ABSTRACT

ABSTRACT BACKGROUND: Non-alcoholic hepatic steatosis (NAS) is characterized by excess fat accumulation in hepatocytes, causing portal and lobular inflammation and hepatocyte injury. OBJECTIVE: We aimed to evaluate the alterations in monocyte count to high-density lipoprotein cholesterol ratio (MHR) in patients with grade 2 or 3 fatty liver disease and the association of this marker with liver function tests and insulin resistance. METHODS: In this retrospective analysis; patients diagnosed and followed for the grade 2 or 3 fatty liver disease were included in the patient group and the patients who had undergone abdominal ultrasound for any reason and who were not having any fatty liver disease were included in the control group. RESULTS: Totally 409 cases were included in the study. Among participants, 201 were in the control group, and 208 were in the NAS group (111 were having grade 2 and 97 were having grade 3 steatosis). The monocyte/HDL ratio was significantly higher in the NAS group compared with the healthy controls (P=0.001). There was a significant positive correlation between the monocyte/HDL ratio and age (r=0.109; P=0.028), ALT (r=0.123, P=0.014) and HOMA-IR (r=0.325, P=0.001) values. CONCLUSION: In conclusion, the monocyte to high-density lipoprotein ratio significantly increases in fatty liver disease and correlates with insulin resistance. Since it was suggested as a prognostic marker in atherosclerotic diseases, elevated MHR values in fatty liver disease should be evaluated cautiously.


RESUMO CONTEXTO: A esteatose não hepática (ENH) é caracterizada pelo acúmulo de gordura nos hepatócitos, causando inflamação portal e lobular e lesões ao hepatócito. OBJETIVO: Avaliar as alterações na contagem de monócitos em relação à proporção de lipoproteína de colesterol de alta densidade (MHR) em doentes com doença hepática gordurosa de grau 2 ou 3 e a associação deste marcador com testes de função hepática e de resistência à insulina. MÉTODOS: Nesta análise retrospectiva os pacientes diagnosticados e seguidos para a doença hepática gordurosa de grau 2 ou 3, foram incluídos no grupo de doentes e os indivíduos que tinham sido submetidos a ecografia abdominal por qualquer motivo e que não tinham qualquer doença hepática gordurosa foram incluídos no de controle. RESULTADOS: Foram incluídos 409 pacientes no estudo. Entre os participantes, 201 estavam no grupo controle e 208 estavam no grupo ENH (111 caracterizados como grau 2 e 97 com esteatose de grau 3). A relação monócito/HDL foi significativamente maior no grupo ENH em comparação com os controles saudáveis (P=0,001). Verificou-se correlação positiva significativa entre a relação monócitos/HDL e a idade (r=0,109; P=0,028), e valores de ALT (r=0,123; P=0,014) e HOMA-IR (r=0,325; P=0,001). CONCLUSÃO: A razão entre monócitos e a lipoproteína de alta densidade aumenta significativamente na doença hepática gordurosa e correlaciona-se com a resistência à insulina. Uma vez que foi sugerido como um marcador prognóstico em doenças ateroscleróticas, os valores elevados de MHR na doença hepática gordurosa devem ser avaliados com cautela.

ABSTRACT

Berberis darwinii Hook es una especie que habita el sur de Chile y la Patagonia. Siendo utilizada por la etnia mapuche para el tratamiento de procesos inflamatorios, estados febriles, y dolor estomacal. El propósito del siguiente estudio fue evaluar in vitro las propiedades del extracto de alcaloides de raíz de B. darwinii sobre respuestas celulares en monocitos desde sangre periférica de rata. Los resultados de la cuantificación del extracto muestran una concentración de alcaloides totales de 1,67 mg/g y la caracterización por HPLC- MS determinó la presencia de berberina y palmatina. In vitro se observó que los extractos disminuyeron la capacidad de adhesión y la actividad fagocítica de los monocitos e inhibieron la translocación del factor nuclear NF-κB asociado a la modulación de la inflamación, pero no así la producción de anión superóxido. Estos resultados indicarían que los alcaloides totales de B. darwinii inhiben algunos mecanismos específicos de defensa celular.


