Resumen: Introducción: La boca del recién nacido se contamina inmediatamente con microorganismos maternos. Caufield determinó la colonización con SGM para niños americanos a los 26 meses de edad. Estudios latinoamericanos la ubican entre los 14,9-18 meses. Objetivos: -Identificar el momento de adquisición y colonización del SGM en una población de niños uruguayos y relacionarlo con factores del niño y maternos. Establecer el tiempo necesario para la recolección de la muestra y estudio de su pérdida. Método: Estudio observacional, longitudinal prospectivo, de 83 pares madre-hijo de un policlínico de Montevideo. De 83 pares examinados, 20 completaron el estudio. Se incluyeron niños de 0-9 meses, con/sin dientes y primera toma salival negativa para SGM. Resultados: Edad promedio de adquisición del SGM 16,4 meses (±4,13) y colonización 18,6 meses (±3,80) de edad. La pérdida de la muestra fue 71,42%. Conclusiones: La colonización resultó similar a la encontrada por Florio (Brasil), observándose diferencias significativas con Caufield (EEUU).

Dos 83 pares examinados, 20 completaram o estudo. Foram incluídas crianças de 0-9 meses com / sem dentes e primeira amostra salivar negativa para SGM. Resultados: Idade média de aquisição do SGM 16,4 meses (± 4,13) e colonização 18,6 meses (± 3,80) de idade. A perda estabelecida da amostra foi 71,42%. Conclusões: A colonização foi semelhante aos de Florio (Brasil), observando diferenças significativas com Caufield (EUA)


Abstract: Introduction: The newborn's mouth is immediately contaminated with maternal microorganisms at birth. Caufield determined that the average age of colonization with Mutants streptococci (MS) for American children was 26 months old. Latin American studies indicate that it occurs at an age between 14.9 and 18 months old. Objectives: To identify the time of MS acquisition and colonization in a population of Uruguayan children and to relate it with various child and maternal factors. To establish the time needed to collect the sample and its loss. Methods: Observational, longitudinal prospective study, of 83 mother-child pairs from a medical center in Montevideo. From the 83 pairs examined, 20 completed the study. Children between 0-9 months old, with/without teeth, whose first saliva sample was negative for MS, were included. Results: The mean age of MS acquisition was 16.4 months (± 4.13), and colonization occurred at 18.6 months (± 3.80) of age. The sample loss was 71.42%. Conclusions: The colonization found was similar to that found by Florio (Brazil) but showed significant differences with Caufield's results (USA).

La aparatología fija usada en ortodoncia dificulta una adecuada higiene bucal, en estas condiciones el de-sarrollo de lesiones cariosas puede ser una complicación, uno de los principales agentes etiológicos involu-crados es Streptococcus mutans. La nanotecnología con plata tiene una aplicación preventiva importante en biomateriales dentales. Objetivo: Se determinó la actividad antimicrobiana sobre Streptococcus mutans de nanopartículas de plata (AgNPs) incluidas en adhesivos ortodónticos a diferentes concentraciones. Materia-les y métodos: En 20 cajas Petri con Streptococcus mutans inoculada en agar Müller-Hinton suplementado con 5% de sangre, se colocaron 6 discos de papel, 4 embe-bidos con AgNPs incluidas en un adhesivo orto-dóntico a concentraciones de 25, 75, 125 y 175 ppm, y 2 discos controles con clorhexidina al 0.12% y agua destilada. Se determinó la capacidad antimicrobiana en función de promedios de halos de inhibición a las 48 horas de incubación, en comparación con la escala de Duraffourd y la clorhexidina 0.12%. Resultados: A 25 ppm no existió capacidad antibacteriana, a 75 ppm el 50% alcanzó sensibilidad límite, pero el 100% de las muestras a 125 y 175 ppm consiguieron sensibilidad límite, sin embargo, la clorhexidina al 0,12% presentó mayor sensibilidad entre media 60% y sumamente sensible 40%. Conclusión: Las nanopartículas de plata con tamaño inferior a 100 nm, incluidas a 125 y 175 ppm en adhesivos de ortodoncia poseen actividad anti-microbiana sobre Streptococcus mutans, sin embargo, en el tiempo de estudio no alcanzan la sensibilidad de la clorhexidina al 0.12%.


