ABSTRACT

Introducción: La hernia de Littré se define como la presencia de un divertículo de Meckel en cualquier saco herniario, su incidencia es de alrededor de un 2 por ciento y se han descrito pocos casos en la literatura. Objetivo: Presentar un caso inusual de cirugía de urgencia con un cuadro de oclusión intestinal por hernia de Littré, operado en el servicio de urgencias del Hospital Militar Dr. Carlos J. Finlay. Caso clínico: Paciente de 79 años de edad que consulta por cuadro de dolor abdominal, vómitos y aumento de volumen en la región crural derecha, operado con diagnóstico presuntivo de hernia crural complicada. Se realiza abordaje preperitoneal, se encontró en el saco herniario, un divertículo de Meckel con compromiso vascular. Fue resecado completamente, efectuada anastomosis término-terminal del íleon y reparado defecto herniario con técnica de Cheatle-Henry. Comentarios: La paciente recibió resección intestinal y reparación tisular del anillo crural, recibió alta hospitalaria al tercer día y médica a los 45 días de la operación(AU)


Introduction: Littré hernia is defined as the presence of a Meckel diverticulum in any hernia sac, the incidence is around 2 percent and few cases have been described in the literature. Objective: To present an unusual case of emergency surgery with intestinal occlusion due to Littré's hernia operated in the emergency service of the Military Hospital Dr. Carlos J. Finlay. Clinical case: A 79-year-old patient who consulted due to abdominal pain, vomiting and increased volume in the right crural region, he was operated with a presumptive diagnosis of complicated crural hernia. A preperitoneal approach was performed, a Meckel diverticulum with vascular compromise was found in the hernia sac. It was completely resected, end-to-end anastomosis of the ileum was performed and hernial defect repaired with the Cheatle-Henry technique. Comments: this patient received intestinal resection and tissue repair of the crural ring. She was discharged on the third day and medically discharge 45 days after the operation(AU)

ABSTRACT

Resumen Objetivo: Presentar un caso de hernia femoral de Littré estrangulada. Caso clínico: Hombre de 86 años, que consultó por cuadro de dolor abdominal periumbilical postraumático de 4 días de evolución que se acompañaba de vómitos y ausencia de deposiciones con tránsito de gases conservado. Destacaba al examen masa dolorosa e indurada de 3 cm de diámetro no reductible bajo arcada inguinal derecha. Se realizó una herniorrafia femoral con abordaje preperitoneal, identificando una hernia de Littré estrangulada y realizándose una resección intestinal más reparación tisular del defecto herniario. Conclusión: Las hernias de Littré femorales son una entidad extremadamente rara y el manejo quirúrgico óptimo no está establecido.


Abstract Objective: To present a case of strangulated Littré femoral hernia. Clinical case: 86 years old man with 4 days of post-traumatic periumbilical abdominal pain accompanied by vomiting and absence of stools with gas transit preserved. Examination highlighted a painful and indurated mass of 3 cm in diameter that it was not reducible below right inguinal arch. A femoral hernia repair with preperitoneal approach was performed by identifying a strangulated Littré hernia performing bowel resection and tissue repair of the hernia defect. Conclusion: Littré femoral hernias are an extremely rare entity and optimal surgical management is not established.

ABSTRACT

Se describe el caso clínico de un adulto de 47 años de edad, quien acudió al cuerpo de guardia del Hospital Provincial Docente Clinicoquirúrgico Saturnino Lora Torres de Santiago de Cuba por presentar, desde hacía 3 días, dolor en epigastrio, de comienzo insidioso, que desapareció en las últimas 24 horas, y apareció nuevamente en la fosa ilíaca derecha y mesogastrio, pero esta vez se trasformó en fijo, punzante, de moderada intensidad, que no se aliviaba con analgésicos, y asociado a náuseas; igualmente, refirió y se corroboró al examen físico la presencia de una hernia umbilical que no lograba reducirse. El paciente fue operado y se encontró una hernia de Littre umbilical estrangulada. Se le realizó resección y anastomosis del íleon a nivel del divertículo de Meckel, así como herniorrafia umbilical. La evolución fue favorable y egresó sin complicaciones


The case report of a 47 years adult is described who went to the emergency room of Saturnino Lora Torres Teaching Clinical Surgical Provincial Hospital in Santiago de Cuba due to epigastric distress of insidious beginning for 3 days that disappeared in the last 24 hours, appearing again in the right iliac cavity and mesogastrium, but this time it was transformed in a fixed, sharp, of moderate intensity pain that was not relieved with analgesics, and associated with nausea; equally, he referred the presence of an umbilical hernia that was not able to decrease and it was corroborated in the physical exam. The patient was operated on and a strangulated umbilical Littre hernia was found. A resection and anastomosis of ilium at the Meckel diverticulum level was carried out, as well as umbilical hernia repair. There was a favorable clinical course and he was discharged without complications

ABSTRACT

La hernia de Littré se define como la presencia de un divertículo de Meckel en el interior de cualquier orificio herniario. Se reportan muy pocos casos en la literatura por lo infrecuente de la patología, con una incidencia de aproximadamente un 2 por ciento de la población general. Presentamos un paciente de 70 años de edad que acudió al servicio de urgencia por presentar una obstrucción intestinal debido a una hernia inguinoescrotal derecha incarcerada. Con este diagnóstico se intervino de forma urgente. Se abordó por vía preperitoneal inguinal derecha y se encontró en el interior del saco herniario un divertículo de Meckel gangrenado y perforado. Se realizó resección intestinal, cierre del cabo distal del íleon terminal cerca de la válvula, anastomosis íleocolon ascendente latero-lateral y reparación del defecto herniario con una malla de polipropileno. El paciente evoluciona bien y es dado de alta a los 7 días(AU)


Littré hernia is defined as the presence of a Meckel's diverticulum inside any hernial orifice. Very few cases are reported in literature because its rarity, with an incidence of approximately 2 percent of the general population. We report a 70-year-old man who came to the emergency department for filing an intestinal obstruction due to an incarcerated right inguinal scrotal hernia. He urgently underwent surgery because of this diagnosis. Surgery was addressed by right preperitoneal inguinal via. A gangrenous and perforated Meckel's diverticulum was found inside the hernia sac. Intestinal resection was performed. The distal end of the terminal ileum was closed near the valve, the íleocolon latero-side up anastomosis was performed and the hernia defect was repaired with a polypropylene mesh. The patient did well and was discharged 7 days later(AU)

ABSTRACT

The term "Littre's hernia" was originally defined by Reinke in 1841 as "the presence of a Meckel's diverticulum in any hernia sac." Littre's hernia is a very rare disease, which accounts for less than 1% of all Meckel's diverticula. We report a case of Littre's hernia experienced in a 45 day-old infant.