Berberis darwinii Hook is a species that inhabits southern Chile and Patagonia. This is being used by the Mapuche ethnic group for the treatment of inflammatory processes, febrile states, and stomach pain. The purpose of the following study was to evaluate in vitro the properties of an alkaloid extract of B. darwinii root on cellular responses in monocytes from the rat peripheral blood. The results of the quantification of the extract showed a total alkaloid concentration of 1.67 mg/g and the characterization by HPLC-MS determined the presence of berberine and palmatine. In vitro, it was observed that the extracts decreased the adhesion capacity and phagocytic activity of the monocytes and inhibited the translocation of the nuclear factor NF-κB associated with the modulation of inflammation, but not the production of superoxide anion. These results indicate that the total alkaloids of B. darwinii inhibit some specific mechanisms of cellular defense.

ABSTRACT

Berberis darwinii H is a native plant of South America, popularly referred to Michay. This species has historically been used by indigenous cultures of Chile as medicinal herb. To preliminarily assess their anti-inflammatory effects was investigated the aqueous and methanolic root extract this plant in human monocytes. The results indicated that the extracts inhibit the production of superoxide anion, the expression of tumor necrosis factor-alpha (TNF-alpha) and interleukin-1-beta (IL-1beta) in monocytes activated with lipopolysaccharide. This result suggests the existence of compounds with potential anti-inflammatory action in these extracts.


Berberis darwinii H. es una planta nativa de América del Sur, conocida popularmente como Michay. Esta especie ha sido históricamente utilizada por las culturas indígenas de Chile como hierba medicinal. Con el fin de evaluar preliminarmente sus efectos anti-inflamatorios, se investigaron dos tipos de extractos; metanólico y acuoso, preparados a partir de la raíz de esta planta. Los resultados indican que estos extractos inhiben la producción de anión superóxido, la expresión del factor de necrosis tumoral-alfa (TNF-alfa) y de interleucina-1beta, (IL-1beta) en monocitos activados con lipopolisacárido. Estos resultados sugieren la existencia de compuestos con potencial acción antiinflamatoria en esta planta.

Studies on host-parasite interaction in Jorge Lobo's disease are scarce, with no report in the literature on the phagocytosis of Lacazia loboi by phagocytic mononuclear cells. Thus, the objective of the present study was to assess the phagocytic activity of blood monocytes in the presence of L. loboi in patients with the disease and in healthy subjects (controls) over 3 and 24 hours of incubation. Statistical analyses of the results showed no significant difference in percent phagocytosis of the fungus between patient and control monocytes. With respect to incubation time, however, there was a significant difference, in that percent phagocytosis was higher at 3 hours than at 24 hours (p <0.01). These results suggest that monocytes from patients with the mycosis are able to phagocyte the fungus, as also observed in control individuals


Os estudos envolvendo a interação hospedeiro-parasita na doença de Jorge Lobo são escassos; até o momento, não existe nenhum trabalho abordando a fagocitose do fungo Lacazia loboi pelas células mononucleares fagocitárias. Assim, o presente estudo teve como objetivo avaliar a atividade fagocítica dos monócitos sangüíneos frente ao L. loboi tanto em pacientes portadores da doença como em indivíduos sadios (grupo controle), utilizando 3 e 24 horas de incubação. A análise estatística dos resultados revelou que não houve diferença significante entre o percentual de fagocitose do fungo pelos monócitos de pacientes e do grupo controle, porém em relação aos tempos de incubação, houve diferença significante, isto é, às 3 horas o percentual de fagocitose foi maior que o obtido às 24 horas (p<0,01). Estes resultados sugerem que monócitos de pacientes portadores da micose são hábeis em fagocitar o fungo, à semelhança do que foi visto em indivíduos do grupo controle