The fixed appliances used in orthodontics make proper oral hygiene difficult, in these conditions the develop-ment of carious lesions can be a complication, one of the main etiologic agents involved is Mutans streptococci. It was determined that silver nanotechnology has an important preventive application in dental biomaterials. Objective: Antimicrobial activity on Mutans streptococci of silver nanoparticles (AgNPs) included in orthodon-tic adhesives at different concentrations. Materials and methods: In 20 Petri dishes with Mutans streptococci inoculated in Müller-Hinton agar supplemented with 5% blood, 6 paper discs were placed, 4 soaked up with AgNPs included in an orthodontic adhesive at concentrations of 25, 75, 125 and 175 ppm, and 2 control discs with 0.12% chlorhexidine and distilled water. Antimicrobial capacity was determined based on averages of inhibition halos at 48 hours of incubation, compared to the Duraffourd scale and 0.12% chlorhexidine. Results:At 25 ppm there was no antibacterial capacity, at 75 ppm 50% reached limit sensitivity, but 100% of the sam-ples at 125 and 175 ppm achieved limit sensitivity, however, chlorhexidine 0.12% showed higher sensitivity be-tween average 60% and highly sensitive 40%. Conclusión: Silver nanoparticles smaller than 100 nm, included at 125 and 175 ppm in orthodontic adhessives, have antimicrobial activity on Strptococcus mutans, however, when studying, they don't reach the sensitivity of chlorhexidine at 0.12%.


Os aparelhos fixos utilizados em ortodontia dificultam uma higiene bucal adequada, nessas condições o desenvolvimento de lesões cariosas pode ser uma complicação, um dos principais agentes etiológicos envol-vidos é o Streptococcus mutans. A nanotecnologia com partículas de prata tem uma importante aplicação pre-ventiva em biomateriais dentários. Objetivo: Determinou-se a atividade antimicrobiana das nanopartículas de prata (AgNPs) incluídas em adesivos ortodônticos em diferentes concentrações sobre Streptococcus mutans. Materiais e métodos: Em 20 placas de Petri com Streptococcus mutans inoculadas em ágar Müller-Hinton suplementado com 5% do sangue, foram colocados 6 discos de papel, 4 ensopados com AgNPs incluídos em um adesivo ortodôntico nas concentrações de 25, 75, 125 e 175 ppm e 2 discos de controle com 0,12% de clorexidina e água destilada. O efeito antimicrobiano foi determinado com base nas médias de halos de inibição às 48 horas de incubação, em comparação com a escala de Duraffourd e com clorexidina a 0,12%. Resultados: A 25 ppm não havia capacidade antibacteriana, a 75 ppm 50% atingiram a sensibilidade limite, mas 100% das amostras a 125 e 175 ppm atingiram a sensibilidade limite, no entanto, 0,12% de clorexidina apresentou maior sensibilidade entre 60% em média e 40% altamente sensíveis. Conclusão: Nanopartículas de prata menores que 100 nm, incluídas em 125 e 175 ppm em adesivos ortodônticos, têm atividade antimi-crobiana em Streptococcus mutans, no entanto, no tempo

ABSTRACT

The aim of this study was to investigate the susceptibility of Mutans streptococci in both planktonic and biofilm states to erythrosine.S. mutans was cultured in brain-heart infusion (BHI) broth. Erythrosine was diluted in BHI broth and prepared at a concentration range of 0.02 – 10000 µg/L. The minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) were measured using the microdilution method. After forming biofilms on 96-well plates, the minimum biofilm inhibitory concentration (MBIC) and minimum biofilm eradication concentration (MBEC) were measured.S. mutans was susceptible to erythrosine in both planktonic and biofilm states. MIC and MBC values were both 19.5 µg/L for the planktonic state, while MBIC and MBEC values were 313 µg/L and 2500 µg/L, respectively, for the biofilm state.Erythrosine (19.5 µg/L) exhibited a bactericidal effect on S. mutans (killing 99.9%) in the planktonic state. For biofilms, erythrosine inhibited biofilm growth and eradicated 99.9% of biofilm bacteria at higher concentrations than MIC and MBC. These MBIC and MBEC concentrations are much lower than known noxious doses, and the MIC, MBC, and MBIC values were even lower than clinical concentrations.

