ABSTRACT

The Rationale: “Frozen Eye” is a very uncommon sequel after treatment of orbital blowout fractures requiring implant placement. Patient Concerns: The implant may faultily impinge on the ocular and extra?ocular muscle(s), causing the abnormality in the movement of the eye. Diagnosis: We present a 56?year?old male whose ocular implant impinged on muscle, causing “frozen eye” and had an infected implant. Treatment Outcomes: The same was removed and surgically corrected. The manuscript describes the details and discusses the possible mechanism that led to the “Frozen Eye”

ABSTRACT

Objective@#To analyze the method and effect of reconstruction of the orbital floor defect with preserved orbital contents after advanced maxillary sinus cancer resection.@*Methods@#Thirty-three patients of T3/T4 stage maxillary sinus cancer with orbital invasion who were treated in Shandong Provincial Hospital from January 2010 to October 2016 were retrospectively analyzed, including 20 males and 13 females, aged from 12 to 80 years old. Patients were treated with appropriate surgical methods according to their clinical manifestation, imaging finding, invasion range, preoperative and intraoperative pathology. Surgical approaches such as maxillectomy, expanded maxillectomy and superstructure maxillectomy were used for patients with Medpor orbit floor repairment. Patients were followed up regularly after surgery, and the surgical efficacy was discussed by descriptive statistical method.@*Results@#Of those 33 cases, 19 were squamous cell carcinoma, 8 were adenoid cystic carcinoma, 5 were inverted papilloma malignant transformation, and 1 was myoepithelial carcinoma in our study. After tumor resection and orbital floor repair, the orbital contents and eyeball function of all patients remained intact. During follow-up time ranged from 3 to 8 years, a total of 6 patients relapsed at 3 years and died of ineffective treatment. The survival rate was 81.8% (27/33).@*Conclusion@#Tumor resection and Medpor orbital floor repair in maxillary sinus cancer patients with orbital invasion can preserve the function of the eyeball well, greatly improve the quality of patient′s life.

ABSTRACT

Objective: By comparing the mechanics of human auricular cartilage, polyurethane elastic material, and high density polyethylene material (Medpor), to produce theoretical proof on choosing optimal artificial auricular scaffold materials. Methods: The experimental materials were divided into 3 groups with 6 samples in each: the auricular cartilage group (group A), the polyurethane elastic material group (group B), and the Medpor group (group C). With an Instron5967 mechanical testing machine, compression and tensile testing were performed to respectively measure values of compression parameters (including yield stress, yield load, elastic modulus, yield compressibility, compressibility within 2 MPa, and compression stress within 10% strain) and values of tensile parameters (including yield stress, yield load, elastic modulus, yield elongation, elongation within 2 MPa, tensile stress within 1% strain) for comparison. Results: Compression testing: no obvious yield points were observed in the whole process in samples of group B, while obvious yield points were observed in samples of groups A and C. There was no significant difference between groups A and C with respect to yield stress and yield load ( P>0.05); while the yield compressibility in group C was significantly lower than that in group A ( P0.05). Tensile testing: the materials in group B had extremely high tensile strength. The yield stress in groups A and B was significantly higher than that in group C ( P0.05). There was no significant difference with respect to yield load among the 3 groups ( P>0.05); but there was significant difference with respect to yield elongation among the 3 groups ( P0.05). Conclusion: Compared with the Medpor, the polyurethane elastic material is a more ideal artificial auricular scaffold material.

ABSTRACT

Objective:To discuss clinical practice,efficacy and surgical techniques for ear reconstruction with Medpor.Methods:Medpor ear reconstructions were used to treat 19 microtia in 17 patients (including 2 bilateral patients) in Xiangya Hospital,Central South University from January 2011 to December 2015.Reconstructions included 2 patients with one-stage ear reconstruction with temporoparietal fascial flap,2 patients with one-stage operation implanting Medpor directly into skin pocket,and 13 patients with two-stage ear reconstruction.Results:After 1-3 years of follow-up,16 microtia in 14 patients gained ideal appearance.Scaffold exposure occurred in 3 unilateral cases,among which one patient who underwent debridement,removal of superficial exposed scaffold and transposition of local flap to salvage exposure was not significantly influenced,and the other 2 patients' scaffolds were still exposed after repairs and finally removed.One stent was removed at the patient's urging because it induced an exacerbation of periarthritis of the right shoulder.Conclusion:Medpor ear scaffold has advantages,such as easy assembly,good immunologic compatibility,fast vascular ingrowth,simple operation,short operative time and ideal appearance of the reconstructed ear.Medpor is an alternative for microtia repair when patients are unwilling to use autologous rib cartilage or costal cartilages are calcified.However,the relatively high incidence of scaffold exposure reminds us that the indications of Medpor should be taken seriously,and measures to avoid skin necrosis and scaffold exposure should be implemented.Long-term follow-up efficacy needs to be proved.

