ABSTRACT

Introducción: La enfermedad por hígado graso no alcohólico es una de las principales causas de afección hepática. La citoqueratina 18 surge como marcador no invasivo para la valoración de fibrosis hepática. El objetivo del trabajo fue validar el uso de la citoqueratina 18 en sangre periférica en el diagnóstico y evolución de los pacientes con enfermedad por hígado graso no alcohólico. Metodología: Para validar la citoqueratina 18 en el diagnóstico se realizó un estudio de tipo caso-control. El grupo caso fueron los pacientes mayores de 18 años, de ambos sexos, con diagnóstico de enfermedad por hígado graso no alcohólico vinculado al síndrome metabólico, captados entre 2/2/2019 al 2/2/2020. El grupo control fueron personas donantes de sangre. Se parearon 1-1 por edad y sexo. Se cuantificó la citoqueratina 18 en sangre periférica de ambos grupos. Para validar la citoqueratina 18 en la evolución de los pacientes con enfermedad de hígado graso no alcohólico se realizó un trabajo prospectivo, longitudinal. El grupo de pacientes captados fueron seguidos durante un año bajo tratamiento estándar, finalizando el mismo se realizó la cuantificación de citoqueratina 18 en sangre periférica. Las variables continuas se expresan con la media y desvío estándar. Se analizó con test de t Student, error α < 5% Resultados: 13 pacientes integran el grupo caso (12 mujeres), de 53 ± 11 años, con IMC 35.01 ± 8.9 kg/m2. El valor de citoqueratina 18 pre-tratamiento fue de 1410 ± 120 UI, y el valor post-tratamiento fue de 117 ± 56, p < 0,005.El grupo control fueron 13 personas (12 mujeres), de 43,4 ± 8,1 años e IMC 28,10 ± 5,4 kg/m2 El valor de citoqueratina 18 fue de 193 ± 7.2 UI, p < 0.005 vs grupo caso pretratamiento. Conclusiones: La citoqueratina 18 es más elevada en los pacientes con enfermedad hígado graso no alcohólico, siendo estadísticamente significativa y disminuye con el tratamiento con significación estadística, pudiendo constituirse en un marcador útil en este grupo de pacientes.


Introduction: Nonalcoholic fatty liver disease is one of the main causes of liver disease. Cytokeratin 18 emerges as a non-invasive marker for the assessment of liver fibrosis. The objective of the work was to validate the use of cytokeratin 18 in peripheral blood in the diagnosis and evolution of patients with non-alcoholic fatty liver disease. Methodology: To validate cytokeratin 18 in the diagnosis, a case-control study was carried out. The case group was patients over 18 years of age, of both sexes, with a diagnosis of non-alcoholic fatty liver disease linked to metabolic syndrome, recruited between 2/2/2019 to 2/2/2020. The control group were blood donors. They were matched 1-1 for age and sex. Cytokeratin 18 was quantified in peripheral blood of both groups. To validate cytokeratin 18 in the evolution of patients with non-alcoholic fatty liver disease, a prospective, longitudinal study was carried out. The group of patients recruited were followed for one year under standard treatment, at the end of which cytokeratin 18 was quantified in peripheral blood. Continuous variables are expressed with the mean and standard deviation. It was analyzed with Student's t test, α error < 5%. Results: 13 patients make up the case group (12 women), 53 ± 11 years old, with BMI 35.01 ± 8.9 kg/m2. The pre-treatment cytokeratin 18 value was 1410 ± 120 IU, and the post-treatment value was 117 ± 56, p < 0.005. The control group was 13 people (12 women), 43.4 ± 8.1 years and BMI 28.10 ± 5.4 kg/m2 The cytokeratin 18 value was 193 ± 7.2 IU, p < 0.005 vs. pretreatment case group. Conclusions: Cytokeratin 18 is higher in patients with non-alcoholic fatty liver disease, being statistically significant, and decreases with treatment with statistical significance, and may become a useful marker in this group of patients.


Introdução: A doença hepática gordurosa não alcoólica é uma das principais causas de doença hepática. A citoqueratina 18 surge como um marcador não invasivo para avaliação de fibrose hepática. O objetivo do trabalho foi validar o uso da citoqueratina 18 no sangue periférico no diagnóstico e evolução de pacientes com doença hepática gordurosa não alcoólica. Metodologia: Para validar a citoqueratina 18 no diagnóstico, foi realizado um estudo caso-controle. O grupo caso foi composto por pacientes maiores de 18 anos, de ambos os sexos, com diagnóstico de doença hepática gordurosa não alcoólica ligada à síndrome metabólica, recrutados entre 02/02/2019 a 02/02/2020. O grupo controle eram doadores de sangue. Eles foram comparados em 1 a 1 por idade e sexo. A citoqueratina 18 foi quantificada no sangue periférico de ambos os grupos. Para validar a citoqueratina 18 na evolução de pacientes com doença hepática gordurosa não alcoólica, foi realizado um estudo prospectivo e longitudinal. O grupo de pacientes recrutados foi acompanhado durante um ano sob tratamento padrão, ao final do qual a citoqueratina 18 foi quantificada no sangue periférico. As variáveis ​​contínuas são expressas com média e desvio padrão. Foi analisado com teste t de Student, erro α < 5%. Resultados: Compõem o grupo caso 13 pacientes (12 mulheres), 53 ± 11 anos, com IMC 35,01 ± 8,9 kg/m2. O valor de citoqueratina 18 pré-tratamento foi de 1410 ± 120 UI e o valor pós-tratamento foi de 117 ± 56, p < 0,005. O grupo controle foi de 13 pessoas (12 mulheres), 43,4 ± 8,1 anos e IMC 28,10 ± 5,4 kg/m2 O valor da citoqueratina 18 foi de 193 ± 7,2 UI, p < 0,005 vs. grupo de casos pré-tratamento. Conclusões: A citoqueratina 18 é maior em pacientes com doença hepática gordurosa não alcoólica, sendo estatisticamente significativa, e diminui com o tratamento com significância estatística, podendo se tornar um marcador útil neste grupo de pacientes.

