ABSTRACT

OBJECTIVE To analyze the effect of pharmaceutical practice of pediatric pharmacists providing internet medication consultation services. METHODS A total of 8 356 children’s medication cases through the “Our Pharmacists” online medication consultation platform (hereinafter referred to as “Our Pharmacists”) were collected from July 2020 to June 2022. The general information, disease type and irrational drug use were analyzed statistically. RESULTS Among 8 356 cases, 4 234 were male and 4 122 were female; the age of children was mainly from 12 months to 3 years old; the consulting diseases were mainly respiratory diseases (46.56%), followed by digestive system diseases (18.41%) and skin system diseases (12.71%). Among respiratory diseases, the proportion of irrational drug use was 92.30%, mainly including the use of antibiotics without indication (83.59%), inappropriate selection of Chinese patent medicines (61.14%) and compound cold medicines (49.77%), etc. Among digestive system diseases, the proportion of irrational drug use was 71.15%, mainly including not using oral rehydration salts (48.85%), inappropriate use of probiotics (31.19%), the use of antibiotics without indication (26.89%), etc. A total of 4 273 evaluations of pharmacist services were received from child guardians, with a five-star rating of 99.88%. CONCLUSIONS Pharmacists provide pediatric medication consultation services through “Our Pharmacists”, providing individualized evidence-based medication suggestions, increasing the safety of drug use and satisfaction with pharmaceutical care, and ensuring the safety of children medication.

ABSTRACT

OBJECTIVE: To summarize the practice of medication consultation service based on the platform of “Our Pharmacist”, and to provide reference for pharmacists participating in pharmaceutical care. METHODS: The 502 medication consultation service for children, pregnant women and lactating women were collected from the platform of “Our Pharmacist” during Oct. 2017-Jul. 2018. General information, main medication, types of drugs, types of questions, types of irrational drug use were analyzed statistically. The practice of rational drug use patient guidance service based on evidence-based concept was introduced with typical examples. RESULTS: Among 502 cases of consultation, 147 (29.3%) were males and 355 (70.7%) were females, and the age was mainly 20-40 years old (291 case, 58.0%). The proportions of patients taking 1-2, 3-4, 5 types or more drugs were 45.4%, 41.4% and 13.2%, respectively, and those who took 5 or more drugs at the same time were all children’s patients. In respect of medication consultation content, children medication consultation, pregnant medication consultation and lactating medication consultation accounted for 42.0%, 40.6%, 17.4%, respectively. Antibiotics, Chinese patent medicines and antivirals accounted for the largest proportion of consulting drugs, being 25.6%, 17.8%, 12.1%, respectively. The question types of medication consultation included indication, dosage and course of treatment, drug safety and ADR, accounting for 31.9%, 17.5%, 13.6%, respectively. The types of irrational drug use mainly included improper drug selection, repeated drug use and no indication, accounting for 44.1%, 21.0%, 14.9%, respectively. In typical cases, pharmacists conducted systematic evidence inquiry and answered patient consultation strictly in accordance with the evidence-based concept. The main evidence-based process included formulating retrieval strategies, retrieving relevant literature, strictly evaluating evidence (authenticity, reliability and adaptability), screening evidence according to its evidence level from high to low, evaluating evidence comprehensively and applying it to practice. Medication guidance information provided by pharmacists based on evidence not only corrected the irrational drug use of patients, but also received good feedback from patients. Five-star score accounts for 95.5% of the satisfaction evaluation. CONCLUSIONS: Based on evidence-based concept, the platform of “Our Pharmacist” provides medication consultation service, which provides references for pharmacists to carry out targeted pharmaceutical care.

ABSTRACT

OBJECTIVE: To improve the quality and efficiency of medication consultation and to promote rational drug use in clinic. METHODS: Totally of 520 cases of medication consultation records from the Affiliated Yueyang Hospital of Integrated Traditional Chinese and Western Medicine, Shanghai University of TCM during Oct. 2015-Oct. 2016 were selected according to random number table, and then summarized and analyzed statistically in respects of consultation content, age and type of consultants, department, consulted medicine types, the number of combination of TCM and Western medicine, satisfactory degree of consultants, etc. RESULTS: The main consultation contents included usage and dosage (41. 92%), combination of TCM and Western medicine (12. 88%), etc. Consultants mainly aged 50-70 years old (48. 27%), and were mainly the patients and their family members (95. 96%). Consultants were primarily diagnosed in cardiology department and endocrinology department. Top 3 consulted medicine types were cardiovascular system medicines, nervous system medicines, digestive system medicines, and types of Chinese patent medicines were mainly coordinating medicines and activating blood and removing stasis medicines. Types of TCM combined with Western medicine were less than or equal to 3 kinds in 420 cases (80. 77%). Most patients were very satisfied with outpatient medication consultation (93. 65%). CONCLUSIONS: Rational drug use consultation improves medication compliance of patients, meets the needs of patients on medication safety knowledge, and provides a strong guarantee for safe and rational drug use in clinic.

