ABSTRACT
Objective@#To sum up the repair techniques of secondary alar deformity after the first phase reconstruction of alar defect.@*Methods@#From January 2010 to December 2017, 17 patients with secondary alar deformity were included. Secondary alar deformities after the first stage reconstruction of alar defect included the abnormalities of alar groove line, absence of alar-cheek groove and the notch of alar rim. Z-plasty, V-Y advanced flaps, local skin flap and other technologies were introduced to repair details.@*Results@#Infection was found in one case of the reconstruction of the alar-cheek groove and the wound was healed by treatment. There was no necrosis occurred in other flap. The shape of the alar was satisfactory and the anterior nostril was not narrow. Patients were followed up for 1 month to 1 year. The average follow-up time was 5 months. No recurrence of nasal alar tumor was found. The patients were satisfied with the appearance.@*Conclusions@#The symmetrical and satisfactory nasal shape can be obtained with local flaps such as Z-plasty, V-Y advancement of flaps for secondary alar deformities.

Objective@#To explore the clinical application and effect of sideburns aesthetic reconstruction with local flaps.@*Methods@#Between June 2012 and June 2017, there were 12 cases of patients with Benign tumor which is in the sideburn , including 7 Male patients, 5 female patients, thier ages are from 37 to 68 years old.The tumor size ranged from 0.5 cm×1.0 cm to 1.5 cm×2.5 cm, and the defects, size ranged from 1.0 cm× 1.2 cm to 1.5 cm× 3.0 cm. Single leaf flaps, double leaves flaps or local flaps which are on the ipsilateral sideburn or in the hairy area behind the ear are designed for sideburns reconstruction according to the size and shape of the defect, The flaps size ranged from 1.5 cm× 2.5 cm to 1.8 cm× 3.5 cm.@*Results@#All flaps with 12 cases of patients survived completely with no flap blood supply obstacles, and the incision healed well, all patients were followed up for 6-24 months with no recurrence of the tumor , and the incision scars are hidden in the sideburns, And the sideburn showed satisfactory effect of aesthetic reconstruction with good symmetric cosmetic effect.@*Conclusions@#The local flaps have less injury in the surgery area , simple surgical approach, hidden incision scars, reliable blood supply and high survival rate, which do not affect the sideburns symmetry.Aesthetic result can be achieved by local flaps in the reconstruction of defect after resection of benign tumor at sideburn.

Objective@#To investigate the clinical effects of immediate nipple reconstruction with a local mastectomy flap in secondary breast reconstruction by deep inferior epigastric artery perforator (DIEP) flap.@*Methods@#A free bipedicle DIEP flap was raised and folded upward to form the breast. Meanwhile, a deepithelialized lower mastectomy flap with a distant skin paddle was elevated and pulled throughout the reconstructed breast. The skin paddle was carefully sutured to the position of future nipple. Three weeks later, the pedicle of mastectomy flap was divided, and the paddle was modeled to form the new nipple.@*Results@#From February 2011 to June 2016, 42 patients who had previously undergone unilateral modified mastectomies underwent the breast reconstructions and immediate nipple reconstructions. All flaps survived postoperatively. The average projection of the reconstructed nipple was measured as (16.3±2.4) mm immediately after the operation and gradually decreased to (8.4±1.3) mm by one year follow up. 35 patients ranked the aesthetic appearance of the reconstructed nipple and breast very good or good.@*Conclusions@#The immediate nipple reconstruction with a local mastectomy flap and breast reconstruction by DIEP flap could achieve a good breast shape and maintain a long term residual nipple projection, which results in considerable patient satisfaction.
ABSTRACT
Objective To investigate the clinical effect of treatment with local skin flap after primary surgical resection of non melanoma skin cancer.Methods A total of 176 patients with head and facial skin cancer who were admitted into our hospital from September 2008 to September 2015 were divided into three groups according to different repair plans.Namely,the suture group (41 cases),the local skin flap group (81 cases) and the skin graft group (54 cases).The repair effect of the three groups were compared after the operation.Results The good rate was 68.39%(28/41) in the suture group,98.77%(80/81) in the local skin flap group,and 83.33%(45/54) in the skin graft group,and it was significantly higher than any of the other two groups with statistically significant difference(P0.05).The 3 groups were followed up for a period of 1 year,as a result,there was no recurrence and distant metastasis.Conclusion For patients with head and facial non melanoma skin cancer,repaired with local skin flap healing treatment after primary surgical resection can not only get high healing rate but also significant improvement of patient satisfaction rate.In addition,it can lower the rate of recurrence and distant metastasis after operation.
