ABSTRACT

Background: HELLP syndrome is life threatening condition. Diagnosis of HELLP syndrome plays an important role in assessment of complication. It can be done by USG and CT scan. Objectives: The aim of this study was to define role of CT scan in assessment of HELLP syndrome. Materials and methods: A case report aimed at evaluation of patient who came with sign and symptoms of HELLP syndrome to Radiology Department of Dhiraj Hospital by using CT scan. Results: A 30 year old patient presented in post-partum state with fever, rigor, nausea and tenderness in the right hypochondrium. Case history showed patient had suffered from pre-eclampsia during childbirth. The Patient had undergone ultrasonography which gave a diagnosis of hepatomegaly with liver abscess. Her Blood investigation showed total count of 22700 with predominant polymorph of 70%, Hb- 4.7 gm, SGPT was 234, Patient has thrombocytopenia with platelet of 1.5 lac. She was subjected to pre and post contrast CT scan in Dhiraj general hospital which showed hepatosplenomegaly, multiple ill-defined hypodense lesions in both lobes of liver (HU value = 52-70) with perilesional oedema, suggestive of multiple liver hematomas with rupture of hematoma into the peritoneal space. Conclusion: CT scan plays the key-role in assessment of complications in pre-eclamptic postnatal patient with HELLP syndrome which may be missed by USG. There by helping in patient management and prognosis.

ABSTRACT

El hematoma subcapsular es una complicación grave elevado riesgo de mortalidad materna y neonatal, se observa como complicación de patologías hipertensivas más frecuentemente, además de otras patologías graves como hígado graso agudo del embarazo, traumatismos, patologías infecciosas. Se instala durante el embarazo o post parto, con manifestaciones clínicas inespecíficas, con grave y rápido deterioro materno que de no diagnosticar y tratar en tiempo y forma. Produce muerte materna en un 75% y fetal 60%. Se presenta un caso de hematoma hepático como complicación de una eclampsia y síndrome de Hellp, que evolucionó en forma favorable. Tratada con electrocoagulación el cual usamos para tratamiento de la hemorragia.


The subcapsular hematoma is a serious complication, with high risk of maternal and neonatal mortality, it is seen as a complication of hypertensive diseases more frequently, busides another serious diseases such as acute fatty liver of pregnancy, trauma, infectious diseases. It is onset occurs during pregnancy or postpartum, with nonspecific clinical manifestations, with severe and rapid material deterioration which of not diagnosed and treated from the beginning could produce maternal death in 75% and fetal 60%. We present a case of liver hematoma as a complication of eclampsia and Hellp syndrome, whose evolution was favorable, treated with electrocoagulation which we use for the treatment of hemorrhage.

ABSTRACT

El hematoma hepático subcapsular es una complicación infrecuente y grave durante la gestación o el período puerperal. Esta patología generalmente se relaciona con preeclampsia o síndrome de HELLP. Su diagnóstico debe confirmarse por tomografía axial computarizada. La precocidad del diagnóstico y tratamiento es importante para evitar la ruptura del hematoma.


Subcapsular hepatic hematoma is a rare and severe complication during pregnancy or postpartum period. This condition is usually related to preeclampsia or HELLP syndrome. Its diagnosis must be confirmed by abdominal computed tomography. An early diagnosis and treatment are important to avoid hematoma rupture.

ABSTRACT

A subcapsular liver hematoma (SLH) is a relatively common lesion in fetuses and neonates. Although an SLH ruptures rarely, it may be life threatening. We report on a term neonate with a delayed rupture of an SLH that occurred on day 7 of life. The infant had been resuscitated with intubation, positive pressure ventilation, and chest compression at birth because of meconium-associated perinatal depression. The SLH was diagnosed by abdominal ultrasonography and paracentesis, and the ruptured SLH was treated operatively. After intensive medical and surgical management, the infant was discharged healthy on day 27 of life. A newborn infant presenting with the sudden onset of extreme shock and pallor associated with abdominal distension should undergo differential diagnosis for SLH and a clinical evaluation concurrent with fluid resuscitation and timely surgery.

ABSTRACT

Spontaneous rupture of subcapsular liver hematoma in pregnancy is rare but potential life threatening complication of preeclampsia. We experienced a case of spontaneous rupture of subcapsular hematoma of liver that was treated with conservative method. So, we present the case with a brief review of literatures as first report in Korea.
