ABSTRACT

Objective To evaluate the effect of extracorporeal photopheresis upon the expression levels of interleukin (IL)-12p70 and T helper cell (Th) 1/Th2-like cytokines in splenic lymphocytes of mouse models undergoing skin transplantation. Methods The C57BL/6 mice were used as the donors and BALB/c mice as the recipients to establish mouse models with skin allograft. The splenic lymphocytes in the C57BL/6 and BALB/c mice (CSP and BSP) were isolated and treated with 8-methoxypsoralen combined long-wave ultraviolet (PUVA-SP). According to the components of intravenous infusion into the recipients, all experimental animals were randomly divided into the PUVA-BSP, PUVA-CSP, BSP, CSP and phosphate buffer solution (PBS) control groups (n=12 for each group). The mice were injected with PUVA-BSP, PUVA-CSP, BSP, CSP or PBS via the caudal vein at preoperative 7 d, upon the day of surgery and at postoperative 7 d, respectively. The apoptosis of the splenic lymphocytes was observed after PUVA treatment. The expression levels of IL-12p70 and Th1/Th2-like cytokines in the peripheral blood of the recipients were quantitatively measured. Results After the skin transplantation, the expression levels of IL-12p70 in the peripheral blood of mice in the PUVA-BSP and PUVA-CSP groups were significantly down-regulated compared with those in the BSP, CSP and PBS control groups (all P<0.01). In the PUVA-BSP and PUVA-CSP groups, the expression levels of Th1-like cytokine IL-2, interferon (IFN)-γwere dramatically lower than those in the BSP, CSP and PBS control groups (all P<0.01). The expression levels of Th2-like cytokine IL-10 in the PUVA-BSP and PUVA-CSP groups were significantly up-regulated compared with those in the BSP, CSP and PBS control groups (all P<0.01). Conclusions Infusion of PUVA-SP at a sufficient dose can induce the low expression level of IL-12p70 and drive the incidence of Th2 immune deviation in the recipient BALB/c mice.

ABSTRACT

Vitiligo é caracterizado por manchas brancas na pele. Na dispensação do metoxisaleno utilizado para tratá-lo, na Farmácia Universitária/UFRJ, os portadores relatavam mal-estares devido à terapia e à doença. A busca pela compreensão da experiência do adoecer conduziu este estudo que adota perspectiva teórico-metodológica socioantropológica. A apreensão da visão de mundo dos sujeitos foi captada em entrevistas entre usuários da FU/UFRJ. A partir do diagnóstico, os sujeitos consideram o vitiligo "um castigo" sentem-se alvo de "chacotas", "preconceito", "discriminações". A experiência da doença impõe rever seus conceitos e filiação religiosa. O fato de o vitiligo não ser concebido como doença grave estabelece um não-lugar para o sofrimento dos portadores. Aspectos simbólicos, emocionais e socioculturais que circunscrevem a doença devem ser considerados para melhorar a atenção à saúde e vida desses sujeitos. Os profissionais de saúde precisam concebê-los para além de suas "manchas".


Vitiligo is characterized by white spots on the skin. During the dispensation of Methoxypsoralen, a drug used for treating it, patients reported discomfort regarding such therapy and their disease. Thus, the study has the motivation of seeking to understand the experience of being ill, using a socio-anthropological, theoretical and methodological approach. Subjects' worldview after the diagnosis of vitiligo was apprehended by interviews with UFRJ pharmacy users. The subjects tend to consider having vitiligo "punishment", and they feel like a target of "mockery", "prejudice" and "discrimination". It is a fact that vitiligo is not taken as a severe condition, and this establishes a "non-place" for the suffering of its bearers. Symbolic, emotional, and socio-cultural aspects of the disease must be taken into account, in order to improve health care for these subjects and their lives. Health professionals need to perceive vitiligo bearers beyond their "spots".


