A duração mais longa das rajadas é provavelmente uma estratégia compensatória e pode estar por trás da dor relatada pelas mães durante a amamentação. Portanto, propomos um método para quantificar objetivamente alguns parâmetros da sucção infantil e demonstramos seu uso para auxiliar na avaliação da anquiloglossia.

Introdução: A laserterapia de alta potência tem estabelecido proporções satisfatórias na odontologia e se tornando a primeira opção de conduta cirúrgica para procedimentos, trazendo qualidade de vida aos pacientes e praticidade aos Cirurgiões. Objetivo: Relatar caso clínico de frenectomia lingual com uso do laser de diodo de alta potência ressaltando os benefícios da conduta. Relato de caso: Paciente sexo feminino, leucoderma, sem alterações sistêmicas, chega à Clínica Escola de Odontologia da Universidade Federal de Campina Grande, com queixa principal de alterações na sua fonologia. No exame intraoral observou-se dificuldade de movimentação lingual e em formato de coração devido a inserção fibrosa do freio, sendo observado a Anquiloglossia. Após explicações da alteração, do tratamento, e assinatura do Termo de Consentimento, foi iniciado a cirurgia: antissepsia, anestesia com vasoconstrictor, e em modo contínuo utilizou-se o laser de diodo de alta potência (TW Surgical- MMO®) configurado em 2W de potência, fibra 600µm, no comprimento de onda infravermelho, de modo que as fibras fossem separadas uniformemente e promovesse mobilidade imediata da língua, por fim realizou-se uma sutura em ponto simples com fio reabsorvível. Para o pós-operatório foi prescrito Dipirona em caso de dor, e estabelecido o acompanhamento. No retorno de 7 dias observou-se cicatrização normal e não ingestão do medicamento, em 30 dias foi visto a completa cicatrização e mobilidade sem desconforto, além de relato da paciente de melhoria na condição de relacionamentos sociais, autoestima e deglutição. Conclusão: O resultado apresentado neste artigo ratifica a benefício da escolha do laser em comparação ao convencional, demonstrando que não há sangramento no trans e pós cirúrgico, além de favorecer o processo de reparação tecidual, da redução do tempo cirúrgico, aumento do conforto, e diminuição de uso de medicamento, desta forma associando o laser cirúrgico a melhoria da qualidade de vida.


Introduction: High-power laser therapy has established satisfactory proportions in dentistry and has become the first choice for surgical procedures, bringing quality of life to patients and convenience to surgeons. Objective: To report a clinical case of lingual frenectomy using high-power diode laser, highlighting the benefits of this approach. Case report: A female patient, leucoderma, without systemic alterations, arrived at the Dental School Clinic of the Federal University of Campina Grande with the main complaint of alterations in her phonology. In the intraoral examination, difficulty in lingual movement and a heart-shaped tongue due to fibrous insertion of the frenum were observed, indicating Ankyloglossia. After explanations of the condition, treatment, and signing of the Informed Consent Form, the surgery was initiated: antisepsis, anesthesia with vasoconstrictor, and continuous mode using a high-power diode laser configured at 2W of power, 600µm fiber, at an infrared wavelength, with the aim of uniformly separating the fibers and promoting immediate tongue mobility. Finally, a simple suture was performed using absorbable thread. For the postoperative period, Dipyrone was prescribed for pain relief, and follow-up was established. At the 7-day follow-up, normal healing was observed, and the patient did not require the medication. At 30 days, complete healing and discomfort-free mobility were observed, along with the patient's testimony of improvement in social relationships, self-esteem, and swallowing. Conclusion: The results presented in this article confirm the benefits of choosing laser therapy over conventional methods, demonstrating the absence of bleeding during and after surgery, promoting tissue repair, reducing surgical time, increasing comfort, and decreasing the use of medication. Therefore, the use of surgical laser is associated with an improvement in quality of life.


Introducción: La terapia láser de alta potencia se ha establecido en proporciones satisfactorias en odontología y se ha convertido en la primera elección para procedimientos quirúrgicos, aportando calidad de vida a los pacientes y practicidad a los cirujanos. Objetivo: Relatar un caso clínico de frenectomía lingual con láser de diodo de alta potencia, destacando los beneficios de este procedimiento. Relato del caso: Paciente del sexo femenino, leucoderma, sin alteraciones sistémicas, llegó a la Clínica de la Facultad de Odontología de la Universidad Federal de Campina Grande con la queja principal de alteraciones en su fonología. El examen intraoral reveló dificultad en el movimiento lingual y lengua en forma de corazón debido a la inserción fibrosa del freno, observándose anquiloglosia. Tras explicar la alteración, el tratamiento y firmar el consentimiento, se inició la cirugía: antisepsia, anestesia con vasoconstrictor y uso continuado de un láser de diodo de alta potencia (TW Surgical- MMO®) configurado a 2W de potencia, fibra de 600µm, a longitud de onda infrarroja, para que las fibras se separaran uniformemente y favorecieran la movilidad inmediata de la lengua, y finalmente una sutura de punto único con hilo reabsorbible. En el postoperatorio, se prescribió dipirona en caso de dolor y se estableció un seguimiento. En el seguimiento a los 7 días, se observó una cicatrización normal y no se tomó medicación. A los 30 días, se observó una cicatrización completa y movilidad sin molestias, así como el informe del paciente de una mejora de las relaciones sociales, la autoestima y la deglución. Conclusión: Los resultados presentados en este artículo confirman el beneficio de la elección del láser frente a la cirugía convencional, demostrando que no hay sangrado durante ni después de la cirugía, además de favorecer el proceso de reparación tisular, reducir el tiempo quirúrgico, aumentar el confort y reducir el uso de medicación, asociando así el láser quirúrgico a una mejor calidad de vida.

