ABSTRACT

Objective @#To investigate the influence of abnormal occlusion factors on the incidence of temporomandibular disorders (TMDs) in junior college students and to provide an etiological basis for the prevention and treatment of TMDs.@*Methods @# We examined the temporomandibular joint (TMJ) and dental occlusion in 754 lower grade college students (male 354, female 400) at Zunyi Medical University (Zhuhai campus). A questionnaire was also administered. We analyzed the correlation between TMD and the other three factors (static abnormal occlusion, dynamical abnormal occlusion and oral parafunctional activity) from the perspective of multivariate unconditioned logistic regression and univariate unconditioned logistic regression with dummy variables.@*Results @#The prevalence of TMD was 31.7%. The incidence of TMD was significantly (P<0.05) associated with sleep bruxism (OR=2.070), clenching (OR=2.553), diurnal (OR=2.642) and anterior teeth overbite (OR=1.228). Univariate unconditioned logistics regression analysis by dummy variables was used to analyze the incidence of TMD at different deep overbites (mild, OR=1.558; moderate, OR=2.189; severe, OR=3.236; P<0.05). @*Conclusion@#The risk factors for TMD in lower grade college students included anterior teeth occlusion, sleep bruxism, clenching, and diurnal treatment. Worse deep overbite might increase the risk of TMD.

ABSTRACT

Isolated masseter spasm is a rare entity encountered in patients with neuromuscular disorders and is frequently associated with use of succinylcholine or halogenated inhalational anaesthetic during induction. We present a case of 32-year old primigravida obstetric patient with kypho-scoliosis posted for lower segment caesarean section that developed masseter muscle spasm (MMS) following administration of a standard dose of succinylcholine. The episode resolved spontaneously after 6-7 min without progression to malignant hyperthermia. The report highlights that an event of masseter muscle spasm, though, rarely encountered in life-time practice of an anaesthesiologist, may be successfully managed by maintaining oxygenation and ventilation till the crisis is tided over

ABSTRACT

Objective: To explore the value of virtual touch tissue imaging quantification (VTIQ) in evaluation on skeletal muscle hardness after injection of botulinum toxin type A for treatment of forearm pronation muscle spasm after stroke. Methods A total of 31 patients with forearm pronation muscle spasm after stroke were collected. VTIQ examinations were performed on bilateral pronator teres and pronator quadratus before and 1 month after ultrasound-guided injection of botulinum toxin type A. Shear wave velocity (SWV) was measured, and the differences between the affected side and the healthy side were compared before and after the treatment. The modified Ashworth scale (MAS) was used to evaluate the muscle tension of upper limb on the affected side before and after treatment, and the differences were compared, and the correlation with SWV was evaluated. Results: SWV of pronator teres of the affected side was higher than that of the healthy side before treatment (P=0.005). One month after treatment, there was no significant difference in SWV of the pronator teres and pronator quadratus between the affected side and healthy side (both P>0.05). SWV of the pronator teres and pronator quadratus on the affected side were lower after treatment than those before treatment (both P<0.05). The difference of MAS score of the affected upper limb before and after treatment were statistically significant (Z=-2.252, P=0.024). MAS score of upper limb on the affected side was positively correlated with SWV of the pronator teres on the affected side before and after treatment (rs=0.629, 0.653, both P<0.001). Conclusion: VTIQ can be used to dynamically observe muscle hardness of pronator teres and pronator quadratus in patients with spasm of forearm pronator after stroke, and quantitatively evaluate the curative effect of injection of botulinum toxin type A for treatment of forearm pronator muscle spasm after stroke.

