ABSTRACT

Mesh-related visceral complications caused by mesh erosion after tension-free inguinal hernia repair are one kind of rare long-term complications, but they are easily neglected. Interval time from initial hernia repair to mesh-related visceral complications by preperitoneal and laparoscopic repair is short. Rutkow and transabdominal preperitoneal repair have the highest reported rate. Lichtenstein has the longest interval time and the lowest reported rate. The most frequently eroded organs are sigmoid colon, bladder and small intestine. The common clinical manifestations of sigmoid colon erosion are hematochezia, abdominal wall fistula and colitis, hematuria and recurrent urinary tract infection in bladder erosion cases, intestinal obstruction and abdominal wall fistula in intestinal erosion case, sigmoid-bladder fistula and intestinal-bladder fistula in multiple organ erosion cases. Resection or repair of corresponding organs with mesh removal have good efficacies in most patients. The authors summarize and analyze researches on mesh-related visceral complications after tension-free inguinal hernia repair from 1994 to 2021, review their advances, in order to raise awareness of such complications in clinicians.

RESUMEN La migración de malla en el posoperatorio alejado de la eventroplastìa y su consecuente infección es una complicación poco frecuente y peligrosa. La malla migrada genera reacción inflamatoria de tipo cuerpo extraño. Puede causar obstrucción intestinal, perforación intestinal o dolor abdominal cróni co. Solo se informan 4 casos en la literatura mundial de migración y uno con compromiso intestinal. Presentamos el caso de un tumor inflamatorio adherido a la pared abdominal, con contenido de poli propileno. El objetivo de esta carta científica es presentar una complicación poco habitual, destacando los aspectos más importantes de su manejo, definiendo algunas recomendaciones y remarcando la importancia del abordaje multidisciplinario.


ABSTRACT Mesh migration with subsequent infection years after incisional hernia repair is an uncommon and dangerous complication. Mesh migration produces an inflammatory foreign body reaction and can cause bowel obstruction, bowel perforation or chronic abdominal pain. Only four cases have been reported in the international literature, one of them with bowel involvement. We report a case of an inflammatory tumor containing polypropylene traces adhered to the abdominal wall. The aim of this scientific letter is to report a rare complication, emphasizing the most relevant aspects about its ma nagement, recommendations, and the relevance of a multidisciplinary approach.

ABSTRACT

ABSTRACT We evaluated a 27-year old male with pneumaturia and fecaluria with a past history of right inguinal hernia repair. Though, cystoscopy and contrast enhanced computed tomography did not furnish any evidence to arrive at a diagnosis, interestingly, colonoscopy revealed a mesh in the sigmoid colon making apparent the diagnosis of colovesical fistula secondary to mesh migration. Later, surgical removal of the mesh from the sigmoid colon with rent closure of the fistulous opening was done successfully. Our case thus, highlights the vital role of common diagnostic tool like colonoscopy in making an uncommon diagnosis.


RESUMO Avaliamos um homem de 27 anos com pneumaturia e fecalúria com antecedentes de reparo da hérnia inguinal direita. Embora a cistoscopia e a tomografia computadorizada com contraste (TCC) não tenham fornecido nenhuma evidência para obter-se um diagnóstico, curiosamente, a colonoscopia revelou uma malha no cólon sigmoide, estabelecendo o diagnóstico de fístula colovesical (FCV) secundária à migração da malha. Mais tarde, foi feita a remoção cirúrgica da malha do cólon sigmoide com fechamento da abertura fistulosa com sucesso. Nosso caso, portanto, destaca o papel vital de uma ferramenta diagnóstica comum, como a colonoscopia, para obter-se um diagnóstico incomum.
