ABSTRACT

ABSTRACT OBJECTIVE Evaluating characteristics of unpaid domestic work and its association with mental disorders, exploring gender differences. METHODS We analyzed cross-sectional data from the second wave of an urban population cohort (n = 2,841) aged 15 and older from a medium-sized city in Bahia (BA). The representative population sample was randomly selected in subsequent multiple steps. We interviewed the survey participants at their homes. This study analyzed sociodemographic, occupational, unpaid domestic work and mental illness data, stratified by sex (gender). We investigated the association between the work-family-personal time conflict, the effort-reward imbalance in domestic and family work and the occurrence of common mental disorders, such as generalized anxiety disorder and depression. We estimated prevalence, prevalence ratios and their respective 95% confidence intervals. RESULTS Among the participants, the unpaid domestic activities were performed by 71.3% of men and 95.2% of women, who were responsible for the investigated activities, except for minor repairs. The percentages of paid work were higher among men (68.1% versus 47.2% among women). The distribution of stressors and conflict experiences showed an inverse situation between genders: men depicted the highest high percentage of low work-family-personal time conflict (39.0%), while among women, the highest percentage was of high conflict (40.0%); 45.8% of the men reported low effort-reward imbalance in domestic and family work, while only 28.8% of women reported low imbalance. The investigated mental disorders were more prevalent among women, who showed a significant association between work-family-personal time conflict and common mental disorders, as well as depression; among men, conflict was positively associated with common mental disorders. The effort-reward imbalance, in turn, was strongly related to CMD (Common Mental Disorders), generalized anxiety disorder and depression among women. Amid men, this discrepancy was only associated to depression. CONCLUSIONS Domestic work persists as a mostly feminine assigned activity. The stressful situations of unpaid domestic work and the work-family-personal time conflict were more strongly associated with adverse effects on the female mental health.


RESUMO OBJETIVO Avaliar características do trabalho doméstico não remunerado e sua associação com transtornos mentais, explorando diferenciais de gênero. MÉTODOS Neste estudo foram analisados dados transversais da segunda onda de uma coorte da população urbana (n = 2.841) com idade a partir dos 15 anos de uma cidade de médio porte da Bahia (BA). A amostra representativa da população foi aleatoriamente selecionada em etapas múltiplas subsequentes. As entrevistas foram realizadas nos domicílios dos participantes do levantamento. O estudo analisou dados sociodemográficos, ocupacionais, do trabalho doméstico não remunerado e adoecimento mental, estratificadas por sexo. Investigou-se associação entre o conflito trabalho-família-tempo para si, o desequilíbrio esforço-recompensa no trabalho doméstico e familiar e a ocorrência de transtornos mentais comuns, de transtorno de ansiedade generalizada e de depressão. Foram estimadas prevalências, razões de prevalência e respectivos intervalos de confiança de 95%. RESULTADOS Entre os participantes, verificou-se que as atividades domésticas não remuneradas eram realizadas por 71,3% dos homens e 95,2% das mulheres, que se mostraram as principais responsáveis pelas atividades de trabalho investigadas, exceto pequenos consertos. A inserção em trabalho remunerado foi maior entre os homens (68,1% contra 47,2% entre as mulheres). A distribuição dos estressores e experiência de conflitos evidenciou situação inversa entre homens e mulheres: o maior percentual entre os homens foi de baixo conflito trabalho-família-tempo para si (39,0%), já entre as mulheres, maior percentual foi de alto conflito (40,0%); entre os homens, 45,8% referiram baixo desequilíbrio esforço-recompensa no trabalho doméstico e familiar, enquanto apenas 28,8% das mulheres relataram baixo desequilíbrio. Os transtornos mentais investigados foram mais prevalentes entre as mulheres, que apresentaram significativa associação entre o conflito trabalho-família-tempo pessoal e os transtornos mentais comuns e a depressão; entre os homens o alto conflito foi associado aos transtornos mentais comuns. Já o desequilíbrio esforço-recompensa se mostrou fortemente relacionado aos TMC, ao transtorno de ansiedade generalizada e à depressão entre as mulheres. Entre os homens, esse desequilíbrio relacionou-se apenas à depressão. CONCLUSÕES O trabalho doméstico persiste como atribuição majoritariamente feminina. As situações estressoras do trabalho doméstico não remunerado e o conflito trabalho-família-tempo para si associaram-se mais fortemente aos efeitos adversos na saúde mental das mulheres.

ABSTRACT OBJECTIVE To investigate the occurrence and factors associated with common mental disorders in pregnancy and depressive symptoms in postpartum, as well as the association between both in the Brazilian Western Amazon. METHODS This is a prospective cohort in the MINA-Brazil study with women who received primary health care in the town of Cruzeiro do Sul, Acre State. We performed two clinical evaluations during pregnancy (the first: 16-20 weeks; the second: 28 gestational weeks) and three postpartum evaluations (at 3, 6 and 12 months), in which demographic and socioeconomic, gestational, lifestyle and clinical data were collected. We used the Self-Reported Questionnaire (score ≥ 8) to screen the gestational common mental disorder and the Edinburgh Postnatal Depression Scale (score ≥ 10) to identify postpartum depressive symptoms. We used adjusted ordinal logistic regression to investigate the relationship between the covariates and the occurrence of common mental disorders in pregnancy and postpartum depressive symptomatology. RESULTS A total of 461 women completed the two clinical evaluations in pregnancy; of these, 247 completed the three postpartum evaluations. The occurrence of common mental disorder during pregnancy was 36.2% and 24.5% in the first and second evaluations, respectively, and the cumulative incidence was 9.2%. In addition, 50.3% maintained the disorder between evaluations. During postpartum, approximately 20% of the mothers presented depressive symptoms during the first year of their children's lives. Parity (≥ 2) was associated with common mental disorders, while low maternal education was associated with postpartum depressive symptoms. Women with a common mental disorder in both evaluations during pregnancy were 5.6 times more likely (95%CI: 2.50-12.60) to develop postpartum depressive symptoms. CONCLUSION The occurrence of common mental disorder at any time assessed during pregnancy, but especially its persistence from the second trimester, was strongly associated with depressive symptoms after childbirth. These findings highlight the need for early screening and monitoring of the mental health of pregnant women at the start of prenatal care in order to reduce possible negative impacts on the health of the mother-child binomial caused by such events.