ABSTRACT

Hábitos alimentares inadequados, sedentarismo e a maior expectativa de vida da população contribuem significativamente para a prevalência da síndrome metabólica. Essa doença predispõe uma pessoa a desenvolver diabetes mellitus tipo 2 e doenças cardiovasculares, as quais têm um amplo impacto na saúde pública, induzindo sobrecarga no sistema de saúde e reduzindo a qualidade de vida dos indivíduos afetados. A síndrome metabólica é uma doença multifatorial e está relacionada ao processo de envelhecimento, contudo, ainda há uma lacuna significativa, em termos de estudos, sobre a prevalência da condição em populações idosas. Nesse contexto, o presente estudo objetivou rastrear a prevalência da síndrome metabólica em participantes da Universidade Aberta da Terceira Idade (UNATI), localizada em Francisco Beltrão, Paraná. Os critérios diagnósticos de síndrome metabólica abordados nesta pesquisa incluem: circunferência abdominal ≥ 90 cm para homens e ≥ 80 cm para mulheres, triglicerídeos ≥ 150 mg/dL, HDL ≤ 40 mg/dL para homens e ≤ 50 mg/dL para mulheres, pressão arterial sistólica ≥ 130 mmHg e/ou pressão arterial diastólica ≥ 85 mmHg ou estar em farmacoterapia para hipertensão, além de glicemia de jejum ≥ 100 mg/dL ou estar em tratamento farmacológico para diabetes. Um total de 44 idosos foram avaliados, apresentando uma média de idade de 66,9 ± 7,1 anos, com uma predominância de mulheres (88%). Os resultados revelaram uma prevalência alarmante de síndrome metabólica, atingindo 36,4% da amostra estudada. Além disso, observou-se uma alta prevalência de condições associadas, como hipertensão arterial (67,2%), sobrepeso (58,6%) e obesidade visceral (31%). Esses achados ressaltam a importância da implementação de medidas preventivas direcionadas à promoção da qualidade de vida saudável e ao controle dos fatores de risco metabólicos.


Inadequate dietary habits, sedentary lifestyle, and increased life expectancy significantly contribute to the prevalence of metabolic syndrome. This condition predisposes an individual to develop type 2 diabetes mellitus and cardiovascular diseases, which have a broad impact on public health, inducing a burden on the healthcare system and reducing the quality of life of affected individuals. Metabolic syndrome is a multifactorial disease and is related to the aging process; however, there is still a significant gap in terms of studies on the prevalence of the condition in elderly populations. In this context, this study aimed to screen the prevalence of metabolic syndrome in participants of the Open University for the Third Age (UNATI), located in Francisco Beltrão, Paraná. The diagnostic criteria for metabolic syndrome addressed in this research include: abdominal circumference ≥ 90 cm for men and ≥ 80 cm for women, triglycerides ≥ 150 mg/dL, HDL ≤ 40 mg/dL for men and ≤ 50 mg/dL for women, systolic blood pressure ≥ 130 mmHg and/or diastolic blood pressure ≥ 85 mmHg or being on pharmacotherapy for hypertension, in addition to fasting glucose ≥ 100 mg/dL or being on pharmacological treatment for diabetes. A total of 44 elderly individuals were evaluated, with a mean age of 66.9 ± 7.1 years, predominantly women (88%). The results revealed an alarming prevalence of metabolic syndrome, affecting 36.4% of the studied sample. Furthermore, a high prevalence of associated conditions was observed, such as arterial hypertension (67.2%), overweight (58.6%), and visceral obesity (31%). These findings underscore the importance of implementing preventive measures aimed at promoting healthy lifestyles and controlling metabolic risk factors.

El síndrome metabólico (SM) se previene controlando sus factores de riesgo, el programa de reforma de vida (PRV) busca el control y regulación de estos. Objetivo: evaluar el impacto del PRV. Materiales y métodos: Investigación aplicativo, longitudinal, prospectivo, diseño longitudinal, inductivo y deductivo, en una población inicial de 104 trabajadores de una universidad del Perú, la población final fue 31 quienes completaron el programa, los factores de riesgo fueron evaluados por un laboratorista clínico, se utilizó fichas de recolección y se procesó con el programa SPSS V.26, prueba de Wilcoxon y T de Student según el criterio de normalidad. Resultados: el PRV disminuye los niveles de triglicéridos, promedio antes 235,6 mg/dl y después 196,1 mg/dl, no se evidencio efectos positivos en el resto de los factores de riesgo. En personas con algún factor de riesgo el PRV impacto positivamente; antes del PRV 2 personas tenían la glucosa >= a 100 mg/dl y después del PRV 1, en cuanto al perímetro abdominal ≥ 90cm M, ≥ 80cm F antes (30), después (19), presión arterial ≥130/85 mmHg antes (06), después (0), triglicéridos ≥ 150 mg/dl antes (30), después (17), C-HDL < 40 M < 50 F antes (29), después (24). Con un p valor de 0,004 el PRV disminuye los niveles de triglicéridos. Conclusión: En la población en general el PRV disminuye el nivel de triglicéridos; en personas con algún factor de riesgo el PRV controla y regula todos los factores SM.

ABSTRACT

A síndrome metabólica é um distúrbio metabólico complexo, caraterizada pela associação de fatores de risco cardiovascular e resistência à insulina. Na Atenção Primária à Saúde, algumas condições laborais as quais os profissionais estão expostos podem ser fonte de adoecimento, e a literatura evidencia que aspectos laborais estão associados à síndrome metabólica, ou seja, que o contexto ocupacional é capaz de ser um fator de exposição para o desenvolvimento deste problema. O objetivo deste estudo foi estimar a prevalência de síndrome metabólica e os fatores associados em Agentes Comunitários de Saúde de uma cidade do Norte de Minas Gerais, Brasil. Estudo transversal, no qual coletaram dados sociodemográficos, estilo de vida, laborais, antropométricos, bioquímicos e aspectos emocionais. A variável dependente síndrome metabólica foi definida conforme o critério do National Cholesterol Education Program Adult Treatment Panel III. Realizaram-se análises descritivas e de regressão múltipla de Poisson, com variância robusta, considerando um nível de significância de 5% (p<0,05) para o modelo final. Foram avaliados 673 Agentes Comunitários de Saúde, a prevalência de síndrome metabólica foi de 20,8% e associou-se à faixa etária ≥40 anos, menor escolaridade (Fundamental e/ou Médio), sobrepeso/obesidade, lipoproteína de baixa densidade ≥130 mg/dl e proteína C-reativa >5,0 mg/dl. Constatou-se uma elevada prevalência de síndrome metabólica em Agentes Comunitários de Saúde. Verifica-se a necessidade de estudos para o aprofundamento sobre a temática e o desenvolvimento de ações que visem a promoção de hábitos comportamentais saudáveis, bem como a prevenção de fatores de riscos.


