ABSTRACT

Objective:To investigate the effects of self-help mindfulness-based stress reduction (MBSR) training on illness uncertainty andsleep quality in patients with atrial fibrillation (AF) after radiofrequency ablation. To provide the basis for improving the quality of life of these patients. Methods:This was a randam controlled Test. A convenient sampling method was used to select 90 patients with atrial fibrillation who underwent radiofrequency ablation in the First Peoples Hospital of Changzhou from January 2021 to December 2021. According to the random number table method. They were divided into the experimental group and the control group, with 45 cases in each group. The control group received routine nursing, and the observation group received home-based self-help MBSR training based on the routine nursing. Five Facet Mindfulness Questionnaire (FFMQ), Mishel Uncertainty in Illness Scale and the Pittsburgh Sleep Quality Index Scale (PSQI) were used to evaluate the scores of each scale before intervention, 8 weeks and 3 months after intervention. Results:A total of 42 cases were completed in the experimental group and 41 cases in the control group. Repeated measurement analysis of variance showed that there were interaction effect ( F interaction = 90.98, 46.27, 39.61, all P<0.01), time effect ( F time = 298.67, 179.06, 76.32, all P<0.01) and inter-group effect ( F intergroup = 19.88, 4.07, 18.70, all P<0.05) on mindfulness score, disease uncertainty score, total score of sleep quality and scores of all dimensionsin between the two groups. Simple effect analysis showed that scores of all scales in experimental groups were significantly better than those in control group after the intervention ( F values were 11.75 to 66.49, all P<0.01). Conclusion:Self-help MBSR training can improve the level of mindfulness, reduce the feeling of uncertainty and improve the quality of sleep in patients with atrial fibrillation after radiofrequency ablation.

ABSTRACT

Objective:To construct a mindfulness-based stress reduction (MBSR) intervention program suitable for uremic hemodialysis patients, and analyze the impact of the program on renal function and quality of life in uremic hemodialysis patients. Methods:This was a randomized controlled trial. The convenience sampling method was used to select 92 uremic patients who underwent hemodialysis in the First People's Hospital of Lianyungang City from March 2018 to March 2019. They were divided into routine group (46 cases, routine care) and MBSR group (46 cases, MBSR of face-to-face guidance combined with WeChat platform supervision) by random number table method. Both groups were intervened for 8 weeks. The Chinese version of the European Five-Dimensional Scale (EQ-5D-3L) was used to evaluate the quality of life of the patients, and the quality of life of the two groups before and after the intervention was compared; and the blood creatinine (Scr) and estimated glomerular filtration rate (eGFR), urea nitrogen (BUN), cystatin C (CysC) levels of the two groups before and after the intervention were analyzed. Results:Before the intervention, there was no significant difference in the levels of Scr, eGFR, BUN, and CysC between the two groups ( P>0.05); after 8 weeks of intervention, the levels of Scr, eGFR, BUN, and CysC were (201.81±14.77) μmol/L, (35.30 ± 2.02) ml/min and (11.47 ± 2.66) mmol/L, (2.41 ± 0.28) mg/L in the MBSR group, (218.37 ± 14.90) μmol/L, (33.99 ± 1.95) ml/min, (12.50 ± 0.76) mmol/L, (2.76 ± 0.30) mg/L in the routine group, the differences were statistically significant between the two groups ( t values were 2.53-5.79, all P<0.05). Before the intervention, there was no significant difference in EQ-5D-3L scores between the two groups ( P>0.05); after 8 weeks of intervention, the pain (discomfort), anxiety (depression), Vasual Analogue Scale (VAS) scores were (1.17 ± 0.34), (1.02 ± 0.35), (88.57 ± 20.28) points in the MBSR group, and (1.46 ± 0.63), (1.30 ± 0.32), (62.69 ± 18.79) points in the routine group, the differences were statistically significant between the two groups ( t=2.75, 4.00, 6.35, all P<0.05). Scr level was negatively correlated with self-care, pain (discomfort), anxiety (depression), mobility, daily activity ability, and VAS ( r values were -0.481 - -0.214, all P<0.05); eGFR level was positively correlated with self-care, pain (discomfort), anxiety (depression), mobility, daily activity ability, and VAS ( r values were 0.199-0.492, all P<0.05). But BUN and CysC levels were not correlated with EQ-5D-3L score (all P>0.05). Conclusions:MBSR can effectively improve the renal function and quality of life of uremic hemodialysis patients, and it is worthy of clinical application.

