ABSTRACT

@#Objective    To analyze the effect of microecological preparation on digestive tract complications and nutritional status after esophageal cancer surgery. Methods    A total of 146 patients with esophageal cancer admitted to the Department of Thoracic Surgery, Renmin Hospital of Wuhan University from October 2017 to June 2019 were selected. There were 91 males and 55 females, aged 65 (61.9±8.2) years. They were randomly divided into two groups (an observation group and a control group, n=73 in each group) according to whether microecological preparation was given when they could take food one week after the surgery. Nutritional status and the incidence of gastrointestinal complications including anorexia, acid reflux, nausea, vomiting and diarrhea (≥3 times per day) in the first month after operation were compared between the two groups. Results    The incidence of anorexia, nausea, diarrhea (≥3 times per day), anastomotic fistula and stenosis in the observation group was lower and the index of nutritional status was higher than those in the control group within one month after the operation (all P<0.05). There was no statistically significant difference in acid reflux, vomiting, lung infection and incision infection between the two groups (all P>0.05). Conclusion    Microecological preparation can regulate gastrointestinal microecological balance, improve nutritional status, reduce the incidence of gastrointestinal complications and accelerate the postoperative rehabilitation.

ABSTRACT

In recent years, with the in-depth research on intestinal microecology, probiotics, probiotics, synbiotics, fecal bacteria transplantation and other microecological therapies have become new clinical treatment and intervention methods. The occurrence and development of some chronic diseases are closely related to the disorder of intestinal flora. Therefore, in clinical practice, the treatment and adjuvant treatment of chronic dis-eases related to intestinal flora disorder with the method of intestinal microecology will bring better treatment op-tions and clinical prognosis. How to better regulate intestinal flora to bring more benefits to such patients, as well as what kind of microecologics and treatment methods are the primary problems and challenges in the inter-vention of chronic diseases with microecologics. Based on the current trend of intestinal microecological inter-vention in chronic diseases, this article proposedthe microecological intervention strategy, in order to improve theunderstanding of this topic.

ABSTRACT

OBJECTIVE: To provide suggestions for improving pediatric medication information in drug instructions of microecological preparations in China. METHODS: Using text analysis method, the pediatric medication information in the instructions of 36 approval number of microecological (29 varieties from 23 enterprises) is statistically analyzed. RESULTS: Among the 36 microecological preparations, there were 2 medicines for children (5.56%). There were 2 granules (5.56%), 9 dispersants (25.00%), 9 tablets (25.00%) and 16 capsules (44.44%); there were 6 national essential medicines (16.67%); there were 13 products in the list of national essential medical insurance (36.11%); there were 22 OTC products (61.11%). The proportion of children’s indications, children’s usage and dosage, child-related adverse reactions, taboos and matters needing attention in the drug instructions for children were 19.44%, 55.56%, 2.77%, 5.56% and 8.33%, respectively. The proportion of specific information about children’s drug use marked in the item of  “children’s drug use” was 25.00%. CONCLUSIONS: There is a lack of information on pediatric medication in the instructions of microecologial preparations in China, and there are some problems such as unclear wording, scattered labeling position of the medication information for children, and the content is not practical. It is recommended that medical institutions should strengthen the rational use of medical guidance for medical personnel, implement the scientific management process of over-instruction medications; the relevant departments should introduce more incentive policies, encourage enterprises to improve the basis of children’s medications, and make standardized labeling of children’s medication information in drug specifications. In addition, a database of information on children’s medications can be established to provide a reference for rational use of pediatric drugs.

ABSTRACT

OBJECTIVE: To investigate the situation and problems of clinical microecological preparations in pediatrics department of China, and to provide reference for rational application of microecological preparations in children. METHODS: Outpatient/emergency prescriptions of microecological preparations during Jan.-Mar. 2019 were retrieved from information system of 5 “Third Grade Class A” children hospitals, and were investigated and analyzed in respects of the type of microecological agents, age distribution, clinical diagnosis, drug combination, off-label medication, etc. RESULTS: A total of 74 210 outpatient/emergency prescriptions were collected, involving 18 kinds of microecological preparations. Among them, Clostridium butyricum combined viable powder ranked first (31 146, accounting for 41.97%), followed by Brucella yeast powder (14 289, accounting for 19.25%) and Compound Lactobacillus acidophilus tablets (10 281, 13.85%). The age of children was less than 18 years old, and the proportion of children aged 1-<3 years old was the highest (33 753, 45.48%). The most common clinical diagnosis     was digestive tract diseases (54 009, 72.78%), followed    by respiratory infectious diseases (11 744, 15.83%). In combination with microecological agents, there were Chinese patent medicine for invigorating the spleen and stomach     (18 546, 24.99%), antibacterial medicine (10 595, 14.28%) and oral rehydration salt Ⅲ (8 238, 11.10%). Off-label drug use was found in 19 532 prescriptions (26.32%), mainly manifesting as super-indications (15 590, 21.01%), super-usage and dosage (8 098, 14.52%), super crowd medication (48, 3.91%) was found in a few prescriptions. CONCLUSIONS: There is a high incidence of off-label use of microecological agents in children. Medical staff should standardize prescription behavior and promote rational use of drugs. At the same time, the improvement of the information of children’s medication in drug instructions to ensure the safety and effectiveness of children’s medication should be encouraged.

ABSTRACT

Human guts harbor abundant microbes that regulate many aspects of host physiology. However, bacterial imbalance or dysbiosis in the gut due to the dietary or environmental changes may cause colorectal cancer (CRC).Therefore,it is theoretically and clinically important to explore the correlation between possible carcinogenic bacteria and CRC and thus reduce CRC incidence by regulating intestinal microecological balance through the application of microecological preparations.