ABSTRACT

Background Diabetes mellitus is one of the serious systemic diseases that may cause general systemic changes, which may be reflected in the oral cavity. The aims of this study were to assess the severity of dental caries, Mutans Streptococci and Lactobacilli in addition to the flow rate and pH among uncontrolled and controlled type 2 diabetic groups in comparison with non-diabetic control group. Materials and Methods Study groups consisted of 25 uncontrolled diabetic patients (HbA1c >7), 25 controlled diabetic patients (HbA1c <7), in addition to 25 nondiabetic healthy looking individuals. Their age was (18–22) years from both genders. The diagnosis and recording of dental caries were according to the severity of dental caries lesion through the application of D1_4MFS and stimulated salivary samples were collected. The salivary flow rate and pH were estimated. Viable count of Mutans streptococci (on Mitis-Salivarius Bacitracin Agar) and Lactobacilli (on Rogosa) was determined. Results The mean values of caries severity were recorded to be highest among study groups compared to the control with statistically highly significant difference (p<0.01). Lowest values of salivary pH and flow rate were among study groups compared to the control with highly significant difference (p<0.01). Concerning Mutans Streptococci and Lactobacilli were found that the mean values of them for uncontrolled diabetic group were highly significant higher than both mean values of controlled diabetic group and control group. Conclusion Dental caries revealed higher percentage of occurrence and severity among uncontrolled diabetic group. Furthermore, there was significant influence of the diabetic and the poor metabolic control on the salivary flow rat, pH, Mutans streptococci and Lactobacilli that have an effect on caries occurrence and severity.

ABSTRACT

The purpose of the study was to investigate the antimicrobial activity of the methanol extract of Coptidis rhizome against the type strains of cariogenic bacteria, Streptococcus mutans and Streptococcus sobrinus, and the periodontopathogens, Porphyromonas gingivalis, Prevotella intermedia, Treponema denticola and Aggregatibacter actinomycetemcomitans. The antimicrobial activities of the crude extract and the methanol extract fractions of Coptidis rhizome separated by silica gel chromatography were evaluated by determining the minimal bactericidal concentration (MBC) values, using the microdilution method. The cell viability test of the extracts of Coptidis rhizome on the KB cells was also studied by methyl thiazolyl tetrazolium (MTT) assay. Our results showed that the 11th fraction (F11) of the methanol extract had the greatest antimicrobial activity against the tested bacteria, with no associated cytotoxicity on the KB cells, upto a concentration of 50 microg/ml. These results suggest that the silica gel chromatography fraction F11 of the methanol extract of Coptidis rhizome, could be useful in the development of oral hygiene products as an antimicrobial agent for the prevention of dental caries and periodontal diseases.

ABSTRACT

Streptococcus mutans is specifically suppressed by intensive treatment with chlorhexidine gel, but the time for recolonization and the effect on other oral bacteria are not totally clear. In this study, recolonization of mutans streptococci was evaluated in nine healthy adult volunteers, who were highly colonized with this microorganism. Stimulated saliva was collected before (baseline) and at 1, 7, 14, 21 and 28 days after application of 1% chlorhexidine gel on volunteers' teeth for two consecutive days. On each day, the gel was applied using disposable trays for 3 x 5 min with intervals of 5 min between each application. Saliva was plated on blood agar to determine total microorganisms (TM); on mitis salivarius agar to determine total streptococci (TS) and on mitis salivarius agar plus bacitracin to determine mutans streptococci (MS). Chlorhexidine was capable of reducing the counts of MS and the proportion of MS with regard to total microorganisms (%MS/TM) (p<0.05), but these values did not differ statistically from baseline (p>0.05) after 14 days for MS and 21 days for %MS/TM. The counts of TM and TS and the proportion of MS to total streptococci did not differ statistically from baseline (p>0.05) after chlorhexidine treatment. The results suggest that the effect of chlorhexidine gel treatment on suppression of mutans streptococci is limited to less than a month in highly colonized individuals.


Streptococcus mutans é especificamente suprimido pelo tratamento intensivo com clorexidina em gel, mas o tempo de recolonização e o efeito em outras bactérias orais não está totalmente claro. Nesse estudo, a recolonização de estreptococos do grupo mutans foi avaliado em nove voluntários adultos saudáveis, os quais eram altamente colonizados por esse microrganismo. Saliva estimulada foi coletada antes (baseline) e 1, 7, 14, 21 e 28 dias após a aplicação de clorexidina em gel a 1% nos dentes dos voluntários por dois dias consecutivos. Em cada dia, o gel foi aplicado utilizando moldeiras descartáveis por 3 x 5 min com intervalos de 5 min entre cada aplicação. A saliva foi inoculada em ágar sangue para determinação dos microrganismos totais (MT); em mitis salivarius ágar para determinação dos estreptococos totais (ET) e em meio mitis salivarius com bacitracina para determinar a contagem de estreptococos do grupo mutans (EGM). O tratamento com clorexidina foi capaz de reduzir as contagens de EGM e a proporção de EGM em relação aos microrganismos totais (%EGM/MT) (p<0,05), mas esses valores não diferiram estatisticamente do baseline (p>0,05) após 14 dias para EGM e 21 dias para %EGM/MT. As contagens de MT e ET e a proporção de EGM em relação a estreptococos totais não difereriram estatisticamente do baseline (p>0,05) após o tratamento com clorexidina. Os resultados sugerem que o efeito do tratamento com clorexidina em gel na supressão de estreptococos do grupo mutans é limitado a menos de um mês em indivíduos altamente colonizados. .