ABSTRACT

BackgroundChronic obstructive pulmonary disease (COPD) is a common chronic respiratory disease, and patients with COPD often experience substantially emotional difficulties, such as anxiety and depression, all of which may cause serious detriment to the prognosis of patients. As a non-pharmacological intervention in clinical practice, group mindfulness-based stress reduction therapy (MBSR) is beginning to emerge, while has rarely been studied in COPD patients with concurrent emotional difficulties. ObjectiveTo evaluate the effects of group MBSR on depression, state of mindfulness and pulmonary function in stable COPD patients, so as to provide references for the application of group MBSR in patients with COPD. MethodsA total of 97 patients with stable COPD who were followed up in the Department of Respiratory and Critical Care Medicine of Mianyang Third People's Hospital from January to October 2019 were selected as the study objects, and they were assigned into study group (n=50) and control group (n=47) by random number table method. All individuals received routine medication therapy and an 8-week health education, based on this, participants in study group partook an 8-week intervention comprising group MBSR. At the baseline, 4 weeks and 8 weeks of intervention, participants were assessed with Self-rating Depression Scale (SDS), Five Facet Mindfulness Questionnaire (FFMQ) and COPD Assessment Test (CAT), as well as the pulmonary function testing. ResultsThere were 41 patients in study group and 42 cases in control group completed the study. The group * time interaction was interpreted as significant between two groups for SDS, FFMQ and CAT scores (F=54.858, 86.161, 69.862, P<0.01). Baseline SDS, FFMQ and CAT scores of the two groups yielded no statistical difference between two groups (F=0.240, 0.052, 0.019, P>0.05), while study group scored lower on SDS and CAT (F=12.900, 38.511, 7.797, 28.824, P<0.01) and higher on FFMQ (F=27.324, 82.412, P<0.01) than those of the control group after 4 and 8 weeks of intervention. With the prolongation of intervention time in study group, participants demonstrated an overall reduction in SDS and CAT scores (F=109.753, 124.144, P<0.01), and an increase in FFMQ scores (F=228.194, P<0.01). There were no between-group differences in forced expiratory volume in one second as percentage of predicted volume (FEV1%pred) after 4 and 8 weeks of intervention (F=0.104, P=0.748) , and the within-group changes in FEV1%pred value over the intervention period in study group was not statistical (F=0.561, P=0.458). ConclusionGroup MBSR may help relieve depressive symptoms, enhance mindfulness level, and alleviate clinical symptoms in stable COPD patients, but has no effect on pulmonary function. ABSTRACT

BACKGROUND: Mindfulness-based stress reduction (MBSR) is a structured group program that employs mindfulness meditation to alleviate suffering associated with physical, psychosomatic, and psychiatric disorders. In this study, we investigate the impact of MBSR on left ventricular (LV) and endothelial function in female patients with microvascular angina. METHODS: A total of 34 female patients (mean age 52.2 ± 13.8 years) diagnosed with microvascular angina underwent a MBSR program with anti-anginal medication for 8 weeks. The global longitudinal strain (GLS) of the LV was used as a parameter to assess myocardial function and reactive brachial flow-mediated dilatation (FMD) was used to assess endothelial function. Symptoms were analyzed by the Symptom Checklist 90 Revised to determine emotional stress. Changes in GLS and FMD between baseline and post-MBSR were analyzed. RESULTS: After 8 weeks of programmed MBSR treatment, stress parameters were significantly decreased. In addition, GLS (−19.5 ± 2.1% vs. −16.6 ± 2.5%, p < 0.001) and reactive FMD significantly improved (8.9 ± 3.0% vs. 6.9 ± 2.6%, p = 0.005) after MBSR compared to baseline. The changes in GLS correlated to changes in FMD (r = 0.120, p = 0.340) and with the changes in most stress parameters. CONCLUSION: MBSR has beneficial impacts on myocardial and endothelial function in female patients with microvascular angina.

