ABSTRACT

Purpose:We investigated the factors associated with medical device-related pressure ulcer (MDRPU) due to lower extremity orthosis in patients undergoing convalescent rehabilitation for stroke.Methods:This retrospective study included patients with stroke who wore ankle foot orthosis in the convalescent rehabilitation ward. We measured the following items at admission:Brunnstrom recovery stage, presence of sensory disturbance, exhibition of unilateral spatial neglect, functional independence measures at admission and discharge, and presence of MDRPU. In the statistical analysis, logistic regression analysis was performed to identify the significant factors associated with MDRPU.Results:Ninety-five participants were enrolled in this study (mean age:54.9 ± 11.6 years, Male:78.9%). In logistic regression analysis, Age (odds ratio=1.05, 95% confidence interval=1.01-1.10, p<0.05) and the presence of sensory disturbance (odds ratio=5.17, 95% confidence interval=1.39-19.28, p<0.05) at admission was extracted as the cause of MDRPU.Conclusion:Sensory disturbance at admission is associated with MDRPU in patients undergoing convalescent rehabilitation for stroke who wear ankle foot orthosis.

ABSTRACT

We investigated annual changes with respect to the period taken from the onset of illness to the prescription of lower extremity orthoses in hemiplegic patients after stroke, retrospectively from 1992 to 2007, in an acute phase hospital. We subdivided the initial period into three parts : the period of time from onset to the beginning of rehabilitation (T1), from the beginning of rehabilitation to the beginning of rehabilitation in the training room (T2), and from the beginning of rehabilitation in the training room to prescription (T3). Analysis was made with respect to the relationship between each of the aforementioned periods and the prescription time. Furthermore, we also analyzed the relationship between the length of stay of patients who had been prescribed orthoses, and the prescription time. The prescription time was found to have gradually shortened over time ; moreover, the period in question shortened in a rapid way in 2001, as compared with previous years (<i>p</i> <0.05). T3 showed the highest correlation coefficient (<i>r</i> =0.99), and the length of stay also exhibited a high correlation coefficient (<i>r</i> =0.97). We assume that the annual changes in prescription time might have been due to various underlying factors, namely, social factors, such as a shortening of the length of stay, caused by the differentiation of roles among hospitals, as well as medical factors such as the development of acute phase rehabilitation, or changes in perception of the ways in which orthoses may be used for stroke patients by rehabilitation staff.

ABSTRACT

@#Patients with spinal cord injury(SCI)required higher level of quality of life(QOL),in fact,the disorder of lower limbs motion is the major factor which restricted the independence of social involvement.Routine rehabilitation training,lower extremity orthosis,functional electrical stimulation(FES)and the combined application of those are still the widely used approaches.Meanwhile,the auto-nerve transferring and auto-tendon operation are also the fields that researchers focus on.In practice,we have to choose proper method individually according to the patients' conditions to restore the lower limbs function,so as to improve the QOL.

ABSTRACT

OBJECTIVE: Lower extremity orthoses are important in the rehabilitation of the patients with stroke and traumatic brain injury. But it is unknown how much they are used in the social activity after the discharge from the hospital. This study was carried out to investigate the status of using orthosis in social activity and complaints about orthosis. METHOD: The questionnaires were given to 42 cases with stroke and 17 cases with traumatic brain injury. RESULTS: The user of lower extremity orthosis in social activity were 55.9%, and the causes of disuse were patient's poor condition, improved gait pattern, inappropriate design and defect of orthosis, and patient's refusal. The degree of satisfaction with orthosis for comfort, external appearance and weight were 79.3%, 86.2%, and 72.4%, respectively. CONCLUSION: Physiatrists should give more attention in orthosis at follow-up of patients with stroke and traumatic brain injury and make efforts to improve function and external appearance of orthosis.