El vitiligo se caracteriza por manchas blancas en la piel. En el momento de la entrega del medicamento correspondiente a esta enfermedad en la farmacia, los pacientes nos informaron sobre incomodidades por que pasaron. La incomodidad en la cosmovisión de los sujetos fue capturada en entrevistas. A partir del diagnóstico médico, los sujetos comenzaron a considerar al vitiligo como "un castigo", blanco de "burlas", 'prejuicios" y "discriminación". Así mismo, la experiencia de la enfermedad les condujo a revisar sus conceptos y religiones. El hecho de que el vitiligo no se considera una enfermedad graveparece establecer un "no lugar" para el sufrimiento y el "dolor espiritual" de sus portadores. Los aspectos simbólicos, emocionales y socioculturales que circunscriben esta enfermedad deben considerarse para mejorar la atención de la salud y la vida de estos sujetos. Por su parte, los profesionales de la salud deben aprender a concebirlos más allá de sus "manchas".

ABSTRACT

Objective: To observe the clinical curative effect of narrow band ultraviolet combined infrared with 8-Methoxypsoralen for the treatment of vitiligo,to seek a new method for the treatment of vitiligo. Methods: Sixty cases of vitiligo patients were randomly divided into experimental group and control group. The experimental group adopted 365-nm narrow band ultraviolet LED combined 850-nm infrared LED with 8-Methoxypsoralen for the treatment of vitiligo, the control group adopted 365-nm narrow band ultraviolet LED irradiation combined with 8-Methoxypsoralen for the treatment of vitiligo, the clinical curative effect of two group were observed when treated with 1 months,2 months. Results:After 2 months of treatment, the efficiency rate of the experimental group was 66.67%, higher than that of the control group with an efficiency of 40%, the difference was statistically significant(x2=4.653, P<0.05). Conclusion: The clinical efficacy of narrow band ultraviolet combined infrared with 8-Methoxypsoralen for the treatment of vitiligo is better than that of narrow band ultraviolet combined with 8-Methoxypsoralen in the treatment of vitiligo.

ABSTRACT

Objective To study the preparation of quercetin compound cream and establish its standard of quality control. Methods The composition of recipe and manufacturing technique were designed.The content of components were determined by HPLC,and its stability tests were carried out.Results The product was a kind of yellow smooth cream.The linear ranges were 0.053-1.696 μg for quercetin(r=0.999 9),0.053-1.696 μg for 8-methoxypsoralen(r=0.999 8)and 0.100-1.000 μg for beta-methasone(r=0.999 9).The average recovery rate were 99.83%,99.52%,and 99.74% of quercetin,8-methoxypsoralen,and beta-methasone(n=9).After 12 months′long term stability test,all the 3 batches of sample preparations were in line with relevant regu-lations.Conclusion The designed recipe was reasonable,and the manufacturing technique was feasible,with stable and controllable quality.

ABSTRACT

Objective To explore the enhancement effect of N-trimethyl chitosan(TMC) on transdermal absorption of 8-methoxypsoralen-loaded liposomal(LMOP) gels in vitro.Methods TMC with quaternization degree of 60% (TMC60) as the enhancer,the LMOP gels were prepared with free TMC60 or TMC60-coating.The enhancement of free TMC60 or TMC60-coating was studied by using Franz diffusion,cells with LMOP gels as the negative control and LMOP gels including 1% Azone + propylene glycol as the positive control.Results Compared with the negative control,enhancement and drug amount in skin of LMOP gels with enhancer all showed significant difference (x2 =8.65,P < 0.05),and the free TMC.60 was the best.Conclusion TMC can enhance the transdermal penetration of LMOP gels in vitro and is valuable to be studied further.

ABSTRACT

Background: Very few studies using the combination of topical 8-methoxypsoralen (8-MOP) and narrow-band ultraviolet B (NBUVB) have been performed, especially in Indian patients. A combination of oral psoralen with NBUVB has been shown to have a superior efficacy as compared with NBUVB alone in psoriasis. Aims: Comparison of the efficacy of topical psoralen NBUVB (combination) versus NBUVB alone in psoriasis. Methods: Thirty patients with plaque psoriasis were taken up for the study and NBUVB phototherapy was given twice weekly. The target lesions on one side were treated with 0.1% topical 8-MOP 15 min before the irradiation. The treatment period was 12 weeks or 24 exposures. Results: The number of treatment sessions and cumulative NBUVB doses were lower in the combination therapy as compared with NBUVB monotherapy, although the differences were not statistically significant. Conclusion: To conclude, topical 8-MOP enhances the therapeutic effects of NBUVB therapy without increasing the incidence of adverse effects.