ABSTRACT

Objective@#To explore the clinical effect by using expander implantation immediately to repair the Medpor exposure after ear reconstruction.@*Methods@#From May 2013 to February 2017, 5 patients were treated with ear reconstructive surgery by Medpor implantation. The previous methods contained not only the auricular posterior fascia flap but also skin grafting. Exposed parts were the upper part of the helix and the wounds were dry without purulent secretions. Bacterial culture did not show bacterial growth. Surgical procedure was to remove the Medpor implant and implant a 50 ml kidney expander. When the wound was healed, we started to inject normal saline into the expander. About 3 months later the total volume of normal saline in the expanders were 80-100 ml.Then we could begin the second operation. We removed the expander and harvested the right side of the 7th and 8th rib cartilage, which were carved into the ear cartilage framework and transplanted into the expansion area. The drainage tube, negative pressure and cotton pad appropriate pressure bandage were needed. 6 days later we removed drainage tube.10 days later we removed the suture.@*Results@#The reconstructed external ear shape in five cases were good. No infection occurred. With 6-20 months of follow-up using telephone and WeChat, no obvious deformation of the shape or cartilage exposure occurred. The five patients did not come back to do the third stage operation for financial considerations. They all left their hometown to work out to reduce economic pressure.@*Conclusions@#In the case of ear reconstruction for Medpor prosthesis exposure, if the wounds were not infected, removing the Medpor and transplanting an expander at the same operative stage can achieve a good therapeutic effect.

Objective@#To investigate the surgical treatment of orbital fractures and ocular traction deformities, and summarize the surgical results.@*Methods@#A retrospective study was conducted on 42 cases of monocular patients with orbital fractures and ocular fractures treated by Medpor Titan and Medpor. Preoperative CT scan and three-dimensional reconstruction were performed to evaluate the maxillofacial fractures, orbital wall defects, orbital content, shift incarceration. During surgery, the periorbital fracture and defect area were exposed, the periorbital shift bone were reduced, Medpor Titan was used to repair the orbital floor, Medpor to correct eyeball retraction. Follow-up observation of postoperative results and to assess the patient′s satisfaction rate, patient’s eyeballs, visual acuity and motor function.@*Results@#All patients had successful operation, the eye trapping deformity were significantly improved or corrected. Patient satisfaction rate was 92.9% (39/42); wound healingswere good, except for 1 infection (4 years after surgery), there was no rejection. Six months after the surgery, the ocular protrusion was reduced in 88.1% (37/42), the degree of diplopia was significantly improved in 90.5% (19/21) and the eye movement disorder corrected by 87.5% (14/16).@*Conclusions@#Medpor Titan combined with Medpor repair orbital wall fractures can achieve good results, with high satisfaction rate. Diplopia and eye movement dysfunction can be significantly improved.

@#AIM: To investigate the clinical effect of reconstruction of orbital blowout fracture by conjunctival incision combined with Medpor Titan implantation under nasal endoscopy. <p>METHODS: Sixteen patients(16 eyes)diagnosed with orbital blowout fracture, include medial fractures, floor fractures and extended fractures, were performed reconstruction by transconjunctival approach with implant material- Medpor Titan under nasal endoscopic-assisted; observation of postoperative visual acuity, eyeball protrusion, extraocular movement limitations, diplopia, orbital CT, occurrence of implanted material rejection was taken. <p>RESULTS: A 3-month follow-up was performed and the therapeutic efficacies of anatomic and functional recovery were evaluated. No further vision loss or infection occurred postoperatively. In the 16 patients, the average postoperative observation was 3mo, enophthalmos were fully corrected, diplopia disappeared. Orbital(CT)did not reveal implant displacement and rejection postoperatively. <p>CONCLUSION: Endoscopic-assisted reconstruction of orbital blowout fracture by conjunctival incision combined with Medpor Titan implantation is a safe and effective surgical method with the characteristics of with direct operation, clear range of fracture exposure and safe and reliable operation, not only restores the patient's visual function and appearance, but also reduces the incidence of complications, avoiding postoperative facial scar.

