ABSTRACT

Introducción: El hombre ha actuado sobre las fracturas óseas de los miembros desde tiempos ancestrales, confirmando a la inmovilización (reducción-estabilidad) como la forma básica de tratar la afección, que se le presenta con la tríada: trauma, dolor e impotencia funcional. Objetivo: El objetivo de este trabajo fue organizar la presentación de los cambios que ocurrían con las innovaciones tecnológicas que se incorporaban en el tratamiento de las fracturas. Métodos: Se presenta una revisión de artículos sobre fracturas publicados en revistas indizadas por el CITMA, libros de autores reconocidos, notas personales de eventos y actividades quirúrgicas, con el fin de analizar las variables seleccionadas y sus vínculos históricos con las distintas etapas que marcaron las nuevas concepciones y modificaciones, surgidas en la aplicación de este tipo de osteosíntesis, lo que nominamos Generaciones. Las variables seleccionadas para el análisis fueron: la reducción de la fractura, la vía de acceso para la colocación de la placa, el procedimiento quirúrgico, los nuevos diseños de implantes, el instrumental, la metalurgia de los implantes y la consolidación curativa. Elementos constitutivos del procedimiento quirúrgico de la osteosíntesis. Relacionando los resultados obtenidos y las fechas de su puesta en práctica, con momentos históricos y la inclusión de Innovaciones Tecnológicas de relevancia, llegamos a reconocer tres etapas o Generaciones donde la influencia de las mismas, explican los adelantos en las concepciones actuales de este proceder. Primera Generación: Clásica, a principios del siglo XX. Segunda Generación: de Compresión, Post Segunda Guerra Mundial. Tercera Generación: Actual, Funcional, y Biológica. Conclusiones: Relacionando los resultados obtenidos y las fechas de su puesta en práctica, con momentos históricos e inclusión de innovaciones tecnológicas de relevancia, llegamos a reconocer tres etapas o Generaciones donde su influencia explica los adelantos en las concepciones actuales de este proceder. Introduction: Man has acted on bone fractures of the limbs since ancient times, confirming immobilization (reduction-stability) as the basic way of treating the condition, which is presented with the triad of trauma, pain and functional impotence. Objective: The objective of this paper was to organize the presentation of the changes occurred with technological innovations, integrated in the treatment of fractures. Methods: A literature review on fractures is presented here. Articles published in journals indexed by CITMA, books by well-known authors, personal notes of events and surgical activities were reviewed, in order to analyze the selected variables and their historical links with the different stages that marked the new conceptions and modifications, arisen in the use of this type of osteosynthesis we call generations. Variables selected for analysis were fracture reduction, access route for plate placement, surgical procedure, new implant designs, instrumentation, implant metallurgy, and healing consolidation. Constitutive elements of the osteosynthesis surgical procedure: By relating the results obtained and the dates of their implementation, with historical moments and the inclusion of relevant technological innovations, we come to recognize three stages or generations and their influence explains the advances in the current conceptions of this approach. First generation: classic, early 20th century. Second generation: compression, post-World War II. Third generation: current, functional, and biological. ABSTRACT

A técnica moderna de fundição laboratorial permite um maior controle das ligas metálicas se comparada aos processos utilizados antigamente. O processo de fundição apresenta muitas variáveis e exige experiência. Durante a transformação de fase do resfriamento, o metal sofre várias modificações, praticamente todas as porosidades são geradas na passagem do estado líquido da liga para o sólido. O ideal é compreender os conceitos da metalurgia que influenciam o aparecimento de porosidades e não apenas decorar as indicações.