RESUMO OBJETIVO Investigar a ocorrência e os fatores associados com os transtornos mentais comuns na gestação e sintomas depressivos no pós-parto, bem como a associação entre ambos na Amazônia Ocidental Brasileira. MÉTODOS Coorte prospectiva no estudo MINA-Brasil com mulheres atendidas na atenção primária à saúde de Cruzeiro do Sul, Acre. Foram realizadas duas avaliações clínicas na gestação (primeira: 16-20 semanas; segunda: 28 semanas gestacionais) e três avaliações no pós-parto (aos 3, 6 e 12 meses), nas quais foram coletados dados demográficos e socioeconômicos, gestacionais, de estilo de vida e clínicos. Utilizou-se o Self-Reported Questionnaire (escore ≥ 8) para rastreamento do transtorno mental comum gestacional e a escala de depressão pós-natal de Edimburgo (escore ≥ 10) para identificação de sintomas depressivos pós-parto. Foi utilizada regressão logística ordinal ajustada para investigar a relação entre as covariáveis e a ocorrência de transtornos mentais comuns na gravidez e a sintomatologia depressiva no pós-parto. RESULTADOS Um total de 461 mulheres completaram as duas avaliações clínicas na gestação; dessas, 247 completaram as três avaliações pós-parto. A ocorrência de transtorno mental comum durante a gestação foi de 36,2% e 24,5% na primeira e segunda avaliações, respectivamente, e a incidência cumulativa foi de 9,2%. Ademais, 50,3% mantiveram o transtorno entre as avaliações. Durante o pós-parto, aproximadamente 20% das mães apresentaram sintomatologia depressiva ao longo do primeiro ano de vida de seus filhos. A paridade (≥ 2) foi associada ao transtorno mental comum, enquanto a baixa escolaridade materna associou-se com sintoma depressivo pós-parto. Mulheres com transtorno mental comum nas duas avaliações na gravidez apresentaram 5,6 vezes mais chance (IC95% 2,50-12,60) de desenvolverem sintoma depressivo pós-parto. CONCLUSÃO A ocorrência de transtorno mental comum em qualquer momento avaliado durante a gravidez, mas principalmente sua persistência a partir do segundo trimestre, foi fortemente associado ao sintoma depressivo posterior ao parto. Tais achados evidenciam a necessidade de rastreamento precoce e monitoramento da saúde mental de gestantes no início do pré-natal, a fim de reduzir possíveis impactos negativos para a saúde do binômio mãe-filho causados por tais eventos.

ABSTRACT

ABSTRACT OBJECTIVE To identify the prevalence of and factors associated with: (1) major depressive episodes; (2) minor psychiatric disorders (MPDs); and (3) suicidal ideation among nursing professionals from a municipality in southern Brazil. METHODS Using a cross-sectional design, we recruited 890 nursing professionals linked to 50 Primary Care units, 2 walk-in clinics, 2 hospital services, 1 emergency room service, 1 mobile emergency care service, and 1 teleconsultation service, in addition to the municipal epidemiological surveillance service and the vacancy regulation center between June and July 2020. We used the Patient Health Questionnaire-9 and the Self-Reporting Questionnaire to evaluate the studied outcomes. Associations between the outcomes and variables related to sociodemographic profile, work, health conditions, and daily life were explored using Poisson regression models with robust variance estimators. RESULTS The observed prevalence of depression, MPDs, and suicidal ideation were 36.6%, 44%, and 7.4%, respectively. MPDs were associated with the assessment of support received by the service as 'regular' (PR: 1.48; 95% CI: 1.19-1.85) or 'poor' (PR: 1.54; 95% CI: 1.23-1.94), with a reported moderate (PR: 1.63; 95% CI: 1.29-2.07), or heavy (PR: 2.54; 95% CI: 2.05-3.15) workload, and with suspected COVID-19 infection (PR: 1.44; 95% CI: 1.25-1.66). Major depressive episodes were associated with a reported lack of personal protective equipment (PR: 1.20; 95% CI: 1.01-1.42), whereas suicidal ideation was inversely related to per capita income > 3 minimum monthly wages (PR: 0.28; 95% CI: 0.11-0.68), and positively related to the use of psychotropic drugs (PR: 3.14; 95% CI: 1.87-5.26). CONCLUSION Our results suggest that nursing professionals' working conditions are associated with their mental health status. The need to improve working conditions through adequate dimensioning, support and proper biosafety measures is only heightened in the context of the COVID-19 pandemic.

ABSTRACT OBJECTIVE To identify the prevalence and factors associated with common mental disorders among farmers living in a medium-sized municipality in Northeastern Brazil between 2019 and 2020. METHODS Trained interviewers applied the standardized questionnaire in 450 participants. Sociodemographic, health, income and working characteristics were assessed. The screening of common mental disorders was performed using the Self-Reporting Questionaire, with the cutoff point ≥ 7 for women and ≥ 5 for men. Poisson regression with robust estimation was applied to verify the prevalence ratios in the bivariate and multivariate analysis. RESULTS The prevalence of common mental disorders among farmers was 55.1% (95%CI: 50.4-59.6). The variables that remained significant and associated with common mental disorders were: men (PR = 1.7), > 60 years old (PR = 0.5), poor or very poor self-assessment of health (PR = 1.4), previous mental health treatment (PR = 1.2), alcohol abuse (PR = 1.2) and loss of production (PR = 1.3). CONCLUSION These results indicate that common mental disorders are associated with individual factors and with the farmers' context of life and work, which shows the importance of social, economic and health services support to this group of workers.


RESUMO OBJETIVO Identificar a prevalência e os fatores associados aos transtornos mentais comuns entre agricultores residentes em um município de médio porte no nordeste do Brasil, entre 2019 e 2020. MÉTODOS Entrevistadoras treinadas aplicaram o questionário padronizado em 450 participantes. Investigou-se características sociodemográficas, de saúde, renda e trabalho. O rastreamento dos transtornos mentais comuns foi realizado mediante a utilização do questionário SRQ-20 ( Self-Reporting Questionaire ), com o ponto de corte ≥ 7 para mulheres e ≥ 5 para homens. Foi aplicada a regressão de Poisson com estimação robusta para verificar as razões de prevalência na análise bivariada e multivariada. RESULTADOS A prevalência dos transtornos mentais comuns entre agricultores foi de 55,1% (IC95% 50,4-59,6). As variáveis que permaneceram significativas e associadas aos transtornos mentais comuns foram: ser do sexo masculino (RP = 1,7), ter mais de 60 anos (RP = 0,5), ter autoavaliação de saúde ruim ou muito ruim (RP = 1,4), ter realizado tratamento anterior para saúde mental (RP = 1,2), fazer uso abusivo do álcool (RP = 1,2) e ter tido perda de produção (RP = 1,3). CONCLUSÃO Esses resultados indicam que os transtornos mentais comuns estão associados a fatores individuais e do contexto de vida e trabalho dos agricultores, o que demonstra a importância do suporte social, econômico e dos serviços de saúde a esse grupo de trabalhadores.

