ABSTRACT

ABSTRACT Objective: To describe the morbidity, mortality, and rate of complete resection associated with robotic surgery for the treatment of non-small cell lung cancer in Brazil, as well as to report the rates of overall survival and disease-free survival in patients so treated. Methods: This was a retrospective study of patients diagnosed with non-small cell lung carcinoma and undergoing resection by robotic surgery at one of six hospitals in Brazil between February of 2015 and July of 2018. Data were collected retrospectively from the electronic medical records. Results: A total of 154 patients were included. The mean age was 65 ± 9.5 years (range, 30-85 years). The main histological diagnosis was adenocarcinoma, which was identified in 128 patients (81.5%), followed by epidermoid carcinoma, identified in 14 (9.0%). Lobectomy was performed in 133 patients (86.3%), and segmentectomy was performed in 21 (13.7%). The mean operative time was 209 ± 80 min. Postoperative complications occurred in 32 patients (20.4%). The main complication was air leak, which occurred in 15 patients (9.5%). The median (interquartile range) values for hospital stay and drainage time were 4 days (3-6 days) and 2 days (2-4 days), respectively. There was one death in the immediate postoperative period (30-day mortality rate, 0.5%). The mean follow-up period was 326 ± 274 days (range, 3-1,110 days). Complete resection was achieved in 97.4% of the cases. Overall mortality was 1.5% (3 deaths), and overall survival was 97.5%. Conclusions: Robotic pulmonary resection proved to be a safe treatment for lung cancer. Longer follow-up periods are required in order to assess long-term survival.


RESUMO Objetivo: Descrever morbidade, mortalidade e completude da ressecção associadas à cirurgia robótica para o tratamento de câncer de pulmão não pequenas células no Brasil, assim como sobrevida global e sobrevida livre de doença. Métodos: Estudo retrospectivo incluindo pacientes com diagnóstico de carcinoma pulmonar de células não pequenas submetidos à ressecção anatômica por cirurgia robótica em seis hospitais brasileiros entre fevereiro de 2015 e julho de 2018. Os dados foram coletados retrospectivamente de prontuários eletrônicos. Resultados: Foram incluídos 154 pacientes. A média de idade foi de 65,0 ± 9,5 anos (variação: 30-85 anos). Adenocarcinoma foi o principal tipo histológico, em 128 pacientes (81,5%), seguido por carcinoma epidermoide, em 14 (9,0%). Lobectomia foi o tipo de cirurgia mais frequente, em 133 pacientes (86,3%), seguida por segmentectomia anatômica, em 21 (13,7%). A média de tempo operatório foi de 209 + 80 min. Complicações pós-operatórias ocorreram em 32 pacientes (20,4%). Fístula aérea foi a principal complicação, em 15 pacientes (9,5%). A mediana (intervalo interquartil) de permanência hospitalar foi de 4 dias (3-6 dias) e a de tempo de drenagem foi de 2 dias (2-4 dias). Houve mortalidade operatória em 1 paciente (0,5%). O tempo médio de seguimento foi de 326 + 274 dias (variação: 3-1.110 dias). A ressecção foi completa em 97,4% dos casos. A mortalidade geral foi de 1,5% (n = 3), com sobrevida global de 97,5%. Conclusões: A ressecção pulmonar anatômica robótica no tratamento do câncer de pulmão demonstrou ser segura. Um maior tempo de seguimento é necessário para avaliar a sobrevida de longo prazo.

RESUMEN Fundamento: el cáncer es una de las causas principales de muerte alrededor del mundo. En el año 2012 hubo 14 millones de casos nuevos, así como 8,2 millones de muertes relacionadas. Dentro de todas las localizaciones del cáncer, es el pulmón el de mayor incidencia y mortalidad para el hombre y en el caso de la mujer ocupa el segundo lugar. Objetivos: realizar una actualización en aspectos epidemiológicos, del diagnóstico, así como del tratamiento del cáncer pulmonar a nivel global, regional y local. Métodos: se realizó una búsqueda en las bases de datos Pub Med, SciELO, Medline, Cochrane, Lilacs, mediante EndNote y Clinical Key, en un período desde el primero de septiembre al 15 de diciembre de 2017. Se emplearon las palabras claves: lung cancer, epidemiology, diagnostic, treatment, screening. Se realizó una revisión bibliográfica de un total 302 artículos de ellos se utilizaron 40 citas la mayoría de ellas de los últimos tres y cinco años. Se consideraron para la revisión, artículos originales, revisiones a texto completo, así como artículos de opinión. Desarrollo: se abordaron aspectos del cáncer pulmonar relacionados con estadísticas globales, regionales y locales, así como de los indicadores de impacto, aspectos relacionados con el diagnóstico precoz a través del tamizaje y elementos novedosos de ensayos clínicos relacionados con la aplicación de la vacuna CIMAvax-EGF. Conclusiones: con el elevado aporte del cáncer de pulmón a la morbilidad y mortalidad general en Cuba y en particular en la provincia de Camagüey, así como el consecuente impacto negativo en la duración y calidad de vida de la población, se considera pertinente y necesario el diseño de un programa integral asistencial organizacional para atender a pacientes con esta terrible y letal enfermedad.