ABSTRACT

O resíduo de fecularia é um subproduto da agroindústria utilizado com frequência na alimentação de bovinos leiteiros pelo alto teor energético e baixo custo. No entanto, possui características propícias ao desenvolvimento de microrganismos patogênicos como o Clostridium botulinum, causador do botulismo. Este relato é o primeiro na literatura consultada a descrever um surto de botulismo em uma propriedade de bovinos leiteiros devido ao consumo de massa de mandioca, alimento ofertado em muitas propriedades do Oeste do Paraná. 16 animais, de um total de 24 morreram em um período de 14 dias, com sinais clínicos de respiração abdominal, sialorreia, paresia de membros pélvicos, decúbito esternal seguido de lateral permanente, diminuição dos tônus mandibular e lingual e morte em até 48 horas. Os animais não eram vacinados para clostridioses e recebiam o resíduo em cocho coletivo. O diagnóstico foi baseado no histórico, na avaliação das condições de armazenamento do alimento na propriedade, no exame físico e na ausência de lesões macroscópicas e histológicas. Conclui-se que o resíduo de fecularia deteriorado é um substrato propício para desenvolvimento do C. Botulinum sendo um alimento passível de provocar surtos de botulismo em bovinos leiteiros com alta morbidade e letalidade.


Cassava is an industry by-product waste used to feeding cattle due to its low cost and high energy. However, this food presents characteristics to development of pathogenic microorganisms such as Clostridium botulinum causer of botulism. This disease results from the ingestion of preformed neurotoxins that act at the neuromuscular junctions causing flaccid paralysis of striated muscles. This is the first report to describe botulism due the intake of cassava by-product (tapioca waste), a common food offered in the west of Parana State. 16 dairy cattle died from a total of 24 animals in 14 days. The clinical signs were dyspnea with abdominal pattern, sialorrhea, pelvic limb paresis, decreased mandibular and tongue tonus and permanent sternal progressing to lateral decubitus and death until 48 hours after the onset of signs. The diagnosis was based on anamnesis, evaluation of food storage conditions, clinical examination, and the absence of macroscopic and histology lesions. It was concluded that cassava by-product has conditions to the development of Clostridium botulinum and can cause botulism outbreak with high morbidity and lethality.


El residuo de almidón es un subproducto de la agroindustria que suele utilizarse para alimentar al ganado lechero por su alto contenido energético y su bajo coste. Sin embargo, tiene características favorables para el desarrollo de microorganismos patógenos como Clostridium botulinum, causante del botulismo. Este informe es el primero en la literatura que describe un brote de botulismo en una hacienda de ganado lechero debido al consumo de pasta de mandioca, alimento ofrecido en muchas propiedades del oeste de Paraná. 16 animales, de un total de 24, murieron en un período de 14 días, con signos clínicos de respiración abdominal, sialorrea, paresia de los miembros pélvicos, decúbito esternal seguido de decúbito lateral permanente, disminución del tono mandibular y lingual y muerte en 48 horas. Los animales no estaban vacunados contra la clostridioses y recibían sus desechos en un comedero comunitario. El diagnóstico se basó en la anamnesis, la evaluación de las condiciones de almacenamiento de los alimentos en la propiedad, el examen físico y la ausencia de lesiones macroscópicas e histológicas. Se concluye que el residuo de almidón descompuesto es un sustrato favorable para el desarrollo de C. botulinum y es un alimento que puede causar brotes de botulismo en ganado lechero con alta morbilidad y letalidad.

ABSTRACT

ABSTRACT Objective: To assess global trends in the publication of studies investigating the association between ankyloglossia and breastfeeding. Material and Methods: An electronic search was performed in the Scopus database without restrictions. Observational studies and clinical trials were included. Bibliometric indices such as publication year, authors, co-authors, journals, field of knowledge, countries, and the most cited keywords were analyzed using the VOSviewer program. Results: The search retrieved 350 studies, and 68 were selected. The first article was published in 2000 in the United States. The United States presented the highest number of publications (n=21), followed by Brazil (n=9) and the United Kingdom (n=9). An increase in publications on this theme was observed in 2013; 2021 was the year with the highest number of publications (n=14). The most common word was "frenulum". The authors with the highest number of publications were Botze and Dollbert from Israel (n=3), Ghaheri, and Mace from the United States (n=3). Among the journals, "Breastfeeding Medicine" presented the highest number of publications (n=7), followed by the "International Journal of Pediatric Otorhinolaryngology" (n=6), "CODAS" (n=5), "Journal of Human Lactation" (n=4) and "Pediatrics" (n=3); the latter published the top-cited studies, with 412 citations. Conclusion: There has been an increase in recent articles evaluating the correlation between ankyloglossia and breastfeeding, indicating the growing interest of researchers in this field.

