ABSTRACT

Objective:To review the characteristics and coping strategies of the rescue and landing medical support mission of Shenzhou-14 manned spacecraft.Methods:The characteristics of rescue and landing medical support mission of Shenzhou-14 manned spacecraft was analyzed, and the coping strategies and experience were discussed.Results:(1) The characteristics of rescue and landing medical support mission of Shenzhou-14 manned spacecraft included: long time in space station and high-intensity space missions; high-density space medical support mission in short term; special environmental factors in severe cold night; complex terrain of landing site; and the young medical support team. (2) The main coping strategies of rescue and landing medical support mission of Shenzhou-14 manned spacecraft included: strengthened the organization and leadership and improved the training model; reinforcement learning the medical treatment plan and strengthened the medical rescue skills training; optimized the carrying equipment and added the heat preservation and lighting measures; improved the medical rescue process and perfected the emergency plan; emphasized on the scientific research as important as mission; and strengthened the physical exercise and cold resistance exercise.Conclusions:The characteristics and coping strategies of rescue and landing medical support mission of Shenzhou-14 manned spacecraft are summarized to provide the experience for space medical rescue and offer the support for China's manned space industry.

ABSTRACT

Objective:To summarize the experience of ambulance medical support in the 19th Hangzhou Asian Games, and provide reference for the ambulance medical support work of large-scale sports events in the future.Methods:According to the actual situation of the Asian Games, the objectives of the medical support were determined according to the principles of highlighting the key points and overall consideration. All kinds of ambulances were allocated according to the principles of combining dynamic and static and regional linkage. The ambulance support was divided into the event side and the city side,with the regional security vehicle as the supplement of the event side and the guarantee of the city side, the implementation of "one pavilion one policy", "one game one policy", "one place one policy" and "one vehicle one plan", and the interconnection of medical points (rooms), designated hospitals, management and command personnel and ambulances were realized through the Asian Games Intelligent Emergency Medical Support Syetem (EMSS).Results:Between September 16, 2023, 08:00, and October 9, 2023, 08:00, a total of 207 ambulances were deployed during the 19th Hangzhou Asian Games. Of these, 88 (42.5%) were at competition venues, 11 (5.3%) at training venues, 30 (14.5%) at non-competition venues, and 78 (37.7%) were regional support vehicles. Among them, 20 venue ambulances participated in regional support tasks during idle times. Ambulances transported patients from event sites 345 times, with the highest number of transports occurring at competition venues (187 times) and the fewest in the assisting city of Huzhou (6 times). The highest ambulance utilization rate was 70.5% at competition venues, while training venues, non-competition venues, and regional support ambulances had utilization rates of 45.5%, 53.3%, and 42.3%, respectively. The median age of transported individuals was 26 (21, 36), with a majority being male (59.1%). Athletes comprised 52.5% of transports, while staff and spectators accounted for 39.7% and 7.8%, respectively. Joint injuries were the most common condition among transported individuals, totaling 58 cases (16.8%). Regional support ambulances participated in 75 transfers and replacements, with 17 (22.7%) transfers and 58 (77.3%) replacements.Conclusions:This study confirms the feasibility and efficiency of the Asian Games' ambulance support plan, highlighting the crucial role of EMSS. The study suggests the need to strengthen medical support for staff, in addition to athletes.

