ABSTRACT

OBJECTIVE: To discuss the clinical effects of clomiphene citrate combined with low molecular weight heparin calcium in the treatment of patients with polycystic ovarian syndrome(PCOS)infertility. METHODS: A total of 105 patients with PCOS infertility who were admitted to the Outpatient Department of Women and Children's Hospital of Qingdao University from September 2016 to April 2018 were taken as research subjects.They were randomly divided into observation group(57 cases)and control group(48 cases).The control group received clomiphene citrate for ovulation treatment,the observation group was given clomiphene citrate combined with low molecular weight heparin calcium for ovulation treatment,and the endometrial receptivity changes,pregnancy rate and the incidence of adverse reactions of patients were compared between the two groups.RESULTS: In the observation group,the endometrial thickness,blood flow index(FI),endometrial spiral arterial blood flow resistance index(RI)and pulsatility index(PI)decreased,and the pregnancy rate was better than that in the control group;the difference was statistically significan(t P0.05).CONCLUSION: Clomiphene citrate combined with low molecular weight heparin calcium in the treatment of polycystic ovary syndrome infertility can improve the treatment efficiency,and is safe and reliable.It is worthy of clinical promotion.

ABSTRACT

Objective@#To analyze the clinical effect of low molecular weight heparin calcium(LMWH) combined with aspirin in preventing portal venous thrombosis (PVT) after devascularization.@*Methods@#Retrospective cohort study was carried out. A total of 61 patients with portal hypertension in Zhongshan Hospital affiliated to Fudan University from December 2014 to April 2017 were included in the study, including 43 males and 18 females, aged 24~74 years. According to anticoagulation methods, the patients were divided into treatment group (31 cases of LMWH combined with aspirin) and control group (30 cases of aspirin). Splenectomy and pericardial devascularization or combined devascularization were performed in both groups. Platelet count, prothrombin time, Child-Pugh score, PVT status and surgical complications were observed after surgery.@*Results@#There were no significant differences for platelet count and prothrombin time between the two groups after 7, 14, 30, 60 and 90 days (P>0.05). Child-Pugh scores at 3, 7, 14, 30, 60 and 90 d after operation of the two groups showed no significant differences (P>0.05). Seven days after the operation, PVT in the treatment group and control group was 7 (22.6%) and 7 (23.3%), respectively. Thirty days after the operation, PVT was detected in 8 patients (25.8%) in the treatment group and 15 patients (50.0%) in the control group. Sixty days after the operation, 6 patients (19.4%) had PVT in the treatment group and 13 patients (43.3%) in the control group. PVT was detected in 4 patients (12.9%) in the treatment group and 12 patients (40.0%) in the control group 90 days after operation. The incidence of PVT in the 30, 60 and 90 days after surgery was lower than that in the control group, with significant difference (P<0.05), and the degree of thrombosis was relatively mild. There were 6 cases (20.0%) of complications in the control group and 6 cases (19.4%) in the treatment group, there were no significant differences between the two groups (P>0.05).@*Conclusion@#LMWH combined with aspirin is safe and effective in preventing PVT after devascularization.

ABSTRACT

Objective To investigate the clinical effect of low molecular weight heparin calcium (MvH) in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with pulmonary heart disease. Methods 128 AECOPD patients with pulmonary heart disease from January 2016 to December 2016 in our hospital were randomly divided into observation group and control group in equal number by random digit table with EXCEL2007. Apart from the basic treatment like anti infection,oxygen,expectorant,maintenance of water and electrolyte balance and cardiotonic treatment ,the observation group was treated with MvH. The two groups were compared in terms of pre-and post-ventricular end diastolic volume(EDV),end systolic volume(ESV),right ventricular ejection fraction(RVEF),pulmonary artery acceleration time(AT),arterial oxygen pressure(PaO2),arterial partial pressure of carbon dioxide(PaCO2),pH value,whole blood viscosity at low shear the whole blood viscosity at high shear rate,plasma viscosity,hematocrit,plasma D- dimer(D-D). Results After treatment,EDV and ESV of the observation group were significantly lower than the control group (P < 0.05)and RVEF and AT were higher than the control group(P < 0.05). PaCO2 was lower than that of the control group(P < 0.05). PaO2,pH values were higher than those of the control group(P < 0.05). After treatment,the whole blood viscosity,whole blood low shear viscosity at high shear rate,plasma viscosity,hematocrit, plasma level of D-D of the observation group were all lower than those of the control group (P < 0.05). Conclusion Subcutaneous injection with MvH to treat the patients with AECOPD complicated with pulmonary heart disease can improve the blood rheology and improve heart function and blood gas level.