ABSTRACT

OBJECTIVES: The role of alloplastic materials in maxillofacial reconstruction is still controversial. Determining the utility of porous, high-density, polyethylene implants as a highly stable and flexible, porous alloplast, with properties such as rapid vascularization and tissue ingrowth, is crucial in cases of maxillofacial deformities and aesthetic surgery. MATERIALS AND METHODS: Thirty high-density porous polyethylene implants were implanted in 16 patients that had been referred to a private office over a three-year period. These implants were used for correcting congenital deformities, posttraumatic defects and improving the aesthetic in nasal, paranasal, malar, chin, mandibular angle, body and orbital areas. RESULTS: The outcomes of the cases in this study showed good aesthetic and functional results. The majority of patients had no signs of discomfort, rejection or exposure. Two implants suffered complications: a complicated malar implant was managed by antibiotic therapy, and an infected mandibular angle implant was removed despite antibiotic therapy. CONCLUSION: Based on the results, the Medpor implant seems to be an excellent biomaterial for correcting various facial deformities. Advantages include its versatility and relatively ideal pore size that allows for excellent soft tissue ingrowth and coverage. It is strong, flexible and easy to shape.

ABSTRACT

AIM: To investigate the effectiveness of minimally invasive conjunctivorhinostomy for complete bicanalicular obstruction, and to evaluate the advantages, complications, and degree of satisfaction after this technique. ●METHODS: Twenty - two patients ( 22 eyes ) with complete bicanalicular obstruction were selected as the research subjects, all patients were treated by minimally invasive conjunctivorhinostomy. The post - operation follow- up lasted for 3 - 27mo. Medical records were reviewed for demographic data, surgical time, length of the tubes, duration of follow-up, patency of the tube, postoperative complications, whether satisfied or dissatisfied with this procedure. Meanwhile, the data was analyzed for evaluating the clinical efficacy of minimally invasive conjunctivorhinostomy. ●RESULTS: The overall operative success rate was 95%(21/22), and the overall patient satisfaction was 68% (15/22 ) . The post - operation complications included:granulation tissue proliferation of the lower end of the tube 2 cases (9%), middle turbinate blocked the opening of tube 3 cases ( 14%) , nasal mucosa covering the opening of tube 1 case ( 5%) , the tube was blocked by secreta 5 cases ( 23%) , foreign body sensation 10 cases (45%), dry eye 2 cases ( 9%), regurgitation of nasal secreta discharge 8 cases (36%). ●CONCLUSION: Minimally invasive conjunctivorhinostomy can treat the complete bicanalicular obstruction. This operation method is characterized by simplicity, high success rate, short operative time, less tissue trauma, less complications, without facial scarring, and it is worth clinical promotion.

ABSTRACT

Objective To study the methods and clinical effects of joint expansion of double fascia flaps wrapping the Medpor to perform full-ear reconstruction on congenital microtia deformity.Methods 60 patients with congenital microtia deformity were selected.including 22 left ears and 38 right ears.Majority had loss of ear auricle or only partial earlobe.Phase Ⅰ procedures included:temporal-parietal branch was used for the vascular pedicle,and mastoid area behind the ear was moved and expanded for fascia flap.Phase Ⅱ included:the joint expansion of double fascia flap wrapped Medpor to do full-ear reconstruction.Results Flaps survived and no complications for all 60 patients.Reconstructed ears had lifelike appearance and clear anatomical structures.Auriculocephalic angulars positions and sizes were consistent with the healthy ears.None of them had exposed prosthesis and scar contraction.Three-dimensional shapes were good.Conclusions Joint expansion of double fascia flap wrapping Medpor could avoid the chest wall deformity and exposed prosthesis.The fine structure of the reconstructed ear is satisfactory.

ABSTRACT

Alloplastic implants have been used to repair orbital wall fractures in most cases. Orbital hemorrhage is a rare complication of these implants and has been reported rarely in Korea. The purpose of this article is to report a late complication case focusing on their etiology and management. A 20-year-old male patient underwent open reduction with Medpor (porous polyethylene) insertion for bilateral orbital floor fractures. The initial symptom occurred with proptosis in the right side as well as vertical dystopia, which had started 4 days earlier, 8 months after surgery. Any trauma history after the surgery was not present. We performed an exploration and removal of hematoma with Medpor titanium meshed alloplastic implant. A case of delayed orbital hematoma following alloplastic implant insertion was identified. It occurred within the pseudocapsule of the implant. One week after surgery, overall symptoms improved successfully, and no complications were reported during the 11-month follow-up period. Although rare, orbital hemorrhage is a potential complication of alloplastic orbital floor implants, which may present many years after surgery. As in the case presented, delayed hematoma should be included in the differential diagnosis of late proptosis or orbital dystopia.