ABSTRACT OBJECTIVE to identify factors associated with common mental disorders (CMD) in a sample of adult women in Southern Brazil. METHODS This population-based study, composed of 1,128 women, investigated socioeconomic, behavioral and health/disease explanatory demographic variables. Five response groups were explored: one group with common mental disorders - cut-off point 6/7 in the Self-Reporting Questionnaire 20 (SRQ-20) - and four others corresponding to the different clusters found using the latent class clustering technique, also from the SRQ-20. These four clusters (low, medium-depressive, medium-digestive and high) were named (denominated) based on the mean scores in the SRQ-20 in each group and on the response patterns of the variables and factorial characteristics. The "low" cluster comprised women with lower SRQ-20 scores and, therefore less likely to present CMD. The "high" cluster, with high mean values in the SRQ-20, was related to higher psychiatric morbidity. We used the Poisson regression technique to compare the findings of the different groups. RESULTS We identified ten variables as factors associated with CMD. Age, education, smoking, physical activity, perception of health and number of medical appointments were the common variables for the cut-off point and cluster-based analyses. Heavy alcohol use was associated only when the sample was evaluated as a cut-off point. Social class, work situation and existence of chronic diseases were associated only when the sample was analyzed by clusters. There was a significant association in the "high" cluster with lower classes (D or E), smoking, physical inactivity, existence of chronic diseases and negative perception of health. CONCLUSION We identified different associated factors according to the response groups considered. New approaches allowing identification of subgroups of individuals with specific characteristics and associated factors may contribute for a more accurate understanding of CMD and provide the basis for health interventions.


RESUMO OBJETIVO Identificar fatores associados aos transtornos mentais comuns (TMC) em uma amostra de mulheres adultas no Sul do Brasil. MÉTODOS Em estudo de base populacional composto por 1.128 mulheres, foram investigadas variáveis explicativas demográficas, socioeconômicas, comportamentais e de saúde/doença. Cinco grupos-resposta foram explorados: um grupo com transtornos mentais comuns - ponto de corte 6/7 no Self-Reporting Questionnaire 20 (SRQ-20) - e outros quatro correspondentes aos diferentes clusters encontrados através da técnica de clusterização por classe latente, também a partir do SRQ-20. Esses quatro clusters (baixo, médio-depressivo, médio-digestivo e alto) foram denominados com base nas pontuações médias do SRQ-20 em cada grupo e nos padrões de resposta das variáveis e características fatoriais. O cluster "baixo" compreendeu mulheres com menores escores no SRQ-20 e, portanto, com menor probabilidade de apresentarem TMC. Já o cluster "alto", com valores médios elevados no SRQ-20, esteve relacionado a maior morbidade psiquiátrica. Utilizou-se a técnica de regressão de Poisson, com comparação entre os achados dos diferentes grupos. RESULTADOS Foram identificadas 10 variáveis como fatores associados aos TMC. Idade, escolaridade, tabagismo, atividade física, percepção de saúde e número de consultas foram as variáveis comuns para as análises com ponto de corte assim como nas baseadas em clusters. Etilismo pesado esteve associado somente quando avaliada a amostra como ponto de corte. Já classe econômica, situação laboral e presença de doenças crônicas estiveram associadas somente quando analisada a amostra por clusters. No cluster "alto", houve associação significativa com classes econômicas mais baixas (D ou E), tabagismo, inatividade física, presença de doenças crônicas e percepção de saúde negativa. CONCLUSÕES Diferentes fatores associados foram identificados segundo os grupos-resposta considerados. Novos enfoques que possibilitem identificar subgrupos de indivíduos com características e fatores associados específicos poderiam contribuir para um entendimento mais apurado dos TMC, servindo de base para intervenções em saúde.

ABSTRACT

ABSTRACT OBJECTIVE To estimate the prevalence of clinical signs and symptoms of severe/extreme stress, anxiety, and depression, as well as their associated factors, among Brazilians during social distancing. METHODS This is a cross-sectional study conducted in April/May 2020 with 3,200 Brazilians over 18 years old. Respondents' sociodemographic and clinical data were collected using an online questionnaire, which also included the 21-item Depression, Anxiety and Stress Scale (DASS-21) to assess emotional symptoms. Unadjusted and adjusted prevalence ratios and their respective 95% confidence intervals were estimated using Poisson regression models with robust variance. RESULTS Our results show the prevalence of severe/extreme stress was 21.5%, anxiety 19.4%, and depression 21.5%. In the final model, sociodemographic, clinical, and Covid-19-related factors were associated with severe/extreme stress, anxiety, and depression in Brazilians during social distancing due to the Covid-19 pandemic. We found the main factors associated with severe/extreme depression to be young women, brown, single, not religious, sedentary, presenting reduced leisure activities, history of anxiety and depression, increased medication use, and Covid-19 symptoms. CONCLUSION This study may help develop and systematically plan measures aimed to prevent, early identify, and properly manage clinical signs and symptoms of stress, anxiety, and depression during the Covid-19 pandemic.

ABSTRACT

RESUMEN Fundamento: Ante el aumento de personas con trastornos psiquiátricos en países de América Latina, se aprecia la necesidad de desarrollar estudios epidemiológicos para conocer la distribución de estos entre los varios grupos etarios; sin embargo, existen diferencias que van desde la elección de los instrumentos, hasta la selección y cálculo de la muestra, lo que afecta los resultados y sus conclusiones, como son el subregistro y el sobre diagnóstico de trastornos, o las dificultades en el acceso a los servicios de salud mental. Objetivo: Analizar diferencias metodológicas entre los estudios relacionados con la morbilidad psiquiátrica en la población general y en servicios de atención primaria, asimismo los diferentes hallazgos obtenidos en esas investigaciones. Conclusiones: Los estudios de morbilidad psiquiátrica a partir del indicador prevalencia realizados en América Latina son heterogéneos e incluyen instrumentos, sistemas de clasificación, sujetos y cálculo de muestras diferentes, así como sus resultados, por lo que es pertinente enfocar el diseño de este tipo de investigación ajustado a las realidades locales y recursos disponibles para de esta manera definir estrategias que aporten a la planificación de servicios y el desarrollo de políticas de salud mental en esta región.