ABSTRACT

OBJECTIVE:To investigate the effects of low molecular weight heparin calcium and rivaroxaban combined with atorvastatin on related indexes in patients with acute pulmonary embolism(APE). METHODS:The data of 72 APE patients were analyzed retrospectively. According to treatment plan,the patients were divided into group A(21 cases),group B(26 cases)and group C(25 cases). Group A was treated with intramuscular injection of Low molecular heparin calcium injection immediately after admission;2 days later,they were given Warfarin sodium tablets;7 days later,Low molecular heparin calcium injection was stopped while warfarin was still administrated,lasting for 3-6 months. Group B was given Low molecular heparin injection(same usage and dose as group A);2 days later,they were additionally treated with Rivaroxaban tablets;7 days later,Low molecular heparin calcium injection was stopped while rivaroxaban was still administrated,lasting for 3-6 months. Based on the treatment in group B,group C was treated with Atorvastatin calcium tablets 20 mg orally,once a day in the evening,lasting for 3-6 months. The time of dyspnea,chest pain and cyanosis disappearance were observed in 3 groups as well as the levels of HR,pa(O2),pa(CO2), CRP,D-dimer,TNF-α,IL-1,IL-6,ET-1 and NO before and after treatment. The occurrence of clinical outcome events and ADR were also observed. RESULTS:The time of dyspnea,chest pain and cyanosis disappearance in group A were longer than group B, and the group B was longer than the group A,with statistical significance(P0.05). After treatment ,HR of 3 groups were all lower than before ,and they showed group Agroup B>group C,with statistical signif-icance(Pgroup A and group B,with statistical significance (P0.05). ET-1 levels in 3 groups were significantly lower than before,and the levels showed group group group B>group A,with statistical significance (P<0.05). The incidence of clinical outcome events in group A was significantly higher than group C, the incidence of ADR in group B was significantly lower than that group A,with statistical significance(P<0.05). CONCLUSIONS:Low molecular weight heparin calcium and rivaroxaban combined with atorvastatin can significantly im-prove clinical symptoms and blood gas indexes,relieve vascular endothelial damage,reduce the levels of inflammatory cytokines and improve the prognosis of patients with APE,without increasing the incidence of ADR.

ABSTRACT

Objective To investigate and analyze clinical analysis of low molecular weight heparin calcium combined with psychological intervention in the treatment of diabetic foot patients. Methods 100 patients with type 2 diabetic foot who were treated in our hospital from February 2015 to November 2016 were selected and randomly divided into the control group and the experimental group, with 50 patients in each group. The patients in the control group and the experimental group were treated with anti infection therapy and insulin hypoglycemic therapy, and the dressing change was carried out. The experimental group was given subcutaneous injection of low molecular weight heparin for psychological intervention, pay attention to the patient's psychological status, strengthen communication with patients, and increase the patient's confidence and treatment compliance. The clinical indexes of the experimental group and the control group were compared and analyzed. Results After the corresponding treatment, 6 cases in the experimental group the wound healing is invalid. The number of ineffective wounds in the control group was 20. The effective rate of treatment in the experimental group was 88.0%, which was significantly higher than that in the control group, and the effective rate was 60.0%, with statistical difference (P<0.05). After treatment, the plasma viscosity and the blood flow velocity of the dorsalis pedis artery in the experimental group were significantly better than those in the control group, with statistical difference (P<0.05). Conclusion Low molecular weight heparin calcium combined with psychological intervention in the treatment of diabetic foot has a good clinical effect, and has the significance of promotion.