El desarrollo de las estructuras faciales está a cargo de células derivadas de la cresta neural, alteraciones en la migración o proliferación celular pueden desencadenar malformaciones nasales. Los defectos nasales son tan antiguos como la humanidad, sin embargo, descripciones anatómicas detalladas de estas malformaciones nasales congénitas y sus reconstrucciones son escasas. La hipoplasia nasal puede definirse como un trastorno en el desarrollo de los huesos y cartílagos nasales, y debe ser diferenciada de la arrinia, la cual consiste en la ausencia de nariz y fosas nasales. Los tejidos autologos son los injertos preferidos para la reconstrucción nasal. El tabique nasal, el cartílago auricular, la calota y el cartílago costal son los más usados; pero, estos recursos son limitados, impredecibles y causan algún grado de morbilidad en el sitio donante. Se presenta el primer caso de ausencia de huesos nasales, septum nasal y cartílagos laterales superiores con cartílagos alares intactos. Adicionalmente se presentan los detalles de la reconstrucción usando aloplasticos de alta densidad (polietileno, MEDPOR)...


Neural crest cells are responsible for the development of facial structures, migration failure, decreased proliferation or increased apoptosis may cause nasal anomalies. Nasal defects have existed as long as man; but, detailed anatomic descriptions of congenital nasal defects are rare, the ones including surgical reconstruction are even fewer. Nasal hypoplasia could be defined as nasal bone and cartilage maldevelopment and must be differentiated from congenital arhinia consisting of an absence of nasal structures and passages. Autogenous tissues have been the gold standard graft material for nasal reconstruction; septum, ear, rib and calvarium are preferred. Here, we report the first case of a non-syndromic absence of nasal bones, upper lateral cartilages and septum, with intact lower lateral cartilages and its reconstruction using high-density polyethylene (Medpor)...

ABSTRACT

Abstrac:Objective To analyze the efficacy and safety of medpor-coated tear drain in lacrimal bypass surgery without skin incision. Methods The data of 7 patients(7 eyes) who underwent no skin incision of lacrimal bypass surgery with medpor-coated tear drain were ret-rospective reviewed. The operation result and complications were observed. Results All patients were followed up for 5~17 months. Com-plete or significant improvement of epiphora was achieved in 5 cases at the last follow-up. Complications included conjunctival granulation hy-perplasia (3 eyes),nasal mucosal granulation hyperplasia (2 eyes),and discomfort (4 eyes). Conclusion The lacrimal bypass surgery with medpor-coated tear drain could be expected to improve epiphora of refractory lacrimal obstruction. The main complications are granulation hyperplasia and discomfort.

AIM: To observe the efficacy of Medpor porous polyethylene implants in treatment of enophthalmos combined with orbital fracture. METHODS: Seventeen cases ( 17 eyes ) with enophthalmos caused by orbital fracture underwent surgical treatment with Medpor porous polyethylene implants. All accepted a 6-mo follow-up and the data of enophthalmos, eyeball movement and diplopia were collected. RESULTS: The average difference of exophthalmos between damaged eyes and undamaged eyes was (3. 4±1. 5 ) mm preoperatively, two cases had residual 1mm enophthalmos 6mo after surgery, while other 15 cases were completely corrected. Seventeen cases suffered from eyeball movement restriction and diplopia preoperatively, 16 cases had normal eyeball movement without diplopia 6mo after surgery, 1 case with limitation of abduction and horizontal diplopia. There was no extrusion, rejection, infection or other complications occurred during follow-up. CONCLUSION: Medpor porous polyethylene implants can effectively improve the orbit volume to repair enophthalmos caused by orbital fracture.

ABSTRACT

AIM: To observe the effects of porous polyethylene ( Medpor) as a spacer graft in the reconstruction of large areas eyelid defect after the operation of malignant tumors of lower eyelids. METHODS: Nineteen cases ( 19 eyes ) of malignant tumors of lower eyelid underwent the eyelid reconstruction were selected. Medpor lower eyelid inserts implantation were used to replace tarsal joint sliding conjunctival flap and pedicle flap, and repaired full -thickness lower eyelid defects then underwent eyelid reconstruction. RESULTS: Appearance of eyelids and functional improvements were satisfactory with no stimulation on the eyeball and no effect on the visual function. Implants is with no absorption, shift, exclusion or infection and no tumor recurrence in all cases during the follow up for 6-36mo. CONCLUSION: Medpor lower eyelid inserts implantation can instead tarsal plate for the reconstruction of medium to large areas lower eyelid defect, which is easy performing with rare complications. It is an ideal alternatives of tarsal plate.