ABSTRACT Background: With the increase in the number of people with psychiatric disorders in Latin American countries, the need to develop epidemiological studies is appreciated to know the distribution of these among the several age groups; however, there are differences ranging from the choice of instruments to the selection and calculation of the sample, which affects the results and their conclusions, such as underreporting and overdiagnosis of disorders, or difficulties in accessing mental health services. Objective: To analyze methodological differences between related studies with psychiatric morbidity in the general population and in primary care health services, also the different findings obtained in these research. Conclusions: Psychiatric morbidity studies based on the prevalence indicator in Latin America are heterogeneous and include different instruments, classification systems, subjects and sample calculation, as well as their results, so it is pertinent to focus the design of this type of research adjusted to local realities and resources available to define strategies that contribute to service planning and development of mental health policies in this region.

RESUMEN Fundamento: varias investigaciones a nivel mundial han demostrado que el embarazo influye en el curso y aparición del trastorno mental. En Cuba, sin embargo, los estudios publicados sobre esta temática son escasos. Objetivo: determinar la prevalencia de trastorno mental en embarazadas. Métodos: se realizó un estudio desde el enfoque cuantitativo, descriptivo y de corte transversal. El universo estuvo constituido por 14 386 embarazadas ingresadas en el servicio de la maternidad del hospital Vladimir Ilich Lenin desde mayo de 2018 a octubre 2019. Se seleccionaron 265 a través de un muestreo no probabilístico. Se utilizó la entrevista semiestructurada y la observación para la evaluación. La variable fue trastorno mental. Los resultados se procesaron en el paquete estadístico EPIDAT 3.1, con las tasas de prevalencias y las frecuencias absolutas y relativas. Resultados: se evidenció una prevalencia de 35,1 % (IC 95 %: 29,1-41,0) de antecedentes psicopatológicos en las gestantes estudiadas. Predominó el intento suicida, la atención por la especialidad de Psiquiatría en alguna ocasión de su vida y el trastorno de la personalidad. La prevalencia estimada de trastorno mental durante la hospitalización fue de 1,8 % (IC 95 %: 1,6-2,1). Se destacaron la ansiedad y la tristeza como alteración emocional, el trastorno de adaptación con ansiedad, y con estado de ánimo deprimido. Conclusiones: la tasa de prevalencia de antecedentes psicopatológicos en las embarazas estudiadas fue relativamente elevada. En cambio, la estimación de la prevalencia de trastorno mental durante la hospitalización se consideró baja. No obstante, se evidenció una prevalencia relativamente alta de ansiedad.


ABSTRACT Background: several researches worldwide have shown that pregnancy influences the course and appearance of the mental disorder. In Cuba, however, published studies are scarce. Objective: to determine the prevalence of mental disorder in pregnant women. Methods: a study was carried out from the quantitative, descriptive and cross-sectional approach. The universe consisted of 14 386 pregnant women admitted to the maternity service of the Vladimir Ilich Lenin hospital from May, 2018 to October, 2019. 265 were selected through a non-probabilistic sampling. The semi-structured interview and observation were used for the evaluation. The variable was mental disorder. The results were processed in the EPIDAT 3.1 statistical package, with prevalence rates and absolute and relative frequencies. Results: a prevalence of 35.1 % (95 % CI: 29.1-41.0) of psychopathological antecedents was evidenced in the pregnant women studied. The suicidal attempt, the attention for the specialty of Psychiatry in some occasion of their lives and the personality disorder predominated. The estimated prevalence of mental disorder during hospitalization was 1.8 % (95 % CI: 1.6-2.1). Anxiety and sadness were highlighted as emotional disturbance, the disorder of adaptation with anxiety, and with depressed mood. Conclusions: the prevalence rate of psychopathological antecedents in the studied pregnancies was relatively high. In contrast, the estimate of the prevalence of mental disorder during hospitalization was considered low. However, a relatively high prevalence of anxiety was evident.

ABSTRACT INTRODUCTION: Adolescence is a stage of great social, family and emotional demands, and the literature has related common mental disorder (CMD) with poor living conditions. OBJECTIVE: To investigate the relationship between CMD and socioeconomic status in Brazilian adolescents aged 12 to 17 years. METHOD: This is a cross-sectional study with data from the Study of Cardiovascular Risk in Adolescents (ERICA - Estudo de Riscos Cardiovasculares em Adolescentes). The outcome was CMD and the exposure was socioeconomic status assessed by race/skin color, maternal schooling, resident/room relationship, type of school, existence of maid and bathroom at home, and work activity. For the calculation of prevalence, the survey mode was used and, in the multivariate analysis, logistic regression with p < 5%, as well as the 95% confidence interval. RESULTS: The prevalence of CMD in girls was 23.3%, and in boys, 11.1%. The variables associated with CMD in girls were age between 15 and 17 years (OR = 1.34; 1.17-1.51), studying in private school (OR = 1.13; 1.01-1.27), having a housemaid (OR = 1.15; 1.00-1.34) and, as a protective factor, unpaid work (OR = 0.64; 0.55-0.75). Boys also had a higher chance of CMD in the highest age group (OR = 1.42; 1.18-1.71) and when they had a housemaid (OR = 1.26; 1.02-1.57), whereas unpaid work decreased this chance (OR = 0.79; 0.67-0.95). CONCLUSION: Socioeconomic variables that were associated with CMD were suggestive of higher economic class, whereas unpaid work favored the mental health of adolescents, results contrary to the literature on socioeconomic status and CMD.