Objective To observe the effect of acupuncture at acupoints selected byZi Wu Liu Zhu Na Jia point-selection card invented by professor ZHOU Ming-xin method (midnight-noon ebb-flow) in improving lower-limb spastic hemiplegia after cerebral stroke.Method Eighty-four patients with lower-limb spastic hemiplegia after cerebral stroke were randomized into a treatment group, a control group, and a blank group by using random number table method, 28 cases in each group. The blank group was only given rehabilitation; the control group was treated with ordinary acupuncture in addition to rehabilitation; the treatment group was intervened by acupuncture at acupoints selected byZi Wu Liu Zhu Na Jia method in addition to the intervention given to the control group. The Ashworth Scale, Activities of Daily Living (ADL) in Barthel Index (BI) and simplified Fugl-Meyer Assessment (FMA) of lower-limb function were observed before and after the intervention. Result After the treatment, the simplified FMA scores were changed significantly in the three groups (P<0.01). After the treatment, the between-group differences in comparing the simplified FMA score were statistically significant (P<0.01), and the treatment group was superior to both the control group and blank group (P<0.01), while the difference between the control group and blank group was statistically insignificant (P>0.05). The ADL from BI and Ashworth scores were significantly changed after the treatment in the three groups (P<0.01,P<0.05). After the treatment, the between-group differences in comparing the ADL score and Ashworth grading were statistically significant (P<0.01), and the therapeutic efficacy was the best in the treatment group (P<0.01,P<0.05).Conclusion Acupuncture at acupoints selected byZi Wu Liu Zhu Na Jia method plus ordinary acupuncture and rehabilitation is effective in improving muscular tension, enhancing lower-limb motor function and the quality of life; theZi Wu Liu Zhu point-selection card is easy-to-operate and has a high accuracy, thus suitable for application and promotion in clinic.

ABSTRACT

Objetivo: determinar la eficacia y la tolerabilidad de la combinación a dosis fija, en una sola tableta, de tiocolchicósido 4 mg más diclofenaco potásico 50 mg en la reducción de la contractura muscular aguda estriada dolorosa comparado contra placebo y el uso de paracetamol tabletas de 500 mg como medicación de rescate. Métodos: fueron reclutados 97 pacientes de 2 ciudades ecuatorianas, Quito y Guayaquil, en tres centros de investigación, públicos y privados, con cervicalgia, dorsalgia y lumbalgia, principalmente de causa funcional. Los pacientes fueron asignados al azar en dos grupos: 1) grupo medicación activa, tiocolchicósido más diclofenaco potásico, 50 pacientes, 2) grupo placebo 47 pacientes. La eficacia en ambos grupos se evaluó por la reducción de la contractura muscular apreciada por inspección, palpación y reducción del dolor medido por una escala visual análoga, después de 5 días de tratamiento. Resultados: la evolución del grado de contractura muscular en el grupo medicación activa por evaluación visual pasó de un 100 por ciento con contractura visible con o sin actividad antiálgica fija a 96 por ciento sin signos visibles de contractura; de un 82 por ciento de contractura moderada a severa con o sin dolor evocado por palpación a un 74 por ciento de contractura leve sin dolor y 26 por ciento de ausencia de contractura. El promedio de dolor según la escala visual análoga disminuyó de 6,66 cm antes del tratamiento a 0,86 cm al finalizar el quinto día de tratamiento. Los efectos adversos fueron leves en el grupo tratado. Conclusiones: la combinación fija de tiocolchicósido 4 mg más diclofenaco potásico 50 mg en una sola tableta, administrado dos veces al día, es eficaz en el manejo de la contractura muscular aguda dolorosa de diversa etiología de manera estadísticamente significativa, bien tolerada y no altera el rendimiento psicomotor(AU)


Objective: to determine the efficacy and tolerability of a combination at a set dose in a single tablet of thiocolchicoside 4 mg plus potassium diclofenac 50 mg in the reduction of painful acute muscle spasm compared with the placebo and the use of 500 mg paracetamol as rescue medication. Methods: ninety seven patients from two Ecuador cities, named Quito and Guayaquil, were recruited in three research centers, both public and private. They suffered cervical pain, low back pain and dorsal pain, mainly of functional cause. The patients were randomly assigned in two groups 1) active medication group with 50 patients treated with thiocolchicoside plus potassium diclofenac and 2) placebo group with 47 patients. The efficacy of both groups was evaluated by the reduction of muscle spams observed in checking, palpation and pain reduction measured in an analogue visual scale after 5 days of treatment. Results: the progress of the muscle spasm degree in the active medication group according to visual evaluation went from 100 percent with visible spasm with or without fixed antialgic activity to 96 percent with no visible signs of spasm; from 82 percent of moderate spasm to severe with or without evoked pain by palpation to 74 percent of mild spasm without pain and 26 percent of spasm-free muscle. The pain average according to the visual scale decreased from 6.66 cm before treatment to 0,86 cm after the 5th day. The adverse effects were mild in t he treated group. Conclusions: the fixed combination of thiocolchicoside 4mg plus potassium diclofenac 50 mg in a single tablet, administered two times a day is efficacious in the painful acute muscle spasm of diverse etiology in a statistically significant way, well-tolerated and with no alteration of the psychomotor performance(AU)