ABSTRACT

Objective:To construct an intelligent medical emergency information platform for the 19th Hangzhou Asian Games events and evaluate its practical application.Methods:Addressing the medical support demands for Asian Games, based on the " Emergency medical service system for large-scale social activities" developed by Zhejiang Laboratory, a collaborative team comprising Hangzhou Emergency Center, the Second Affiliated Hospital of Zhejiang University,Zhejiang Lab and College of Biomedical Engineering and Instrument Science of ZhejiangUniversity developed an intelligent medical emergency information platform for the Asian Games. This system was implemented during the 19th Hangzhou Asian Games, collecting basic information, initial diagnosis locations, prognoses, severity grading, and occurrences of infectious diseases for all patients treated at support locations from September 16th to October 8th, 2023. The Modified Early Warning Score (MEWS) was employed for assessing patient conditions, and a symptom monitoring system was used to alert the occurrence of infectious diseases. Post-event, a satisfaction evaluation (on a 5-point scale) regarding the system's application was conducted.Results:The successful construction of the intelligent medical emergency information platform comprised a data dashboard display platform, a comprehensive command system on the management center, and a mobile platform covering on-site medical support, ambulances, and designated hospitals. ①A total of 11 068 patients were treated using this system, including 2 979 athletes, 3 429 staff members, 1 421 volunteers,1 051 spectators, and 2 188 others. The proportion of patients seeking treatment due to illnesses was significantly higher than those due to injuries. Among injury-related cases, athletes constituted 41.37%, while staff accounted for 32.5% of illness-related cases. ②The area under the ROC curve for MEWS regarding hospitalization rates was 0.81 for disease patients and 0.60 for injury patients. ③ There were 211 reported cases of infectious diseases during the event, constituting 1.91% of all treated cases.④ Satisfaction ratings: User-friendliness scored 4.47, completeness of functions scored 4.28, data security scored 4.38, and data accuracy scored 4.49. Compared to traditional paper-based reporting methods, 98.11% of users found the system more convenient, 92.45% were satisfied overall, and the recommendation rating was 4.36.Conclusions:The study findings confirm the critical role played by the information platform in enhancing comprehensive, accurate, timely, secure medical support information dissemination for the Asian Games and in early warning and control of infectious diseases.

ABSTRACT

Objective:To discuss the medical support strategy by summarizing the experience of medical support in Dongfeng landing site.Methods:According to the current situation of medical support at Dongfeng landing site and the practice and experience of previous medical support, a retrospective summary study was conducted.Results:The main methods were summarized: (1) formulate a feasible plan; (2) strictly screen skilled personnel; (3) provide reasonable medicinal materials and equipments; (4) complete the modification of the rescue carrier; (5) attach importance to the training of first aid techniques; (6) strengthen political and ideological education; (7) carry out physical fitness training; (8) seriously participate in actual combat training. Three points of experience were put forward: (1) the importance of thinking and action is the premise of completing the task; (2) the cooperation between training and actual combat is the basis for completing the task; (3) equal emphasis on education and management is the bottom line for completing the task.Conclusions:The medical support experience of the Dongfeng landing site is summarized to provide support and experience for the long-term in-orbit operation of China's space station and on-site medical rescue after the landing of the manned spacecraft.

ABSTRACT

This study focused on the socialist camp's North Korean medical support and its effects on North Korean medical field from liberation to 1958. Except for the Soviet assistance from liberation to the Korean War, existing studies mainly have paid attention to the ‘autonomous’ growth of the North Korean medical field. The studies on the medical support of the Eastern European countries during the Korean War have only focused on one-sided support and neglected the interactions with the North Korean medical field. Failing in utilizing the materials produced in North Korea has led to the omission of detailed circumstances of providing support. Since the review of China's support and the North Korea-China medical exchanges has been concentrated in the period after the mid-1950s, the impacts of China's medical support on North Korea during the Korean War period and the post-war recovery period have not been taken into account. In terms of these limitations, this study examined the medical activities by the Socialist camp of the Eastern European countries in North Korea after the Korean War. The medical aid teams from Hungary, Romania, Bulgaria, Czechoslovakia, Poland, and East Germany that came to North Korea in the wake of the Korean War continued to stay in North Korea after the war to build hospitals and train medical personnel. In the hospitals operated by these countries, cooperative medical care with North Korean medical personnel and medical technology education were conducted. Moreover, medical teams from each country in North Korea held seminars and conferences and exchanged knowledge with the North Korean medical field staffs. These activities by the Socialist countries in North Korea provided the North Korean medical personnel with the opportunity to directly experience the medical technology of each country. China's support was crucial to North Korea's ‘rediscovery’ of Korean medicine in the mid-1950s. After the Korean War, North Korea began to apply the Chinese-Western medicine integration policy, which was performed in China at that time, to the North Korean health care field through China's medical support and exchanges. In other words, China's emphasis on Chinese medicine and the integration of the Chinese-Western medicine were presented as one of the directions for medical development of North Korea in the 1950s, and the experiences of China in this process convinced North Korea that Korean medicine policy was appropriate. The decision-makers of the North Korean medical policies, who returned to North Korea after studying abroad in China at that time, actively introduced the experiences from China and constantly sought to learn about them. This study identified that a variety of external stimuli had complex impacts on the North Korean medical field in the gap between ‘Soviet learning’ in the late 1940s and the ‘autonomous’ medical development since the 1960s. The North Korean medical field was formed not by the unilateral or dominant influences of a single nation but by the stimulation from many nations and the various interactions in the process.