ABSTRACT

Objective To study the effect of low molecular weight heparin calcium combined with Bailing capsule in treatment of children with Henoch-Schonlein purpura nephritis and the efficacy of serum hs-CRP, MMP-9, APN and CysC.MethodsFrom January 2013 to December 2015, 84 children with Henoch-Schonlein purpura nephritis were enrolled in our hospital.According to the order of admission, the observation group and control group were divided into observation group and control group.The control group were given low molecular weight heparin calcium, the observation group were treated with low molecular weight heparin calcium combined with Bailing capsule for treatment.The levels of serum hs-CRP, MMP-9, APN, CysC, inflammatory factors and immune levels were observed in the two groups.ResultsAfter treatment, the clinical effective rate of the observation group was significantly higher than that of the control group[92.86% (39/42) vs 69.05% (29/42)] (P<0.05).There were no significant differences in hs-CRP, MMP-9, APN and CysC levels between the two groups.After treatment, the levels of hs-CRP, MMP-9 and CysC in the two groups were significantly higher than those before treatment (P<0.05).APN was significantly higher than that before treatment (P<0.05).Compared with the control group, the levels of hs-CRP, MMP-9 and CysC in the observation group were lower than those in the control group (P<0.05), and the APN level was lower than that in the control group (P<0.05).There were no significant differences in TNF-α, IL-6, IL-10 and IL-18 levels between the two groups before treatment.After treatment, The levels of TNF-α, IL-6 and IL-18 in the two groups were significantly lower than those before treatment (P<0.05).There were no significant differences in IgA, IgG and IgM levels between the two groups.After treatment, the levels of IgA, IgG and IgM in the two groups were significantly lower than those before treatment (P<0.05).The levels of IgA, IgG and IgM in the observation group were lower than those in the control group (P<0.05).There was no significant difference in adverse reaction rate between the two groups.ConclusionLow molecular weight heparin combined with Bailing capsule can effectively reduce hs-CRP, MMP-9, CysC level and APN level in children with Henoch-Schonlein purpura nephritis, improve clinical symptoms, good clinical efficacy and high safety.

ABSTRACT

OBJECTIVE:To compare the clinical efficacy and safety of rivaroxaban and low molecular weight heparin calcium (LMWHA)in the prevention of deep venous thrombosis(DVT)after total hip arthroplasty(THA). METHODS:A total of 100 THA patients selected from orthopedics department of our hospital as research objects were divided into control group and observa-tion group according to random number table,with 50 cases in each group. Control group was treated with LMWHA injection 0.4 mL subcutaneously,qd;observation group was given Rivaroxaban tablet 10 mg orally,qd. Both groups received treatment on the first day after surgery,for consecutive 14 d. Coagulation indexes(PT,APTT,Fib,TT,D-D),VAS score,the incidence of DVT and PE were observed in 2 groups. The postoperative bleeding volume and ADR as hematoma and gastrointestinal bleeding were compared between 2 groups. RESULTS:Before treatment,there was no statistical significance in coagulation indexes or VAS scores between 2 groups(P>0.05). After treatment,PT,APTT,TT and D-D levels,VAS scores of 2 groups were decreased sig-nificantly,while Fib levels were increased significantly;VAS score of observation group was significantly lower than that of con-trol group,with statistical significance(P0.05). The incidence of DVT and PE in observation group were 8.00% and 7.50% ,which were significantly lower than 12.00% and 4.00% of control group,with no statistical significance(P>0.05). The postoperative bleeding volume of observation group was(298.31±52.18)mL,which was significantly lower than(327.40±54.20)mL of control group,with statistical signifi-cance(P0.05). CONCLUSIONS:Rivaroxaban and LMWHA can significantly improve coagulation state,prevent the generation of DVT after THA. While rivaroxaban is better in shortening pain time without increasing the risk of ADR.