OBJECTIVE:To study the percutaneous permeability of methoxypsoralen liposomal gel METHODS:The preparations were applied on the rat skin in vitro and in vivo with common gel and Mingbailing tincture as control,and the drug concentrations in the receiver,skin,blood and other organs were determined by RP-HPLC RESULTS:In vitro experiment,liposomal gel showed the lowest percutaneous permeation rate and the highest retention in skin In vivo studies illuminated that the skin depositions of drug encapsulated in liposome was(4 37?0 91) and (3 36?0 58) times those of the common gel and tincture respectively CONCLUSION:Methoxypsoralen liposomal gel showed an evident targeting effect on skin

ABSTRACT

BACKGROUND: An alternative approach to oral PUVA therapy for psoriasis, psoralen bath plus UVA therapy(bath PUVA therapy) that avoids the adverse effects associated with oral PUVA therapy has increasingly been used during recent years. OBJECTIVE: This study was performed to evaluate the efficacy and safety of bath-8-MOP-UVA therapy in the treatment of psoriasis. METHODS: Twenty patients were enrolled in this study after determination of the minimal phototoxic doses(MPD). We evaluated the total treatment number, duration, final UVA dose and total cumulative UVA dose of bath-8-MOP-UVA therapy to reach grade 4 response and categorized each patient into clearing, improvement, or failure groups based on the therapeutic efficacy. We measured the PASI score at two week intervals. Blood samples were obtained from all twenty patients 2 hours after bath-8-MOP and plasma levels of 8-MOP were quantified by a reverse phase high performance liquid chromatography. RESULTS: The following results were obtained from this study. 1. Phototoxicity testing with bath-8-MOP-UVA elicited mean MPD value of 3.5+/-1.3J/cm2. 2. The mean PASI score at 10 weeks was significantly decreased to 5.8+/-1.3 from baseline PASI score 20.1+/-4.3. 3. Among 20 patients, clearing was shown in 13 patients(65%), 6 patients(30%) were improved and 1 patients(5%) showed failure. 4. In clearing and improvement groups, the mean treatment number, duration, final dose of UVA and total cumulative UVA dose reaching grade 4 were 19.3+/-5.4, 49.9+/-13.5days, 5.2+/-1.3J/cm2 and 68.6+/-30.1J/cm2, respectively. 5. Five patients experienced side-effects. Two patients had intense tan, one withdrawing because of it and the other continued treatment. Three had pruritus that was controlled on oral antihistamines. Phototoxic or other classic adverse effects of oral PUVA therapy, such as nausea, vomiting and headaches, were not observed in any of our 20 patients. 6. All twenty patients had an undetectable plasma 8-MOP level. The lower limit of detection level was 20ng/ml. CONCLUSION: Bath-8-MOP-UVA therapy for psoriasis is a very effective and safe alternative to oral PUVA therapy.

ABSTRACT

One adverse effect of PUVA therapy is the development of severe dermatitis. Only a few cases of photoallergy to psoralens during PUVA therapy have been reported. We describe herein a patient with photoallergic dermatitis induced by PUVA with 8-methoxypsoralen(8-MOP). A 38-year old woman with generalized vitiligo had acute pruritic exanthematous maculopapular lesions in the treated areas after PUVA therapy with 8-MOP. A patch test and photopatch test were performed with 8-MOP, 5-MOP, and 4,5,8-trimethylpsoralen(TMP). The patch test carried out with these psoralen derivatives were all negative, but the photopatch test showed a positive reaction to 8-MOP. The patient consequently had PUVA therapy with 5-MOP and she had no further experience of a photoallergic reaction.

