ABSTRACT

Background and objectives: Moderator band is a specialized bridge present between the base of the anteriorpapillary muscle and interventricular septum. It carries the right branch of the bundle of HIS with it. The band isknown to prevent the over distension of the right ventricle during the diastolic phase. There is a need of lot ofresearch and studies on the septomarginal trabecula as it proves to be important clinically. Here we measuredthe length, breadth, height, angle with the interventricular septum, and the superficial marking of Moderatorband on the sternocostal surface of the right ventricle is done. This paper describes the morphological variationsfound in its origin and insertion.Materials and Methods: 15 formalin-soaked cadaveric hearts were taken from the Department of Anatomy ofKVG Medical College, Sullia. The foetal hearts, the hearts with gross pathology, if any are excluded from the study.Result: The morphometric parameters are tabulated and the morphological variations are noted. The results arecompared with the results of the previous studies on the moderator band.Conclusion: It is confirmed that the thickness of moderator band varies at its origin and insertion. The surfacemarking of moderator band on sternocostal surface of the heart will help in locating the moderator band throughthe echocardiography and during cardiac surgeries on right ventricle. The possibility of second moderator band,multiple origin or insertion is more than 20%.

ABSTRACT

Objective: To facilitate personalized treatment for major depressive disorder (MDD) among adults, a systematic review was conducted to explore the moderators of treatment outcome between pharmacotherapy and psychotherapy. Methods: A systematic search of papers, published in English before April, 2017, was conducted in Pubmed, PsycINFO and EMbase. Totally 754 papers were screened in accordance with the predefined inclusive and exclusive criteria. Then the selected literatures were scored according to randomized controlled trial (RCT), statistical, and the sample-size quality criteria, and analyzed to find the moderators. Results: A total of 30 papers were included. Totally 11 got " + " in all four domains in RCT quality, 25 got the score of 4 in statistical quality, and 15 got 6 in sample-size quality. And the moderators were tested across 7 categories, namely, social demographic and contextual factors, symptom severity, preference, symptom subtype, comorbid personality traits or disorders, comorbid emotional disorders or emotional reactivity, and biological factors. Hypometabolism of the right anterior insula, positive summed functional connectivity, preference for psychotherapy, a greater number of events, married/co-habiting status, unemployment, therapy sites with better psychotherapists, more prior antidepressant exposures, and childhood trauma might be associated with better outcomes with psychotherapy, while preference for medicine, melancholic features, high levels of neuroticism, comorbid personality disorders, comorbid anxiety emotional disorders predicted better outcomes in pharmacotherapy than in psychotherapy. Conclusion: Patient characters have a moderation effect on outcomes between pharmacotherapy and psychotherapy, thus these characters should be considered in the selection of the two therapies. Due to the limited quantity of these reviewed papers, however, the results should be retested by future studies.

ABSTRACT

Objective To explore the relationship between big-five personality,and public mood of volunteers,and to find out the moderator effect of justice sensitivity.Methods A total of 1 022 volunteers were investigated.The Big-Five Inventory (BFI),Public Mood Scale (PMS),Justice Sensitivity Inventory (JSI) were applied to all subjects.Results The average score was (19.77±4.49) for extraversion,(26.69± 4.13) for agreeableness,(23.33±5.37) for conscientiousness,(12.49±4.91) for neuroticism,(24.95±5.44)for openness,(8.78±3.23) for positive public mood,(3.46±3.06) for negative public mood and correlated with (26.85± 10.55) for observer sensitivity.The extraversion,agreeableness,conscientiousness and openness were correlated with positive public mood(β=0.31,P<0.01;β=0.25,P<0.01;β=0.29,P<0.01;β=0.26,P<0.0 1),and negatively correlated with public mood (β=-0.14,P<0.01;β=-0.29,P<0.01;β=-0.20,P<0.01;β=-0.08,P<0.01).Neuroticism was negatively correlated with positive public mood (β=-0.27,P<0.01),and positively correlated with negative public mood (β=0.31,P<0.01).The interaction effect of agreeableness and observer sensitivity negatively correlated with positive public mood,negative public mood (β=-0.09,P<0.01;β=-0.07,P<0.05).The interaction effect of neuroticism and observer sensitivity was positively correlated with positive public mood (β=0.08,P<0.01).Conclusion Observer sensitivity plays a regulative role between big-five personality and mental health,and can slow the negative impact of low agreeableness to positive public mood,strengthen the impact of low agreeableness to negative public mood,and slow the negative impact of high neuroticism to positive public mood.