Satoyoshi syndrome is a rare autoimmune disease characterized by alopecia, painful muscle spasms, diarrhea and secondary skeletal changes. We report a 11- year old girl presenting with the typical features of alopecia totalis, severe muscle spasm and skeletal deformities.

ABSTRACT

@#Objective To observe the effect of Motomed Gracile on spastic cerebral palsy. Methods 48 cerebral palsy children were divided into two groups: intervention group (n=24) and control group (n=24). The children in the control group accepted comprehensive rehabilitation, while the children in the intervention group were trained with Motomed Gracile in addition. They were evaluated with Ashworth Assessment and manual muscle testing (MMT) 6 months after treatment. Results The scores of intervention group improved more significantly than those in control group (P<0.05). Conclusion Motomed Gracile can facilitate the recovery of children with spastic cerebral palsy.

@#Objective To observe the effect of manipulation treatment on headache induced by pure pillow linea nuchae inferior adhesion muscle spasm.Methods 36 headache patients caused by pure pillow linea nuchae inferior adhesion muscle spasm were treated by manipulate the methods of finger press and palm root circle press in base on fixing Ash points according to the symptoms and physical signs.Results After 5 to 10 times by manipulation,of 36 patients,25 cases were recovery,10 cases were excellent,1 case was effective,and the excellence effective rate was 97.22%.Conclusion The manipulation treatment is effect on headache induced by pure pillow linea nuchae inferior adhesion muscle spasm;the curative effect is satisfactory and stable.

ABSTRACT

Objective To probe into the causes, clinical features, diagnosis and treatment of ocular neuromyotonia (ONM). Methods To deeply analyze and discuss the causes, clinical features, diagnosis an treatment of this disease by reporting 4 cases of ONM and refor a pituitary tumor. The spasms occurred in the primary position of gaze and were induced by eccentric gaze. A 72 year-old man had chronic arachnoiditis after myelography with radioactive thorium dioxide many years ago. Spasms of muscles innervated by a third nerve and a sixth nerve were produced by eccentric gaze. Treatment with carbamazepine stopped the ONM in both patients. A 47 year-old man had ONM of a lateral rectus muscle when he looked in eccentric gaze several years after irradiation for a cerebellar medulloblastoma. The spasms occurred when treatment for seizures with carbamazepine was decreased, and resolved with the dosage was increased.A 21 year-old man had recurrence of a congenital hemangioma of the neck, face and brain that caused ONM of a superior oblique muscle many years after it had been partially excised. Downward gaze cau sed the spasms, and they resolved spontaneously. Conclusion ONM begins months to years following treatment for intracranial lesions. Typically, the treatment is radiation for an intracranial tumor, but other eauses of damage to the cranial nerves can produce the syndrome. Spasm of extraocular muscles innervated by the third, fourth or sixth cranial nerves produce strabismus and diplopia, lasting from a few seconds to a few minutes. The spasms are spontaneous or induced by ecceutric gaze that stimulates the affected cranial nerve. Neuroradiologic tests might show abnormalities in or near the cavernous sinus, but usually do not show recurrence of the original tumor. The syndrome rarely resolves spontaneously, but responds well to treatment with carbamazepine.

ABSTRACT

Satoyoshi syndrome(generalized Komuragaeri disease) is a rare disorder of unknown cause, characterized by progressive, painful, intermittent muscle spasms and alopecia. Endocrinopathy with amenorrhea, secondary skeletal abnormalities, and diarrhea or unusual malabsorption are frequently seen. It seems that autoimmunity may play a role in its pathogenesis. We report a 13-year-old girl with characteristic manifestations of the syndrome. She was treated with intravenous gammaglobulin and Prednisolone. Painful muscle cramps were gradually improved, but the scalp condition did not change. Satoyoshi syndrome should be considered in children with unexplained muscle spasms and alopecia.
