ABSTRACT

Objective To explore the correlation between serum lipoprotein-related phospholipase A2 (Lp-PLA2), cystatin C (Cys-C) levels and disease severity in patients with hepatitis B cirrhosis (HBC). Methods Clinical data of 110 HBC patients in the hospital from October 2017 to May 2019 were retrospectively analyzed. According to Child-Pugh classification criteria of liver cirrhosis, they were divided into grade A (n=42), grade B (n=37) and grade C groups (n=31). Another 30 healthy controls during the same period were enrolled as control group. The levels of serum Lp-PLA2 and Cys-C were detected. And their correlation with disease severity was analyzed. Results Levels of serum Lp-PLA2 and Cys-C in HBC group were higher than those in control group (P<0.05). The levels of serum Lp-PLA2 and Cys-C were the highest in grade C group, followed by grade B group and grade A group (P<0.05). The areas under the ROC curve (AUC) of serum Lp-PLA2 combined with Cys-C for evaluating grade A and B, grade B and C HBC were 0.875 and 0.837, which were higher than those of Lp-PLA2 (0.772, 0.750) and Cys-C (0.750, 0.691) alone (P<0.05). Spearmann rank correlation analysis showed that levels of serum Lp-PLA2 and Cys-C were positively correlated with disease severity (r=0.659, 0.561, P<0.05). Conclusion The levels of serum Lp-PLA2 and Cys-C are significantly increased in HBC patients, which are gradually increased with the aggravation of HBC. The two indexes are positively correlated with disease severity, which are of diagnostic efficiency for the classification of liver cirrhosis.

ABSTRACT

Objective Few studies are reported about the values of the levels of plasma lipoprotein related phospholipase A2 (LP-PLA2) and serum resistin in predicting the prognosis of atherosclerotic cerebral infarction (ACI).This article aims to evaluate the predictive values of LP-PLA2 and serum resistin in the prognosis of ACI.Methods This study included 136 cases of ACI diagnosed and treated in Huaihe Hosptial from September 2013 to September 2014.The patients were followed up for 2 years,during which 48 were found with adverse outcomes (the poor prognosis group) 76 without disease progression (the good prognosis group).We analyzed the influencing factors on prognosis using the Cox proportional hazard model and evaluated the sensitivity and specificity of these factors in predicting the prognostic risks of the patients by ROC curve analysis.Results The rate of poor prognosis was 38.71% among the included patients.Analysis with the Cox proportional hazard model showed significant impacts of LP-PLA2 (OR =2.105,95% CI:1.878-2.413) and serum resistin (OR=1.784,95% CI:1.509-2.213) on the prognosis of the patients.Compared with the good prognosis group,the poor prognosis group exhibited markedly higher levels of LP-PLA2 ([128.78±76.22] vs [268.65±89.02] mg/L,P<0.01)and serum resistin ([20.71±6.15] vs [24.36±4.87] mg/L,P<0.01).The sensitivity and specificity of LP-PLA2 combined with serum resistin were 81.35% and 78.26%,respectively.Conclusion The combination of LP-PLA2 with serum resistin has a good predictive value for the prognosis of atherosclerotic cerebral infarction and is expected to be widely applied as a routine index in clinical practice.

ABSTRACT

Objective To investigate the value of combinedevaluation of serum lipoprotein associated phospholipase A2 (LP-PLA2) and cystatin C (CysC) on systemic lupus erythematosus (SLE) patients with lupus erythematosus.Methods A total of 177 patients with SLE were enrolled in the General Hospital of People's Liberation Army from June 2000 to February 2017,87 cases in lupus group (SLE group),90 cases in lupus nephritis group (LN group),60 cases in healthy control group.The concentration of LP-PLA2 and CysC were measured.The ROC curves was established for estimation of the cutoff values of two indexes in SLE patients with LN,and the diagnostic efficacy of different diagnostic criteria was compared.Results The LP-PLA2 and CysC concentration in the LN group were higher than those in the SLE group (Z=-2.512,-5.688,all P<0.05).The cutoff value of LP-PLA2 was 245.5 U/L and the cutoff value of CysC was 1.235 mg/L on the risk assessment of in SLE patients with LN.When using the parallel method for combined LP-PLA2 and CysC to evaluate the risk of LN,the sensitivity and negative predictive value would be significantly improved (x2 =9.461,4.377,all P< 0.05),the efficiency was better.Conclusion Parallel assessment of serum LP-PLA2 and CysC have clinical value on assessment of the risk of LN in SLE patients.
