ABSTRACT

We describe the case of a 25-year-old woman who presented with monoarthritis of the proximal interphalangeal articulation of the fourth left finger 4 days after receiving the second dose of the Oxford-AstraZenecaR SARS-CoV-2 vaccine. She had no abnormalities in her X-ray or blood exams, but she had a cousin diagnosed with juvenile rheumatoid arthritis. The patient had an excellent response to anti-inflammatory medication, the arthritis was transient and left no sequelae. Studies have shown some vaccines may be associated with acute arthritis, in particular the measles-mumps-rubella vaccine. Young women such as our patient seem to be more susceptible to post-vaccination arthritis. Most of the cases reported were transient and left no articular sequelae, thus we did not contraindicate further doses of the Oxford-AstraZenecaR SARS-CoV-2 vaccine (should they be recommended in the future) in this case.


Describimos el caso de una mujer de 25 años que presentó monoartritis de la articulación interfalángica proximal del cuarto quirodáctil izquierdo 4 días después de recibir la segunda dosis de la vacuna Oxford-AstraZenecaR SARS-CoV-2. No presentaba anomalías en sus radiografías ni en los exámenes de sangre, pero tenía una prima diagnosticada con artritis reumatoide juvenil. La paciente tuvo una excelente respuesta a la medicación antiinflamatoria, la artritis fue transitoria y no dejó secuelas. Los estudios han demostrado que algunas vacunas pueden estar asociadas con la artritis aguda, en particular las vacunas contra el sarampión, las paperas y la rubéola. Las mujeres jóvenes como nuestra paciente parecen ser más susceptibles a la artritis posvacunación. La mayoría de los casos notificados fueron transitorios y no dejaron secuelas articulares, por lo que no contraindicamos nuevas dosis de la vacuna Oxford-AstraZenecaR SARS-CoV-2 (en caso de recomendarse en el futuro) en este caso.

ABSTRACT

Introducción: el dolor articular agudo es una emergencia reumatológica. Con pocas excepciones, todas las patologías articulares pueden presentarse inicialmente como dolor agudo mono o poliarticular. Objetivo: revisar elementos asociados al diagnóstico temprano de la monoartritis aguda en pacientes con o sin evidencia de trauma para que sea identificada como una urgencia clínico quirúrgica. Desarrollo: la monoartritis aguda es la inflamación de una sola articulación con un dolor intenso, que se caracteriza por enrojecimiento de la articulación, calor, tumefacción y limitación de los movimientos con una duración aproximada hasta 6 semanas. Conclusiones: para realizar el diagnóstico y poder efectuar una correcta atención de urgencia es necesario tener en cuenta un amplio espectro de patologías en el diagnóstico diferencial, por tanto, es esencial llevar a cabo un buen interrogatorio, obtener información sobre la historia de la enfermedad actual y hacer un examen físico detallado. Ante estos eventos es preciso que las instituciones y servicios implicados disponibles de forma inmediata, prevista y coordinada las condiciones físicas y materiales necesarias para recibir al paciente e iniciar el inicio el tratamiento indicado bajo la supervisión de los especialistas(AU)


Introduction: Acute joint pain is a rheumatological emergency. With few exceptions, all joint pathologies can present initially as acute mono or polyarticular pain. Objective: To review elements associated with the early diagnosis of acute monoarthritis in patients with or without evidence of trauma to be identified as a surgical clinical emergency. Development: Acute monoarthritis is the inflammation of a single joint with intense pain, characterized by redness of the joint, heat, swelling and limitation of movements with an approximate duration of up to 6 weeks. Conclusions: In order to make the diagnosis and be able to carry out a correct emergency care, it is necessary to take into account a wide spectrum of pathologies in the differential diagnosis, therefore, it is essential to carry out a good interrogation, obtain information about the history of the current disease and do a detailed physical examination. In view of these events, it is necessary that the institutions and services involved immediately available and planned and coordinated the physical and material conditions necessary to receive the patient and initiate the indicated treatment under the supervision of the specialists(AU)