ABSTRACT

This study investigated the antimicrobial activity of methanol extract of mulberry leaf against 16 strains of mutans streptococci and four species of periodontopathogens: Porphyromonas gingivalis, Prevotella intermedia, Fusobacterium nucleatum, and Aggregatibacter actinomycetemcomitans. The antimicrobial activities of the crude extracts or silica gel chromatography fractions of methanol-extracted mulberry leaf were evaluated by determining minimal inhibitory concentrations using an established microdilution method. The cytotoxicity of the extracts of mulberry leaf on KB cells was tested by the methyl thiazolyl tetrazolium assay. Chromatography fraction 12 displayed the most potent antimicrobial activity against all 16 strains of mutans streptococci, P. gingivalis, and P. intermedia. No KB cell cytotoxicity was evident up to 128 microg/ml of fraction 12. The methanol extract had no antimicrobial activity against F. nucleatum and A. actinomycetemcomitans. These results suggest chromatography fraction 12 methanol extract of mulberry leaf could be useful in the development of oral hygiene products, such as dentifrice and oral hygiene solution, for the prevention of dental caries.

Objectives: To compare mutans streptococci levels in saliva, before and after consumption of probiotic ice-cream and curds. Materials and Methods: Forty caries free children in the age group of 12-14 years were selected and equally divided into four groups I, II, III, IV. Children in group I and II were given 100 ml probiotic ice-cream and plain ice cream respectively and group III and IV were given 100 ml probiotic curd and plain curd respectively for a period of 7 days. Saliva samples were assessed at baseline, 1 hour after consumption and after 7 days intervention period using Mitis salivarius Bacitracin agar. The number of colonies was counted and subjected to statistical analysis. Results: The study revealed a reduction in salivary mutans streptococci (MS) counts after 1 hour in all the groups. However after 7 days, probiotic ice-cream and curd showed a statistically significant (P < 0.001) reduction in MS counts as compared to the control ice cream and curd. The difference in the reduction of MS counts with probiotic ice-cream and probiotic curd at 1 hour and 7 days was not statistically significant. Conclusion: The use of probiotic products could be an alternative strategy of displacing pathogenic microorganisms by probiotic bacteria and can thus be exploited for the prevention of enamel demineralization.

Objective: The aim of this study was to determine in a randomized, double-blind, placebo-controlled clinical trial the effects of typified propolis and chlorhexidine rinses on salivary levels of mutans streptococci (MS) and lactobacilli (LACT). Methods: One hundred patients were screened for salivary levels of MS >100,000 CFUs/mL of saliva. All patients presented with at least one cavitated decayed surface. Sixty patients met entry criteria. Subjects were adults 18-55 years old. After restoration of cavitated lesions patients were randomized to 3 experimental groups:1) PROP-alcohol-free 2% typified propolis rinse (n= 20); 2) CHX- 0.12% chlorhexidine rinse; 3) PLplacebo mouthrinse. Patients rinsed unsupervised 15 mL of respective rinses twice a day for 1 min for 28 days. Patients were assessed for the salivary levels of MS (Dentocult SM) and LACT (Dentocult LB) at baseline, 7-day, 14-day, and at 28-day visits (experimental effects) and at 45-day visit (residual effects). General linear models were employed to analyze the data. Results: PROP was superior to CHX at 14-day and 28-day visits in suppressing the salivary levels of MS (p < .05). PROP was superior to PL at all visits (p < .01). The residual effects of PROP in suppressing the salivary levels of MS could still be observed at the 45-day visit, where significant differences between PROP and CHX (p < .05), were demonstrated. PROP was significantly superior than CHX in suppressing the levels of salivary LACT at the 28-day visit (p < .05). Conclusion: Typified propolis rinse was effective in suppressing cariogenic infections in caries-active patients when compared to existing and placebo therapies.