ABSTRACT

BACKGROUND: PUVA therapy has been used to treat psoriasis, vitiligo, and various cutaneous diseases. Psoralens are metabolized in the liver and thus can produce liver damage. OBJECTIVE: This study was done to observe the effect of 5-methoxypsoralen on liver function during photochemotherapy. METHODS: Serum transaminase levels were determined before and during photochemotherapy in eighty patients with vitiligo or psoriasis. RESULTS: Four of the eighty patients who had received 5-MOP-PUVA showed elevation of serum transaminase levels. CONCLUSION: Although there have been few cases showing transient elevated values of serum transaminase during 5-MOP-PUVA, it is advisable to obtain liver function tests before and serially during photochemotherapy.

ABSTRACT

Objective To study the effects of 8-methoxypsoralen (8-MOP) on a ctivation, proliferation and melanin synthesis of hair-follicle melanocytes. Me thods The cultured human hair follicle melanocytes were treated with various con centrations of 8-MOP(10~500 ?mol/L) in vitro. The effects of 8-MOP were obse rved on cellular morphologic changes, proliferation, tyrosinase activity and mel anin contents of cultured melanocytes. Results Seven days after treatment with 8-MOP(50 ?mol/L), striking changes were found in cellular morphology with many elongated dendrites, and deposition of brown granules in cytoplasm. ABSTRACT

Objective To explore the influence factors and experimental conditions of phototoxicity test using pure UVA light source.Method The variations of light intensity associated with the exposure duration,the influence of exposure duration to UVA radiation on the temperature of the exposed area were both observed.The phototoxicity of positive control of 8_methoxypsoralen(8_MOP)to skin of healthy adult guinea pigs exposed to UVA radiation at dose of 10 J/cm2 for about 50min was observed also.Results The intensities of UVA light source were stable in the range of 3.3~3.4 mW/cm2 during the period of 100 minute_radiation.After a 100 minute_exposure to UVA radiation at 18 ℃,the temperature of the irridiated area increased 1.5 ℃,8_MOP revealed significant phototoxicity.Conclusion This method for phototoxicity test using pure UVA light source could examine the phototoxicity of a certain chemicals accurately,which was scientific and feasible.

ABSTRACT

We have observed the dark effect of 8-methoxypsoralen(8-M(P) on the viability and DNA synthesis in human lymphocyte cultures after stimulaticn with phytohemagglutinin (PHA) in the absence of ultraviolet A radiation. The concentratioiis of 8-MOP was 0.5-32 ug/ml. We have also measured the LDH activity in supernatant. of lymphocyte cultures treated with 8-MOP. The results were as follows: 1. There was no 8-MOP dose-dependent decrease in the viability of lymphocytes up to 8- MOP 32pg/ml. 2. There was a 8-MOP dose-dependent decrease in PHA-induced DNA synthesis of lymphocytes from the concentration of 8-MOP 2 ug/ml. 3. There was a time-dependent decrease in PHA-induced DNA synthesis of lymphocytes at the concentration of 8-MOP 32 ug/ml. 4. There was no LDH release in supernatant of lymphocyte ciltu es after incubation with 8-MOP up to 8-MOP 32ug/ml.

ABSTRACT

Allergic contact dermatitis due to 8-MOP is a rarely known si(ie effect of this widely used drug. Other known adverse reactions due to 8-MOP such as the oallergic dermatitis as well as some isolated cases of exanthema, papular eruptions, and astloma like symptoms are also sporadically reported. A 52-year-old man with vitiligo developed erythema to the UVA exposed 0.3% Oxoralen cream applied area. Prior to this episode, the patient had history of generalized burns after systernic PUVA therapy in 1983. Even after this experience, the patient had few more episodes of erythema at the site of 0.3%. Oxoralen cream application. We performed patch test and photopatch tests with Scandinavian series, 0.3% Oxoraler or am (as is), and diluted 8-MOP, 5-MOP, TMP solution. The result showed positive reactivity to 6-methylcoumarin, 8-MOP, as well as to 0.3% Oxoralen cream. The size of erythema was same in both irradiated areas which indicates an allergic contact dermatitis rather than photoallergic dermatitis or phototoxic dermatitis.