RESUMO INTRODUÇÃO: A adolescência é uma fase de grande demanda social, familiar e emocional, e a literatura tem relacionado o transtorno mental comum (TMC) com piores condições de vida. OBJETIVO: Investigar a relação entre TMC e a condição socioeconômica em adolescentes brasileiros de 12 a 17 anos. MÉTODO: Estudo seccional com os dados do Estudo de Riscos Cardiovasculares em Adolescentes (Erica). O desfecho foi o TMC e a exposição foi a condição socioeconômica avaliada por raça/cor, escolaridade materna, relação morador/cômodo, tipo de escola, existência de empregada e banheiro no domicílio e atividade laboral. Para o cálculo das prevalências, foi utilizado o modo survey e, na análise multivariada, a regressão logística com p < 5%, assim como o intervalo de confiança de 95%. RESULTADOS: A prevalência de TMC em meninas foi 23,3% e em meninos, 11,1%. As variáveis associadas ao TMC nas meninas foram ter idade entre 15 e 17 anos (OR = 1,34; 1,17-1,51), estudar em escola privada (OR = 1,13; 1,01-1,27), ter empregada doméstica (OR = 1,15; 1,00-1,34) e, como fator de proteção, o trabalho não remunerado (OR = 0,64; 0,55-0,75). Os meninos também apresentaram maior chance de TMC na faixa etária mais alta (OR = 1,42; 1,18-1,71) e quando tinham empregada (OR = 1,26; 1,02-1,57), enquanto o trabalho não remunerado diminuiu essa chance (OR = 0,79; 0,67-0,95). CONCLUSÃO: As variáveis socioeconômicas que estiveram associadas ao TMC foram sugestivas de classe econômica mais elevada, enquanto o trabalho não remunerado favoreceu a saúde mental dos adolescentes, resultados contrários à literatura sobre condição socioeconômica e TMC.

ABSTRACT OBJECTIVE: To explore the association of occupational pesticide exposure with acute and mental health symptoms. METHODS: Cross-sectional survey carried out with 78 Brazilian family farmers, who were pesticide applicators and helpers conveniently selected. Symptoms and exposure data were collected by interviews, and mental health outcomes by the Self-Reporting Questionnaire. Blood samples were analyzed to assess cholinesterase levels. Exposure indicators and symptoms were compared between applicators and helpers, and Poisson regression was performed to estimate prevalence ratios. RESULTS: Farmers reported exposure to multiple pesticides from early ages; they worked without safety training, technical support, and full protective equipment, and they had a high prevalence of acute and mental health symptoms (e.g., headache, mucosal irritation, tachycardia, and depressive signs). Applicators had more cholinesterase changes than helpers, but less symptoms. Helpers used less personal protection and had significantly higher prevalence ratio of headache, dyspnea, wheezing, cough, poor digestion, tiredness, and feeling worthless, after adjustment. CONCLUSIONS: Acute and mental health symptoms were observed, both among farmers and helpers. Thus, surveillance actions must be reinforced in Brazil, technical support and safety training improved, focused on applicators and helpers, who are occupationally and environmentally exposed to pesticides. Agricultural practices of these groups with less pesticide use should receive incentive.

ABSTRACT OBJECTIVE: This study aims to investigate the prevalence of adolescents and young adults who were victims of sexual violence at some point in their lives and to compare the presence of depressive and anxious symptoms, quality of life, and use of alcohol, tobacco, and illegal drugs among this population and those who were not abused. METHODS: Validated questionnaires and instruments were applied in a group of university students to assess: sexual profile and behavior, socioeconomic status, presence or not of sexual violence (Questionnaire on Exposure to Traumatizing Events), depressive (Beck Depression Inventory) and anxious symptoms (Beck Anxiety Inventory), quality of life (World Health Organization's Quality of Life Assessment) and the use or abuse of tobacco, alcohol, and illegal drugs (Smoking, Alcohol, and Substance Involvement Screening Test). RESULTS: Out of the 858 students who participated, 71 (8.3%) were victims of sexual violence, 52 girls (73.2%). In the victims of violence group there were more students who already had the first sexual intercourse (p = 0.029), students who already had become pregnant (p = 0.001), students with higher scores for depressive (p < 0.001) and anxious symptoms (p = 0.001), students with worse quality of life (p < 0.001), and who used more tobacco (p = 0.008) and marijuana (p = 0.025) as well as abused hypnotics or sedatives (p = 0.048) than in the non-victim group. CONCLUSION: The abuses are presented in several forms and affect, even in long term, the survivors' life. The sexual violence theme should be addressed and widely discussed in all spheres of society in order to mobilize, to sensitize, and provide society with knowledge, demystifying this subject and drawing attention to this important social issue.


RESUMO OBJETIVO: Investigar a prevalência de adolescentes e adultos jovens que foram vítimas de violência sexual em algum momento da vida e comparar a presença de sintomas depressivos e ansiosos, qualidade de vida e uso de álcool, tabaco e drogas ilícitas entre esta população e a que não sofreu abuso. MÉTODOS: Aplicaram-se questionários e instrumentos validados, em uma população de estudantes universitários, para avaliar: perfil e comportamento sexual, nível socioeconômico, presença ou não de violência sexual (Questionário sobre a Exposição a Eventos Traumatizantes), sintomas depressivos (Inventário de Depressão de Beck) e ansiosos (Inventário de Ansiedade de Beck), qualidade de vida (World Health Organization's Quality of Life Assessment) e o uso ou abuso de tabaco, álcool e drogas ilícitas (Teste para Triagem do Envolvimento com Fumo, Álcool e Outras Drogas). RESULTADOS: Dos 858 alunos que responderam à pesquisa, 71 (8,3%) foram vítimas de violência sexual, sendo 52 meninas (73,2%). No grupo vítima de abuso havia mais alunos que já tinham tido a coitarca (p = 0,029), alunas que já engravidaram (p = 0,001), estudantes com maiores escores para sintomas depressivos (p < 0,001) e ansiosos (p = 0,001), alunos com pior qualidade de vida (p < 0,001) e que usavam mais tabaco (p = 0,008) e maconha (p = 0,025) bem como abusavam de hipnóticos ou sedativos (p = 0,048) que no grupo não vítima. CONCLUSÃO: Os impactos causados pelo abuso são diversos e afetam, mesmo no longo prazo, a vida dos sobreviventes. Abordar o tema e o discutir, amplamente, em todas as esferas da sociedade é uma forma de mobilizar, sensibilizar e instrumentalizar o coletivo, desmistificando o assunto e chamando atenção para essa importante questão social.