ABSTRACT

La hemorragia producida por lesión de la arteria lingual en la base de la lengua por cirugías o por tumores es infrecuente. La mayor frecuencia en la indicación de abordajes transorales para tratar diferentes patologías que afectan la orofaringe requiere que el equipo quirúrgico tenga experiencia en el manejo de esta complicación. La ligadura de la arteria lingual en el cuello es una técnica quirúrgica muy eficaz para solucionar la hemorragia, pero es importante conocer las posibles variantes anatómicas que puede tener la arteria en su trayecto cervical. Debido a su baja incidencia se propone como objetivo describir dos casos clínicos de pacientes que tuvieron hemorragias graves por lesión de la arteria lingual en la base de la lengua, producidas por daño quirúrgico y por erosión por tumor. The bleeding caused by injury to the lingual artery at the base of the tongue due to surgery or tumors is infrequent. The increased frequency in the indication of transoral approaches to treat different pathologies affecting the oropharynx requires the surgical team to have experience in managing this complication. Ligation of the lingual artery in the neck is a very effective surgical technique to solve the bleeding; however, it is essential to be aware of the possible anatomical variants the artery may have in its cervical trajectory. Due to its low incidence, we propose to describe two clinical cases of patients who had severe bleeding due to a lesion of the lingual artery at the base of the tongue, produced by surgical damage and erosion due to a tumor. ABSTRACT Purpose To analyze the effects of lingual frenotomy on the breastfeeding of infants, based on the electrical activity of the masseter and suprahyoid muscles and assessment of the breastfeeding. Methods Observational study developed between October 2017 and June 2018 with a sample of 20 newborns and infants who attended a dental clinic and were diagnosed with ankyloglossia. Another 20 were excluded for meeting some of the following exclusion criteria: babies more than 6 months old, who were not on exclusive or mixed breastfeeding, who had other clinical impairments that interfered with breastfeeding, who had other foods introduced into their diet, who had neurological changes and/or craniofacial deformities, and/or who did not finish all the stages of the study. Breastfeeding was assessed with the UNICEF Breastfeeding Assessment and Observation Protocol, while the muscle electrical activity was assessed with the Electrical Activity Assessment Protocol for the Masseter and Suprahyoid Muscles in Newborns During Breastfeeding. The same speech-language-hearing therapist conducted the two assessments both before the conventional frenotomy and 7 days after it. Results The signs suggestive of breastfeeding difficulties changed 7 days after the surgery, with a p-value ≤ 0.002 for general observation of the mother, position of the infant, latch, and sucking. The maximum voluntary contraction of the masseter was the only integral parameter with a difference, as the electrical activity had decreased. Conclusion Behaviors favorable to breastfeeding increased 7 days after the frenotomy in all the breastfeeding assessment categories, whereas the electrical activity of the masseter decreased.


RESUMO Objetivo Analisar os efeitos da frenotomia lingual na amamentação de recém-nascidos, com base na atividade elétrica dos músculos masseter e supra-hióideos e avaliação da mamada. Método Estudo observacional, desenvolvido entre outubro de 2017 e junho de 2018. Amostra de 20 recém-nascidos e lactentes de uma clínica odontológica, diagnosticados com anquiloglossia; 20 foram excluídos por se encaixarem em algum dos critérios de exclusão: ter mais de seis meses de vida, não fazer aleitamento materno exclusivo ou misto, apresentar outro comprometimento clínico que interfira na amamentação, ter iniciado a ingestão de outros alimentos, alterações neurológicas e/ou deformidades craniofaciais e não ter finalizado todas as etapas do estudo. A avaliação da amamentação seguiu o Protocolo de Observação e Avaliação da Mamada da UNICEF, e a avaliação da atividade elétrica muscular o Protocolo de Avaliação da Atividade Elétrica dos Músculos Masseter e Supra-hióideos em Recém-Nascidos Durante a Alimentação, ambas realizadas pela mesma fonoaudióloga antes da frenotomia convencional e sete dias após. Resultados Os sinais sugestivos de dificuldade na amamentação mudaram sete dias após a cirurgia, com valor de p≤0,002 para a observação geral da mãe, posição do bebê, pega e sucção. Houve diferença apenas no parâmetro integral da Contração Voluntária Máxima do masseter, com diminuição da atividade elétrica. Conclusão Sete dias após a frenotomia observou-se aumento de comportamentos favoráveis ​​à amamentação em todas as categorias de avaliação da mamada e diminuição da atividade elétrica do masseter.

ABSTRACT

【Objective】 To investigate the common etiology, characteristics and treatment of iatrogenic ureteral stricture. 【Methods】 The clinical data of 226 patients with ureteral stricture repaired during May 2019 and Mar. 2022 were retrospectively analyzed, including 68 cases of iatrogenic ureteral stricture. According to the etiology, the patients were divided into urinary group and non-urinary group. 【Results】 There were 42 females and 26 males, aged 25 to 67 (average 49.0±10.4) years. Upper ureteral stricture was detected in 24 (35.3%) cases, who received oral mucosal repair of the ureter. Middle ureteral stricture was detected in 12 (17.6%) cases, who underwent ileal ureterography. Lower ureteral stricture was observed in 24 (35.3%) cases, who were treated with vesical wall flap ureteroplasty. Full-length stricture was observed in 8 (11.8%) cases,who were treated with ileal ureterography. There were significant differences in age, gender, stenosis side, stenosis location and length, surgical methods and types between patients in the urinary group and non-urinary group (P<0.05). During the follow-up of 8 to 20 (average 12.3±5.6) months, the symptoms and renal function of all patients improved, and no recurrence occurred. 【Conclusion】 Invasive endourological surgery is the most common cause of iatrogenic ureteral stenosis. Different treatment strategies should be adopted according to patients’ condition, time of diagnosis and location and length of ureteral injury.

ABSTRACT

For a long time, urological surgeons have been troubled by the treatment of long proximal and mid ureteral stricture, and the commonly used ileal ureteral substitution and autotransplantation have certain shortcomings. In recent years, the development of autologous graft technique has brought a new hope for these patients. The commonly used autologous grafts or flaps include oral mucosa, intestinal tissue, urogenital tissue, etc. The feasibility and safety of autologous graft technique have been preliminarily verified, but each kind of graft or flap has different advantages and disadvantages. The research progress of autologous grafts or flaps ureteroplasty in this article was reviewed and the related problems of this technique were discussed.

ABSTRACT

Ureteral calculi after lingual mucosal ureteral reconstruction are rare. In this paper, we reported a case of a male patient who had undergone robotic-assisted laparoscopic lingual mucosal right ureteroplasty. Calculi were found in the right reconstructed ureteral segment 4 months after surgery. Then the patient underwent transurethral ureteroscopic holmium laser lithotripsy combined with a stone retrieval basket, and postoperative urological CT showed no residual calculi in the right ureter. No recurrence of right ureteral calculi or complications were observed during 20 months of follow-up.