Metabolic syndrome is a complex metabolic disorder, characterized by the association of cardiovascular risk factors and insulin resistance. In Primary Health Care, some work conditions to which professionals are exposed can be a source of illness and the literature shows that work aspects are associated with metabolic syndrome, that is, that the occupational context is capable of being an exposure factor for the development of this problem. The objective of this study was to estimate the prevalence of metabolic syndrome and associated factors among community health workers in a city in the North of Minas Gerais, Brazil. Cross-sectional study, in which sociodemographic, lifestyle, work, anthropometric, biochemical, and emotional aspects of data were collected. The dependent variable metabolic syndrome was defined according to the National Cholesterol Education Program Adult Treatment Panel III criteria. Descriptive and multiple Poisson regression analyzes were carried out with robust variance, considering a significance level of 5% (p<0.05) for the final model. 673 community health workers were evaluated, the prevalence of metabolic syndrome was 20.8% and was associated with the age group ≥40 years, lower education (elementary and/or secondary), overweight/obesity, low-density lipoprotein ≥130 mg/dl and C-reactive protein >5.0 mg/dl. A high prevalence of metabolic syndrome was found in community health workers. There is a need for studies to delve deeper into the topic and develop actions aimed at promoting healthy behavioral habits, as well as preventing risk factors.

Introdução: O diagnóstico precoce quanto às perdas auditivas é essencial para minimização do impacto social em relação à rotina laboral e na qualidade de vida. Objetivo: Caracterizar a associação entre a perda auditiva em trabalhadores com doenças metabólicas. Método: Estudo transversal retrospectivo de dados secundários de prontuário com o tratamento das doenças metabólicas, os dados foram coletados em duas clínicas de saúde ocupacional (C1 e C2) em Florianópolis - Santa Catarina (Brasil), no período de janeiro de 2020 a dezembro de 2022, considerando exames referenciais a partir do ano de 2005. Os dados foram organizados em planilhas do programa Microsoft Excel® e, posteriormente, exportados e analisados no software MedCalc® Statistical Software versão 22.006. Resultados: Foram analisados dados de 97 pacientes (71 homens e 26 mulheres), expostos ao ruído ocupacional (p = 0,0047), com diagnóstico de ao menos uma doença metabólica (41,20%) e prevalência de medicamentos da classe ATC H (p = 0,0465) e Losartana® (OR = 1,6976). Conclusão: O ruído ocupacional é o principal fator de risco auditivo nas empresas analisadas, e a presença de doença metabólica poderá influenciar em alterações dos limiares auditivos. Para reduzir a vulnerabilidade dessa população, é necessário a promoção, educação e conscientização dos trabalhadores nos aspectos de saúde. (AU)


Introduction: Early diagnosis of hearing loss is essential to minimize the social impact in relation to work routine and quality of life. Objective: Analyze the association between hearing loss in workers and metabolic diseases. Method: Retrospective cross-sectional study of secondary data on the use of medications in metabolic diseases, the data were collected in two occupational health clinics (C1 and C2) in Florianópolis - Santa Catarina (Brazil), from January 2020 to December 2022, considering references from exams from the year 2005. The data were organized in Microsoft Excel® spreadsheets and subsequently exported and analyzed using the MedCalc® statistical software version 22.006. Results: The data of 97 patients (71 men and 26 women) exposed to occupational noise (p = 0.0047), diagnosed with at least one metabolic disease (41.20%) and prevalence of ATC H class medications (p = 0.0465) and Losartan® (OR = 1.6976). Discussion andConclusion: Occupational noise is the main auditory risk factor, and the presence of metabolic disease can influence hearing thresholds. To reduce the vulnerability of this population, it is necessary to promote, raise awareness and educate, using approaches related to health aspects at work. (AU)

Los datos fueron organizados en hojas de cálculo Microsoft Excel®. y posteriormente exportados y analizados. utilizando el software estadístico MedCalc® versión 22.006. Resultados: Se analizaron los datos de 97 pacientes (71 hombres y 26 mujeres) expuestos a ruido ocupacional (p = 0,0047), diagnosticados con al menos una enfermedad metabólica (41,20%) y prevalencia de medicamentos clase ATC H (p = 0,0465). y Losartan® (OR = 1,6976). Discusión y Conclusión: El ruido ocupacional es el principal factor de riesgo auditivo y la presencia de enfermedad metabólica puede influir en los umbrales auditivos. Para reducir la vulnerabilidad de esta población es necesario promover, sensibilizar y educar utilizando enfoques relacionados con aspectos de salud en el trabajo. (AU)

ABSTRACT

Objective.To assess the effectiveness of an educational intervention on perceived stress and metabolic syndrome parameters among adults with type 2 diabetes mellitus. Method. Fifty-one adults (aged 48.73±7.84; 86.3% of women) were included in a non-randomized clinical trial performed in a healthcare unit for six months (Brazilian Clinical Trial Registry: RBR-43K52N). All participants were diagnosed with type 2 diabetes mellitus and metabolic syndrome (intervention group, n=26; control group, n=25). The intervention consisted of a nurse-led educational health-promoting program with a multidisciplinary approach organized in seven workshops. The primary outcome was decreased perceived stress, and the secondary outcome was improvement in metabolic syndrome parameters according to perceived stress levels. These outcomes were assessed at two points in time, at the baseline and follow-up. Results. Participation in the intervention program resulted in a significant decrease in perceived stress (p=0.028). The stressed participants in the intervention group experienced a significant decrease in blood glucose levels (p=0.001) and a significant increase in high-density lipoprotein-cholesterol (p=0.003) concentrations after the six-month intervention. Conclusion.The nurse-led educational health-promoting program decreased perceived stress among adults with type 2 diabetes mellitus and metabolic syndrome, improving fasting blood glucose and high-density lipoprotein cholesterol among the stressed participants in the intervention group.