Objective: To identify the prevalence of common mental disorders (CMD) in medical students at Vila Velha University (UVV), ES, and its associated factors. Methods: A cross-sectional study was conducted with 360 medical students from February to April 2018. A self-administered, confidential and online questionnaire was used, including sociodemographic, economic data, family support, friends' network, physical activity and academic performance, as well as Self- Reporting Questionnaire (SRQ-20). Statistical analysis was performed using the STATA program, version 13.0. The Ethics Committee on Research with Humans approved the study, under number 2.108.290. Results: The prevalence of CMD among students was 45.6%. Risk factors were mental disorder in the family (RR 1.24, 95% CI 1.01-1.54), poor sleep quality (RR 1.49, 95% CI 1.17-1.90), fear that impaired the academic result (RR 1.33, 95% CI 1.01-1.77), feeling rejected by friends (RR 1.45, 95% CI 1.07-1.96), thinking about giving up the course (RR 1.67, 95% CI 1.29-2.17) and physical discomfort during the test (RR 1.63, 95% CI, 1.21-2.20). Conclusions: The overall prevalence of CMD among students was high, and the risk factors were significant. It is recommended that educational institutions and responsible government agencies be able to formulate and subsidize preventive actions and care for the mental health of medical students.

Recomenda-se que as instituições de ensino e órgãos governamentais responsáveis possam formular e subsidiar ações preventivas e um cuidado à saúde mental dos estudantes de medicina.

Conclusiones: La prevalencia general de TMC entre los estudiantes fue alta, y los factores de riesgo fueron expresivos. Se recomienda que las instituciones de enseñanza y órganos gubernamentales responsables puedan formular y subsidiar acciones preventivas y un cuidado a la salud mental de los estudiantes de medicina.

ABSTRACT OBJECTIVE To assess the prevalence of some mental disorders and suicide risk, and the association between them in youths. METHODS Data from the 1993 Pelotas Birth Cohort (Brazil) was used. The prevalence of mental disorders at 22 years [major depressive disorder (MDD), generalized anxiety disorder (GAD), social anxiety disorder (SAD), attention-deficit/ hyperactivity disorder (ADHD), bipolar disorders type 1 and 2 (BD1; BD2), post-traumatic stress disorder (PTSD), and antisocial personality disorder (APD)] and of suicide risk were assessed using the Mini International Neuropsychiatric Interview (n = 3,781). Comorbidity between disorders was also assessed. Association of each mental disorder and the number of disorders with suicide risk was assessed using Poisson regression. RESULTS The prevalence of any mental disorder was 19.1% (95%CI 17.8-20.3), and GAD was the most prevalent (10.4%; 95%CI 9.5-11.4). The prevalence of current suicide risk was 8.8% (95%CI 5.9-9.7). All disorders (except APD) and the suicide risk were higher among women. Mental disorders were associated with a higher suicide risk, with the highest risks being observed for MDD (RR = 5.6; 95%CI 4.1-7.8) and PTSD (RR = 5.0; 95%CI 3.9-6.3). The higher the number of co-occurring mental disorders, the higher the risk of suicide. CONCLUSIONS Our findings showed that about 20% of the youths had at least one mental disorder. However, this prevalence is underestimated since other relevant mental disorders were not assessed. Mental disorders were associated with higher suicide risk, especially the comorbidity between them.

ABSTRACT

ABSTRACT OBJECTIVE To identify the prevalence of 12-month self-reported pain and chronic pain in a general population and to describe their clinical profile to assess if chronic pain is associated with 12-month mental disorders. METHODS The data used comes from the São Paulo Megacity Mental Health Survey, a population-based study assessing adult (≥ 18 years) residents of the São Paulo metropolitan area, Brazil. We have assessed the respondents (n = 5,037) using the Composite International Diagnostic Interview (CIDI 3.0), with a global response rate of 81.3%. Descriptive analyses have been performed, and crude and adjusted odds ratios (OR) have been calculated with logistic and multinomial regression and presented with respective 95% confidence intervals (95%CI). RESULTS The prevalence of pain and chronic pain in the past 12 months were 52.6% (95%CI 50.3-54.8) and 31.0% (95%CI 29.2-32.7), respectively. Joints (16.5%, 95%CI 15.4-17.5) and back or neck (15.5%, 95%CI 14.2-16.9) were the most frequently reported anatomical sites of chronic pain. On a 10-point analogue scale, the mean intensity of the worst pain was 7.7 (95%CI 7.4-7.8), and the mean average pain was 5.5 (95%CI 5.2-5.6); the mean treatment response was 6.3 (95%CI 6.0-6.6). Mean pain duration was 16.1 (95%CI 15.6-17.0) days a month and 132 (95%CI 126-144) minutes a day. Chronic pain was associated with 12-month DSM-IV mental disorders (OR = 2.7, 95%CI 2.3-3.3), anxiety disorders (OR = 2.1, 95%CI 1.9-3.0), and mood disorders (OR = 3.3, 95%CI 2.4-4.1). CONCLUSIONS A high prevalence of chronic pain in multiple sites is observed among the general adult population, and associations between chronic pain and mental disorders are frequent.

ABSTRACT OBJECTIVE To estimate the prevalence of common mental disorders and to analyze the associated factors in migrant and sugarcane workers. METHODS This is a cross-sectional study carried out with 110 workers. Common mental disorders were evaluated using the Self-Reporting Questionnaire (SRQ-20), and sociodemographic, occupational, and lifestyle variables were studied. The CAGE questionnaire was used to detect the abuse of alcoholic beverages. RESULTS The prevalence of common mental disorders affected 40% of the workers and the association showed statistical significance for the positive result of the CAGE test, sickness, absence from work, and medical care during the harvest period. CONCLUSIONS The suspected cases of problem drinkers and the control mechanisms used by the mill for workers who miss work or become ill are factors that can cause common mental disorders.


RESUMO OBJETIVO Estimar a prevalência de transtornos mentais comuns e analisar os fatores associados em trabalhadores migrantes e canavieiros. MÉTODOS Estudo transversal realizado com 110 trabalhadores. Os transtornos mentais comuns foram avaliados por meio do Self-Reporting Questionnaire (SRQ-20) e foram estudadas variáveis sociodemográficas, ocupacionais e de estilo de vida. Para detectar o uso abusivo de bebidas alcoólicas, foi utilizado o questionário CAGE. RESULTADOS A prevalência de transtornos mentais comuns afetou 40% dos trabalhadores e a associação mostrou significância estatística para a positividade no teste CAGE, adoecimento, falta ao trabalho e atendimento médico durante o período da safra. CONCLUSÕES A suspeita de casos de bebedores-problemas e os mecanismos de controle utilizados pela usina para os trabalhadores que faltam ou adoecem são fatores que podem causar transtornos mentais comuns.