Objective To test the effects of human-computer interaction intelligent compression package for heparin injection in patients with orthopedic surgery.Methods From July 2015 to June 2016,108 patients with orthopedic surgery were enrolled in the study,and were divided into the experimental group (54 cases) and the control group(54 cases) by random number table.The experimental group was given heparin injection with human-computer interaction intelligent compression package for 3 min,while the control group was given manual compression for 3 min.We evaluated the incidence and severity of subcutaneous hemorrhage and nurses' operating time of two groups.Results The incidence of subcutaneous hemorrhage was 3.9% in the experimental group,12.4% in the control group(P<0.05).The operating time was(100.4±8.7 s) for the experimental group and(233.8±15.3 s) for the control group (P<0.01).Conclusion Using human-computer interaction intelligent compression package can reduce the incidence of subcutaneous hemorrhage.It can also reduce the working hours of nurses and optimize human resources.

Objective To explore the clinical effect of low molecular weight heparin calcium combined with psychological intervention in the treatment of recurrent spontaneous abortion. Methods 60 patients were treated with recurrent miscarriage after treatment in our hospital from October 2016 to June 2017, and these patients were randomly divided into two groups: control group and observation group.Each group of 30 cases. First of all, the two groups of patients were treated with low molecular weight heparin calcium, and then the control group of patients to choose the most commonly used care methods for nursing operations,the observation group of patients choose to use more targeted psychological intervention care for nursing work.The clinical effects of the two groups were observed and compared. Results There was no significant difference between the two groups. The adverse reaction rate was 10.00% in the observation group and the control group The adverse reaction rate was 26.67%. The adverse reaction rate of the observation group was significantly lower than that of the control group.The difference between the two groups was statistically significant (P<0.05). Conclusion Recurrence of recurrent miscarriage after treatment of patients with more targeted psychological intervention in nursing care,compared to conventional care methods better,while effectively reducing the low molecular weight heparin calcium treatment of recurrent miscarriage of adverse reactions.The incidence of psychological intervention in the care of the clinical way is a worthy of use and to promote the treatment.

ABSTRACT

Objective To explore the preventive effect of low molecular weight heparin calcium in severe burn patients with venous thrombosis. Methods 64 patients of lower extremity burns complicated by systemic inflammatory response in Taizhou Enze medical center (Group) Enze hospital from March 2016 to 2017 March as the research object, were received once daily subcutaneous injection of 2850 U (0.3 mL) low molecular weight heparin calcium treatment, continuous injection of 10 d. The control group was given the routine nursing on the basis treatment, the observation group was given nursing intervention, the incidence rate of deep venous thrombosis and nursing satisfaction rate were compared between two groups. Methods FIB and blood D-dimer in the seventh day after treatment were improved (P<0.05) , PT and APTT in the seventh day after treatment were increased (P<0.05) , the level of FIB in the seventeenth day after treatment was significantly lower than the first day and the seventh day. The incidence of deep vein thrombosis in the observation group was significantly lower than the control group, nursing satisfaction rate in the observation group was higher than the control group, the difference was statistically significant (P<0.05). Conclusion Subcutaneous injection of low molecular weight heparin calcium in patients who are in high blood coagulation state after moderately severe burns SIRS can effectively prevent deep venous thrombosis, auxiliary nursing intervention is helpful to promote the rehabilitation of patients and improve nursing satisfaction.