Objetivo. Evaluar la efectividad de una intervención educativa sobre el estrés percibido y los componentes del síndrome metabólico en adultos con diabetes mellitus tipo 2. Métodos. Se incluyeron 51 adultos (48.73±7.84 años; 86.3% mujeres) de un estudio no-ensayo aleatorizado realizado en una unidad de salud durante seis meses, con Registro Brasileño de Ensayos Clínicos: RBR-43K52N, todos los participantes fueron diagnosticados con diabetes mellitus tipo 2 y síndrome metabólico (grupo intervención, n=26; grupo control, n=25). La intervención consistió en un programa educativo de promoción de la salud con enfoque multidisciplinario, liderado por una enfermera, estructurado en siete talleres grupales. El resultado primario fue la reducción del estrés percibido y el secundario, la mejora de los componentes del síndrome metabólico influenciados por el nivel de estrés percibido, evaluado en dos momentos, al inicio y después del seguimiento. Resultados. La participación en el programa de intervención resultó en una reducción significativa del estrés percibido en comparación con el grupo control (p=0.028). Los participantes estresados en el grupo de intervención tuvieron, respectivamente, una disminución y un aumento significativos en las concentraciones séricas de glucosa (p=0.001) y lipoproteínas de alta densidad-colesterol (p=0.003) después de seis meses de intervención. Conclusión. Un programa educativo de promoción de la salud liderado por enfermeras fue eficiente para reducir el estrés percibido entre adultos con diabetes mellitus tipo 2 y síndrome metabólico, además de mejorar la glucemia en ayunas y el colesterol unido a lipoproteínas de alta densidad en los participantes del grupo estresado de intervención.

Foram incluídos 51 adultos (48.73±7.84 anos de idade; 86.3% mulheres) em um ensaio clínico não-randomizado realizado em uma unidade de saúde durante seis meses, com Registro de Ensaio Clínico Brasileiro: RBR-43K52N.Todos os participantes apresentavam diagnóstico de diabetes mellitus tipo 2 e síndrome metabólica (grupo intervenção, n=26; grupo controle, n=25). A intervenção consistiu em um programa educativo de promoção da saúde com abordagem multidisciplinar, liderado por enfermeiro, estruturado em sete oficinas em grupo. O desfecho primário foi a redução do estresse percebido, e o secundário, a melhora dos componentes da síndrome metabólica conforme influência do nível de estresse percebido, avaliados em dois momentos, na condição basal e após o acompanhamento. Resultados. A participação no programa de intervenção resultou na redução significativa do estresse percebido em comparação com o grupo controle (p=0.028). Os participantes estressados do grupo intervenção tiveram, respectivamente, diminuição e aumento significativos das concentrações séricas de glicose (p=0.001) e da lipoproteína-colesterol de alta densidade (p=0.003) após seis meses de intervenção. Conclusão. Um programa educativo de promoção da saúde liderado por enfermeiros foi eficiente para reduzir estresse percebido entre adultos com diabetes mellitus tipo 2 e síndrome metabólica, além de causar melhora da glicemia de jejum e e da lipoproteína-colesterol de alta densidade dos participantes estressados do grupo intervenção.

RESUMEN Introducción: Existe evidencia reciente que establecería a la hipoperfusión muscular como causa primaria de trastornos metabólicos en respuesta a la sobrealimentación. Esta concepción centrípeta del desarrollo de trastornos metabólicos podría implicar no solo alteraciones en la microvasculatura, sino también afectación en las arterias de conductancia. Objetivos: 1) Determinar la asociación entre diámetro basal de la arteria humeral (D-Hum) y la vasodilatación mediada por flujo (VDMF) 2) Analizar la asociación de ambos parámetros conforme aumenta de la masa corporal 3) Evaluar asociaciones entre el D-Hum/VDMF con componentes del síndrome metabólico (SM) 4) Evaluar la asociación independiente de ambas variables con el SM. Material y métodos: Se evaluaron 3493 pacientes. Se excluyeron pacientes < 18 y >80 años, con patología cardiovascular previa, insuficiencia renal crónica (IRC), colagenopatías, y tratados con estatinas. Se determinó presión arterial (PA), parámetros antropométricos y perfil metabólico, y se clasificó a los sujetos de acuerdo con la presencia de SM según AHA/NHLBI 2019. Se midieron D-Hum en mm y VDMF en %. Se analizó la asociación lineal entre D-Hum y VDMF y se analizaron ambas variables según decilos de índice de masa corporal (IMC). Se evaluaron asociaciones entre D-Hum/VDMF con la PA, glucemia (Glu), triglicéridos (TG) y colesterol de alta densidad (HDLc). Se realizaron dos regresiones logísticas con SM como variable dependiente y D-Hum o VDMF más edad, sexo, IMC y factores de riesgo coronario (FRC) como independientes. Resultados: Ingresaron 1995 pacientes (48,2 ± 11 años, 56 % hombres). El D-Hum y la VDMF presentaron una asociación inversa (r= -0,42; p < 0,0001). El D-Hum aumentó según decilos del IMC (p < 0,000001); la VDMF mostró relación inversa con los decilos crecientes de IMC (p < 0,000001). El D-Hum presentó correlación directa con PA, Glu y TG; e inversa con HDLc (p < 0,05 en todos los casos). La VDMF mostró correlación inversa con PA, Glu y TG; y directa con HDLc (p < 0,05 en todos los casos). El D-Hum se asoció en forma independiente con el SM ajustado por edad, sexo, IMC y FRC (OR 1,42, p = 0,0019), mientras que la VDMF no (OR 0,98, p = 0,217). Conclusión: El remodelado vascular excéntrico se asoció con un compromiso en la adaptación vascular ante aumentos en la demanda de flujo sanguíneo y con alteraciones metabólicas a lo largo del incremento de la masa corporal. Así, el compromiso dinámico de la vasculatura podría tener un rol determinante en el desarrollo de alteraciones metabólicas en forma sincrónica con la ganancia de peso.