Participants in con-trol group received no intervention, while participants in experimental group received MBSR once a week for 8 weeks. All participants were assessed with the Zarit Caregiver Burden Interview ( ZBI,measuring caregiver burden) ,Beck Depression Inventory( BDI,measuring depression level) and Hamilton Anxiety Scale( HAMA, measuring anxiety level) before and after intervention. Results After 8-week intervention, participants in experimental group got significantly lower total scores of ZBI,BDI and HAMA compared with pretest ((22.31 ±5.76) vs (25.98±6.00),(8.47±3.21) vs (11.87±3.51),(14.98±4.75) vs (17.93±4.65), P0.05).Caregiver burden played mediating effect in MBSR for improving depression, and mediating effect was 56.6% of the total effect. Conclusion MBSR can effectively improve anxiety and depression with the spou-ses of breast cancer patients, and the caregiver burden is the intermediary variable in the effect of MBSR for improving depression.

ABSTRACT

Psychological stress including depression and anxiety are independent risk factors for cardiovascular disease morbidity and mortality, especially in women. Emotional regulation plays a mediating role in the development of depression and physical illness, and can alter resting physiologic responses associated with the stress response. Mindfulness-based stress reduction (MBSR) is a structured group program that employs mindfulness meditation to alleviate suffering associated with physical, psychosomatic, and psychiatric disorders. MBSR was originally developed for the management of chronic pain, which is now used widely to reduce psychological morbidity associated with chronic illnesses and to treat emotional and behavioral disorders. In cardiovascular disease, MBSR may be helpful for controlling several risk factors for coronary heart disease such as hypertension, type 2 diabetes mellitus, dyslipidemia, oxidative and psychosocial stress, obesity, and smoking, and improvements in submaximal exercise responses and heart rate variability. Although the most effective mode of stress reduction therapy is yet to be established, increasing recognition is being given to MBSR therapy.

The coping style, the hope level and the quality of life between the two groups were compared by Simplified Coping Style Questionnaire, Herth Hope Scale, European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ- c30). Results Before the treatment, two groups of positive and negative coping had no statistically significant difference (P>0.05). After the treatment of the observation group actively respond to score as follows: (25.44 ±2.61) points, significantly higher than the control group:(19.42±3.33) points, negative coping scores as follows: (9.76±1.89) points, significantly lower than the control group:(11.99±2.43) points, the differences between the two groups were significant (t=-10.468, 7.572, both P0.05). The scores of the reality and the future of positive attitude, positive action, keep close relationship with others were (14.5±2.6), (15.1±2.4), (15.6±2.1) points after the treatment in the observation group, and (10.1±2.7), (10.5±2.3), (11.6±2.5) points in the control group, and there were significant differences (t=4.965, 5.569, 3.659, all P0.05). The scores of physical function, role function, emotional function, cognitive function, social function, fatigue, pain, general health questionnaire, shortness of breath, insomnia, loss of appetite and the economic difficulties were (69.3±15.5), (74.1±22.6), (68.3±21.5), (79.7±23.4), (72.6±25.2), (42.1±26.1), (30.1±26.2), (55.6±15.6), (35.2±27.4), (36.2±28.7), (33.6±28.3), (25.6±24.3) points after the treatment in the observation group, and (58.6±21.2), (61.4±26.2), (75.6±20.4), (65.4±22.3), (55.4±28.7), (48.5±25.3), (37.6±29.2), (30.2±11.3), (41.6±28.7), (44.6±31.3), (40.2±30.4), (59.6±32.4) points in the control group, and there were significant differences (t=-39.369-15.621, P<0.01 or 0.05). Conclusions The mindfulness- based stress reduction can significantly improve the coping style in patients with liver cirrhosis and ascites, the level of hope and improve quality of life of patients, it is worth clinical promotion.