ABSTRACT

Introdução: O alumínio é um metal proveniente da bauxita, sendo o Brasil um importante produtor mundial. Estudos descrevem distúrbios osteomusculares relacionados ao trabalho entre trabalhadores que transformam o alumínio em produto para consumo. Objetivo: Avaliar as condições ergonômicas e as queixas osteomusculares em trabalhadores metalúrgicos de alumínio. Métodos: Estudo transversal realizado em indústria transformadora de alumínio na cidade de Embu das Artes, São Paulo. Foram aplicados checklists para a avaliação simplificada do risco de lombalgia e para a avaliação simplificada do risco de distúrbios musculoesqueléticos de membros superiores relacionados ao trabalho no setor de coquilha da empresa. Os 38 trabalhadores do setor responderam ao Questionário Nórdico de Sintomas Osteomusculares (QNSO). Resultados: O posto de trabalho apresenta um risco moderado para agravamento/desencadeamento de lombalgia e distúrbios musculoesqueléticos de membros superiores relacionados ao trabalho. Nos últimos 12 meses, 15,8% dos trabalhadores se queixaram de dores lombares, e, nos últimos sete dias, 5,3% relataram queixas de dores em punhos/mãos/dedos. Conclusão: Condições biomecânicas do trabalho podem estar relacionadas ao desenvolvimento de queixas osteomusculares. A gestão dos riscos ocupacionais nesse ramo industrial se faz necessária para promover produtividade e qualidade de vida entre os trabalhadores.


Background: Aluminum is a metal from bauxite and Brazil is a major producer worldwide. Studies describe work-related musculoskeletal disorders among workers in aluminum industry. Objectives: Evaluating ergonomic conditions and musculoskeletal complaints among workers in aluminum metallurgy. Methods: Cross-sectional study carried out in manufacturing aluminum in Embu das Artes, São Paulo. Checklists were applied to simplified evaluation of the risk of low back pain and simplified evaluation of the risk of musculoskeletal disorders of the upper limbs related to work in a specific sector. The 38-sector workers answered to the Nordic Musculoskeletal Questionnaire (NMQ). Results: The ergonomic conditions present moderate risk for low back pain and upper limb disorders. In the last 12 months, 15.8% of workers complained of back pain and in the last seven days, 5.3% of workers reported complaints of pain in wrists/hands/fingers. Conclusion: Biomechanical conditions of work can be related to musculoskeletal complaints. The management of occupational hazards in this branch of industry is needed to promote productivity and quality of life among workers.

ABSTRACT

Objetivos. Determinar los niveles de plomo (Pb) en gestantes y neonatos en la ciudad de La Oroya, Perú. Materiales y métodos. Se realizó un estudio transversal. La población estuvo conformada por gestantes residentes durante al menos dos años en la ciudad de La Oroya, cuando operaban normalmente las fundiciones de Pb, cobre y zinc. Se obtuvo muestras de sangre de la gestante antes del parto y luego del parto se tomaron muestras de placenta y sangre del cordón umbilical. El nivel de Pb de dichas muestras fue evaluado por espectrometría de absorción atómica con horno de grafito. Se determinaron regresiones cuadráticas y correlaciones de Pearson. Resultados. Se evaluó cuarenta partos normales. Los niveles promedio de Pb en la sangre materna, cordon umbilical y placenta fueron 27,4 ± 15,6 ug/dL; 19,0 ± 12,6 ug/dL y 319,0 ± 215,9 ug/100 g, respectivamente. El 67,5% de los neonatos tuvieron más de 10 ug/dL de Pb. El nivel de Pb de la sangre del cordón umbilical representó el 69,4% del nivel de Pb en sangre materna. Los coeficientes de correlación entre el Pb de la sangre materna y cordón umbilical, sangre materna y placenta, placenta y sangre del cordón umbilical fueron 0,36; 0,48 y 0,33, respectivamente. Conclusiones. Cuando funcionaba la fundición, las gestantes y recién nacidos tenían concentraciones elevadas de Pb en sangre. Las concentraciones de Pb en la placenta y cordón umbilical tuvieron una correlación moderada con el Pb de la sangre materna. El Pb de la sangre materna y cordón umbilical fue 1,5 veces más alto que los de zonas más alejadas.