ABSTRACT Background: Recent evidence would establish muscle hypoperfusion as the primary cause of metabolic disorders in response to overfeeding. This centripetal concept on the development of metabolic disorders could involve not only alterations in the microvasculature, but also affect the conductance arteries. Objectives: The aim of this study was 1) to determine the association between baseline brachial artery diameter (BAD) and flow-mediated vasodilation (FMVD), 2) To analyze the association of both parameters throughout the increase in body mass, 3) To evaluate associations between BAD/FMVD with components of the metabolic syndrome (MS) and 4) To evaluate the independent association of both variables with MS. Methods: A total of 3493 patients were evaluated. Patients <18 and >80 years old, those with previous cardiovascular disease, chronic kidney disease (CKD), collagenopathies, or treated with statins were excluded from the study. Blood pressure (BP), anthropometric parameters and metabolic profile were determined, and the subjects were classified according to the presence of MS conforming AHA/NHLBI 2019 criteria. BAD was measured in mm and FMVD as percentage. The linear association between BAD and FMVD was assessed, and both variables were analyzed according to deciles of body mass index (BMI). Associations between BAD/FMVD with BP, glucose (Glu), triglycerides (TG) and high-density cholesterol (HDL-C) levels were evaluated. Two logistic regression analyses were performed with MS as dependent variable and BAD or FMVD plus age, gender, BMI, and coronary risk factors (CRF) as independent variables. Results: A total of 1995 patients (48.2 ± 11 years, 56% men) were admitted in the study. An inverse correlation was found between BAD and FMVD (r= -0.42; p < 0.0001). BAD increased according to deciles of BMI (p < 0.000001), while FMVD showed an inverse relationship with increasing deciles of BMI (p < 0.000001). BAD exhibited a direct correlation with BP, Glu and TG; and an inverse relationship with HDL-C (p < 0.05 in all cases). FMVD presented an inverse correlation with BP, Glu and TG; and a direct correlation with HDL-C (p < 0.05 in all cases). BAD was independently associated with MS adjusted for age, gender, BMI and CRF (OR 1.42, p=0.0019), while FMVD was not (OR 0.98, p = 0.217). Conclusion: Eccentric vascular remodeling was associated with vascular adaptation to increased blood flow demand and with metabolic alterations throughout the increase in body mass. Thus, the dynamic compromise of vasculature could play a decisive role in the development of metabolic alterations occurring synchronously with weight gain.

Introducción: La obesidad, uno de los problemas de salud pública más grave del siglo XXI, afecta de manera general a todos los países. Objetivo: Caracterizar desde el punto de vista clínico y humoral a niños y adolescentes obesos con síndrome metabólico. Métodos: Se realizó un estudio observacional, descriptivo y de corte transversal de enero de 2018 a diciembre de 2022 en 106 niños y adolescentes con el diagnóstico de síndrome metabólico, atendidos por consulta externa de obesidad del Hospital Militar Central "Dr. Luis Díaz Soto". Las variables a medir fueron: edad, sexo, grado de obesidad según índice de masa corporal, hipertensión arterial, glucemia en ayunas alteradas, hipertrigliceridemia y la HDL-c. Resultados: Predominó el grupo de edades entre 15-18 años (51,9 %), el sexo masculino (58,5 %), mientras que la hipertrigliceridemia (61,3 %) y la obesidad moderada (60,4 %) tuvieron mayor frecuencia. Por otro lado, la hipertensión arterial, glucemia en ayuna alterada, y la dislipidemia fueron más frecuentes en los pacientes con obesidad moderada e intensa. Conclusiones: Los componentes clínicos del síndrome metabólico, como el índice de masa corporal y la hipertensión arterial; y los humorales, como la dislipidemia y la glucemia en ayunas alterada, tienen mayor frecuencia en la medida que aumenta el grado de obesidad.


Introduction: Obesity, one of the most serious public health problems of the 21st century, generally affects all countries. Objective: To characterize obese children and adolescents with metabolic syndrome from a clinical and humoral point of view. Methods: An observational, descriptive and cross-sectional study was conducted from January 2018 to December 2022 in 106 children and adolescents with the diagnosis of metabolic syndrome attended by the obesity outpatient clinic of the Central Military Hospital "Dr. Luis Díaz Soto". The variables to be measured were: age, sex, degree of obesity according to body mass index, arterial hypertension, altered fasting blood glucose, hypertriglyceridemia and HDL-c. Results: The age group between 15-18 years (51.9%), male sex (58.5%) predominated, while hypertriglyceridemia (61.3%) and moderate obesity (60.4%) had a higher incidence. On the other hand, arterial hypertension, altered fasting blood glucose and dyslipidemia were more frequent in patients with moderate and severe obesity. Conclusions: The clinical components of the metabolic syndrome such as body mass index and arterial hypertension, and the humoral components such as dyslipidemia and altered fasting blood glucose have a greater incidence as the degree of obesity increases.

Introducción: Se reconoce la asociación entre los factores de riesgo aterogénico y las alteraciones microvasculares de la retina, pero no hay consenso sobre si estas afectaciones en la retina preceden o son una respuesta fisiopatológica a dichos factores. Objetivo: Determinar si la presencia de los factores de riesgo aterogénico predice las alteraciones vasculares retinianas, a través del fondo de ojo y la retinografía. Métodos: Estudio trasversal en 55 sujetos mayores de 19 años de edad, de cualquier sexo, sin opacidades en los medios transparentes del ojo. Se estudiaron las variables edad, sexo, dislipidemia, hábito de fumar, consumo de alcohol, hipertensión arterial, diabetes mellitus tipo 2, presión arterial sistólica y diastólica, índice de masa corporal, colesterol, glicemia, triglicéridos, creatinina, lipoproteínas de alta densidad, urea, eritrosedimentación y conteo leucocitario. Resultados: El 65,45 % presentó alteraciones en el fondo de ojo: aumento del brillo arteriolar (53,03 %) y disminución del calibre arteriolar generalizado (52,24 %). La retinografía mostró daño en el 58,18 %: rectificación de los cruces arteriovenosos (65,71 %), tortuosidad venosa (28,21 %) y cruces arteriovenosos con aplastamiento (85,71 %). El aumento del colesterol sérico (p= 0,003) se asoció con la presión arterial sistólica (p= 0,037) en el fondo de ojo, y con el antecedente de hipertensión arterial (p= 0,023) en la retinografía. Conclusiones: El colesterol sérico, las cifras elevadas de tensión arterial sistólica y antecedentes de hipertensión arterial son los factores de riesgo que mejor predicen el daño vascular retinal.