Objetivo: O objetivo deste estudo foi determinar, em um estudo randomizado, duplo-cego, placebo-controlado os efeitos da própolis tipificada e clorexidina sobre os níveis salivares de estreptococos do grupo mutans (EM) e lactobacilos (LACT). Métodos: Cem pacientes foram selecionados para níveis salivares de MS > 100.000 UFC/mL de saliva. Todos os pacientes apresentaram pelo menos uma superfície cariada com cavitação. Sessenta pacientes preencheram os critérios de inclusão. Os indivíduos eram adultos com 18-55 anos de idade. Após a restauração das lesões cavitadas, os pacientes foram randomizados para três grupos experimentais: 1) PROP- bochecho livre de álcool de 2% de própolis tipificada (n = 20), 2) CHX- bochecho de clorexidina 0,12%, 3) PL- bochecho placebo. Os participantes bochecharam sem supervisão 15 mL dos enxaguatórios duas vezes por dia, durante 1 minuto, durante 28 dias. Os pacientes foram avaliados para os níveis salivares de MS (Dentocult SM) e LACT (Dentocult LB) na linha de base, e após 7 dias, 14 dias, 28 dias (efeitos experimentais) e 45 dias (efeitos residuais). Modelos lineares foram utilizados para analisar os dados. Resultados: PROP foi superior ao CHX nas visitas de 14 dias e de 28 dias na supressão dos níveis salivares de SM (p < 0,05). PROP foi superior ao PL em todas as visitas (p < 0,01). Os efeitos residuais de PROP na supressão dos níveis salivares de MS ainda foi observado na visita de 45 dias, onde diferenças significativas entre PROP e CHX (p < 0,05) foram demonstradas. PROP foi significativamente superior a CHX na supressão dos níveis salivares de LACT na visita de 28 dias (p < 0,05). Conclusão: O enxaguatório de própolis tipificada foi eficaz na supressão de infecções cariogênicas em pacientes com atividade de cárie quando comparado a terapias existentes e ao placebo

This study investigated the presence of the black-pigmented bacteria Prevotella nigrescens and Prevotella intermedia, the non-black-pigmented bacteria Actinomyces spp and particularly the cariogenic pathogen Streptococcus mutans in the dental biofilms of patients with or without black extrinsic tooth stains, using the multiplex polymerase chain reaction (PCR) technique. Analysis of the dental biofilms of patients with (n=26) or without (n=26) black tooth stains was performed using duplex PCR for the 16S ribosomal RNA gene (P. nigrescens, P. intermedia, Actinomyces spp) and glucosyltransferase-I gene for S. mutans. P. nigrescens and S. mutans were the most frequent bacteria detected in both groups. The least frequently detected were P. intermedia and Actinomyces spp. The similar bacterial composition of dental biofilms of black tooth stains and healthy tooth surfaces indicates that black tooth stains are not free of cariogenic bacteria.


O objetivo deste estudo foi investigar a presença das bactérias pigmentadoras de negro Prevotella nigrescens e Prevotella intermedia, da não pigmentadora de negro Actinomyces spp e particularmente a bactéria cariogênica Streptococcus mutans, no biofilme dentário de pacientes com ou sem manchas dentárias extrínsecas negras, utilizando a técnica multiplex PCR (reação em cadeia da polimerase). Análises do biofilme dentário de pacientes com manchas (n=26) e sem manchas (n=26) foram realizadas utilizando a multiplex PCR para o gene 16S RNA ribosomal (P. nigrescens, P. intermedia, Actinomyces spp) e o gene glucosiltransferase-I para S. mutans. P. nigrescens e S. mutans foram as bactérias mais frequentemente detectadas em ambos os grupos. As menos frequentemente detectadas foram P. intermedia e Actinomyces spp. A similaridade entre a composição bacteriana dos biofilmes dentários das manchas dentárias extrínsecas negras e das superfícies dentárias sem manchas indicam que as manchas dentárias extrínsecas negras não estão livres de bactérias cariogênicas.