ABSTRACT OBJECTIVE To investigate the association of intimate partner violence against women reported in the last 12 months and seven years with the incidence of common mental disorders. METHODS A prospective cohort study with 390 women from 18 to 49 years, registered in the Family Health Program of the city of Recife, State of Pernambuco; from July 2013 to December 2014. The Self Reporting Questionnaire-20 (SRQ-20) assessed mental health. Intimate partner violence consists of concrete acts of psychological, physical or sexual violence that the partner inflicts on the woman. Poisson regression was used to estimate crude and adjusted relative risks (RR) of the association between common mental disorders and intimate partner violence. RESULTS The incidence of common mental disorders was 44.6% among women who reported intimate partner violence in the last 12 months and 43.4% among those who reported in the past seven years. Mental disorders remained associated with psychological violence (RR = 3.0; 95%CI 1.9–4.7 and RR = 1.8; 95%CI 1.0–3.7 in the last 12 months, and seven years, respectively), even in the absence of physical or sexual violence. When psychological violence were related to physical or sexual violence, the risk of common mental disorders was even higher, both in the last 12 months (RR = 3.1; 95%CI 2.1–4.7) and in the last seven years (RR = 2.5; 95%CI 1.7–3.8). CONCLUSIONS Intimate partner violence is associated with the incidence of common mental disorders in women. The treatment of the consequences of IPV and support for women in seeking protection for themselves for public services is essential.


RESUMO OBJETIVO Investigar a associação da violência por parceiros íntimos relatada contra as mulheres nos últimos 12 meses e últimos sete anos com a incidência dos transtornos mentais comuns. MÉTODOS Estudo de coorte prospectivo com 390 mulheres de 18 a 49 anos, cadastradas no Programa Saúde da Família da cidade do Recife, PE, entre julho de 2013 e dezembro de 2014. A saúde mental foi avaliada pelo Self Reporting Questionnaire-20 (SRQ-20). A violência por parceiro íntimo foi definida por atos concretos de violência psicológica, física ou sexual infligidos à mulher pelo parceiro. A regressão de Poisson foi utilizada para estimar os riscos relativos (RR) brutos e ajustados da associação entre transtorno mental comum e violência por parceiro íntimo. RESULTADOS A incidência dos transtornos mentais comuns foi de 44,6% entre as mulheres que relataram violência nos últimos 12 meses e de 43,4% nas que relataram violência nos últimos sete anos. Os transtornos mentais mantiveram-se associados à violência psicológica (RR = 3,0; IC95% 1,9–4,7 e RR = 1,8; IC95% 1,0–3,7 nos últimos 12 meses, e sete anos, respectivamente), mesmo na ausência de violência física ou sexual. Quando a violência psicológica esteve combinada com violência física ou sexual, o risco dos transtornos mentais comuns foi ainda mais elevado, tanto nos últimos 12 meses (RR = 3,1; IC95% 2,1–4,7) quanto nos últimos sete anos (RR = 2,5; IC95% 1,7–3,8). CONCLUSÕES A violência por parceiro íntimo está associada à incidência de transtornos mentais comuns nas mulheres. É fundamental o tratamento das consequências da VPI e o apoio às mulheres na busca de proteção para si pelos serviços públicos.

ABSTRACT

ABSTRACT OBJECTIVE To analyze the interaction between the psychosocial aspects of work and the occurrence of common mental disorders among health workers. METHODS This is a cross-sectional study conducted with a representative sample of workers of the primary health care of five municipalities of the State of Bahia, Brazil, in 2012. The variable of outcome were the common mental disorders evaluated by the SRQ-20, and the variables of exposure were high demand (high psychological demand and low control over the work) and low social support in the workplace. Interaction was checked by the deviation of the additivity of the effects for the factors studied from the calculation of excess risk from interaction, proportion of cases attributed to interaction, and the synergy index. RESULTS The global prevalence of common mental disorders was 21%. The group of combined exposure has shown higher magnitude (high demand and low social support), reaching 28% when compared to the 17% in the situation of no exposure (low demand and high social support). CONCLUSIONS The results strengthen the hypothesis of interaction between the factors investigated, directing to the synergy of the effects.


RESUMO OBJETIVO Analisar a interação entre aspectos psicossociais do trabalho e a ocorrência de transtornos mentais comuns entre trabalhadores da saúde. MÉTODOS Estudo transversal conduzido em amostra representativa de trabalhadores da atenção básica de cinco municípios da Bahia em 2012. As variáveis desfecho foram os transtornos mentais comuns avaliados pelo SRQ-20, as de exposição foram a alta exigência (alta demanda psicológica e baixo controle sobre o próprio trabalho) e o baixo apoio social no trabalho. A interação foi verificada pelo afastamento da aditividade dos efeitos para fatores estudados a partir do cálculo do excesso de risco devido à interação, proporção de casos atribuída à interação e índice de sinergia. RESULTADOS A prevalência global de transtornos mentais comuns foi de 21%. Apresentou maior magnitude no grupo de exposição combinada (alta exigência e baixo apoio social), chegando a 28% quando comparada a 17% na situação de nenhuma exposição (baixa exigência e alto apoio social). CONCLUSÕES Os resultados fortalecem a hipótese de interação entre os fatores investigados, com direção para a sinergia dos efeitos.