ABSTRACT

Background: The moderator band has the right bundle of His within the right ventricle. It also is known to prevent the over dilatation of RV. It is important because of its involvement in RV infarcts, in differentiating RV from LV in congenital anomalies, in VPDs & VTs and in VSDs. The present literature is very much lacking in its morphometry. Materials and Methods: Moderator band was looked for in 50 hearts and only in this study did we find it in all the hearts. The length, the thickness, the distance from the tricuspid valve along with the presence of any trabeculations in the septal end or in the papillary muscle end were looked for and recorded. Results: The average length was 14.71±4.99mm, the average thickness was 4.97±1.64 mm, the average distance from the tricuspid orifice was 19.85±5.92 mm. The attachment to the anterior papillary muscle was found to be branched in 8 hearts that is having an incidence of 14% . The septal end were branched and separated as two in only two specimens. Only one specimen had branched attachments on both the apical and septal ends. Conclusion: The distance from tricuspid valve and knowledge of branching are important in corrective surgeries of the VSDs. An awareness of length and thickness helps the clinician to differentiate moderator band from the other structures, which may result in misdiagnosis during echocardiography. Its presence may in fact be of significance in preventing over distension of the heart in RV myocardial ischaemia.

ABSTRACT

Objective: optimism is an important variable that has consistently been shown to affect adjustment to quality of life in chronic diseases. This study aims to clarify if dispositional optimism exerts a moderating or a mediating influence on the personality traits-quality of life association, in Portuguese chronic patients. Methods: multiple regression models were used to test the moderation and mediation effects of dispositional optimism in quality of life. A sample of 729 patients was recruited in Portugal's main hospitals and completed self-reported questionnaires assessing socio-demographic and clinical variables, personality, dispositional optimism, quality of life (QoL) and subjective well-being (SWB). Results: the results of the regression models showed that dispositional optimism did not moderate the relationships between personality traits and quality of life. After controlling for gender, age, education level and severity of disease perception, the effects of personality traits on QoL and in SWB were mediated by dispositional optimism (partially and completely), except for the links between neuroticism/openness to experience and physical health. Conclusion: dispositional optimism is more likely to play a mediating, rather than a moderating role in personality traits-quality of life pathway in Portuguese chronic patients, suggesting that "the expectation that good things will happen" contributes to a better quality of life and subjective well-being. .


Objetivo: o otimismo tem sido demonstrado como uma variável importante no ajustamento da qualidade de vida de pessoas com doenças crônicas. O estudo tem como objetivo verificar se o otimismo exerce um efeito moderador ou mediador entre os traços de personalidade e a qualidade de vida, em portugueses com doenças crônicas. Métodos: os modelos de regressão linear múltipla foram usados para avaliar o efeito de moderação e mediação do otimismo na qualidade de vida. A amostra, constituída por 729 doentes, recrutados nos principais hospitais de Portugal responderam a questionários de autorresposta avaliando questões sócio-demográficas e clínicas, personalidade, otimismo disposicional, qualidade de vida e bem-estar subjetivo. Resultados: os resultados encontrados mostraram que o otimismo disposicional não exerce um papel moderador entre os traços de personalidade e a qualidade de vida. Controlando por idade, sexo, nível de escolaridade e percepção da severidade da doença, o efeito dos traços de personalidade na qualidade de vida e no bem-estar subjetivo foi mediado pelo otimismo (parcial e total), expecto para as associações, neuroticismo/abertura à experiência e à saúde física. Conclusão: o otimismo disposicional exerce apenas um papel mediador entre os traços de personalidade e qualidade de vida, em pessoas com doenças crônicas, sugerindo que 'a expectativa de que coisas boas vão acontecer' contribui para uma melhor qualidade de vida e melhor bem-estar subjetivo. .

ABSTRACT

We report herein a case of 35-year-old male with a history of complex congenital heart disease, consisting of double-chambered right ventricle (DCRV) with ventricular septal defect and presenting with transient 2:1 atrioventricular (AV) block on electrocardiogram. This unique presentation has not yet been described in literature, hence it was worth describing this rare complication of DCRV.