Objectives. To determine blood-lead (Pb) levels in pregnant women and neonates in a metallurgical city of Peru. Materials and methods. A cross-sectional study was carried out, the population of which consisted of pregnant women who had lived for at least two years in the city of La Oroya, at the time when the lead, copper and zinc foundries operated normally. A blood sample was obtained from the pregnant women before delivery and, after delivery, from the neonate’s umbilical cord and placenta. Lead levels in those samples were measured by graphite furnace atomic absorption spectrophotometry. Pearson’s squared regressions and correlations were determined. Results. Forty normal deliveries were studied. The average blood lead levels in the mother, umbilical cord and placenta were 27.4 ± 15.6 ug/dL; 19.0 ± 12.6 ug/dL and 319.0 ± 215.9 ug/100 g, respectively. 67.5% of the neonates registered levels higher than 10 ug/dL. Pb levels in the umbilical cord accounted for 69.4% of Pb level in maternal blood. The correlation coefficients between Pb levels in maternal blood and umbilical cord, maternal blood and placenta, and placenta and umbilical cord were 0.36; 0.48 and 0.33, respectively. Conclusions. At the time the foundry operated, pregnant women and newborns had high blood lead concentrations. Pb concentrations in the placenta and umbilical cord had moderate correlation with the Pb levels in maternal blood. Pb levels in maternal blood and umbilical cord were 1.5 times as high as in more distant areas.

ABSTRACT

Estudo epidemiológico transversal realizado em indústria metalúrgica, no interior do estado de São Paulo, com o objetivo de identificar o perfil sociodemográfico, aspectos de saúde, como índice de massa corporal, horas de sono, tabagismo, etilismo e estilo de vida dos trabalhadores. Foram entrevistados 182 trabalhadores que responderam questionário com dados sociodemográficos, estilo de vida e saúde. A maioria era do sexo masculino (75,8%), com idade média de 33,8 anos, realizava atividade de lazer (88,5%) e fazia hora extra (72,5%). Faziam atividade física (55%); fumavam (17,6%) e relataram ingestão de bebida alcoólica (59,3%). A investigação dos aspectos de saúde e trabalho é de grande importância visando à implantação de medidas de proteção e prevenção relacionadas ao trabalho e à vida pessoal de cada trabalhador.


This is a cross-sectional study, carried out in a metallurgical industry in the state of São Paulo, Brazil, aiming to identify the demographic profile, health aspects such as body mass index, hours of sleep, smoking, alcohol consumption and lifestyle of the workers. We interviewed 182 workers who answered a questionnaire with socio-demographic, lifestyle and health. The majority was male (75.8%), with a mean age of 33.8 years, did leisure activities (88.5%) and did overtime (72.5%). Physical activity was referred by 55% of the workers, 17.6% smoked and 59.3% reported drinking alcohol. The investigation of health and work aspects is important in order to implement prevention and protection measures related to work and the personal life of each worker.


Estudio epidemiológico realizado en una industria metalúrgica en el estado de São Paulo, Brasil, con el objetivo de identificar el perfil socio demográfico, aspectos de la salud, tales como el índice de masa corporal, horas de sueño, el tabaquismo, el consumo de alcohol y estilo de vida de trabajadores. Entrevistamos a 182 trabajadores que respondieron a un cuestionario con el estilo de vida socio demográficas y de salud. La mayoría eran hombres (75,8%), con edad media de 33,8 años, oscilando, actividades de ocio (88,5%) y que las horas extraordinarias (72,5%). La actividad física fue de 55%, el 17,6% fumaba y el 59,3% dijeron haber bebido alcohol. La investigación de los aspectos de la salud y el trabajo es de gran importancia para la aplicación de las medidas de prevención y protección relacionadas no sólo trabajo, sino también la vida personal de cada trabajador.

ABSTRACT

Se presenta el estudio de diagnóstico y seguimiento de los niveles de mercurio en ambiente y en fluidos biológicos de los trabajadores de la metalurgia de Minas de Almadén (España) con el fin de aportar los insumos necesarios para que el Área de Prevención de Riesgos Laborales implantara un método que permitiera establecer unos “niveles críticos” para adoptar acciones preventivas en base a éstos. El estudio se enmarcó dentro de un programa de recolección de datos sobre 15 puestos de trabajo (hasta 70 operarios) como condiciones de vida del trabajador, niveles de exposición al mercurio mediante muestreos ambientales y personales, horarios y turnos de trabajo, reconocimientos médicos, profesiogramas, controles biológicos mensuales en condiciones basales (utilizando el Análisis por Espectrofotometría de Absorción Atómica en Vapor Frío) y valores biológicos máximos permisibles. Se analizaron los niveles de mercurio en la instalación, considerada globalmente, en puestos de trabajo con jornada continua y jornada alterna y, finalmente, en el puesto de trabajo como unidad operativa. Los resultados arrojaron que, a partir de la automatización de la planta en 1997, la mejora de los niveles de mercurio en ambiente de trabajo fue evidente y que la reducción de jornada, considerándola desde la perspectiva del riesgo al mercurio, no estaba justificada. Estableciendo dos periodos (1986-1997 y 1998-2001), los indicadores de mercurio, obtenidos en relación al puesto de trabajo como unidad operativa, experimentaron una notable disminución y evidenció que era necesario prestar especial atención a los puestos de trabajo de operador de hollines y peón de servicios varios.