ABSTRACT

Objective: To explore the protective effect of hydrogen (H2) inhalation through respiratory tract on monoarthritis (MA) rats. Methods: Forty male SD rats were randomly divided into 5 groups: MA group, MA+H2 0-14 d group, MA+H2 0-3 d group, MA+H2 4-14 d group, and sham+H2 0-14 d group (n = 8). Complete Freund's adjuvant (CFA) was injected into the left ankle joint of rats to construct MA model. The rats in the MA group did not inhale H2, and the rats in other four groups inhaled 65% H2 1.5 h per day on the day of modeling and 1-14 days after modeling, 1.5 h per day on the day of modeling and 1-3 days after modeling, 1.5 h per day on the 4th-14th day after modeling, and 1.5 h per day on the day of sham operation and 1-14 days after modeling, respectively. Paw withdrawal threshold (PWT) was detected by von Frey method 0, 1, 3, 5, 7, 10, and 14 d after modeling. Another 15 rats were randomly divided into 5 groups and there were 3 rats in each group. Lumbar enlargement tissues were taken from the spinal cord of the inflammatory side on the 10th day after modeling or sham operation. The contents of superoxide dismutase (SOD), catalase (CAT) and malondialdehyde (MDA) were detected. Results: In the MA group, the posterior paw PWT of the inflammatory side was lower than that of the healthy side on the first day after modeling, reached the lowest on the 3th day and remained stable until the 14th day (P0.05). The levels of SOD and CAT in the spinal cord of the inflammatory side in the MA+H2 0-14 d group were higher than those in the MA group (all P0.05). Conclusion: Inhalation of 65% H2 via the respiratory tract at the initial stage of MA rat modeling can alleviate mechanical hyperalgesia and oxidative stress. Pre-administration of H2 can inhibit the formation of mechanical hyperalgesia in MA.

Background: Monoarthritis is a common rheumatological complaint. Inspite of investigations, many cases remain undiagnosed. Prompt investigation and treatment is important in acute arthritis especially septic arthritis else joint destruction, permanent disability or even death can result. This study was conducted to etiologically categorise patients as inflammatory, non-inflammatory and infective arthritis and to study the outcome.Methods: This observational prospective study conducted at a tertiary care hospital in Mumbai enrolled 40 patients above the age of 12 yrs presenting with first episode of mono-articular arthritis. They were treated with standard treatment guidelines and followed up every 3 monthly for one year. Outcome was assessed using ESR, CRP values and Health Assessment Questionnaire.Results: Mean age at diagnosis was 38 years with a male to female ratio of 1.4:1. Acute and chronic mono-articular arthritis cases were 16.2% and 83.7% respectively. Knee joint was most commonly involved (38%). Etiologically inflammatory, infectious and non-inflammatory cases were 59.5%, 29.7% and 10.8% respectively. In 21% cases etiology was tuberculosis. 27 % evolved into oligoarthritis over one year. The serial ESR, CRP values and Stanford Health Assessment Questionarre scores decreased significantly across all etiological groups with treatment.Conclusions: Knee is the most commonly affected joint in mono-articular arthritis. Tuberculosis is the most common etiology. Irrespective of the etiology, if patients are treated according to standard guidelines promptly mono-articular arthritis has a good response to therapy as assessed by the health assessment questionnaire (HAQ) and serial measurements of proinflammatory markers like ESR, CRP.

ABSTRACT

RESUMOLipoma arborescens (LA) é definido como uma proliferação vilosa lipomatosa da membrana sinovial com substituição difusa do tecido sinovial por células gordurosas maduras. A lesão é muito rara, envolve a articulação do joelho (região suprapatelar) e se associa a doenças articulares degenerativas. Descrevemos o caso de um paciente de sexo masculino, portador de espondilite anquilosante com quadro de monoartrite de joelho direito e aumento de volume importante na região suprapatelar. O diagnóstico de lipoma arborescens foi confirmado a partir da ressonância magnética do joelho direito.


ABSTRACTLipoma arborescens (LA) is defined as a villous lipomatous proliferation of the synovial membrane with diffuse substitution of the synovial tissue by mature fat cells. The injury is very rare, involving the knee joint (suprapatellar region) and is associated with degenerative joint disease. We describe the case of a male patient, suffering from ankylosing spondylitis with monoarthritis of the right knee and swelling in suprapatellar region important. The diagnosis of lipoma arborescens was confirmed from the MRI of the right knee.

ABSTRACT

La monoartritis es un reto diagnóstico para el clínico, ya que es extensa la lista de patologías asociadas. En pacientes con diagnóstico establecido de enfermedad articular inflamatoria, se acepta que la monoartritis corresponde a la exacerbación de la enfermedad de base; sin embargo, ignorar el abordaje sistematizado de las monoartritis puede generar omisiones e implicaciones diagnósticas erróneas. En este reporte se analiza el abordaje de un caso de artritis seudoséptica, simulando un ataque agudo de artritis por urato monosódico recurrente en un paciente con retención de cuerpo extraño intraarticular.