ABSTRACT

Objective: To evaluate the colonization of group mutans streptococci according to age, measuring the amount of bacteria in saliva with a semi-quantitative method in a population attended in public and private dental centers of the Metropolitan Region, Santiago, Chile. Patients and Methods: Saliva samples were obtained from 14,649 patients aged 5 to 40 years, in one public and 5 private dental centers. Bacteria concentration was estimated by the comparison with a standard counting-chart. The concentration of group mutans streptococci in saliva was test by a 3-way ANOVA. Results: Bacterial concentration of Streptococcus mutans related with the age of patients was significant (p < 0.001). Bacterial concentration in the preschool age was 4,7 x 10(5) CFU/mL at 5 years, while 6,0 x10(5) CFU/mL at 12 years of age, with a decrease in patients over 30 years. Bacterial concentration was significantly different in the six centers of the study. Conclusions: The semi-quantitative method was useful to determine the colonization by Streptococcus mutans according to age. This could help for identifying population at high risk of dental caries and to develop oral health prevention programs in specific populations.


Objetivo: Evaluar el nivel de colonización Streptococcus grupo mutans según la edad, midiendo en saliva, con un método semi-cuantitativo, la cantidad de bacterias presente en una población que acude a centros odontológicos públicos y privados de la Región Metropolitana de Chile. Pacientes y Métodos: Se obtuvieron de 14.649 muestras de saliva de pacientes incluyendo hombres y mujeres, de 5 a 40 años, en un centro de atención estatal y en cinco de atención privada. El método de recuento fue la comparación con una tabla normada de concentraciones. El análisis estadístico incluyó ANOVA. Resultados: Se encontró que el recuento de Streptococcus grupo mutans en relación a las edades de los pacientes fue significativo (p < 0,001). El recuento de bacterias en los preescolares de 5 años fue alrededor de 4.7x10(5) ufc/ mL de saliva mientras que en los niños de 12 años fue de 6.0x10(5) ufc/mL, observándose además una disminución de la carga bacteriana en los pacientes de 30 años. A edades superiores se apreció un descenso paulatino. La concentración bacteriana fue significativamente distinta en los seis centros estudiados. Conclusiones: Este método de recuento de colonias a través de la espátula permitió determinar el grado de colonización producida por Steptococcus grupo mutans según las edades, es de utilidad para identificar la población de alto riesgo de caries dentales y su aplicación ayudaría a desarrollar programas de prevención en salud oral en poblaciones especificas.

Background: Streptococcus mutans (S. mutans) is associated with the onset of caries. Since root exposure in patients affected by periodontitis leads to higher caries rates, progressively more severe forms of periodontal disease might associate with elevated counts of S. mutans. Aim: To determine whether increasingly destructive forms of periodontal disease are associated with higher counts of S. mutans in untreated patients. Methods: 206 subjects aged 20-75 were classified into three groups according to the severity of periodontal disease: 1) gingivitis, 2) chronic slight periodontitis and 3) chronic moderate or chronic severe periodontitis. S. mutans counts (cfu/mL) were obtained by direct counting on selective agar plates from saliva samples. A cumulative proportional logistic regression model was adjusted for S. mutans counts. Results: The model failed to show differences by gender, but periodontal diagnosis had a significant effect on S. mutans counts depending on age. While in the group with moderate and severe periodontitis the probability of having high counts of S. mutans significantly increased with age, the probability remained unchanged in individuals with chronic slight periodontitis or gingivitis. Conclusion: High S. mutans levels appear directly co-associated with increased severity of periodontal disease at older ages in untreated patients.

Aim: Since deaf children are unable to comprehend or cooperate with dental treatment due to lack of communication, preventive measures may be an important way to control the high prevalence of dental caries in these patients. The aim of the this study was to evaluate the effect of an intensive treatment with chlorhexidine (CHX) gel, containing either saccharin or aspartame, in deaf children highly infected with mutans streptococci (MS). Methods: Eighteen children were randomly divided into two groups, according to the sweetener used to improve the CHX gel bitter taste: saccharin or aspartame. Before CHX treatment, saliva samples were collected to establish baseline microbial data for MS. CHX gel was applied on two consecutive days, four times the first day and three times the second day. Saliva samples were then taken after 7, 30, 60, 90 and 120 days to evaluate MS oral recolonization. Results: CHX gel containing saccharin was not effective on the reduction of MS levels, while the gel containing aspartame decreased significantly MS levels after treatment (P<.05). Conclusions: Although a new CHX application may be necessary after 60 days to control caries risk and MS levels, CHX treatment should be individually controlled because of variations in the response of subjects.