ABSTRACT

Objective To investigate the clinical efficacy and value of low molecular weight heparin calcium combined with vertebroplasty in the treatment of osteoporotic vertebral compression fractures in elderly patients .Methods 67 elderly patients with osteoporotic vertebral compression fractures in our hospital from January 2013 to January 2016 were selected and randomly divided into control group 30 cases and treatment group 37 cases. All patients were treated with percutaneous vertebroplasty, the control group was given calcium carbonate D3 conventional treatment, the treatment group was treated with low molecular weight heparin calcium anti osteoporosis treatment, the course of treatment was one month.Measurement of bone mineral density before and after the operation of different follow-up time points ( BMD ) and the changes of the oswestry disability index ( ODI ) evaluation activities and the visual analogue scale (VAS) to assess the degree of pain, and the changes of vertebral height before and after operation were analyzed. Results All patients were followed up for an average of 16.4 months (12 to 21 months).Compared with before vertebroplasty, the BMD of the patients in the treatment group was higher than that in the control group at one month, three months and six months, the difference was statistically significant (P<0.05), the low molecular weight heparin treatment after January, treatment group VAS and ODI scores were significantly lower than the control group, and the subsequent different follow-up time points ODI and VAS score can effectively maintain, the difference was statistically significant ( P<0.05) .Compared with before vertebroplasty, postoperative anterior vertebral height, middle height better than preoperative vertebral height, the difference was statistically significant (P<0.05), and after one months of treatment, the treatment group of anterior vertebral height, middle height were better than the control group, the difference was statistically significant (P <0.05).Conclusion After treatment with low molecular weight heparin calcium, the healing process of vertebral body after vertebroplasty, to a certain extent, it can increase the bone mass and prevent bone destruction, increase bone strength, and further achieve the treatment of osteoporosis and prevention of osteoporotic fracture in elderly patients with osteoporotic vertebral compression fractures.

Objective To analyze the clinical value of low molecular heparin calcium combined with early rehabilitation intervention in the prevention of deep vein thrombosis after internal fixation of intertrochanteric fractures.Methods 100 patients with internal fixation for treatment of femoral intertrochanteric fracture from January 2014 and October 2015 were selected and randomly divided into two groups:the control group of 50 cases, treated with low molecular weight heparin low molecular weight heparin by abdominal subcutaneous injection of drug therapy and routine nursing intervention service, observation group of 50 cases, in the control group based on the use of early rehabilitation intervention.The incidence of deep venous thrombosis after operation in two groups was recorded, and the changes of platelet count and coagulation markers were observed before and after intervention in the two groups.Results The observation group of patients with postoperative deep venous thrombosis incidence rate of 4%, significantly lower than the control group 16%, the difference was statistically significant (P<0.05), the two groups after the intervention each time the platelet count was higher than that of before intervention, four weeks after the intervention, the observation group of platelet count (239.51 ±34.62) ×109/L, significantly higher than the control group (235.23 ±28.87) ×109/L, the difference was statistically significant (P<0.05), the two groups before the intervention of each marker concentration, the difference was not statistically significant, patients in the observation group after the intervention, the blood coagulation markers concentrations were significantly lower than before treatment, and were significantly lower than the control group after the intervention.The comparison between the two groups, the difference was statistically significant (P<0.05).There was no chest pain, chest tightness, dyspnea and other symptoms of pulmonary embolism in the two groups , and no adverse reactions were found.Conclusion The application of low molecular weight heparin calcium combined with early rehabilitation intervention can effectively reduce the incidence of deep venous thrombosis after intertrochanteric fracture fixation.

ABSTRACT

Objective To observe the effect of low molecular weight heparin calcium combined with astraga-lus injection in the treatment of diabetic nephropathy.Methods From January 2013 to December 2014,78 patients with stage III diabetic nephropathy in our hospital were enrolled into the study.They were randomly divided into con-trol group(41 cases) and treatment group(37 cases).The control group received general therapy,such as low protein diets,compound alpha keto acid,insulin,irbesartan,simvastatin.The treatment group was treated with low molecular weight heparin calcium and astragalus injection on the basis of general therapy.The changes of urinary albumin and 24 hours urinary protein after treatment in the two groups were observed.Results After treatment,the negative rate of urinary micro albumin in the treatment group ( 75.68%) was significantly higher than that of the control group (29.27%)(χ2 =16.765,P<0.01).24 hour urinary protein of the treatment group[(176.2 ±82.3)mg/24h]was significantly lower than that of the control group[(223.8 ±87.8)mg/24h](t=2.471,P<0.05).Conclusion Low molecular weight heparin calcium combined with astragalus injection can reduce the proteinuria of diabetic nephropa-thy,it is an effective intervention therapy for early diabetic nephropathy.

