ABSTRACT

Objective To analyze the MRI findings of pure mucinous breast carcinoma and to investigate the causes of misdiagnosis.Methods MRI findings of 1 3 patients with pure mucinous breast carcinoma were analyzed retrospectively.All patients were verified by surgery and histopathology.Results All the 13 cases were presented as a mass,among which 12 cases (92.3%)had lobular contour and 1 (7.7%) had irregular shape.11 cases (84.6%)showed low or iso-signal intensity and 2 (15.4%)showed heterogeneous iso or high intensity on T1WI.All the 13 cases showed high signal intensity on fat suppression T2WI,and 5 cases(38.5%)showed low signal fiber separation inside. 5 cases (38.5%)showed homogeneous high signal intensity and 8 cases (61.5%)showed heterogeneous high signal intensity on diffusion weighted imaging(DWI)and apparent diffusion coefficient(ADC)map,the ADC value was (2.002±0.478)× 10-3mm2/s.11 cases (84.6%)showed rim enhancement,among which 8(61.5%)showed persistent enhancement and 2 (15.4%)showed heterogeneous enhancement.Time-signal intensity cure(TIC)of 11 cases(84.6%)were monophasic or biphasic and 2 cases(15.4%0)were wash-out type. 5 cases(38.5%)were misdiagnosed on preoperative MRI.Conclusion There are several MRI features in pure mucinous breast carcinoma,such as lobular contour,high signal intensity on DWI and ADC map,persistent enhancement and monophasic or biphasic on TIC.Comprehending these MRI features is the key to reduce misdiagnosis.

ABSTRACT

Objective To explore the mixed mucinous breast carcinomas(MMBC)and pure mucinous breast carcinomas(PMBC) imaging features,and evaluate the diagnostic value of ultrasound,X-ray and MRI for mucinous breast carcinoma(MBC).Methods Totally 48 cases of MBC proved by postoperative pathology were analyzed retrospectively.Manifestations of ultrasonography,X-ray and MRI of MMBC and PMBC were summarized,and diagnostic values of three methods were compared.Results Most PMBC showed regular shaped (round,oval or lobulated)mass with clear edge,but most MMBC showed irregular shaped mass with unclear edge in three imaging examinations.On sonogram,MBC mainly showed mixed cystic-solid echo,most PMBC showed low blood signal with rear echo enhancement,and most MMBC showed rich blood signal with various rear echo.On MRI,MBC usually demonstrated high or strongly high signal intensity on T2 WI.On DCE-MRI,PMBC was mainly characterized by rim enhancement with persistent enhancement pattern, and MMBC was exhibited more heterogeneous or rim enhancement with plateau enhancement pattern.Three imaging methods were no significant difference in the accuracy of diagnosis between PMBC and MMBC.There was no statistical significance in three imaging methods.Conclusion PMBC and MMBC with different the appearances of ultrasonography,X-ray and MRI have some characteristics.Fully aware of imaging features can not only identify two types of MBC,but also reduce missed diagnosis and misdiagnosis of MBC.

ABSTRACT

Mucinous carcinoma of the breast is a relatively rare malignancy accounting for 2% of all breast cancers. Mucinous carcinoma of the breast has a favourable prognosis and is usually seen in postmenopausal women. Here, we report a 60 year old female patient presented with right breast lump was diagnosed on cytology as ductal carcinoma of the breast, on histopathology diagnosed as mucinous carcinoma with neuroendocrine differentiation, which was subsequently confirmed on IHC marker synaptophysin and chromogranin A positive.

ABSTRACT

Objective To explore clinical characteristics,pathology,prognosis and proper treatment of pure mucinous breast carcinoma(PMBC).Methods Retrospective analysis Was carried out on patients with PMBC who were admitted from Jan.2004 to Dec.2010.Results PMBC patients accounted for 2.6%of all the patients with breast cancer treated at the same period.The mean age of PMBC patients was(55.28±15.73)years,ranging from 32 years to 81 years.The tumor diameter was from 1 to 10 cm and axillary lymph node involvement was 0%.Immunohistochemieal detection showed the positive rate of estrogen receptor(ER),progesterone receptor(PR),HER-2 and p53 was 96%,92%,0%and 32%respectively.All patients underwent operation and polychemotherapy(cyclophosphamide+Adriamycin,Paclitaxel+Adriamyein).12 cases received tamoxifen,13 cases received letrozole and 8 cases received postoperative irradiation.All the patients were followed up from 1 month to 6 years and the overall survival rate was 100%.Conclusions PMBC is a favorable histological type of breast carcinoma with good prognosis.Proper surgical and adjuvant therapy is important to improve survival rate and life quality.Molecular biologic parameters should be given enough consideration into prognosis evaluation.

ABSTRACT

PURPOSE: Mucinous carcinoma of breast was known to have a lower lymph node metastasis and better prognosis than other type of breast cancer. This study was performed to clarify the clinicopathologic features and prognosis of mucinous breast carcinoma. MATERIALS AND METHODS: We reviewed clinicopathologic features of 40 patients diagnosed mucinous breast carcinoma from Jan. 1985 to Dec. 1995 and compared 31 patients of unmixed mucinous breast carcinoma with 1128 patients of infiltrating ductal carcinoma group. RESULTS: Of total 40 mucinous ca., 31 cases were unmixed type showing no component of invasive ductal component(IDC) and 9 cases were mixed type showing IDC. The average age of the patients was 47 ranging from 31 to 71 years. The most common symptom was palpable breast mass(38 cases, 95%). According to TNM classification, the numbers of unmixed type were following, stage I 14 cases(45%), stage II 15 cases(48%), and stage III 2 cases(7%), and of mixed type, stage I 1 cases(11%), and stage II was 8 cases(89%). Axillary lymph node metastasis was shown to be negative in 26 cases of unmixed mucinous carcinoma(84%), whereas it did in 544 cases of infilterating ductal carcinoma(48%)(p<0.05). Because all patients with unmixed mucinous carcinoma survived except 2 patients who died of cancer unrelated causes, it was impossible to analyze the difference of outcome in unmixed mucinous carcinoma and infilterating ductal carcinoma(5YSR: 81%). CONCLUSION: Considering unmixed mucinous carcinoma had fewer axillary lymph node metatasis than infiterating ductal carcinoma, it may be concluded that unmixed mucinous carcinoma had better prognosis.
