ABSTRACT

Objective:To explore the effect of correcting atrophic and loose breast in women with golden section theory and minimally invasive suspension and silicone prosthesis through areola incision.Methods:A total of 126 female patients with postpartum breast atrophy and ptosis were enrolled in Nanjing Kangmei and Changsha Aiste Medical Beauty Hospital from March 2012 to March 2019. The 126 cases, aged from 25 to 46 years, had a history of 2 to 16 years. The glandular suspension was performed after the implantation of the prosthesis through the incision above the areola according to the golden section.Results:A total of 252 breasts were followed up for 6 to 84 months. No severe complications such as scar, infection or skin necrosis occurred. Among them, 69 cases were very satisfied, 45 cases were satisfied, and 12 cases were basically satisfied. The satisfactory rate was 90.4%.Conclusions:This method is effective with small wound and less complications for micromastia patients with mild to moderate mastoptosis, which is worthy of clinical application.

ABSTRACT

Objective To investigate a creative breast augmentation of autologus fat in combination with implant in reconstruction of normal breast and prevention of implant related contracture.Methods The autologous fat was harvested and purified for use.A 3.0 to 4.0 cm long incision was made in the axillary crease.The implant placement cavity was created underlying the pectoralis major after subcutaneous blunt dissection through the incision.The properly selected silicone breast implant of certain type and volume was inserted into the previously created cavity through the subcutaneous tunnel.The autologous fat was transplanted overlying the pectoralis major in multiple planes and the fat was located over the implant in an umbrella form.Results A total of 12 cases obtained obvious breast increase,reconstruction of breast,fullness of breast and softness of breast without obvious contracture after the autologous fat in combination with implant in reconstruction of normal breast.The result was satisfactory during a follow-up between 6 months and 3 years.Conclusions The transplanted fat overlying the silicone implant can augment the breast,increase breast,promote regeneration of breast,reduce or resist immune response of silicone breast implant,and then prevent breast implant related contracture.The newly built breast is soft and natural.The procedure is safe and effective.

ABSTRACT

Objective To explore the key points of surgical techniques in individualized breast augmentation via inframammary fold (IMF) incision.Methods Fifty-six patients of our department from March 2014 to March 2017 in this group were followed up for 3-18 months,with average 12 months.According to the patients' characteristics to determine the size and dimensions of the implant and the exact position of new IMF,via IMF incision we inserted an implant part under the pectoralis major muscle and part posterior to the breast after precisely stripping and forming the implant pocket,which were the dual-plane augmentation mammoplasty.Results Compared with traditional endoscope-assisted breast augmentation via axillary incision,the breast augmentation via inframammary fold incision was more accurate,the operation time was shortened by an average of one hour,the intraoperative hemorrhage decreased by about 20 ml on average,the postoperative drainage fluid was reduced by about 70 ml on average,the hospitalization time was shortened by about 3 days,the postoperative breast shape was naturally upright,and the incision was hidden at the new inframammary fold.No hematoma,infection,significant hypertrophic scar,capsule contracture and other complications were noted.Conclusions The individualized dual-plane augmentation mammoplasty via IMF incision can hide the incision in the inframammary fold.The operation under direct vision and no use of special equipments make it an ideal breast augmentation surgery.

ABSTRACT

Objective To seek an effective surgical procedure to treat patients with the varying degrees of breast ptosis and micromastia.Methods Patients were classified into Ⅰ-Ⅳ degrees based on different breast ptosis,and treated by different methods.Degree Ⅰ was treated with implanting prosthesis to the post-pectoralis major space; degree Ⅱ patients were repaired with avulsion of deeper mammary gland and pectoralis major and hanging fixed breast tissue,based on the degree Ⅰ procedure; degree Ⅲ was fixed with breast tissue flap,excised redundant epidermis by the method of double-rings and the complex of nipple and areola was shifted,based on the degree Ⅱ procedure; degree Ⅳ was repaired with implanting prosthesis and the method of lines to hanging fixed breast.Results In 116 cases of this study,there were no hematoma,infection and nipple and areola necrosis by the combined strategies.The follow-up period after the surgery was 6-31 months (mean 13.3 months).All cases had voluptuous and upright breasts,rectified breast ptosis,with the normal sense of nipple and areola.Conclusions For breast ptosis and hypoplasia,the combined strategies have better clinical therapeutic effects.

ABSTRACT

Objective To investigate the expression of the cyclin D1 in pubertal mammary hypertrophy and micromastia. Methods The expression of cyclin D1 was detected by SP immunohisto-chemistry in 18 cases of pubertal mammary hypertrophy and 12 cases of micromastia. Results The expression rate of cyclin D1 within breast tissue was 83. 33 % and 16. 67 % in pubertal mammary hypertrophy and micromastia, respectively. There was significant difference between the two groups (P<0. 01). Conclusion The expression level of the cyclin D1 within breast tissue in pubertal mammary hypertrophy is higher than that in micromastia.
