ABSTRACT

Objective:To understand the medical experience of patients with early lung cancer in pre-hospitalization, find out the problems and shortcomings in the service process, and provide the basis for improving the quality of medical service under the pre-hospitalization mode.Methods:Through phenomenological research method, 21 pre-hospitalized patients with early lung cancer from September to October 2020 in the First Hospital Affiliated to Guangzhou Medical University were selected by purpose sampling method for semi-structured interviews, and the research data were analyzed by Colaizzi seven-step method.Results:There were four themes in the medical experience of early lung cancer pre-hospitalized patients: the turnover rate of beds was fast, the length of stay and cost of stay were reduced, the inclusion criteria of pre-hospitalization lacked the humanization, admission and examination procedures needed to be optimized and insufficient doctor-patient communication during pre-hospitalization.Conclusions:The implementation of pre-hospitalization mode can make patients′ medical treatment more convenient and efficient, and reduce hospitalization expenses. However, there are still some shortcomings in the management of pre-hospitalization of early lung cancer patients. The pre-hospitalization process should be further standardized, and the communication and management of pre-hospitalization patients during waiting period for out-of-hospital examination should be strengthened.

ABSTRACT

Due to its own internal laws of development, Chinese medicine (CM) seems more inclined to empirical medicine in a relatively long historical period. It is considered to be lacking objective and unified clinical practice guidelines (CPGs), and the difficulties in diagnosis and therapeutic effect evaluation comes with it, have restricted its further inheritance, development and international communication. Over the years, our research group has been committed to improving the standardization theory and methodology of CM, also perfecting relative techniques for further application, which are all based on the stratified evidence scoring method. We have already applied this method to 45 issued guidelines, including 5 national guidelines, 3 industrial guidelines, and 37 formulation/revision social organization guidelines. The stratified evidence scoring method has been recognized and used widely. It helps scholars and applicators to study, formulate, publish and popularize the acupuncture therapy clinical practice guidelines better, thus further promotes the development of acupuncture therapy.

ABSTRACT

BACKGROUND: The purpose of this study is to elucidate the context of medical experience and the perception of unmet healthcare of elderly people with chronic diseases based on in-depth interview data. METHODS: We carried out in-depth interviews with 10 elderly people with chronic diseases using semi-structured questionnaires based on literature review. The in-depth interview data were analyzed using thematic analysis; one qualitative research methodology, three core meaning categories, and four attributes associated with unmet healthcare were ultimately derived. RESULTS: The context of the medical experience were based on the following three categories: (1) discomfort due to diseases and high medical needs, (2) the poor community medical environment and difficulties in accessing to metropolitan medical institutions, and (3) inconvenience caused by long waiting time and side effects of medicine. In addition, the elderly with chronic disease realized the unmet healthcare as (1) the availability related to the desired medical institutions at the right time, (2) the affordability related to their economic capacity, (3) the effectiveness of the medical services they experienced, and (4) the appropriateness related to receiving medical services in a pleasant environment. CONCLUSION: The perception of unmet healthcare among the elderly with chronic disease is the result of interaction of multi-level and multi-dimensional factors related with their medical experience.

ABSTRACT

Objective: To research a intelligent mobile APP information platform is used to carry out remote diagnosis and treatment services for the patients with diabetes so as to improve the medical experience and treatment compliance of the patients with diabetes. Methods: The patient's physiological information data that were obtained from home wearable medical devices were collected through the Bluetooth technique by using wireless mobile network. And the APP system was used to carry out remote online multi-channel communication between doctor and patient, and was applied to establish relevant health records. Results: The control rate of blood sugar, blood pressure, blood lipid and other indicators of patients with diabetics have been increased from 33% to 70%, and the filing rate of health records of patients also has been increased from 29% to 85%. Conclusion: The application of multi-channel multimedia interconnect platform and the integration of health data of patients with diabetes can ensure patients to complete treatment in least process, and it can provided convenient communication in real time for doctor and patients. And it also can greatly enhance the efficiency for the management of diabetes, improve satisfaction of patient, reduce the risk of hypoglycemia and the complications of diabetes.

ABSTRACT

Objective To explore the effect of psychological counseling skills on medical treatment and satisfaction degree of patients in hospital.Methods Toally 60 discharged patients during December 2015 were assigned as the control group and another discharged one during January 2016 as the intervention group.Training on psychological counseling skills was done for the nurses by the psychological counselor in the intervention group while the control group was not.The two groups were compared on the basis of investigation with the patient's consultation experience scale as well as the medical satisfaction questionnaire.Result The satisfaction levels of consultation experience and medical consultation satisfaction in the intervention group were both higher than those of the control group (all P<0.01).Conclusion Psychological counseling skills used in the nurse patient communication can enhance the patient's medical experience and their satisfaction with medical treatment.
