ABSTRACT

Se presenta el caso clínico de un paciente de 21 años de edad, quien ingresa en el Servicio de Cardiología del Hospital Provincial Docente Clinicoquirúrgico Saturnino Lora Torres por presentar pericarditis aguda de evolución tórpida. Al realizarle la tomografía axial computarizada del mediastino se diagnosticó un teratoma asociado a derrame pleuropericárdico severo por ruptura de ambas estructuras. Fue intervenido con circulación extracorpórea y se confirmó el diagnóstico histológico


The case report of a 21 years patient is presented who is admitted in the Cardiology Service of Saturnino Lora Torres Teaching Clinical Surgical Provincial Hospital due to acute pericarditis of torpid course. When the computerized axial tomography of the mediastinal cavity was carried out, a teratoma associated with severe pleural pericardial stroke due to rupture of both structures was diagnosed. He was treated with extracorporeal circulation and the histologic diagnosis was confirmed

ABSTRACT

Paciente de 57 años, fumador, antecedentes de salud que acudió a consulta por presentar tos seca de 3 meses de evolución. Se le diagnosticó unteratoma mediastinal, tumor poco frecuente en esa edad. En la radiografía de tórax se observó una masa mediastinal anterior que se confirmó con la tomografía computarizada y donde la localización y las características imaginológicas sugirieron el diagnóstico. Se realizó esternotomía media con exéresis del tumor, confirmando por anatomía patológica la presencia de un teratoma quístico maduro en el mediastino(AU)


A 57 year old male smoking patient, with healthy history presented to the clinic for having dry cough for 3 months of evolution. Mediastinal teratoma was diagnosed, a rare tumor at that age. An anterior mediastinal mass was observed on the chest X-ray, which was confirmed by computed tomography. The location and imaging characteristics suggested the diagnosis as well. A median sternotomy was performed with tumor excision, the pathological anatomy confirmed the presence of a mature cystic teratoma in the mediastinum(AU)

ABSTRACT

Mediastinal teratoma is an infrequent germ cell tumour and comprises of 1 to 5% of all mediastinal tumours. We report a case of mediastinal mature teratoma in a 12 year old boy who presented to us with persistent non-productive cough, fever and dyspnoea for the past 7 months. Computed tomographic scan of thorax revealed a large anterior mediastinal mass measuring 11.2x9.9x14cm with calcification within. He subsequently underwent a median sternotomy with left subcostal extension (L-incision) and excision of tumour. Histopathology of the tumour revealed a mature cystic teratoma. We would like to report a case of successful surgical management of a large mediastinal mature teratoma in a child.

ABSTRACT

This case describes a contiguous mediastinal and retroperitoneal mature teratoma in a congenital diaphragmatic defect, a combination that is hitherto unreported in literature. It substantiates embryological chronology of events during the cephalad migration of the primordial germ cells through the developing diaphragm, prior to its closure.

ABSTRACT

Patients with mediastinal teratoma are usually asymptomatic, but may develop symptoms by rupture into adjacent structures which result in pneumonia, hemoptysis, pleural effusion, pericardial effusion, or pneumothorax. Rarely, life-threatening acute respiratory distress require a emergenc y surgery. Rupture into pleural cavity may result in pleuritis and pleural effusion with severe anterior chest or back pain. The symptom must be differentiated from other common intrathoracic distress diseases. Clinical, cytologic and radiologic examinatio ns of pleural effusion, and moreover, measurement of enzymes such as amylase or insulin, which is secreted from pancreatic tissues, in pleural effusion and cystic fluid enabled us to make the diagnosis of rupture of mediastinal teratoma preoperatively.