@#Lack of alveolar bone height is a major challenge for dental implants. In recent years, the use of "sandwich" osteotomy to increase alveolar bone height has become a topic of discussion within the research community. In theory, "sandwich" osteotomy is a U-shaped osteotomy in the bone defect area, to preserve the blood supply of the mucoperiosteal on the lingual side and to create an artificial "four-wall bone bag" to build a favorable space for osteogenesis and to increase the height of the alveolar bone. Histological studies have shown that the osteogenesis speed of "sandwich" osteotomy is fast, and the bone is good. Sandwich osteotomy is suitable for buccal-lingual alveolar bone height defects less than 50% of the implant length or for unilateral defects more than 50% of the implant length. In the operation of "sandwich" osteotomy, the horizonal incision should be 10-12 mm below the crest of the buccal alveolar ridge. The design of the osteotomy line should ensure the height of the osteotomy block and that the mandibular canal does not sustain damage and that it fits the shape of the bone defect. There was no significant difference in the osteogenic effect of different types of bone graft materials used for "sandwich" osteotomy. The osteotomy block was rigorously fixed by a titanium plate, titanium nail, implant and other materials, and finally, the intraoperative area was tensioned and sutured. The effect of bone augmentation was evaluated and compared with other bone augmentation techniques; the evaluation showed that sandwich osteotomy was better for moderate vertical bone defects. This technique is highly sensitive and postoperative transient sensory loss is common. With advances in technology, the application of digital technology and ultrasonic bone knives, the risk of complications is greatly reduced and advances in digital osteotomy will promote apply of "sandwich" osteotomy, which will become a popularized technique for clinical alveolar bone augmentation.

To evaluate breastfeeding in babies up to six months of age before and after frenotomy surgery in a reference hospital in a city from Santa Catarina state. Methods: An observational quality improvement study, carried out with babies up to six months of age undergoing frenotomy and their mothers in a reference hospital in southern Santa Catarina state. A questionnaire was applied to the mothers in two moments (before and after the surgery), with information regarding breastfeeding, sociodemographic, anthropometric and behavioral characteristics of them and of the babies. Descriptive analyzes were performed and the association between breastfeeding and the independent variables was assessed through Pearson's chi-square and Fisher's exact tests, using a 5% significance level. Results: A total of 74 children were studied, with 48 of them returning after surgery. 83.8% were breastfed before surgery and 64.9% after surgery. Before surgery, 58.1% of children effectively took the breast at once. After surgery, this prevalence was 83.3% (p=0.015). Before surgery, 75.9% of the mothers reported not feeling pain, while, after surgery, almost all of them (95.8%) reported this (p=0.004). Most mothers reported improvement in grip (83.3%), increase in the duration of breastfeeding (69.0%), improvement of baby's breathing (75.0%), and an increase in the frequency of breastfeeding (51.7%). Conclusions: There was a decrease in the prevalence of breastfeeding after frenotomy. However, there was an improvement in the babies 'grip and breathing and a reduction in the mothers' pain when breastfeeding. It is emphasized the need to implement multidisciplinary actions in both primary and hospital care to assist mothers in order to prolong the duration of exclusive breastfeeding

RESUMO Buscou-se investigar e conhecer a frequência e a distribuição de frenotomias e fenectomias realizadas no Sistema Único de Saúde (SUS) após os dispositivos legais que instituíram o teste da linguinha, bem como entender quais profissionais estão realizando o procedimento cirúrgico. Estudo descritivo analítico, retrospectivo, com dados secundários, disponíveis no Sistema de Informações Ambulatoriais do DataSUS, extraídos por meio do TabWin. Observou-se um aumento considerável no número de procedimentos, porém, com flutuações nos anos seguintes. Das frenectomias registradas no SUS, no período, 33% foram realizadas por dentistas na atenção primária. Os resultados apontaram que houve um aumento substancial no número de procedimentos cirúrgicos em dois momentos, em 2014 e em 2017, possivelmente em decorrência da publicação da Lei nº 13.002/2014 e da Nota Técnica (NT) nº 09/2016. A NT nº 35/2018 deslocou o diagnóstico e o tratamento para a média complexidade, possivelmente resultando em filas, principalmente em se tratando de cirurgias que poderiam ser realizadas na atenção primária. Apesar de não haver unanimidade quanto à correlação anquiloglossia e desmame, não parece haver um monitoramento para evitar cirurgias desnecessárias, uma vez que a ordenha pode não ser afetada e o acompanhamento ser realizado para confirmar ou não essa necessidade.


ABSTRACT This study sought to investigate and to know the frequency and the distribution of frenotomies and frenectomies performed in the Unified Health System (SUS) after the legal provisions that instituted the tong test, and to understand which professionals are performing the surgical procedure. This is an analytical, descriptive, retrospective study, with secondary data, available in the DATASUS Information System. It was observed that a considerable increase in the number of procedures, but with fluctuations in the following years. Out of the frenectomies registered in the SUS in the period, 33% were performed by dentists in primary care. The results showed that there was a substantial increase in the number of surgical procedures in two moments, in 2014 and in 2017, possibly as a result of the publication of Ordinance nº 13.002/2014 and Technical Note (NT) No. 09/2016. NT No. 35/2018 shifted diagnosis and treatment to medium complexity, possibly resulting in queues, especially in the case of surgeries that could be performed in primary care. Although there is no unanimity regarding the correlation between ankyloglossia and weaning, there seems to be no monitoring to avoid unnecessary surgeries, since milking may not be affected and follow-up is carried out to confirm or not this need.

