ABSTRACT

AIM:To observe the clinical efficacy of modified Buyang Huanwu Decoction in treating non-proliferative diabetic retinopathy(NPDR)of qi and yin deficiency and stagnation of collaterals, and to quantitatively analyze the changes in peripapillary vessel density before and after treatment using optical coherence tomography angiography(OCTA).METHODS:A randomized controlled trial was used to collect a total of 58 patients(99 eyes)with qi and yin deficiency and stagnation of collaterals NPDR who visited our hospital from June 2022 to November 2022, and patients were randomly divided into an observation group(n=29, 51 eyes)and a control group(n=29, 48 eyes). The control group received basic treatment according to the recommendations for DR published by the American Academy of Ophthalmology in 2019(blood glucose control, diabetes health education, and regular follow-up for patients with mild NPDR; and add local/grid-like laser photocoagulation if necessary for patients with moderate NPDR), while the observation group received modified Buyang Huanwu Decoction in addition to the basic treatment for 1mo. The best-corrected visual acuity(BCVA), traditional Chinese medicine(TCM)efficacy, peripapillary telangiectasia vessel density(ppVD), and changes in peripapillary retinal nerve fiber layer(pRNFL)thickness were compared between the two groups before and after treatment.RESULTS:The BCVA(LogMAR)of the observation group was 0.20(0.10, 0.30)after 1mo of treatment, which was significantly improved compared with that of the control group of 0.30(0.20, 0.40; P<0.05). The TCM efficacy in the observation group after 1mo of treatment was better than that in the control group(P<0.05). The ppVD in all quadrants of the observation group showed a significant improvement at 1mo after treatment, and the ppVD in all quadrants of the observation group was higher than that of the control group(P<0.05). The pRNFL thickness in the superior, temporal, and average peripapillary areas of the observation group increased after 1mo of treatment, and the pRNFL thickness in the superior, temporal, inferior quadrants, and average peripapillary area of the observation group was higher than that of the control group(P<0.05).CONCLUSION:Modified Buyang Huanwu Decoction can improve visual acuity and enhance TCM efficacy in patients with NPDR of qi and yin deficiency and stagnation of collaterals. It may be related to its ability to improve ppVD and reduce damage to the pRNFL.

ABSTRACT

Objective To observe the therapeutic effect of modified Buyang Huanwu decoction combined with betahistine hydrochloride in the treatment of vertebral-basilar artery ischemic vertigo and its influence on hemorheological index. Methods 80 patients with vertebral-basilar artery ischemic vertigo were randomly divided into observation group and control group,40 cases in each group. The control group was given betahistine hydrochloride 12 mg,three times a day.The observation group was given modified Buyang Huanwu decoction on the basis of the control group, 4 weeks for a course. The DARS and DHI were measured before and after 4 weeks of treatment. The blood flow velocity was detected by transcranial Doppler ultrasonography, the difference of clinical curative effect between the two groups was compared. Results After treatment, the DARS and DHI scores of the observation group and control group were significantly decreased(t=14.716 and 5.258,20.662 and 14.609,all P<0.01).The DARS and DHI scores in the observation group were significantly lower than those in the control group (t=3.644,7.116,all P<0.05).After treatment, the average blood flow velocity of the vertebral artery, right vertebral artery and basilar artery in the observation group and the control group were significantly increased(t=11.216 and 7.447,10.103 and 7.204,11.303 and 6.642,all P<0.01).The average blood flow velocity of the vertebral artery, right vertebral artery and basilar artery in the observation group were significantly higher than those of the control group (t=4.405,4.761,5.824,all P<0.01).The total effective rate of the observation group was 95.00% (38/40),which was significantly higher than that of the control group (77.50%,31/40), the difference was statistically significant (Z=-2.000,P<0.05).Conclusion Modified Buyang Huanwu decoction combined with betahistine hydrochloride is effective in the treatment of vertebro-basilar ischemic vertigo. It can improve the blood supply to the brain and relieve the symptoms of vertigo, which is worthy of popularization and application.

ABSTRACT

OBJECTIVE:To establish the method for simultaneous determination of paeoniflorin, amygdalin, ferulic acid and ligustrazine in Modified buying huanwu decoction.METHODS:RP-HPLC method was adopted. The determination was performed on YMC C18 column with the mobile phase consisted of acetonitrile-0.1% phosphoric acid (gradient elution) at the flow rate of 1.0 mL/min.The detection wavelengths were set at 320 nm (femlic acid),230 nm (paeoniflorin),207 nm (amygdalin),280 nm (ligustrazine). The column temperature was 30 ℃,and sample size was 10 μL.RESULTS:The linear ranges of paeoniflorin, amygdalin, femlic acid and ligustrazine were 0.191 2-1.912 μg/mL (r=0.999 6),0.117 4-1.174 μg/mL (r=0.999 6),0.011 5-0.115 μg/mL (r=0.999 8) and 0.001 66-0.016 6 μg/mL(r=0.999 7),respectively.The limits of quantitation were 1.912,1.174,0.115,0.016 6 μg/mL,and the limits of detection were 0.25,0.40,0.05,0.008 5 μg/mL,respectively.RSDs of precision, stability and reproducible tests were all lower than 2.0%.The recoveries were 96.9%-100.3% (RSD=1.3%,n=6),95.1%-100.3% (RSD=2.2%,n=6),95.3%-100.2% (RSD=2.0%,n=6)and 97.0%-100.0% (RSD=1.3%,n=6).CONCLUSIONS:The method is reliable, simple and accurate, and is suitable for simultaneous determination of paeoniflorin, amygdalin, ferulic acid and ligustrazine in Modified buyang huanwu decoction.

ABSTRACT

Objective To study the effects of modified Buyang Huanwu decoction on adjuvant-induced arthritis, and the mechanism of this decoction in treating rheumatoid arthritis. Methods Adjuvant-induced arthritis(AA) model was induced by plantar injection of Freundis complete adjuvant and Observed the level of interlinkin-1(IL-1),tumor necrosis factor-α(TNF-α),circulating immunecomplexes(CIC),malondialdehyde(MDA) and antioxidizing activity of superoxide dismutase(SOD)in AA rats treated with the decoction. Results The contents of IL-1,TNF-α,CIC,MDA obviously decreased and the activity of SOD was increased by Buyan Huanwu Decoction(P<0.01)in AA rats. Conclusion Modified Buyang Huanwu decoction shows visibly anti-inflammatory function on AA rats, which is associated with its mechanism of suppressing free radical injure and modifying autoimmune physiological and pathological reaction.