ABSTRACT

Objective To investigate the status of empowerment and medical support in patients with type 2 diabetes mellitus,and to explore the relationship between empowerment and medical support.Methods A multi-centered collaboration and cross-sectional survey was used in this study.With the method of simple random sampling,1512 patients with type 2 diabetes mellitus from 18 tertiary hospitals in Hunan Province were investigated from January 2016 to April 2016.General information,empowerment and medical support were investigated using general information questionnaire,Diabetes Empowerment Scale-DAWN Short Form,Patient Assessment of Chronic Illness Care DAWN Short Form.Results The mean score of empowerment ability for patients with type 2 diabetes mellitus was (53.20±16.40).The mean score of medical support for patients with type 2 diabetes mellitus was (34.56±11.64).There was positive correlation between the empowerment ability and medical support in patients with type 2 diabetes mellitus.Conclusion There is positive correlation between empowerment and medical support in patients with type 2 diabetes mellitus.But the received medical support is not enough.It is necessary to increase the supply of medical support to promote the empowerment of patients with type 2 diabetes.

ABSTRACT

Objective To understand the demands of elderlies in a community in Hangzhou for the elder care with medical support and the current supply, and analyze the influencing factors. Methods A questionnaire survey was conducted among 1 416 elderlies at 12 communities of 3 neighborhoods in Jianggan district of Hangzhou city, using stratification sampling. Results For the elderlies with demands for medical care, such services as medical advice in community, pharmacy in community and annual physical examination were made available and the highest satisfactions, being 70. 4%, 67. 1% and 64. 0% respectively. In contrary, less than 30% of the rest eight medical care demands were met. For the elderlies with demands for nursing care in community, supply of nine elderly care services fell below 30%. For the elderlies with demands for rehabilitation services, supply of six community rehabilitation services fell below 15%. Age, self-care ability and the number of chronic diseases were the main factors affecting old people′s demand for medical-nursing combined care(P <0. 05). Conclusions A gap is found between elderlies′demands for elderly care with medical support and the services available. Medical care, nursing care and rehabilitation service deserve further development efforts. Those of advanced age, with declining self-care ability, and those afflicted with many diseases should attract major attention.

ABSTRACT

The huge shortage of nursing assistants in the industry, has made it imperative and feasible to build and develop a team of nursing assistants. This paper studied the present situation and problems of the nursing assistants in China, naming such setbacks as the personnel shortage, low competence, and poor professionalism. In comparison, overseas experiences in teambuilding were introduced with recommendations raised for China. These recommendations include higher social approval of elderly care industry, innovative development mechanism of nursing assistants, appraisal mechanism for nursing assistants performance, and better fund raising mechanisms for their incentives and training.

ABSTRACT

The authors analyzed the opportunities and challenges faced by nurses in the time of elderly care with medical support using SWOT analysis, for the purpose of suggestions for the development of nurses to cope with such new scenario. Advantages of the nurses in such a time are as follows: professional nursing care, holistic care ability, and standardized assessment skill. Their disadvantages include: poor team leadership, insufficient business and management skills, and poor motivation for personal development. Their opportunities include: guidance of the national strategy, innovation of " Internet plus", and flexible working hours. Their challenges include: room of improvements for both quantity and quality of nursing personnel, better efficiency for integration and allocation of resources, and better capability to leverage" Smart elderly care". Four suggestions are raised in this regard, i. e., to strengthen the ability for elderly care with medical support, to improve multidisciplinary coordination, to cultivate their entrepreneurship, and to improving the ability of information application.