Monoarthritis is a diagnostic challenge for the clinician, as the list of associated conditions is quite long. It is accepted that in patients with a diagnosis of inflammatory joint disease monoarthritis represents exacerbation of the underlying disease. However, ignoring the systematized approach to monoarthritides may lead to omissions and mistaken diagnostic implications. This report describes the approach to a case of pseudoseptic arthritis that mimicked an acute episode of recurrent arthritis due to monosodium urate in a patient with retention of an intraarticular foreign body.

ABSTRACT

Nowadays, tumor necrosis factor-alpha (TNF-alpha) blockers are used for treatment of rheumatoid arthritis, inflammatory bowel diseases, ankylosing spondylitis, psoriatic arthritis, and psoriasis. Paradoxically, there are some reports on the appearance of psoriasis after administration of TNF-alpha blockers. Here, we report on a patient with monoarthritis in a knee joint who experienced psoriasis after TNF-alpha blocker therapy (adalimumab and etanercept). Oral medication was not a treatment option due to patient intolerance; thus, we tried ustekinumab, an anti-interleukin (IL)-12/23 monoclonal antibody used for treatment of psoriasis. Following ustekinumab injection, psoriatic skin lesions and joint symptoms were much improved. However, in the following period, joint pain and swelling became aggravated and synovial fluid cytokine levels including IL-6 and IL-17 were elevated. The treatment was changed to tocilizumab, a humanized monoclonal antibody against IL-6 receptor. After injection, knee joint swelling rapidly subsided without worsening of psoriatic skin lesions.

ABSTRACT

La sinovitis villonodular pigmentada (SVNP) es una proliferación sinovial benigna, asociada a depósitos de hemosiderina, que se origina en las articulaciones, bursas o vainas tendinosas. La articulación más afectada es la rodilla (80% de los casos) y menos frecuente hombro, codos y tobillos. La resonancia magnética, permite la detección de la SVNP y el estudio histopatológico hace la confirmación del diagnóstico. Describimos el caso de una paciente con monoartritis de rodilla izquierda que, después de múltiples estudios, la biopsia fue compatible con sinovitis villonodular pigmentada


Pigmented villonodular synovitis (PVNS) is a benign synovial proliferation associated with hemosiderin deposits, which originates in joints, bursa or tendon sheaths. The knee is the joint most commonly affected (80% of cases), and less frequently it can affect the shoulder, ankle and elbow. Magnetic Resonance Imaging (MRI) is used to detect PVNS, and histological analysis is used to confirm the diagnosis. We describe the case of a patient with monoarthritis of the left knee, who after multiple biopsy studies, it was found to be compatible with PVNS.

ABSTRACT

Abstract Arthritis in childhood is common. The pattern, presentation and duration of arthritis help differentiate between the various possible diagnoses. When only one joint is involved, i.e., monoarthritis, it may be difficult to make a diagnosis as there are many possibilities both acute and chronic in nature. A detailed history and clinical examination is important to reach a correct diagnosis and the single most important investigation when a child presents acutely is a joint aspiration to rule out septic arthritis that may destroy the joint in hours. Inflammatory markers, antinuclear antibody testing, test for tuberculosis and imaging (in specific cases) play an important role in the diagnosis of a child that presents with a chronic monoarthritis. In this article we provide a clinical approach to the diagnosis of monoarthritis in a child.

Artrites são manifestações clínicas de uma série de doenças. Sua classificação etiológica é muitas vezes difícil e depende de história clínica e exame físico cuidadosos. Artrite séptica e gota se apresentam mais comumente como monoartrite aguda e quadros reacionais são geralmente poliarticulares. A internação hospitalar é fator de risco para o desenvolvimento tanto de artrites reacionais quanto para crises de gota e artrite séptica. O diagnóstico precoce é muito importante a fim de iniciar o tratamento precocemente, alívio dos sintomas e preservação da funcionalidade articular. A punção do líquido sinoviale sua análise são de fundamental importância diagnóstica nos quadros de monoartrite aguda.


Arthritis are clinical manifestations of plenty of diseases. Its etiological classification is many timesdifficult and depends on careful clinical history and physical examination. Gout and skeptical arthritispresents commonly as acute monoarthritis and reactional arthritis are often polyarticular. Hospitalaradmission is a risk factor to the development of reactional arthritis, crisis of gout and septical arthritis.Early diagnosis is imperative to start early treatment, symptom relief and articular function preservation.Synovial fluid aspiration and its analysis are of critical diagnostic importance in cases of acute monoarthritis.