Objetivo: Caracterizar as alterações na fala decorrentes da anquiloglossia, por meio de revisão integrativa da literatura. Métodos e Procedimentos: Levantamento bibliográfico realizado em fevereiro de 2020, delimitado segundo os idiomas inglês, português, espanhol e idade a partir de 6 anos. Foram selecionados artigos disponíveis em quatro bases eletrônicas: PubMed, SciELO, Scopus, Web Of Science. Palavras-chave utilizadas: freio lingual; distúrbios na fala; anquiloglossia. Foram consideradas publicações de 2010 a 2020 mediante análise de metadados, a partir do título e resumo, para identificar pertinência à pesquisa. Foram excluídos estudos publicados há mais de dez anos, que não permitiram acesso ao texto integral, repetidos por sobreposição dos descritores, discrepantes do tema. Resultados e Discussão: Foram localizados 276 artigos, que após aplicados os critérios de inclusão e exclusão resultaram em 27. Os resultados encontrados indicam que sujeitos com alterações no frênulo lingual, principalmente na anquiloglossia, utilizam estratégias compensatórias variadas de lábios, língua e mandíbula para a produção dos fonemas 't', 'd', 'l', 'n', 's', 'z', 'r' e de grupos consonantais, que poderão apresentar distorção, substituição e/ou omissão, por serem de difícil produção com frênulo curto. Aos profissionais otorrinolaringologistas, ortodontistas e fonoaudiólogos é recomendada realização de exame clínico cuidadoso, que possibilite diagnóstico com objetivo de obter resultados satisfatórios em menor tempo e indicação de intervenções cirúrgicas, quando necessárias. Conclusão: A revisão integrativa da literatura aponta para a relação entre anquiloglossia e alterações na fala.


Objective: To characterize the speech disorders resulting from ankyloglossia, through an integrative literature review. Methods and Procedures: Bibliographic survey carried out in February 2020, delimited language (English, Portuguese, Spanish) and age (from 6 years old). Articles available in four electronic databases were selected: PubMed, SciELO, Scopus, Web of Science. Keywords used: lingual frenum; speech disorders; ankyloglossia. Manuscripts published from 2010 to 2020 were included based on metadata analysis that considered title and abstract to identify the relevance for this research. Studies published over ten years ago, those that did not allow access to the full text, manuscripts repeated due to overlapping of descriptors or diverging from the topic were excluded. Results and Discussion: 276 articles were initially identified; after the inclusion and exclusion criteria were applied, 27 studies were considered. The results showed that subjects with lingual frenulum alterations, mainly ankyloglossia, use varied compensatory strategies of lips, tongue and mandible for the production of the phonemes 't', 'd', 'l', 'n', 's', 'z', 'r' and consonant clusters, which may present distortion, substitution and/or omission, as their production is hindered by the short frenulum. Otorhinolaryngologists, orthodontists and speech therapists are recommended to conduct careful clinical examination, allowing for diagnosis, in order to obtain satisfactory results in less time and indication of surgical interventions, when necessary. Conclusion: The integrative literature review shows the relationship between ankyloglossia and speech disorders

Resultados y Discusión: Se localizaron 276 artículos, que luego de aplicar los criterios de inclusión y exclusión resultaron 27. Los resultados encontrados indican que los sujetos con alteraciones en el frenillo lingual, principalmente en anquiloglosia, utilizan variadas estrategias compensatorias de labios, lengua y mandíbula para la producción de los fonemas 't', 'd', 'l', 'n', 's', 'z', 'r' y grupos consonánticos, que pueden presentar distorsión, sustitución y/u omisión, porque son difíciles de producir con un frenillo corto. Se recomienda a los otorrinolaringólogos, ortodoncistas, logopedas profesionales que realicen examen clínico cuidadoso, que permita un diagnóstico con el objetivo de obtener resultados satisfactorios en menos tiempo e indicación de intervenciones quirúrgicas, cuando sea necesario. Conclusión: La revisión integrativa de la literatura apunta a la relación entre la anquiloglosia y los trastornos del habla.

Introdução: A remoção cirúrgica dos terceiros molares inferiores apresenta o risco potencial de lesões a estruturas nobres como o nervo lingual devido sua localização anatômica. O grau de comprometimento do nervo indicará se a lesão é transitória ou permanente. A injúria nervosa pode motivar o paciente a acionar judicialmente o cirurgião-dentista, e sendo evidenciada a culpa profissional, caberá a reparação pecuniária dos danos causados ao paciente, sejam de natureza material ou moral. Objetivo: Analisar nove decisões judiciais de 1ª e 2ª instância, disponíveis no site do Tribunal de Justiça de São Paulo (TJSP), motivadas pela lesão do nervo lingual durante a exodontia de terceiro molar inferior. Metodologia: Após a leitura e análise foram extraídas informações como: perfil das partes, tempo de duração dos processos, procedência ou não da ação, valor da indenização, responsabilidade profissional e presença de TCLE. Resultados: Na primeira instância, seis dos nove casos foram julgados improcedentes; em segunda instância, quatro destas sentenças foram reformadas para procedência. No total, após decisão de segunda instância, houve procedência da ação em sete dos nove casos. Nas ações procedentes, as indenizações para dano moral variaram de R$ 5 mil a R$ 50 mil, sendo a imperícia e negligência as modalidades de culpa mais atribuídas aos profissionais. Conclusão: Sabendo-se que a lesão do nervo lingual durante a extração de terceiro molar inferior, mesmo previsível, nem sempre é evitável, verifica-se a importância do TCLE (ou documento similar) na prática clínica como instrumento de defesa jurídica do profissional, diante da alegação de falha de informação. Este documento foi mencionado em apenas três dos nove casos processuais, sendo em que em dois destes não houve constatação de falha técnica ou de informação (ação improcedente nas duas instâncias)


