ABSTRACT

OBJECTIVE@#To observe the clinical efficacy of methycobal iontophoresis combined with balance acupuncture in the treatment of peripheral facial paralysis.@*METHODS@#A total of 108 patients with peripheral facial paralysis were randomly divided into a methycobal iontophoresis combined with balance acupuncture group (a combined group), a methycobal iontophoresis group and a simple balance acupuncture group, 36 cases in each one. Basic medical treatment were given in the three groups. The simple balance acupuncture was applied at contralateral lumbago acupoint, rhinitis acupoint, stomachache acupoint in the simple balance acupuncture group. Methycobal through iontophoresis anodic introduction therapy was given in the methycobal iontophoresis group. On the basic treatment of methycobal iontophoresis, in the combined group, acupuncture was supplied at contralateral lumbago acupoint, rhinitis acupoint and stomachache acupoint. The treatment in all groups was given once a day, for 2 weeks. The House-Brackmann grading scale and the modified portmann score (RPA) method were used to observe the degree of nerve function and facial paralysis before and after treatment, and the clinical efficacy of each group was evaluated.@*RESULTS@#The total effective rate of the combined group was 97.2% (35/36), which was higher than 83.3% (30/36) in the methycobal iontophoresis group and 88.9% (32/36) in the simple balance acupuncture group (all 0.05). The RPA score of the combined group was higher than those in the methycobal iontophoresis group and simple balance acupuncture group (both 0.05).@*CONCLUSION@#Compared with methycobal iontophoresis and simple balance acupuncture therapy, methycobal iontophoresis combined with balance acupuncture therapy can effectively improve the clinical symptoms and signs of peripheral facial paralysis.

ABSTRACT

Objective To investigate the effects of methycobal on the expression of Caspase-3 in brain tissue after cerebral ischemia reperfusion in rats. Methods Rats were randomly divided into sham-operation group, model control group, nimodipine group and low-dose methycobal group, high-dose methycobal group(n=30 in each group).Rats in the sham-operation group and model control group were administered intragastrically with 0.9% sodium chloride solution, rats in the nimodipine group were treated with 1 mg . kg-1 . d-1 of nimodipine, rats in the low- and high-dose of methycobal groups were given 50 and 100 μg.kg-1 .d-1 of methycobal, respectively. The rat model of cerebral ischemia reperfusion was established by middle cerebral artery occlusion with suture method for 3 h.Neurological deficit scores were evaluated 24 h after reperfusion. The apoptosis of perifocal cortex cells was detected by TUNEL method and the expression of Caspase-3 was analyzed by RT-PCR 6, 12 and 24 h after reperfusion. Results Neurological deficit scores in model control group, nimodipine group, low-dose methycobal group and high-dose methycobal group were 2.70±0.52, 1.30±0.51, 2.20±0.75 and 1.30±0.81, respectively.Compared with model control group, neurological deficit scores were significantly different in the nimodipine group, low-dose methycobal group and high-dose methycobal group(P0. 05 ) . There was a significant difference between the high-dose methycobal group and low-dose methycobal group( P0.05).There were significant differences between the high-dose methycobal group and low-dose methycobal group at the end of 24 h(P<0.05). Conclusion Methycobal can protect the brain cells from injury after cerebral ischemia reperfusion by adjusting the expression of Caspase-3m RNA, and the high-dose methycobal is more effective.

ABSTRACT

Objective To evaluate the clinical efficacy of Dan-Hong Injection and Methycobal combined therapy in treating diabetic peripheral neuropathy(DPN).Methods 72 cases of peripheral neuropathy and type 2 diabetes(conformed to ADA standard published in 1999)were randomly separated into an experimem group and a control group.Members in the experiment group were treated with Dan-Hong injection 20ml/day i.v.and Methycobal 500ug/day i.m.Members in the control group were treated with Methycobal only.At the end of 2 weeks'therapeutic courses, motor nerve conducdon velocity (MNCV)and sural nerve conduction velocity(SNCV)were evaluated. Results The overall effective rate was 92.1% in the experiment group and 67.6% in the control group, showing significant difference between the two groups(P<0.01).The post-therapeutic values of both SNCV and MNCV were significantly different from those of previous treatment.(P<0.01)The post-therapeutic values of SNCV and MNCV of the experiment group were also significantly different from those of the control group(P<0.05).No evidently relative adverse effects were observed in both groups. Conclusions Combined therapy of Dan-Hong injection and Methycobal was more effective in controlling the clinical activity of diabetic peripheral neuropathy.

ABSTRACT

@#ObjectiveTo investigate the effect of varied hyperglycemia and durations on preventing peripheral neuropathy (PN) with methylcobal in diabetic rats.Methods80 Sprague Dawley rats were selected and randomly chosen 16 as normal control (NC) group, others 64 animals as diabetic model induced by alloxan. The model rats were evenly divided into relatively good control (GC) group and relatively poor control (PC) group based on levels of hyperglycemia regulated with exogenous insulin. 16 rats of each GC/PC group were intramuscularly injected with methylcobal (500 μg/kg). The sensory nerve conduction velocity (SNCV), motor nerve conduction velocity (MNCV) and evoked potential amplitute (EPA) of sciatic nerve of rats were detected by evoked electromyogram respectively at 2nd, 8th and 12th week after onset of diabetes. Fructosamine in serum was also detected.ResultsThe rats injected with methylcobal had a significantly delayed reduction of SNCV, MNCV and EP in GC group (fructosamine,1.0 mmol/L) than that in PC group (fructosamine, 1.2 mmol/L) ( P<0.05~0.01). Up to 8th week after onset of diabetes, there was no obvious difference in physioelectroparameters between the GC group and NC group. At 12th week after onset of diabetes, physioelectroparameters in the PC group fell to the level of non-methylcobal injected group. No effect of methylcobal on blood glucose revealed in any groups ( P>0.05).ConclusionMethylcobal has a beneficial effect on delaying the onset of diabetic PN, and is of high efficacy with a good control of diabetic state, especially at early stage.