Introduction: The association between atherogenic risk factors and retinal microvascular alterations is recognized, but there is no consensus on whether these retinal disorders precede or are a pathophysiological response to these factors. Objective: To determine if the presence of atherogenic risk factors predicts retinal vascular alterations, through fundus examination and retinography. Methods: Cross-sectional study in 55 subjects over 19 years of age, of either sex, without opacities in the transparent media of the eye. The variables studied were age, sex, dyslipidemia, smoking habit, alcohol consumption, arterial hypertension, type 2 diabetes mellitus, systolic and diastolic blood pressure, body mass index, cholesterol, glycemia, triglycerides, creatinine, high-density lipoproteins, urea, erythrocyte sedimentation rate and leukocyte count. Results: 65.45% presented alterations in the fundus of the eye: increased arteriolar brightness (53.03%) and decreased generalized arteriolar caliber (52.24%). Retinography showed damage in 58.18%: rectification of arteriovenous crossings (65.71%), venous tortuosity (28.21%), and arteriovenous crossings with crushing (85.71%). The increase in serum cholesterol (p= 0.003) was associated with systolic blood pressure (p= 0.037) in the fundus, and with a history of arterial hypertension (p= 0.023) in retinography. Conclusions: Serum cholesterol, high systolic blood pressure and a history of hypertension are the risk factors that best predict retinal vascular damage.

Abstract Diabetic-metabolic syndrome (MetS-D) has a high prevalence worldwide, in which an association with the rupture of the intestinal epithelium barrier function (IEBF) has been pointed out, but the functional and morphological properties are still not well understood. This study aimed to evaluate the impact of acute hyperglycemia diabetes on intestinal tight junction proteins, metabolic failure, intestinal ion and water transports, and IEBF parameters. Diabetes was induced in male Rattus norvegicus (200-310 g) with 0.5 mL of streptozotocin (70 mg/kg). Glycemic and clinical parameters were evaluated every 7 days, and intestinal parameters were evaluated on the 14th day. The MetS-D animals showed a clinical pattern of hyperglycemia, with increases in the area of villi and crypts, lactulose:mannitol ratio, myeloperoxidase (MPO) activity, and intestinal tissue concentrations of malondialdehyde (MDA), but showed a reduction in reduced glutathione (GSH) when these parameters were compared to the control. The MetS-D group had increased secretion of Na+, K+, Cl-, and water compared to the control group in ileal tissue. Furthermore, we observed a reduction in mRNA transcript of claudin-2, claudin-15, and NHE3 and increases of SGLT-1 and ZO-1 in the MetS-D group. These results showed that MetS-D triggered intestinal tissue inflammation, oxidative stress, complex alterations in gene regulatory protein transcriptions of intestinal transporters and tight junctions, damaging the IEBF and causing hydroelectrolyte secretion.

ABSTRACT

Accumulation of visceral adipose tissue is associated with metabolic syndrome (MS), insulin resistance, and dyslipidemia. Here we examined several morphometric and biochemical parameters linked to MS in a rodent litter size reduction model, and how a 30-day fish oil (FO) supplementation affected these parameters. On day 3 post-birth, pups were divided into groups of ten or three. On day 22, rats were split into control (C) and small litter (SL) until 60 days old. Then, after metabolic disturbance and obesity were confirmed, FO supplementation started for 30 days and the new groups were named control (C), FO supplemented (FO), obese (Ob), and obese FO supplemented (ObFO). Comparison was performed by Student t-test or 2-way ANOVA followed by Tukey post hoc test. At the end of the 60-day period, SL rats were hyperphagic, obese, hypoinsulinemic, normoglycemic, and had high visceral fat depot and high interleukin (IL)-6 plasma concentration. Obese rats at 90 days of age were fatter, hyperphagic, hyperglycemic, hypertriacylgliceromic, hipoinsulinemic, with low innate immune response. IL-6 production ex vivo was higher, but in plasma it was not different from the control group. FO supplementation brought all biochemical changes to normal values, normalized food intake, and reduced body weight and fat mass in obese rats. The innate immune response was improved but still not as efficient as in lean animals. Our results suggested that as soon MS appears, FO supplementation must be used to ameliorate the morpho- and biochemical effects caused by MS and improve the innate immune response.

Abstract Background: There are few studies dedicated to the characterization of the geriatric population with psoriasis, which has particularities in terms of clinical manifestations and therapeutic limitations. As psoriasis is a chronic disease, presenting a higher prevalence with age, the increase in life expectancy in Brazil demands knowledge about the behavior of the disease among the elderly. Objectives: To characterize elderly people with psoriasis from a tertiary service, from the clinical-epidemiological point of view, presence of comorbidities, physical frailty, and affective impact, and to compare these aspects with adults with psoriasis and elderly people without the disease. Methods: Cross-sectional study of 64 elderly patients with psoriasis, 64 adults with psoriasis, and 64 elderly patients without the disease. Clinical-demographic aspects, the Beck depression scale, and Skindex-16 were evaluated. Indicators of physical frailty were evaluated in elderly patients: handgrip, sit-to-stand test, fatigue, and weight loss >5%. Results: In the elderly, the mean age (SD) of psoriasis onset was 44 (10) years, men represented 47% of the sample, the prevalence of arthritis was 22%, and ungual involvement occurred in 72%. Topical corticosteroids were used more often among elderly people with psoriasis (100%) than among adults with the disease (86%), with no difference among other systemic treatments. Diabetes mellitus occurred in 30% of the elderly. Hypertension (59%), dyslipidemia (52%), depression (34%), and fatigue (59%) were more prevalent among the elderly with psoriasis than among the healthy controls.

Objective To investigate the clinical features of patients with recurrent acute pancreatitis(RAP)complicated with metabolic syndrome(MS)and the influencing factors of severe disease.Methods The clini-cal data of 382 RAP patients admitted to the hospital from June 2012 to June 2022 were retrospectively analyzed,and they were divided into the MS group(n=142)and the non-MS group(n=240)according to whether they were combined with MS,and into the severe group(n=29)and the non-severe group(n=353)according to the severity.The general data,serological parameters[triglyceride(TG),total cholesterol(TC),white blood cell count(WBC),neutrophil to lymphocyte ratio(NLR),blood calcium,D-dimer(D-D),lactate dehydrogen-ase(LDH),ALT,AST]and ICU occupancy rate and total length of stay were compared among all groups.Bi-nary logistic regression was used to analyze the independent influencing factors of RAP development into se-vere disease,and receiver operating characteristic(ROC)curve was used to analyze the predictive value of each indicator for RAP development.Results Hyperlipidemia was the most common cause of RAP in MS group(66.2%),and biliary origin was the most common cause of RAP in non-MS group(44.6%).There was sig-nificant difference among different causes was statistically significant(P<0.05).There were significant differences in age,gender,proportion of hypertension,diabetes,MS,length of stay,LDH,blood calcium,D-D and NLR levels between the critical and non-critical groups(P<0.05).The area under the curve(AUC)of blood calcium,D-D,LDH,NLR and combined diagnosis were 0.759,0.777,0.710,0.621,and 0.841,respec-tively.The AUC of single diagnosis had a certain predictive value,but combined diagnosis had a higher predic-tive value.Conclusion The most common cause in the MS group was hyperlipidemia,and the most common cause in the non-MS group was biliary.Blood calcium.D-D,LDH,NLR are reliable indicators to predict the development of RAP into severe disease,and the accuracy of combined diagnosis is higher.