Introduction: Surgical removal of lower third molars presents the potential risk of injury to noble structures such as the lingual nerve due to its anatomical location. The level of nerve involvement will indicate whether the injury is transient or permanent. Nervous injury can motivate the patient to sue the dentist, and if professional guilt is evidenced, it will be up to pecuniary compensation for the damage caused to the patient, whether of a material or moral nature. Objective: To analyze nine court decisions of 1st and 2nd instances, available on the Court of Justice of State of São Paulo (Brazil) website, motivated by lingual nerve injury during lower third molar extraction. Methodology: After reading and analysis, information was extracted such as: profile of the litigating parties, duration of the processes, judge decision for the action, indemnity value, professional responsibility and presence of TCLE. Results: In the first instance, six of the nine cases were dismissed, the sentence was reformed in four cases, confirming the merits of the action in seven of the nine cases. In the cases of valid actions, compensation for moral damages ranged from R$ 5 thousand to R$ 50 thousand, with malpractice and negligence being the types of fault most attributed to professionals. Conclusion: Knowing that lingual nerve injury during mandibular third molar extraction, even if predictable, is not always avoidable, the importance of the informed consent (or similar document) in clinical practice as an instrument for the professional's legal defense is mandatory, to avoid the allegation of miss information. This document was mentioned in only three of the nine cases, and in two of these there was no finding of technical or information failure (unfounded action in both instances)

RESUMEN Introducción: La lengua vellosa negra es un trastorno benigno adquirido, de presentación inusual en pediatría. Consiste en una glositis benigna caracterizada por una hipertrofia de las papilas filiformes linguales, con aumento de su queratinización, lo cual condiciona que la superficie adquiera un aspecto velloso con un color que varía de pardo negruzco a negro, debido a la presencia de microorganismos productores de sustancias cromógenas. Objetivo: Describir las principales características de esta entidad y revisar los conceptos más actuales. Presentación del caso: Lactante de 10 meses que es llevado a consulta al notar la madre que el niño presentaba una lengua de color negro, con antecedentes de haber sido tratado con cefalexina debido a un impétigo contagioso 10 días antes. El examen físico mostró un aumento de volumen de las papilas filiformes linguales, posiblemente vinculada al uso del antibiótico, asociada a una defectuosa limpieza de la lengua. En el estudio microbiológico se aisló Cándida albicans, que se consideró como hallazgo concomitante. El tratamiento consistió en medidas de limpieza de la lengua con buena respuesta. Conclusiones: La lengua vellosa negra es una enfermedad autolimitada rara en lactantes, de diagnóstico clínico y buen pronóstico. El tratamiento debe ir encaminado al empleo de medidas higiénicas, como terapéutica de primera línea. El tratamiento antifúngico por lo general es innecesario. Hasta donde conocemos, este es el primer caso en Cuba, documentado en la literatura médica.


ABSTRACT Introduction: Black hairy tongue is an acquired benign disorder, of unusual presentation in pediatrics. It consists of a benign glossitis characterized by a hypertrophy of the lingual filiform papillae, with increased keratinization, which conditions that the surface acquires a hairy appearance with a color that varies from blackish brown to black, due to the presence of microorganisms that produce chromogenic substances. Objective: Describe the main characteristics of this entity and review the most current concepts. Case Presentation: A 10-month-old infant who is taken to the consultation when the mother notices that the child had a black tongue, with a history of having been treated with cephalexin due to a contagious impetigo 10 days before. Physical examination showed an increase in volume of the lingual filiform papillae, possibly linked to the use of the antibiotic, associated with a defective cleaning of the tongue. In the microbiological study, Candida albicans was isolated, which was considered as a concomitant finding. The treatment consisted of tongue cleaning measures with good response. Conclusions: Black hairy tongue is a rare self-limiting disease in infants, with a clinical diagnosis and a good prognosis. Treatment should be aimed at the use of hygienic measures, as first-line therapy. Antifungal treatment is usually unnecessary. As far as we know, this is the first case in Cuba, documented in the medical literature.

Abstract Introduction Cochlear implantation is an effective treatment method for severe to profound hearing loss. Many factors that may influence cochlear implantation success have been explained in previous studies. Apart from those, minor differences in size of normal cochlear nerves may affect postoperative performance. Objective To investigate whether the minor differences in cochlear nerve size in normal cochlear nerves affect postoperative cochlear implant performance. Methods 30 pediatric prelingually deaf patients who were treated with cochlear implantation were included in this study. From the reconstructed parasagittal magnetic resonance images, the diameter and cross-sectional area of the cochlear nerve on the ipsilateral and contralateral side were measured. Auditory evaluations were performed 1, 3, 6 and 12 months following the first fitting. All the analysis was performed by using EARS®, evaluation of auditory responses to speech tool. Correlation between cochlear nerve diameter, cross-sectional area and postoperative auditory perception was analyzed to determine whether variation in cochlear nerve size contributes to postoperative auditory performance. Results The mean diameter of the cochlear nerve on the ipsilateral side was 718.4 μm (504.5 − 904.3 μm) and mean cross sectional area was 0.015 cm2 (0.012 − 0.018 cm2). On the contralateral side the mean cochlear nerve diameter was 714.4 μm (502.6 − 951.4 μm) and mean cross sectional area was 0.014 cm2 (0.011 − 0.019 cm2). The correlation between the diameter and cross-sectional area of the ipsilateral and contralateral cochlear nerve revealed no significance. Mean score at first month monosyllable-trochee-polysyllable test, MTP1, was 0.17 (0.08 − 0.33), at 6th month with 6 words test, 6th month MTP6 was 0.72 (0.39 − 1.0), at 6th month with 12 words, 6th month MTP 12 was 0.46 (0.17 − 0.75) and at 12th month with 12 words, 12th month MTP12 was 0.73 (0.25 − 1.0). There was no correlation between the monosyllable-trochee-polysyllable test, values at any time with the diameter of the ipsilateral cochlear nerve. However, the first month MTP, 6th month MTP6 and 12th month MTP12 correlated with the cross-sectional area of the ipsilateral cochlear nerve. Conclusion Measuring the cross sectional area of the normal- appearing cochlear nerve may give important prognostic knowledge on cochlear implant outcomes. In patients with a larger cross sectional area the auditory performance was better and faster. Although normal appearing, slight differences on cross sectional area of the cochlear nerve may affect performance. Measuring the size of the cochlear nerve on parasagittal magnetic resonance images may provide beneficial information on the postoperative rehabilitation process.