ABSTRACT

OBJECTIVE:To provide reference for medication consultation Wechat platform in the hospital. METHODS:The medication consultation Wechat platform of our hospital was introduced,including main method,content and effect. The consulting pharmacist team was set up,and Wechat public number was applied and operated experimentally;information pharmacist was re-sponsible for its daily maintenance. We released pharmacy news,accumulated consumers and expanded influence through other me-dia at regular intervals. RESULTS & CONCLUSIONS:According to the medical practice,two modules“medication consultation”and“pharmacy news”were set up. From Dec. 2014 to Jul. 30th 2015,medication consultation system had accumulated 1 121 con-sumers,with an increase of 5 to 8 ones everyday;average number of consultation records was 10 everyday;we also had published 22 batch of medication news(79 messages),including 31 original messages and 3 video messages. Several messages gained much attention from network media and offline media. As new pharmaceutical care mode,Wechat platform contribute to realize a individ-ual,convenient and mobile medication consultation service,medication health science popularization,and improves medication compliance of patients and the awareness of medication safety in field of pharmaceutical administration Wechat platform,so as to improve pharmaceutical care effect of“taking patients as the center”and promote the propaganda of hospital brand. Thus it has a good prospect of application and popularization.

ABSTRACT

OBJECTIVE:To explore appropriate pharmaceutical care methods for hemophilia children,and to improve rational drug use. METHODS:Drug consultation hemophilia records collected from our hospital during Nov. 2014 to Jul. 2015 were summa-rized in respects of personal structure,form and content;key cases were analyzed specially. RESULTS:During Nov. 2014-Jul. 2015,clinical pharmacists received 130 times/cases of medication consultation that related to hemophilia in our hospital;patient’s family members occupied the main part (70.0%);most of drug consultation form was information platform for consultation (58.5%);consultation content mainly covered drug combination and auxiliary medication (77.7%). Clinical pharmacists have re-duced the risk of bleeding,improved the quality of life,reduced the economic burden of families and strengthened the awareness of medical staff and patient’s family member on safe drug use through finding out medication error timely,paying attention to the component of compound drug,guiding medication direction and optimizing therapy plan. CONCLUSIONS:Clinical pharmacists ac-tively provide pharmaceutical care and promote rational drug use during hemophilia treatment from medication consultation,ratio-nal drug use,etc.

ABSTRACT

OBJECTIVE:To discuss the effect of performance management in medication consultation service in hospital,and to improve the service quality in medication consultation center. METHODS:The performance management mode of medication consultation center was implemented in our hospital through formulating consultation pharmacist performance evaluation content and accounting rules from four dimensions of business skills,cultural literacy,work attitude,workload,conducting daily evaluation and check,developing training,setting development plan,etc. The effects of the mode were evaluated. RESULTS:In performance evaluation,business skills,cultural literacy,work attitude and workload took up 20,15,15 and 50 points,respectively;they were subdivided into 16 specific items and extra point items. After the implementation of above performance management,average weekly clients increased from 104 persons to 247 persons (increasing by 138%);the proportion of patient medication education mainly provided by clinical pharmacists increased from 18.30% to 46.34%(increasing by 153%);the complex and professional problems of medication consultation such as drug interaction rose from 0.96% to 5.28%;the problem of drug usage and dosage in-creased from 6.73% to 23.17%. The performance management mode not only improved the service consciousness and the enthusi-asm of consultation pharmacists,but also improved the level of pharmaceutical care. CONCLUSIONS:Reasonable and effective performance management mode can promote the service quality and ability of medication consultation center,and play more impor-tant role in hospital pharmaceutical care.

ABSTRACT

OBJECTIVE:To improve the quality of drug consultation.METHODS:The drug consultation records of a total of 1 663 cases in our hospital from July 2004 to July 2007 were analyzed statistically by a retrospective method.RESULTS:Of the drug consultation records of a total of 1 663 cases,the chief questions consulted were adverse drug reactions and long-term medication safety(25.2%),dosage and administration(21.1%),medication for special populations(9.7%),drug interactions(8.1%),pharmacological action and indications(7.1%),and generic name and composition of drugs(5.2%).CONCLUSION:The drug consultation service in our hospital achieved a remarkable efficacy in guiding physicians and pharmacists on rational drug use;however,it has its problems and remains to be improved further.