ABSTRACT

La sinovitis villonodular pigmentaria es una neoplasia benigna con proliferación sinovial y depósito de hemosiderina. Se presenta usualmente entre la tercera y cuarta década de la vida. Típicamente es monoarticular y puede afectar cualquier articulación, pero más frecuentemente compromete las grandes articulaciones de la extremidad inferior. El tratamiento se basa en la erradicación del tumor mediante cirugía o radioterapia.


Pigmented villonodular synovitis is a benign neoplasm with synovial proliferation and hemosiderin deposition. It occurs usually between 3th and 4th decades of life. Typically, it is monoarticular and can affect any joint but most commonly involves the large joints of the lower extremity. Treatment is based on the eradication of the tumor by surgery or radiotherapy.

Em quadros de monoartrite crônica devem ser investigadas doenças inflamatórias como a artrite reumatoide (AR), doenças infecciosas como a tuberculose e outras doenças que causem espessamento sinovial e derrame articular como sinovite vilonodular pigmentada (SVNP), hemangioma sinovial, osteocondromatose sinovial e lipoma arborescente sinovial. Relatamos o caso de uma jovem paciente com quadro de monoartrite em ombro, cujo exame por imagem mostrou sinovite e cujo exame histopatológico obtido através de artroscopia com biópsia revelou tratar-se de SVNP. RELATO DO CASO: J C M, 15 anos, sexo feminino, branca, estudante, foi encaminhada ao reumatologista com hipótese diagnóstica de artrite idiopática juvenil (AIJ) pauciarticular. Apresentava, há um ano, dor no ombro D, que melhorava com o uso de anti-inflamatório não-esteroidal (AINE) em dois dias. Teve nesse período de cinco a seis destes episódios que duravam poucos dias. Negava outras queixas articulares ou sistêmicas. Trazia exames normais ou negativos: hemograma, VHS, proteína C reativa, fator reumatoide, sedimento de urina. O FAN era positivo 1/80, pontilhado fino. Trazia Ressonância Magnética do ombro indicativa de sinovite glenoumeral com derrame articular com conteúdo expansivo de tecidos moles na bursa subescapular, segundo o laudo, podendo corresponder a pannus. Como história e exame físico não eram compatíveis com AIJ, foi realizada outra RM que mostrou aumento da lesão já descrita. Foi então encaminhada à artroscopia para biópsia, que revelou SVNP. Durante o procedimento, foi realizada sinovectomia, e uma nova RM feita após nove meses mostrou ausência de sinovite. A SVNP do ombro é incomum e a sinovectomia foi curativa nesse caso.


Chronic monoarthritis demand an investigation of inflammatory diseases, such as rheumatoid arthritis (RA), infectious diseases like tuberculosis; and other diseases that cause synovitis and joint effusion, such as pigmented villonodular synovitis (PVNS), synovial hemangioma, synovial osteochondromatosis and arborescence lipoma. We report the case of a young patient with chronic right shoulder monoarthritis, who's magnetic resonance imaging (MRI) showed synovitis. Arthroscopy was performed and the biopsy revealed PVNS. CASE REPORT: J C M, 15 years-old, female, Caucasian, student. She was sent to a Rheumatologist along with a diagnosis of juvenile idiopathic arthritis (JIA). The patient presented, for one year, a mild pain of insidious onset in her right shoulder, with relief of the symptoms in two days under nonsteroidal anti-inflammatory therapy (NSAIDs). During this year, the patient presented five or six episodes of pain with the same characteristics. No other signs and symptoms were related. The following tests showed normal or negative results:complete blood count, ESR, C-reactive protein, rheumatoid factor and urinalysis. The antinuclear antibody (ANA) was 1/80 speckled pattern. The MRI of the shoulder showed glenohumeral synovitis with joint effusion and soft tissue swelling in the subscapular bursa, which could correspond to pannus. As the medical history and physical examination were not compatible to JIA, a second MRI was performed, which showed an increase of the synovitis. The patient was submitted to an arthroscopy with biopsy and the histopathological examination showed PVNS. A complete synovectomy was performed and a new MRI, nine months later, showed no synovitis. PVNS of the shoulder is uncommon, and synovectomy was curative in this case.

ABSTRACT

Primary involvement of carpal bones in septic arthritis is a rare event. We report here a case of idiopathic septic arthritis with isolated involvement of the scaphoid-trapezium joint. An elderly patient presented with collapse and symptoms of septicaemia, and right wrist that was swollen and tender. Radiographs revealed erosion of the scaphoid-trapezium joint and increased joint space. The joint was aspirated yielding frank pus. Swelling and redness improved, but the patient died due to ongoing septicaemia. Localised septic arthritis of the scaphoid-trapezium joint, however rare, can be fatal if not aggressively treated in a timely fashion.