Distilled water rinsed group served as a control. Paraffin-stimulated whole saliva samples were collected at baseline, 5 weeks, and 10 weeks after rinsing to determine the level of salivary MS by culturing on Mitis Salivarius Bacitracin agar. The statistical significance was calculated by Kruskal Wallis, Mann Whitney U, and Wilcoxon signed-rank tests at a significant level of P< 0.05.</p><p><b>RESULTS</b>Significant reductions in MS count were observed in subjects using 0.05% NaF + 12.5% xylitol over other groups within 5 weeks and after 10 weeks and 12.5% xylitol alone after 10 weeks compared with baseline.</p><p><b>CONCLUSIONS</b>The present study provides evidence for the inhibitory effect of xylitol, used in combination with fluoride, delivered in the form of mouthrinse, on salivary MS in the group of schoolchildren.</p>

ABSTRACT

Dental caries may be defined as a complex multifatorial disease in that a broad group of biological, socio-economic and cultural factors interact directly or indirectly in the establishment and colonization of cariogenic microorganisms within the microbial community of the dental biofilm. Innate and adaptative immunity are two fundamental aspects of the immune system response against infections, such as dental caries. Besides, the majority of pathogenic infectious agents enter the organisms by the oral route. Consequently, the mucosal tissue, associated exocrine glands and saliva contributes to the protection of the oral cavity because contain cells responsible for antigen internalization and antibodies specific to oral bacteria. Macrophages are phagocytic cells that can internalize and kill bacteria by several mechanisms of internalization, including endocytosis, macropinocytosis and phagocytosis. Streptococcus mutans is the major pathogen of dental caries due to its ability to adhere and accumulate on tooth surfaces, using different virulence factors (AgI/II, Gtf, Gbps). Recent studies demonstrated protection against experimentally induced dental caries for vaccines containing intact or peptides from antigen I/II, Gtf or Gbp and vaccines containing a combination of antigens. The present review summarizes the fundamental mechanisms of host immune responses to oral bacteria and the main perspectives of a vaccine against dental caries.


A cárie dentária pode ser definida como uma doença complexa multifatorial causada por fatores biológicos, socioeconômicos e culturais que interagem direta ou indiretamente na colonização e estabelecimento de microrganismos cariogênicos na comunidade microbiana do biofilme dentário. As imunidades inata e adaptativa são os dois aspectos fundamentais de resposta do sistema imune contra infecções, como a cárie dentária. Além disso, a maioria dos agentes infecciosos patogênicos entra no organismo por via oral. Consequentemente, o tecido mucoso, associado com as glândulas exócrinas e a saliva contribuem para a proteção da cavidade bucal por conterem células responsáveis pela internalização de antígenos ou anticorpos contra as bactérias bucais. Os macrófagos são células fagocíticas que podem internalizar e eliminar bactérias por diversos mecanismos de internalização, como a endocitose, macropinocitose e fagocitose. Streptococcus mutans é o principal patógeno da cárie dentária por sua habilidade em aderir e acumular nas superfícies dentárias, usando diferentes fatores de virulência (AgI/II, Gtf e Gbps). Estudos recentes têm demonstrado proteção contra cárie induzida experimentalmente utilizando vacinas contendo antígenos intactos ou peptídeos a partir de AgI/II, Gtf ou Gbps ou uma combinação de antígenos. A presente revisão sumariza os mecanismos fundamentais de resposta imune contra bactérias bucais e as principais perspectivas de uma vacina anticárie.

OBJECTIVE: To estimate the effects of bracket material type on enamel decalcification during orthodontic treatment, this study analyzed the adhesion level of mutans streptococci (MS) to orthodontic bracket materials in vivo. METHODS: Three different types of orthodontic bracket materials were used: stainless steel, monocrystalline sapphire, and polycrystalline alumina. A balanced complete block design was used to exclude the effect of positional variation of bracket materials in the oral cavity. Three types of plastic individual trays were made and one subject placed the tray in the mouth for 12 hours. Then, the attached bacteria were isolated and incubated on a mitis salivarius media containing bacitracin for 48 hours. Finally, the number of colony forming units of MS was counted. The experiments were independently performed 5 times with each of the 3 trays, resulting in a total of 15 times. Mixed model ANOVA was used to compare the adhesion amount of MS. RESULTS: There was no difference in colony forming units among the bracket materials irrespective of jaw and tooth position. CONCLUSIONS: This study suggested that the result of quantitative analysis of MS adhesion to various orthodontic bracket materials in vivo may differ from that of the condition in vitro.