Objective To determine the quantitative diagnostic criteria of phlegm-stasis interjunction syndrome of metabolic syndrome.Methods Based on the literature research,the Expert Consultation Questionnaire for MS Syndrome of Phlegm-stasis Interjunction based on Analytic Hierarchy Process was developed.Experts were invited to fill out the questionnaire,and the Diagnostic Criteria for MS Syndrome of Phlegm-stasis Interjunction(Draft)was constructed by using the expert group decision algorithm.The patients with MS were prospectively collected,and the diagnostic criteria for phlegm-stasis interjunction syndrome of MS were validated and revised by conditional probability conversion method and maximum likelihood discrimination,and the quantitative diagnostic criteria for phlegm-stasis interjunction syndrome of MS were finally formulated.Results The sensitivity,specificity and accuracy of the established quantitative diagnostic criteria for MS phlegm-stasis interjunction syndrome were 95.7%,84.0%,91.6%,positive likelihood ratio 5.98,negative likelihood ratio 19.46.Conclusion The quantitative diagnostic criteria for MS phlegm-stasis interjunction syndrome established based on literature research,expert consultation and diagnostic tests have good diagnostic efficacy.

Objective:To analyze the current research status and hot spots of metabolic syndrome after renal transplantation, and provide reference for domestic research in this field.Methods:Computer retrieval of the literature related to renal transplantation metabolic syndrome in the Chinese Biomedical Literature Service System and the Web of Science core collection database was conducted from January First, 2002 to December 31, 2022. The retrieval results were analyzed using Citespace.6.1.R3c software.Results:After screening, a total of 1024 papers related to metabolic syndrome of renal transplantation were included, including 409 Chinese papers and 615 English papers. In the past 20 years, the number of papers related to metabolic syndrome of renal transplantation in foreign countries has increased progressively, and the overall domestic literature has not increased significantly. Domestic and international research focuses mainly on the epidemiology, pathogenesis, risk factors and related hazards of metabolic syndrome in renal transplantation.Conclusions:The research on metabolic syndrome in renal transplantation has received more and more attention, and still has great research prospects. The risk factors and intervention methods of metabolic syndrome in renal transplantation have been the research focus of scholars at home and abroad in recent years. Chinese scholars can further explore on the basis of previous research, strengthen the exchange and cooperation between different fields, institutions and countries, so as to optimize and improve the related research of metabolic syndrome in kidney transplantation.

ABSTRACT

Objective:To investigate the diagnostic value of non-high-density lipoprotein cholesterol (non-HDL-C),ratio of triglyceride to high-density lipoprotein cholesterol (TG/HDL-C), and triglyceride glucose index (TyG) on metabolic syndrome (MS) in adult women.Methods:This was a cross-sectional study. A total of 24 410 adult women who received health examination in health management center of the Affiliated Hospital of Southwest Medical University were selected from January 2019 to December 2021 as subjects. The subjects′ basic information, physical examination results, and laboratory examination data were collected retrospectively. The relationship between non-HDL-C, TG/HDL-C, TyG, and MS in adult women were examined using multivariate logistic regression analysis. The receiver operating characteristic (ROC) curves were constructed and the area under the curve (AUC) were calculated to evaluate the diagnostic value of each indicator for MS in adult women.Results:Among 24 410 adult females, 800 (3.3%) were found to have MS. After adjusting for age, body mass index, waist circumference, hip circumference, systolic blood pressure, diastolic blood pressure, blood uric acid, history of hypertension, history of diabetes, fatty liver, non HDL-C ( OR=1.608), TG/HDL-C ( OR=1.311), TyG ( OR=13.288) were all risk factors for MS in adult women. non-HDL-C, TG/HDL-C, and TyG, as well as their combined AUC of ROC, were 0.795 (95% CI: 0.742-0.776), 0.909 (95% CI: 0.902-0.917), 0.942 (95% CI: 0.937-0.948), and 0.944 (95% CI: 0.937-0.950), respectively. TyG had the highest diagnostic value for MS in adult women among the three indicators, the optimal cutoff value for TyG was 8.237, with a sensitivity of 93.5% and a specificity of 85.5%. Conclusion:non-HDL-C, TG/HDL-C, TyG, as well as their combination, all demonstrate good diagnostic value for MS in adult women.

ABSTRACT

Objective:To investigate the association between quantified CT (QCT)-measured body composition and metabolic syndrome (MS) components in obese populations before bariatric surgery.Methods:A cross-sectional study. A retrospective analysis was conducted on a cohort of 97 obese patients scheduled for weight-loss surgery at the First Affiliated Hospital of Wannan Medical College from January 2021 to March 2023. The patients′ body mass index (BMI), biochemical parameters and body composition measurements obtained by QCT were recorded. The patients were stratified into groups based on gender, obesity severity and the number of MS components. Differences in body composition among the groups were compared. Additionally, the correlations between each body composition parameter and metabolic indicators were analyzed. The diagnostic efficacy of each body composition parameter for identifying obese individuals with different MS components was assessed using receiver operating characteristic (ROC) curve analysis.Results:There were 75 females (77.3%). Male obese patients had higher total abdominal fat area [(693.23±148.90) vs (574.99±114.89) cm 2, t=-3.958, P<0.001], visceral fat area [(289.65±57.67) vs (195.60±57.37) cm 2, t=-6.753, P<0.001], fat content of pancreatic head [27.45%(21.65%, 45.48%) vs 21.60%(17.6%, 26.9%), Z=-2.675, P=0.007], and skeletal muscle index [73.36(68.74, 81.26) vs 61.52(55.74, 66.41) cm 2/m 2, Z=-5.246, P<0.001]. With the increase of obesity, abdominal fat mainly increases in subcutaneous fat. With the increase of MS components (MS2 group, MS3 group, MS4 group, MS5 group), the abdominal fat area, abdominal fat/subcutaneous fat, liver fat content, pancreatic head fat content, and skeletal muscle index of patients all increased accordingly. In diagnosing the presence of two components of MS, area under the curve of visceral fat area was the largest (AUC=0.706, 95% CI=0.577-0.834). For diagnosing the presence of three, four and five components of MS, area under curve of liver fat content were all the largest (MS3=0.712, 95% CI=0.605-0.818; MS4=0.652, 95% CI=0.537-0.766; MS5=0.706, 95% CI=0.576-0.836). Conclusion:There are differences in QCT body composition among obese patients with different MS components, and there is a correlation between each body composition and MS component. Among them, intra-abdominal fat area and liver fat content are of great value in evaluating obese patients with different MS components.