ABSTRACT

OBJECTIVE:To observe the safety and therapeutic efficacy of low molecular weight heparin calcium in the preven-tion of venous thromboembolism (VTE) of ICU patients. METHODS:572 VTE patients were randomly divided into trial group and control group,with 286 cases in each group. Trial group was given Low molecular weight heparin calcium injection 0.3-0.6 ml, im,qd;control group was given Rivaroxaban tablet 10 mg,po,qd. The incidence of VTE,platelet count,coagulation function, quality score of life and the occurrence of ADR were compared between 2 groups. RESULTS:The incidence of VTE in trial group (0.3%)was significantly lower than control group(2.1%),with statistical significance(P0.05). The physical health,body function and role,general health,emotional role function,mental health and other aspects of trial group were improved significantly,compared to control group,with statistical significance(P>0.05). No obvious ADR was found in 2 groups. CONCLUSIONS:Low molecular weight heparin calcium can effectively prevent VTE and improve the quality of life, while doesn’t influence platelet count and coagulation function with good safety.

ABSTRACT

Objective To explore the clinical therapeutic effect of combined treatment of low molecular weight heparin calcium and breviscapine in elder pulmonary heart disease .Methods 303 patients in elder pulmonary heart disease in the hospital from May 2011 to June 2013 were divided into three groups .100 patients in group A were received the routine therepy ,102 patients of group B were received routine therapy combined with low molecular weight heparin calcium only ,101 patients of group C were received routine therapy combined with low molecular weight heparin calcium and breviscapine .the changes of clinical symptom ,blood gas a-nalysis ,hemorheology in groups before and after treatment were observed .Results The effective rates of group A ,B ,C were 59 .0% ,73 .5% and 90 .0% ,the results showed that the blood gas analysis and hemorheology parameter in groups B and C had more improved than group A (P<0 .05) ,and better in group C than group B(P<0 .05) .Conclusion The combined treatment of low molecular weight heparin calcium and breviscapine can decrease the press of lung artery ,relieve the bear of right ventricle and improve the arterial partial pressure of oxygen ,mean pulmonary arterial pressure and hemorheology parameter ,and has less adverse reaction .

ABSTRACT

Objective To evaluate the clinical effect of clopidogrel combined with low molecular weight heparin (LMWH) on chronic pulmonary heart disease in patients with acute exacerbation.Methods Eighty cases of patients with chronic pulmonary heart disease with exacerbation were randomly divided into treatment group (40 cases) and control group (40 cases).Patients in control group were administrated with conventional treatment including infection control,cough,phlegm,relieving spasm and asthma,persistent low flow oxygen,maintain airway patency,strong heart,diuresis,anti arrhythmia.Patients in treatment group were treated with subcutaneous injection of low molecular weight heparin and oral clopidogrel therapy beside routine treatment.Plasma prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time (TT),fibrinogen(Fb) and pulmonary function(FEVI) were measured.Results (1) After treatment,Fb in treatment group and control group were (3.92 ± 1.11) g/L,(4.54 ± 1.41) g/L respectively.While PT,APTT,TT and FEVI in treatment group were (13.02 ± 1.52) s,(33.21 ± 4.03) s,(14.03 ± 2.52) s,(45.73 ± 7.38) % respectively,significantly higher than the control group ((11.31 ± 2.01) s,(30.13 ± 3.59) s,(12.88 ± 2.11) s and (40.12 ± 6.99) %,P <0.05).(2) In control group,FEVI after treatment significantly improved than that of before treatment (t =4.821 ; P < 0.05),while PT,APTT,TT were no significant changes (P > 0.05).PT,APTT,TT,FEVI in treatment group were significantly improved after treatment than that of before treatment(P < 0.05).(3) The clinical effective rate in treatment group was 80.0% (32/40),higher than that in control group (65.0%(26/40) x2 =4.815; P <0.05).Conclusion The therapy scheme of clopidogrel combined with low molecular heparin calcium is proved to be an efficacy method for reducing sputum viscosity,antiinflammatory,anti allergic effect,platelet aggregation,increasing pulmonary blood flow,improving the condition of patients with pulmonary heart diseasethe.And its clinical effect is significant,worthy to promote and apply.