Resumo Introdução O implante coclear é um método de tratamento eficaz para a perda auditiva grave a profunda. Muitos fatores que podem influenciar o sucesso do implante coclear foram explicados em estudos anteriores. Além desses fatores, pequenas diferenças no tamanho dos nervos cocleares normais podem afetar o desempenho pós-operatório. Objetivo Investigar se pequenas diferenças no tamanho dos nervos cocleares normais afetam o desempenho pós-operatório do implante coclear. Método Foram incluídos neste estudo 30 pacientes pediátricos surdos pré-linguais, tratados com implante coclear. A partir de imagens de ressonância magnética parassagitais reconstruídas, foram medidos o diâmetro e a área de seção transversal do nervo coclear no lado ipsilateral e contralateral. As avaliações auditivas foram feitas 1, 3, 6 e 12 meses após a primeira adaptação. Todas as análises foram feitas com a ferramenta EARS® (do inglês evaluation of auditory responses to speech). A correlação entre o diâmetro do nervo coclear, a área transversal e a percepção auditiva pós-operatória foi analisada para determinar se a variação no tamanho do nervo coclear contribui para o desempenho auditivo pós-operatório. Resultados O diâmetro médio do nervo coclear no lado ipsilateral foi de 718,4 μm (504,5 a 904,3 μm) e a área da seção transversal média foi de 0,015 cm2 (0,012-0,018 cm2). No lado contralateral, o diâmetro médio do nervo coclear foi de 714,4 μm (502,6 a 951,4 μm) e a área da seção transversal média foi de 0,014 cm2 (0,011 a 0,019 cm2). A correlação entre o diâmetro e a área transversal do nervo coclear ipsilateral e contralateral não revelou qualquer significância. O escore médio do teste monosyllable-trochee-polysyllable no primeiro mês, denominado MTP1, foi de 0,17 (0,08-0,33), no sexto mês com teste de 6 palavras, MTP6, foi de 0,72 (0,39-1,0), no sexto mês com 12 palavras, MTP12, foi de 0,46 (0,17-0,75) e no 12° mês com 12 palavras, MTP12, foi de 0,73 (0,25-1,0). Não houve correlação entre os valores do teste monosyllable-trochee-polysyllable em qualquer momento com o diâmetro do nervo coclear ipsilateral. Entretanto, o teste monosyllable-trochee-polysyllable do primeiro mês, do 6° mês, e o do 12° mês correlacionaram-se com a área transversal do nervo coclear ipsilateral. Conclusão Medir a área da secção transversal do nervo coclear de aparência normal pode fornecer conhecimento prognóstico importante sobre os resultados do implante coclear. Em pacientes com área da secção transversal maior, o desempenho auditivo foi melhor e mais rápido. Embora o nervo coclear pareça normal, pequenas diferenças na área da secção transversal do nervo coclear podem afetar o desempenho, de forma que a medida do tamanho do nervo coclear nas imagens de ressonância magnética na projeção parassagital pode fornecer informações benéficas sobre o processo de reabilitação pós-operatória.

SUMMARY: The aim of this study was to determine the morphologic characteristics of the lingual foramen and lateral lingual foramen using cone-beam CT in elderly Korean. Cone-beam CT images were obtained from 80 Korean older than 50 years (mean age, 65.2 years). The prevalence of the lingual and lateral lingual foramina at the lingual aspect of the mandible was determined. The diameter and height to the upper margin of the foramina from the mandibular inferior margin, and the bone height to the alveolar crest from the mandibular inferior margin were measured. In addition, the location of the lateral lingual foramen, the direction of its canal, and the presence of communication with the mandibular canal were evaluated. All of elderly Korean possessed at least one lingual foramen, with two or three foramina occurring in 77.5 % of Korean. A lateral lingual foramen was observed in 91.3 % of Korean, with the prevalence being highest at the second premolar in dentulous cases (21.6 %; 33/153). The very high frequencies of these foramina were attributable to high frequencies of relatively small-diameter inferior lingual foramen and lateral lingual foramen in the incisor region. The prevalence of a large-diameter (≥1 mm) superior lingual foramen was high, at 31.0 %. A large-diameter lateral lingual foramen in the premolar region occurred at a frequency of 17.0 %; communication with the mandibular canal was observed in 70.0 % of these cases. These quantitative data on the lingual and lateral lingual foramina of the mandible provide valuable information that could help to avoid surgical complications during implant placement in elderly Korean.