Objective To analyze the internal and external factors of the portable integrated remote medical examination chest during military medical support to facilitate telemedicine service in the military hospital.Methods The self-developed chest had its advantages, disadvantages, opportunities and threatening factors analyzed qualitatively with SWOT method. Results The chest had advantages in meeting military and practical requirements as well as fulfilling military-civilian integration, disadvantages in deficiency of specialized mobile network, specific signs acquisition devices and mature operation mechanism, opportunities in national health program, military development and mobile internet as well as the threatening factors in non-unified interface standard,untimely policy issuing and unclear definition of responsibility,rights and interests.Conclusion Considerations have to be taken on internet application and policy institution to facilitate military telemedicine support.

Results The system shared casualty treatment data with servicemen electronic tag and medical commanding information with logistics integrated command and control platform,so that the requirements for medical information were met in the institution.Conclusion The system improves the field medical support institution in medical support ability,treatment rate and efficacy.

Objective To design a whole process management information system of the field medical equipment.Methods According to the problems found in the management of medical equipment, the functional requirements of different user systems were determined.The system was developed with combined Java and C languages,Eclipse and Visual Studio 2012, and was connected into military integrated information network according to security requirements.Results By applying the whole process management system of the field medical equipment, all process data could be managed from purchasing, warehousing,distribution,use,maintenance,R&D and so on.Conclusion By applying this system,the management of the field medical equipment is improved, functional requirements are met of all levels of health equipment management, the platform for data sharing among relevant departments is set up,and the static mode is transformed into a real-time,dynamic and whole process one for field medical equipment management.

Object To quantitatively analyze the rationality of medical support process and resource allocation on a certain model of ship.Methods Based on the queuing theory, the simulation model was built and analyzed by using simulation software. The medical system operational effectiveness was investigated through process parameters, such as the wounded waiting time, the resource utilization of major equipment.Results The simulation results showed that the design scheme of medical system was reasonable.Conclusion The quantitative process parameters of the system operating efficiency can be obtained at the design stage by using simulation analysis technology, which is of great practical value for the rationality of medical system design on ship.

Object To execute medical information informatized acquisition, transmission, storage and procession to facilitate the submarine medical rescue team to implement medical treatment on the rescue ship.Methods A submarine rescue medical information system based on information and material flows was developed with PowerBuilder 11.5 platform, C/S architecture and Oracle 11g database, which was composed of six modules of personnel management, equipment management, drug and consumables management, examination and evaluation, medical treatment and system management. In medical treatment module information sharing and real-time communication were implemented between the groups of casualty triage, casualty holding, surgical treatment and severe injury treatment.Results The system realized information acquisition, procession, application and integrated sharing during mass casualty treatment on the rescue ship.Conclusion The system enhances the military hospital in medical support to multi military missions, medical commanding and comprehensive treatment.

Objective To develop a clinical laboratory vehicle with advantages in mobility,deployment,automation and informatization to meet the requirements of disaster relief.Methods Ford transit V348 ambulance was used as the base,which was equipped with routine blood analyzer,routine urine analyzer,biochemical analyzer and etc.The same instruments,agents and procedures were adopted as those in the clinical laboratory department of rear hospital,and LIS,air cleaning system and necessary auxiliary units were also involved in the modification.Results The clinical laboratory vehicle increased the working efficiency while decreased the errors.Conclusion The vehicle fulfills mobile medical support for military operations other than war such as disaster relief,and thus is worthy promoting practically.

ABSTRACT

The particularity and complexity of medical support in the United Nations peacekeeping operations bring various ethical problems to medical staff of the second-grade peacekeeping hospitals during their work.This paper analyzed the ethical dilemma in terms of the relevant provisions of the United Nations,the medical humanitarianism principle,lack of medical resources and the principle of fairness,emergency treatment and informed consent,cultural differences and the principle of optimal health,occupational protection requirements and respect to the human rights.In addition,it discussed the countermeasures from personnel selection,the laws and regulations,the training of medical norms and foreign customs,the concept of “patient-centered”,and the humanitarian treatment,providing a theoretical basis for solving the problems of medical support in peacekeeping operations.