ABSTRACT

Objective:To investigate the correlation between metabolic syndrome (MS)and recurrent acute pancreatitis (RAP).Methods:From June 1, 2012 to June 1, 2023, 463 patients with RAP (recurrent group) who visited the First Affiliated Hospital of Chongqing Medical University were retrospectively collected. The clinical data of 463 patients without RAP (non-recurrent group) during the same period were randomly selected. The patients in the recurrence group were treated at the First Affiliated Hospital of Chongqing Medical University for both initial acute pancreatitis and RAP. The differences in the proportion of patients combined with MS and its components (obesity (body mass index ≥25 kg/m 2), fasting blood triglyceride (TG) ≥1.7 mmol/L, hypertension, hyperglycemia, fasting blood high-density lipoprotein cholesterol (HDL-C) <1.04 mmol/L) between the two groups were analyzed by Chi-square test. Binary logistic regression analysis was used to identify the risk factors of acute pancreatitis recurrence. Results:Among the 463 patients in the recurrent group, there were 221 patients (47.7%) with MS, 276 patients (59.6%) with obesity, 223 patients (48.2%) with hypertension, 185 patients (40.0%) with hyperglycemia, and 365 patients (78.8%) with fasting blood TG ≥1.7 mmol/L. Among the 463 patients in the non-recurrence group, there were 95 patients (20.5%) with MS, 245 patients (52.9%) with obesity, 115 patients (24.8%) with hypertension, 92 patients (19.9%) with hyperglycemia, and 301 patients (65.0%) with fasting blood TG ≥ 1.7 mmol/L. The percentages of MS, obesity, hypertension, hyperglycemia, and fasting blood TG ≥1.7 mmol/L of the recurrent group were all higher than those of the non-recurrent group, and the differences were statistically significant ( χ2=76.27, 4.22, 54.35, 44.55, 21.90; P<0.001, =0.040, <0.001, <0.001, <0.001). There was no statistically significant difference in the percentage of patients with fasting blood HDL-C <1.04 mmol/L between the recurrent group and the non-recurrent group (68.5%, 317/463 vs. 65.4%, 303/463, P>0.05). The results of binary logistic regression analysis showed that combined with MS ( OR=3.538, 95% confidence interval (95% CI) 2.647 to 4.728), hypertension ( OR=2.700, 95% CI 2.025 to 3.602), hyperglycemia ( OR=2.228, 95% CI 1.633 to 3.039), and fasting blood TG ≥1.7 mmol/L ( OR=1.757, 95% CI 1.276 to 2.421) all significantly increased the risk of acute pancreatitis recurrence ( P<0.001, <0.001, <0.001, =0.001). Obesity was not an independent risk factor for RAP ( OR=0.967, 95% CI 0.727 to 1.286, P=0.816). Conclusion:The combination of MS, hypertension, hyperglycemia, and fasting blood TG ≥1.7 mmol/L are independent risk factors for RAP.

ABSTRACT

Objective@#To explore the effects of cyclic training on metabolic syndrome among obese adolescents, so as to provide a reference for reducing physical burden and maintaining physical health for the obese adolescent population.@*Methods@#The study selected 42 obese adolescents aged 10-17 from Foshan City from June 1 to September 1, 2021. The subjects were randomly divided into a training group (n=24) and a control group (n=18). The training group implemented a 12week training plan (4 times weekly), including two major events and six movements, namely resistance training (push ups, sit ups, forward lunge) and aerobic exercise (high leg lifts, stationary runs,jumping jack), and each action lasted for 25 seconds with a 15 second interval, completing 4 groups. The control group maintained daily living habits.The ttest and Chisquare test were used for statistical analysis.@*Results@#After intervention, the weight, body mass index (BMI), body fat percentage and fat mass of the training group [(72.3±1.8)kg, (26.4±2.0)kg/m2, (31.9±2.3)%, (17.5±2.8)kg] were all lower than before intervention [(73.8±2.1)kg, (30.3±1.8)kg/m2, (34.4±2.5)%, (20.0±3.3)kg], and the muscle mass and physical fitness index of the training group [(32.4±1.1)kg, (67.3±5.3)%] were higher than before intervention [(31.5±1.5)kg, (63.1±6.9)%] (t=-2.75,-7.10,-3.61,-2.83,2.37,2.36, P<0.05). After intervention, the blood glucose, cholesterol, lowdensity lipoprotein cholesterol , waist circumference and hip circumference of the training group [(4.6±0.5)mmol/L, (8.2±0.4)mmol/L, (2.8±1.2)mmol/L, (73.5±4.2)cm, (93.2±5.8)cm] were all lower than those before intervention [(4.9±0.4)mmol/L, (8.7±0.8)mmol/L, (3.9±1.1)mmol/L, (77.8±3.9)cm, (99.5±5.1)cm], and highdensity lipoprotein cholesterol was higher than that before intervention[(3.2±0.3)mmol/L,(2.9±0.8)mmol/L](t=-2.30,-2.74,-3.31,-3.68,-4.00,2.29, P<0.05). There was no statistically significant differences between groups in the parameters of each indicator before intervention (P>0.05), while all indicators in the training group were lower than those in the control group except for muscle mass, physical fitness index and highdensity lipoprotein cholesterol, which were higher than those in the control group after intervention (P<0.05).@*Conclusions@#The cyclic training method for 12 weeks is helpful to prevent the metabolic syndrome disease in obese adolescents.It is possible to appropriately increase the intervention methods of cyclic training to reduce the occurrence of metabolic syndrome among obese adolescents.