RESUMEN: El objetivo de este estudio fue determinar las características morfológicas del foramen lingual y del foramen lingual lateral mediante TC de haz cónico en adultos mayores coreanos. Se obtuvieron imágenes de TC de haz cónico de 80 coreanos mayores de 50 años (edad media, 65,2 años). Se determinó la prevalencia de los forámenes linguales y linguales laterales en la cara lingual de la mandíbula. Se midió el diámetro y la altura hasta el margen superior de los forámenes desde el margen inferior mandibular, y la altura ósea hasta la cresta alveolar desde el margen inferior mandibular. Además, se evaluó la ubicación del foramen lingual lateral, la dirección de su canal y la presencia de comunicación con el canal mandibular. Todos los adultos mayores coreanos tenían al menos un foramen lingual, con dos o tres forámenes en el 77,5 %. Se observó un foramen lingual lateral en el 91,3 %, siendo la prevalencia más alta en el segundo premolar en casos dentados (21,6 %; 33/ 153). Las mayores frecuencias de estos forámenes se atribuyeron a altas frecuencias de foramen lingual inferior y foramen lingual lateral de diámetro relativamente pequeño en la región de los incisivos. La prevalencia de un foramen lingual superior de gran diámetro (≥1 mm) fue alta, del 31,0 %. Un foramen lingual lateral de gran diámetro en la región premolar ocurrió con una frecuencia del 17,0 %; se observó comunicación con el canal mandibular en el 70,0 % de estos casos. Estos datos cuantitativos sobre los forámenes linguales y linguales laterales de la mandíbula proporcionan información valiosa que podría ayudar a evitar complicaciones quirúrgicas durante la colocación de implantes en adultos mayores coreanos.

ABSTRACT

La ortodoncia lingual ha acercado a la consulta ortodóncica a pacientes que nunca antes hubie- ran aceptado un tratamiento ortodóncico. Los tratamientos estéticos son una de las opciones más solicitadas en este siglo por los pacientes adultos, que no solo desean un resultado estético sino que solicitan, también, aparatología estética. Se presenta el caso de una paciente adulta para el que se eligió un tratamiento híbrido: aparatología superior lingual (brackets linguales 2D) y apara- tología inferior vestibular (brackets autoligables, prescripción Roth pasiva-activa). Los brackets 2D son de autoligado, por lo que se utilizan fuerzas biológicas, suaves, constantes y continuas. Es importante en estos casos que ambos maxilares se trabajen con el mismo tipo de fuerzas.


Lingual orthodontics has brought to the orthodontic consultation patients who would never have accepted orthodontic treatment before. In this century, aesthetic treatments are one of the most requested options by adult patients who not only want an aesthetic result but also aesthetic appliances. It is presented the case of an adult patient to which a hybrid treatment was chosen: upper lingual appliances (2D lingual brackets) and lower vestibular appliances (self-ligating brackets, passive-active Roth prescription). 2D brackets are self-ligating, so constant and continuous biological gentle forces are used. In these cases, it is important that both jaws are worked with the same type of forces.

ABSTRACT

Introduction: The World Health Organization (WHO) establishes that breast-feeding must be promoted, supported, and protected due to its benefits in maternal and child health and for the economic, environmental, family and community benefits it produces. Case Report: Ankyloglossia in the newborn can cause early cessation of breastfeeding. A clinical case of a 2-month-old baby, referred to the surgical team of the Dental Service of the Pereira Rossell Hospital Center, with difficulties in breastfeeding due to ankyloglossia is presented. Diagnosis and laser surgical treatment was performed. In the immediate postoperative period, there was an improvement, achieving breastfeeding without the aid of devices, and a progressive improvement during the month following the intervention occurred. Conclusion: Early diagnosis and timely intervention collaborate in the maintenance of lactation and in the progressive improvement of the process.


Introducción: La Organización Mundial de la Salud (OMS) establece que la lactancia materna debe ser promovida, apoyada y protegida por sus beneficios demostrados en la salud materno-infantil y por las ventajas económicas, ambientales, familiares y comunitarias que produce.Reporte de Caso: La anquiloglosia en el recién nacido puede provocar el abandono temprano de la lactancia. Se presenta un caso clínico de un bebe de 2 meses, derivado al equipo quirúrgico del Servicio Odontológico del Centro Hospitalario Pereira Rossell, por dificultades en el amamantamiento a causa de anquiloglosia. Se realizó el diagnóstico y tratamiento quirúrgico con láser. En el postoperatorio inmediato se registró una mejoría en el amamantamiento, logrando la lactancia sin intermediario y una mejoría progresiva en la praxis durante el mes siguiente a la intervención.Conclusión: El diagnóstico precoz y la intervención oportuna colaboran en el mantenimiento de la lactancia y en la mejora progresiva de la praxis.

Abstract Introduction The tongue plays an important role in the development of craniofacial structures. At rest, the light and constant pressure of the tongue against the hard palate, counterbalanced by the pressure provided by proper lip sealing, serves as a guide for maxillary growth. Ankyloglossia makes tongue coupling against the hard palate difficult, impacting maxillary development, which may lead to breathing disorders. Objective To verify the effect of lingual frenotomy on the resting position of the tongue and lips in infants with ankyloglossia. Methods The sample consisted of 334 infants aged between 1 and 60 days old diagnosed with ankyloglossia. The groups were divided in: a) experimental group (EG), which consisted of infants whose mothers agreed with lingual frenotomy; b) control group (CG), which consisted of infants whose mothers either refused lingual frenotomy or were waiting for surgery. Both the position of the lips and of the tongue at rest were assessed while the infants were sleeping during the quiet sleep phase. For mothers who refused their infants to undergo the surgical procedure, a follow-up of the infants was proposed to verify possible interference of the frenulum with the resting position of the tongue and lips. Infants whose mothers agreed with surgery were referred for lingual frenotomy. Results Regarding the position of the tongue and lips at rest at the initial and final assessments, the statistical analysis demonstrated significant differences between both groups. Conclusion Lingual frenotomy enabled infants diagnosed with ankyloglossia to maintain both tongue coupling against the hard palate and closed lips at rest.