ABSTRACT

OBJECTIVES: The purpose of this study is to identify the role of individual innovation to demographic variables for determining IT adoption behaviors. This study also examines the effect of individual innovation on IT adoption behaviors across IT types. METHODS: To verify the invariant effect of individual innovativeness, two groups of persons working in the health care field were surveyed. The first study subject group was radiologists and their adoption of e-purchasing the second group was emergency rescue crews and their adoption of GPS. RESULTS: Adopter categories in innovations (ACI) as the measurement of individual innovation were a significant variable in both studies. Innovative adopters were more likely to use new IT tools than the majority of early adopters, and the early majority was more likely to adopt IT than the laggards. After merging the two data sets into one for testing the role of IT types as a moderator, the significance of ACI did not change, compared to the two separate analyses. In the merged data set, innovative adopters were 2.34 times more likely to be adopters than the early majority. The early majority was 2.32 times more likely to be adopters than laggards. Moreover, there were no moderating effects of IT types. Thus, there were no reversed adoption rates according to levels of ACI and demographic variables. CONCLUSIONS: ACI has invariant effects on IT adoption behaviors regardless of IT types and demographic differences. To implement a new innovation, understanding individual innovativeness will provide more sophisticated implementation strategies for health care organizations and appropriate education programs for their employees.

ABSTRACT

PURPOSE: This study sought to determine whether abdominal obesity is a risk factor for impaired fasting glucose (IFG) and hypertriglyceridemia and to verify whether moderate effect of abdominal obesity on the relationship between IFG and hypertriglyceridemia in Korea. MATERIALS AND METHODS: Data from the Korean National Health and Nutrition Examination Survey was used for the analysis. The study population included 5,938 subjects aged 20 year old drawn from non-diabetic participants in a health examination survey. The subjects were classified according to the presence of abdominal obesity based on waist circumference, IFG based on their fasting blood glucose level, and hypertriglyceridemia on their fasting triglyceride. RESULTS: The multivariate-adjusted odds ratios for the occurrence of hypertriglyceridemia were 2.91 in the abdominal obesity group as compared with the nonobesity group and 1.31 in subjects with IFG compared with the normoglycemia controls. Abdominal obesity was found to be positively moderated in the interaction between waist circumference and fasting blood sugar. CONCLUSION: The moderate effect between abdominal obesity and IFG contributes to the development of hypertriglyceridemia in Korea.

Objective To explore the relationship of hardiness,stress and psychological symptoms.Methods 622 college students participated the present study.Hardiness Scale,Chinese College Student Psychochological symptoms(r=-0.240,P<0.01).Stress had significantly positive correlation with psychological sympefficient of interaction between stress and hardiness had significant predictability on psychological symptoms(△R2=0.041,P<0.01).Conclusion Stress had a significant predictability on psychological symptoms.Hardiness was a moderator on stress and psychological symptoms,which can relieve adverse effects induced by stress.

Objective;To explore the regulative effect of perceived social support between personality and mental health.Methods;With Perceived Social Support Scale,NEO Five Factor Inventory and General Health Questionnaire-GHQ 20,1800 college students from 9 Hunan colleges and universities were inverstigated.Results;①Significant positive correlation existed between the factors of GHQ-20 and NEO-N,while significant negative correlation between the factors of GHQ-20 and NEO-E,NEO-C and all factors of perceived social support;perceived social support and its factors had significant negative correlation with NEO-N,while significant positive correlation with NEO-E and NEO-C.②There was significant interaction effects between perceived social support and personality existing in mental health.Conclusion;Students' perceived social support play a regulative role between personality and mental health.

ABSTRACT

The specimens were taken from two adults' hearts, thirty-four and twenty-eight years of age respectively. The A-V bundle was taken from the middle part of postero-inferior edge of the membranous part of the ventricular septum. The moderator band were cut from its superficial and deep part. The pseudochordae tendineae were collected from the left ventricle. All of the samples were fixed by 4% glutaraldehyde. The chief results were as follows:There are many Purkinje cells in the A-V bundle, moderator band and pseudochordae tendineae. The shape of the Purkinje cells is broader and shorter than the ordinary myocardial cells. The myofibrils and myofiliments of the Purkinje cells are less than of those of the ordinary myocardial cells. The mitochondria of the Purkinje cells were often situated around the nucleus. In the A-V bundle, a few transitional cells and general myocardial cells may be observed. Therefore the His bundle may be consisted of Purkinje cells transitional cells and a few general myocardial cells. Our findings are not in accordance with that of Glomset et al. They observed that the A-V bundle only consisted of the general ventricle myocardial cells. The intercalated discs present between the Purkinje ceils in the A-V bundle were not typical. Besides the intercalated discs, we observed two sorts of the junction types in the A-V bundle: (1) An intercalated disc was observed between a branch of a transitional cell and the lateral surface of the other adjacent transitional cell. (2) Through the collagenous septum, there was a junction between a transverse branch of transitional cell and opposite transitional cell.