This paper presents the monitoring and follow-up of environmental and biological mercury levels among metallurgy workers of the Almadén Mines in Spain, which provided the foundation for the mine’s occupational health department to implement a method that allowed establishment of "critical levels" that would trigger specific preventive interventions.The study was part of an information-gathering project targeting 15 jobs (up to 70 workers), focused on worker living conditions, environmental and personal mercury concentrations, job schedules and shifts, medical surveillance, job profiles, monthly baseline biological controls (using Cold Vapor Atomic Absorption Spectrophotometry) and maximum allowable biological exposure levels. Mercury levels were analyzed at several levels: facility-wide, in workplaces with continuous and alternating shifts, and, lastly, at the job level. Results showed that, since plant processes were automated in 1997, there had been a definite improvement in workplace mercury levels, and that a reduction of shifts, from the standpoint of the risk posed by exposure to mercury, was not justified. Between two periods (1986-1997 and 1998-2001), mercury exposures at the job level decreased significantly, although a closer evaluation of the soot operator and miscellaneous laborer jobs was warranted.

ABSTRACT

O objetivo do presente estudo foi comparar as características superficiais de amostras de Titânio (Ti) porosas (Ti-P) e de Ti Denso (Ti-D), usando MEV e rugosímetro 3D. O experimento foi realizado em triplicata (n= 3). Inicialmente foram obtidas imagens em MEV de alta definição das superfícies. Na sequência, as amostras eram analisadas em rugosímetro 3D utilizando parâmetros de amplitude (Sa e St) que indicam a rugosidade média, um parâmetro espacial (Str), que indica a uniformidade da textura, e um parâmetro híbrido (Sdr) que indica a área superficial disponível. Fotomicrografias mostraram que o Ti-P apresentava poros abertos e amplos. Além disso, nesta superfície foram observados valores de Sa e de St bastante altos (35.3 ± 1.20 μm e 315.67 ± 14.36 μm, respectivamente), valor de Str de 0.81 ± 0.08 (tendendo a 1, ou seja, isotrópico) e de Sdr de 42.57% ± 0.67, enquanto o Ti-D apresentava uma superfície lisa e valores de rugosidade bastante baixos (Sa e St de 0.17 ± 0.04 e de 5.31 ± 0.49, respectivamente), Str de 0.00 ± 0.00 (anisotrópico) e Sdr de apenas 0.14% ± 0.04. Portanto, pode-se concluir que as superfícies de Ti poroso apresentaram características topográficas bastante distintas do Ti Denso, como poros abertos e amplos, maior rugosidade, isotropia e maior área superficial disponível.


The aim of this study was to compare surface topographical features of porous Ti (Ti-P) and Dense Ti (Ti-D) samples, using SEM and 3D perfilometer. The study was performed in triplicate (n = 3). First, SEM high-definition images of both surfaces were obtained. Then, the samples were analysed by 3D perfilometer, using amplitude parameters (Sa and St), a Spatial parameter (Str) and a Hybrid parameter (Sdr). Open and wide pores were observed on Ti-P surface by photomicrographs. Furthermore, Ti-P showed higher values of amplitude roughness (Sa and St values of 35.3 ± 1.20 μm and 315.67 ± 14.36 μm, respectively), higher Str value (0.81± 0.08, tending to 1, i.e, isotropic) and Sdr value of 42.57% ± 0.67. On the other hand, Ti-D presented a smooth surface and very low roughness values (Sa and St, 0.17 ± 0.04 and 5.31 ± 0.49, respectively), Str 0.00 ± 0.00 (anisotropic) and Sdr 0.14% ± 0.04. So, it can be concluded that Ti-P andTi-D surfaces presented very distinct topographic features, especially open and wide pores, higher roughness, isotropy and higher available surface area.
