ABSTRACT

In China, gastric cancer is the top three malignant tumors with high incidence and mortality, which seriously endangers the lives and health of people. The comprehensive treat-ment based on surgery is still the main treatment for gastric cancer. Through the unremitting efforts and hard works of gastric cancer experts, the treatment effect of gastric cancer in China has made great progress. However, gastric cancer surgery still faces some outstanding problems. How to further improve preoperative accurate diagnosis? How to further rationalize the treatment of early gastric cancer? Where is the future development of minimally invasive techniques for gastric cancer surgery? How to break through the bottleneck of conversion therapy? These are the challenges for gastric cancer surgery. Based on relevant literature and combined with own clinical practice, the author explores the hot issues of gastric cancer surgery.

ABSTRACT

Pancreatic surgery is an important specialty in the field of general surgery. As the anatomical location of pancreas is deeply and complex, and the diseases of pancreatic surgery is various kinds of and serious with high difficulty in surgery, the development of pancreatic surgery is slowly in the past. In the recent 20 years, with the deepening of basic and clinical researches in related fields, researchers' cognition of pancreatic diseases has gradually deepened. A series of new con-cepts and techniques have been applied to pancreatic surgery, making pancreatic surgery enter a rapid development stage. At the same time, researchers also recognize that the existing medical concepts and technical means are not sufficient to solve all clinical problems. Therefore, the cancer of pancreatic surgery in China can be alive and flourishing by grasping the opportunities, making continuous exploration and innovation.

ABSTRACT

Since entering the 21st century, colorectal surgery in China has developed rapidly, with new technologies emerging and ideas innovating constantly. While conforming to the world trend, our colorectal surgery colleagues in China have actively carried out and popularized new technologies, conducted relevant high quality researches, and formulated corresponding surgical treatment guidelines and expert consensus for colorectal cancer based on the received evidence-based medical evidences. Nowdays, the treatment of colorectal cancer in China has entered the era of individualized precision minimally invasive medicine. The author makes a systematic review of colorectal surgery development in China in the past two decades, and furthermore prospects the future of colorectal surgery in China.

ABSTRACT

Objective To study the EnCor vacuum negative pressure rotary system in ultrasonic BI-RADS 2~3 scale mammary gland disease application value.Methods Under the guidance of ultrasound, the application EnCor vacuum negative pressure rotary cutting system for 142 patients, 366 cases of breast lesion resection, specimens sent to pathological examination.Results A total of 366 breast lesions, 359 complete resection, the success rate was 98.1%, 2 transit operation, and transfer rate was 0.5%, 5 partial nephrectomy, residual rate was 1.4%.359 complete resection of the pathological results for breast malignant lesions in 4, the infiltrating ductal carcinoma 1, 3 ductal carcinoma in situ;breast benign lesions in 355, of which 132 breast fibroadenoma, adenosis with adenoma formation of 60, 99 breast adenosis, breast adenosis with duct expansion 43, 11 breast intraductal papilloma, one with four individual catheter atypical increase the living.Two transit operation pathological results were fibroadenoma, 5 disease of partial nephrectomy for adenosis with duct expansion.Conclusions The excision of breast neoplasm EnCor vacuum negative pres-sure rotary cutting system, not only small trauma, good cosmetic effect, and reduce misdiagnosis and im-prove the patients'satisfaction.

ABSTRACT

Objective Pedode foxatoon at the level of fracture vetebral technology is widely used in rebuilding spnal fracture recently.The purpose of minimally invasive surgery for thoracolumbar fractures is to reduce the injury , fix segments, and rebuild the stability of the spine .This study aimed to investigate the short-term clinical effect of pedicle fixation at the fractured vertebra via the pa-ravertebral intermuscular approach for the treatment of thoracolumbar fractures . Methods We treated 24 cases of thoracolumbar fracture by pedicle fixation at the fractured vertebra via the paravertebral intermuscular approach , rebuilt spinal stability , and corrected kyphotic deformity .We evaluated the improvement of pain symptoms and life quality using The Visual Analogue Scale ( VAS) and The MOS 36-Item Short-Form Health Survey (SF-36), and assessed the corrected status of vertebral height loss and kyphotic deformity by radiographic measurement .All the patients were followed up for over 12 months. Results The VAS score of the patients was signifi-cantly higher while the SF-36 score remarkably lower before surgery than the scores in 12 months which were ([7.65 ±0.13] -[1.54 ±0.07], P<0.05) and ([90.21 ±2.02]-[117.21 ±1.02], P<0.05).Imaging evaluation showed desirable correction of the reduced height of the injured vertebrae and kyphotic deformity . Conclusion Pedicle fixation at the fractured vertebra via the pa-ravertebral intermuscular approach , with its advantages of minimal invasiveness and fewer segments of fixation , can effectively correct vertebral height reduction and kyphotic deformity .