ABSTRACT Background: cancer is one of the leading causes of death around the world. In 2012, there were 14 million of new cases, as well as 8.2 million related deaths. Within all cancer locations, the lung is the one with the highest incidence and mortality for men and in the case of women, it occupies the second place. Objectives: to carry out an update on epidemiological aspects of diagnosis, as well as the treatment of lung cancer at a global, regional and local level. Methods: a search on Pub Med, SciELO, Medline, Cochrane, Lilacs databases using EndNote and Clinical Key was carried out, from September 1 to December 15, 2017. The following keywords were used: lung cancer, epidemiology, diagnostic, treatment, screening. A bibliographic review of a total of 302 articles was made, of them 40 citations were used, most of them from the last three and five years. Original articles, full-text reviews, as well as opinion articles were considered for the review. Development: aspects of lung cancer, related to global, regional and local statistics, as well as impact indicators, aspects related to early diagnosis through screening and novel elements of clinical trials related to the application of the vaccine CIMAvax-EGF were addressed. Conclusions: with the high contribution of lung cancer to morbidity and general mortality in Cuba and in particular in Camagüey province, as well the negative consequent impact on the duration and quality of life of the population, it is considered necessary to design a comprehensive organizational assistance program to care for patients with this terrible and lethal disease.

ABSTRACT

ABSTRACT OBJECTIVE To describe the trend in cancer mortality rates in Brazil and regions before and after correction for underreporting of deaths and redistribution of ill-defined and nonspecific causes. METHODS The study used data of deaths from lung cancer among the population aged from 30 to 69 years, notified to the Mortality Information System between 1996 and 2011, corrected for underreporting of deaths, non-registered sex and age , and causes with ill-defined or garbage codes according to sex, age, and region. Standardized rates were calculated by age for raw and corrected data. An analysis of time trend in lung cancer mortality was carried out using the regression model with autoregressive errors. RESULTS Lung cancer in Brazil presented higher rates among men compared to women, and the South region showed the highest death risk in 1996 and 2011. Mortality showed a trend of reduction for males and increase for women. CONCLUSIONS Lung cancer in Brazil presented different distribution patterns according to sex, with higher rates among men and a reduction in the mortality trend for men and increase for women.


RESUMO OBJETIVO Descrever a tendência da mortalidade por câncer de pulmão no Brasil e regiões, antes e após as correções por sub-registro de óbitos, redistribuição de causas mal definidas e causas inespecíficas. MÉTODOS Foram utilizados dados de óbitos por câncer de pulmão da população de 30 a 69 anos, notificados ao Sistema de Informação sobre Mortalidade, entre 1996 e 2011, corrigidos para sub-registro de óbitos, declaração de sexo e idade ignorados e causas com códigos mal definidos e inespecíficos segundo sexo, idade e região. Foram calculadas taxas padronizadas por idade para dados brutos e corrigidos. Realizou-se análise da tendência temporal da mortalidade por câncer de pulmão por meio do modelo de regressão com erros autorregressivos. RESULTADOS O câncer de pulmão no Brasil apresentou taxas mais elevadas em homens que em mulheres e a região Sul foi a que apresentou maior risco de morte em 1996 e 2011. A mortalidade tendeu a reduzir para o sexo masculino e a aumentar para o sexo feminino. CONCLUSÕES O câncer de pulmão no Brasil apresenta padrão de distribuição diferente segundo sexo, com taxas mais elevadas em homens e com redução da tendência de mortalidade para o sexo masculino e aumento das taxas para o sexo feminino.