ABSTRACT

The purposes of this study were to detect S. mutans and S. sobrinus by polymerase chain reaction (PCR) amplification, and to relate their presence to the incidence of dental caries in 42 Brazilian preschool children. Dental plaque samples were collected from the cervical margin of all erupted teeth of 5-6 years old children with primary dentition, using a sterile explorer. Examination of the dmft (decayed, missing, filled teeth) index, performed following the World Health Organization (WHO) caries diagnostic criteria, showed a 2.71 score. Prevalence of S. mutans and S. sobrinus was respectively, of 85.7 percent and 14.3 percent; no dental plaque sample was either positive or negative for both bacterial species. Children harboring either S. mutans or S. sobrinus presented the same caries prevalence. PCR showed good discriminative ability for differentiation between these species, and suggested that it is a technique suitable for epidemiological studies on mutans streptococci.


A presença de Streptococcus mutans e S. sobrinus, detectadas através da técnica da reação em cadeia da polimerase (PCR) foi comparada com a incidência de cárie dental em 42 crianças pré-escolares brasileiras. As amostras da placa dental foram obtidas utilizando-se um explorador estéril, o qual foi passado na margem cervical de todos os dentes erupcionados nas crianças de 5-6 anos de idade, as quais possuíam a dentição primária. Foi realizado um exame dental para a avaliação do índice ceod (índice de dentes decíduos cariados, extraídos por cárie ou obturados) de acordo com os critérios de diagnóstico de cárie preconizados pela Organização Mundial de Saúde (OMS). O índice ceod foi de 2,71. A presença de S. mutans e S. sobrinus foi detectada em 85,7 por cento e 14,3 por cento das crianças analisadas respectivamente, nenhuma amostra da placa dental foi positiva ou negativa para a presença das duas bactérias ao mesmo tempo. As crianças que apresentavam S. mutans ou S. sobrinus isoladamente apresentaram o mesmo índice de cárie. A técnica de PCR apresentou um grande poder discriminatório entre as duas espécies o que sugere que ela é adequada para estudos epidemiológicos no grupo mutans.

ABSTRACT

The aim of this study was to compare the species and biotypes of mutans streptococci isolated from dental plaques sampled from the interfaces between the bracket and tooth surface and smooth tooth surfaces in orthodontic patients. Dental plaque was collected from the interfaces between brackets and teeth (test group), and from smooth tooth surfaces distant from brackets by more than 2 mm (control group). The dental plaque collected by a sterilized curette was transferred into a vial of 1 X PBS. The sample in the vial was vigorously vortexed for 1 min and plated on mitis-salivarius bacitracin (MSB) agar plate using cotton tips. The agar plates were incubated at 37 degrees C in a candle jar for 2 days, and again incubated for 1 more day at anambient temperature. Individual colonies were cultured in TH broth at 37 degrees C CO2 incubator. The PCR-RFLP based on dextranase gene was performed for the identification of mutans streptococci at the species-level. For biotyping of mutans streptococci, biochemical tests were performed. There was no significant difference of the species of mutans streptococci isolated from both test and control groups. However, the biotypes of the mutans streptococci isolated from test and control groups were different. These results may offer the basic data to verify the relationship between the mutans streptococci biotype and enamel decalcification or dental caries in orthodontic patients with fixed appliances.

ABSTRACT

The purpose of this study is to survey the frequency of mutans streptococci species and biotypes isolated from dental plaque in Korean adolescents and adults and the relationship between species of mutans streptococci and DMFT index. The dental plaque samples were collected from the anterior and molar teeth of both of jaws in the 47 human subjects (aged 14 to 29.3 years, average age was 20.2 years). A dental examination was performed for DMFT with the WHO caries diagnostic criteria. The mutans streptococci were cultured selectively on mitis-salivarius bacitracin (MSB) agar plates. The bacterial colonies growing on the MSB plates were then identified with biochemical tests (for biotyping) as well as polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) targeting dextranase gene (dex) (for the determination of species). The data showed that the prevalence of Streptococcus mutans, S. sobrinus, and S. downei were 93.6%, 12.8%, and 8.5%, respectively. The biotype I (87.4%) and biotype IV (19.1%) were the most frequently detected. The DMFT scores of adolescents and adults harboring both S. mutans and S. sobrinus were significantly higher than those for S. mutans alone (p < 0.05). Above results revealed that S. mutans and biotype I were frequently detected in Korean adolescents and adults. In addition, the results suggest that subjects having plaques compared of both S. mutans and S. sobrinus may be more susceptible to caries than those having S. mutans alone.
