ABSTRACT

BACKGROUND: Proper implementation of the logistics management information system (LMIS) would facilitate access to essential pharmaceutical products. It also prevents wastage at health posts. The aim of this study was to assess the implementation of the LMIS and the availability of tracer drugs at health posts in rural Ethiopia. METHODS: We employed a cross-sectional descriptive design with a mixed-method approach. The data used for this paper was collected from March to May 2019 as part of the National HEP assessment. The study involved 343 health posts randomly selected from nine regions of Ethiopia. Women's Development Army members and household heads participated in the qualitative study (i.e. in FGD and KII). The quantitative data were exported from Open Data Kit (ODK) to Stata 15.1 for statistical analysis, and the qualitative data were entered into NVivo 12 and analyzed using thematic content analysis. RESULTS: Of the health posts, 59.4% had a space for storing drugs; less than half (41.9%; 95% confidence interval (CI) [36%, 48%]) had a functioning refrigerator. The mean percentage of the availability of selected tracer drugs at health posts was 59.6%, with a 95% CI (58.9%, 60.3%). Bin cards were available at 43% (95% CI [40%, 46%]) of health posts, and among these, only 27.5% of the health posts adequately used the bin cards. CONCLUSION: The absence and poor use of LMIS tools was observed at health posts. Proper implementation of the LMIS has the potential to improve the availability of essential drugs that, in turn, improve health post level delivery of health services.

ABSTRACT

High-energy radiation derived from X-ray, γ-ray, neutrons, and other radioisotopes has been widely used for disease diagnosis and treatmentin clinical practice. Notably, high-energy radiation has been proven to increase the cure rate, prolong the survival time, and improve the quality of life among patients with malignant tumors. However, radiation poses huge threats to human health and life. Establishment of effective emergency management information systems for medical radiation is therefore of great significance to evaluate the radiation safety, predict the leakage of radioactive materials, and propose effective responses. This review summarizes the development and application of currently main emergency management information systems for medical radiation, so as to provide a reference for the establishment of sensitive and effective hospital-based radiation emergency management information systems.

ABSTRACT

Background: In 2005, the Government of India implemented the National Rural Health Mission for reduction of maternal mortality. One of the major impediments in improving maternal health since then has been a poor management of the Health Management Information System (HMIS) at grass-roots level which could integrate data collection, processing, reporting, and use of information for necessary improvement of health services. Objective: The paper identifies the challenges in generating information for HMIS and its utilization for improvement of maternal health program in the tribal-dominated Jaleswar block in Odisha, India. It also aims to understand the nature and orientation of the HMIS data generated by the government for the year 2013–2014. Methods: The study is a cross-sectional type which used observation and interview methods. Primary data were gathered from health professionals to understand the challenges in generating information for HMIS and its utilization. Next, to understand the nature and orientation of HMIS, data pertaining to tribal block were analyzed. Results: The findings show that there are challenges in generation of quality data, capacity building of workforce, and monitoring of vulnerable tribal population. The discrepancies between HMIS data and field reality display the gap in formulation of policy and its implementation. Conclusion: The study unearths the existing politics of knowledge generation. This shows highly standardized procedures and information gathering by use of dominant biomedical concepts of maternal health with limited inclusion of local birthing conceptions and needs of vulnerable tribal pregnant women.

ABSTRACT

Background: Effective monitoring and supervision of health care programs depend on complete, accurate, and timely flow of data. Mother and Child Tracking System (MCTS) is a centralized information technology (IT)-based application launched in 2009 for improving the delivery of maternal and child health care services through name-based tracking. There is minimal evidence in the literature evaluating the operational aspects of such a name-based tracking system even after 5 years of its implementation. Objective: The present study was thus conducted to understand the opportunities and challenges in the operationalization of MCTS strategy in a district in Haryana and to understand the stakeholder's perspectives. Materials and Methods: Performance of Routine Information System Management (PRISM) framework was used. This cross-sectional study was conducted in Shahzadpur block of Ambala district, Haryana, India involving in-depth interviews of health care providers and clients in 12 subcenters (SCs) and two primary health centers (PHCs). Results: Lack of appropriate training, overburdened data entry operator (DEO) and auxiliary nurse midwife (ANM), poor Internet connectivity, slow server speed, and frequent power failures were revealed as major limitations for the effective implementation of MCTS. Nearly 18% of the clients reported receiving short message service (SMS) and only 6% could understand the SMS. Conclusion: MCTS has led to accountability and improved supervision of health workers, apart from empowering the community.

ABSTRACT

OBJECTIVE: The purpose of this study was to evaluate the effectiveness of a web-based intervention program for depressive symptoms management among Korean adults. METHODS: After the first author developed a user-centered design website intended to manage the depressive symptoms of the general public, two hundred and seventy two participants were screened and randomly assigned into experimental and control groups. These subjects underwent a 12-week nonequivalent pretest and post.test evaluation program to determine changes in their depression scores, which were measured using the Korean Depression Scale. Subjects' unique IP addresses were used for monitoring their access periods on the website. RESULTS: There were no significant differences in the baseline and outcome characteristics of both experimental and control groups. Moreover, findings revealed that the total depression score of the experimental group (who accessed the developed web-site three times a week with at least an hour per session) showed to have decreased immensely with a mean of 5.57. On the contrary, only a 0.09 point difference resulted between the pre and post tests of the control group (who accessed other depression management websites generally available). CONCLUSIONS: Web-based interventions could be an effective and inexpensive means of managing depressive symptoms of Koreans. Findings from this effectiveness study of an internet-based program could also be used by a variety of audiences including government agencies, educators, and health care professionals concerned with mental health promotion and prevention.

ABSTRACT

OBJECTIVE: Using the integrated database in hospital information systems, this study tried to analyze inpatient hospital fees for stroke patients and evaluate the impact of patients' demographic and clinical characteristics on the fees. METHODS: Inpatient charge data from patient management information system were merged to each patient's clinical data from electronic medical record. Demographic, clinical, health service utilization, and hospital fee data of 540 stroke patients were collected in the years 2000 and 2001. Inpatient charges were compared according to patient characteristics; in addition, the impact of those characteristics on hospital charges was analyzed using a multiple regression model. RESULTS: The average length of stay (LOS) was 19.6 days and the total inpatient charge was USD 4 418. Inpatient charges were significantly higher in patients with previous stroke history, diabetes, hemorrhagic type, and severity, respectively. Simultaneously, LOS, ICU use, surgical operations, stroke type, hopeless discharge, lesion location, echocardiography and hypertension were significant factors influencing patient hospital charges (R(2)=0.847). LOS was the most significant factor explaining 81% of variance accounted for the charges. The strategies of reducing LOS need to be developed. CONCLUSION: Hospital information systems and their databases play an important role in a broad spectrum of research in clinical medicine, and can be useful to support retrospective studies and prospective clinical trials. Active utilization of the integrated database in hospital information systems is suggested for knowledge extraction.