ABSTRACT OBJECTIVE To describe the prevalence of common mental disorders in Brazilian adolescent students, according to geographical macro-regions, school type, sex, and age. METHODS We evaluated 74,589 adolescents who participated in the Cardiovascular Risk Study in Adolescents (ERICA), a cross-sectional, national, school-based study conducted in 2013-2014 in cities with more than 100,000 inhabitants. A self-administered questionnaire and an electronic data collector were employed. The presence of common mental disorders was assessed using the General Health Questionnaire (GHQ-12). We estimated prevalence and 95% confidence intervals of common mental disorders by sex, age, and school type, in Brazil and in the macro-regions, considering the sample design. RESULTS The prevalence of common mental disorders was of 30.0% (95%CI 29.2-30.8), being higher among girls (38.4%; 95%CI 37.1-39.7) when compared to boys (21.6%; 95%CI 20.5-22.8), and among adolescents who were from 15 to 17 years old (33.6%; 95%CI 32.2-35.0) compared to those aged between 12 and 14 years (26.7%; 95%CI 25.8-27.6). The prevalence of common mental disorders increased with age for both sexes, always higher in girls (ranging from 28.1% at 12 years to 44.1% at 17 years) than in boys (ranging from 18.5% at 12 years to 27.7% at 17 years). We did not observe any significant difference by macro-region or school type. Stratified analyses showed higher prevalence of common mental disorders among girls aged from 15 to 17 years of private schools in the North region (53.1; 95%CI 46.8-59.4). CONCLUSIONS The high prevalence of common mental disorders among adolescents and the fact that the symptoms are often vague mean these disorders are not so easily identified by school administrators or even by health services. The results of this study can help the proposition of more specific prevention and control measures, focused on highest risk subgroups.


RESUMO OBJETIVO Descrever a prevalência de transtornos mentais comuns em adolescentes escolares brasileiros, segundo macrorregiões, tipo de escola, sexo e idade. MÉTODOS Foram avaliados 74.589 adolescentes participantes do Estudo de Riscos Cardiovasculares em Adolescentes (ERICA), estudo transversal, nacional, de base escolar, realizado em 2013-2014 em municípios com mais de 100 mil habitantes. Utilizou-se questionário autopreenchível e coletor eletrônico de dados. Presença de transtornos mentais comuns foi avaliada por meio do General Health Questionnaire (GHQ-12). Estimaram-se prevalências e intervalos de confiança de 95% de transtornos mentais comuns por sexo, idade e tipo de escola, no Brasil e nas macrorregiões, considerando o desenho da amostra. RESULTADOS A prevalência de transtornos mentais comuns foi de 30,0% (IC95% 29,2-30,8), sendo mais elevada entre meninas (38,4%; IC95% 37,1-39,7), quando comparadas aos meninos (21,6%; IC95% 20,5-22,8) e entre os adolescentes de 15 a 17 anos (33,6%; IC95% 32,2-35,0), em relação àqueles entre 12 e 14 anos (26,7%; IC95% 25,8-27,6). As prevalências de transtornos mentais comuns aumentaram conforme a idade, para ambos os sexos, sempre maior nas meninas (variando de 28,1% aos 12 anos, até 44,1% aos 17 anos), do que nos meninos (variando de 18,5% aos 12 anos até 27,7% aos 17 anos). Não houve diferença importante por macrorregião ou tipo de escola. Análises estratificadas mostraram maior prevalência de transtornos mentais comuns entre meninas de 15 a 17 anos de escolas privadas da região Norte (53,1; IC95% 46,8-59,4). CONCLUSÕES A elevada prevalência de transtornos mentais comuns entre os adolescentes e o fato de os sintomas serem muitas vezes vagos fazem com que esses transtornos sejam pouco identificados por gestores escolares ou mesmo serviços de saúde. Os resultados deste estudo podem ajudar na proposição de medidas de prevenção e controle mais específicas e voltadas para os subgrupos sob maior risco.

Objetivo Estimar a prevalência de transtorno mental comum e seus fatores associados em serviço de atenção primária.Métodos Estudo transversal que incluiu 607 indivíduos em serviço de atenção primária. O instrumento de pesquisa foi o questionário Self Report Questionnaire 20.Resultados Dos sujeitos entrevistados, 31,47% apresentaram maior probabilidade para transtorno mental comum. Foram associadas à menor probabilidade de desenvolvimento do Transtorno Mental Comum as variáveis preditoras: gênero, estado civil solteiro, ocupação estudante e com carteira assinada, maior nível de escolaridade e renda acima de quatro salários mínimos. E, à maior probabilidade de desenvolvimento do Transtorno Mental Comum as variáveis referir ocupação autônoma, do lar, ter filhos, menor escolaridade e baixa renda.Conclusão A prevalencia de Transtorno Mental Comum foi alta e os fatores associados foram: no gênero feminino, divorciado ou separado, cor da pele amarela, idade de 18 a 59 anos, ocupação do lar, com filhos, com quatro a sete anos de estudo, renda de até um salário mínimo e residindo em moradia emprestada ou doada.


Objective To assess the prevalence of common mental disorder and its related factors in primary health care.Methods Cross-sectional study with 607 individuals in a primary health care service. The instrument of the study was the Self Reporting Questionnaire 20.Results Out of the interviewed subjects, 31.47% showed greater probability of occurrence of a common mental disorder. The following predictive variables were associated with a lower probability of occurrence of common mental disorder: sex, being single, being a student or a worker with signed labor, having higher education levels and income over four times the minimum wage. The variables associated with a higher probability of occurrence of a common mental disorder were being self-employed, housewife, with children, having lower education level and low income.Conclusion The prevalence of a common mental disorder was high and the associated factors were: being female, divorced, Asian, aged between 18 and 59, housewife, with children, having four to seven years of education, income up to one minimum age and living in a borrowed or donated house.

ABSTRACT

O objetivo deste estudo foi estimar prevalência dos transtornos mentais comuns (TMC) e bebedores problemas (BP) e também a utilização de serviços, a partir de estudo seccional, populacional com amostragem aleatória. Incluiu-se sujeitos com 16 anos e mais, avaliando-se aspectos sócio-demográficos, uso de álcool, saúde mental e utilização de serviços. A prevalência de TMC foi 21,7%, de BP foi 4,4%. Na ultima quinzena 14,6% procuraram serviço de saúde. As prevalências observadas confirmam achados da literatura: TMC e BP são importantes problemas de saúde pública, com implicações para a organização de serviços e formação de recursos humanos.


The aim of this cross-sectional study was to estimate the prevalence of common mental disorders (CMD) and problem drinking (PD), as well as health services use in an unselected populational sample. Subjects over 16 years were included and aspects related to socio-demographic characteristics, alcohol use, mental health and services use were studied. The prevalence of CMD was 21.7%, of PD 4.4%. In the two weeks preceeding the interview, 14.6% had used a health service. The results are in accordance with the literature: CMD and PD are important health problems, with implications for health services organization and formation of health professionals.