Objective To analyze and discuss the possible reasons of the bone resorption beneath the prostheses after chin augmentation.Methods Twelve patients were admitted to our department for further correction after chin augmentation with materials.The bone resorption was observed through the clinical research and X-ray examination.Results All the patients were underwent the removal of the materials,genioplasty was performed in 8 patients,and two patients were treated by chin augmentation with polyethylene.All the patients were satisfied with their facial contouring.Mild bone resorption was found in seven patients (depth of bone resorption ≤2 mm),in which five patients were used with silicone materials,two patients were performed with expanded polytetrafluoroethylene implants.Moderate bone resorption was seen in three cases.All of them were used with silicone implants (2 mm < depth of bone resorption ≤4 mm).Severe bone resorption happened in two patients (depth of bone resorption >4 mm).One was used with silicone implant,and the other one was carried out with expanded polytetrafluoroethylene implant.Conclusions The imbalance among mentalis muscle,materials and underlying bone might be one of the key reasons.Thus for mild and moderate microgenia cases,chin augmentation with material is suitable,while long-term fellow-up study is necessary.But for the cases of severe mirogenia or microgenia and micrognathia with dentofacial deformity or mentalis muscle hyperactivity,genioplasty might be performed as well to correct their deformities.

Mean postoperative exophthalmometry differences between the eyes were 0.49 +/- 1.04 mm and 0.37 +/- 0.62 mm in Medpor(R) and Mesh plate(R) groups, respectively, showing no statistically significant difference (independent t-test, p = 0.512). Postoperative complications such as inflammation or implant malposition were observed only in 3 patients in the Medpor(R) group. CONCLUSIONS: No difference was observed between Medpor(R) and Mesh plate(R) in terms of surgical results during the postoperative 6 month period after reconstruction of orbital medial wall fracture. However, postoperative complications were observed in 3 patients in the Medpor(R) group.

CONCLUSIONS: No difference was observed between Medpor(R) and Mesh plate(R) in terms of surgical results during the postoperative 6 month period after reconstruction of orbital medial wall fracture. However, postoperative complications were observed in 3 patients in the Medpor(R) group.

ABSTRACT

Objective To explore the clinical methodology and comparative analysis of both autogenous septum cartilagineum nasi or Medpor surgical implant in secondary unilateral cleft lip rhinoplasty.Methods Thirty-six patients with varied secondary nasal deformities of unilateral cleft lip were treated,autogenous septum cartilagineum nasi or Medpor surgical implant were implanted to supply the powerful supporting strength for nasal constructions.An anthropometry method based on the quantitative analysis of the characteristics of nasal deformity was used to evaluate and compare the preoperative and postoperative measure of the patients.Results After 6 to 24 months follow-up,all 36 cases received satisfactory results.20 of them used autologous septum cartilagineum nasi to supply the powerful supporting strength for nasal construcions,and other used Medpor surgical implant.No rejection and exposure of implants were found.The postoperative quantitative measurement results supported that both of the implants could effectively correct nasal deformities.Conclusions Both of the implants could effectively correct nasal deformities of unilateral cleft lip in secondary rhinoplasties.The quantitative measurement is a scientific approach to evaluate the outcome.

ABSTRACT

Objective To study Medpor combined with autogenous outer table of calvarial in management of deformities secondary to orbitozygomatic fracture.Methods In the reconstruction of the orbitozygomatic fracture,we firstly did the operation of osteotomies to reset reliable internal fixation,then Medpor combined with autogenous outer table of calvarial was used to repair wall and in correction of depressed orbitozygomatic region.Results From January 2007 to December 2010,13 cases of deformities secondary to orbitozygomatic fracture were repaired.The enlarged orbital volume was reduced,enophthalmos and facial asymmetry were corrected or improved.Facial deformity was significantly improved and no complications occurred.Conclusions Medpor combined with autogenous outer table of calvarial is benefit to most of secondary deformities to orbitozygomatic fracture.It is an effective solution to the orbital content shift and double vision problem,and it also can reduce the autologous damage and foreign body rejection.
