ABSTRACT

Autoimmune pancreatitis type 2, an increasingly recognized etiology of pancreatitis in young patients without elevated IgG4. We report the case of a young woman admitted for acute pancreatitis, whose initial etiological record was negative. The evaluation of gravity by CT scan showed a slightly hipertrophic pancreas with the discovery of an aspect of ulcerative colitis "pancolitis". This association helped to guide the etiological diagnosis and treatment management. The patient was put on steroids with a rapidly favorable course, without recurrence and a 12-month follow-up.

ABSTRACT

Objective To explore the clinical application and effect of Flow-through anterolateral thigh flap transfer combined with bone transport technique for repairing the segamental mutilating injury in lower extremity.Methods From June,2010 to June,2016,Flow-through anterolateral thigh flap transfer combined with Ilizarov technique were applied to repair the segamental mutilating injury in lower extremity in 10 cases.Of the 10 cases,there were 8 males and 2 females (their ages ranged from 26 to 55,36 on average).Four caused by road accident,4 caused by crush injury,2 cases caused by squeeze injury.First stage,Flow-through anterolateral thigh flap transfer were applied to revascularize the lower limb and repair the soft tissue defect in emergency.Second stage,bone transfort with external fixator were applied to repair bone defect in second stage.The area of the flap ranged from 12.0 cm×15.5 cm to 20.0 cm×25.0 cm.The repair of bone defect ranged from 4.0 cm to 10.0 cm.Time interval between two stages ranged from 2 to 4 months,3 months on average.Followed-up method was regular outpatientcare after discharge.The main contents include:the presence of red swollen with pus,the condition of infection in the pin,if the fixed pin was loosening,and adjust the speed of bone transport according to the mineralization of new bone.Results All cases were successfully repaired in 10 cases.Ten cases were followed-up with a mean followed-up time of 42 months.All flaps survived,in with 6 cases were healed in first stage,4 case were healed in second stage (healing time ranged from 14 to 30 days),and bone transfort time ranged from 6 to 16 months.The docking site union occurred in 6 cases and were healed by use of bone debridement,bone graft and compression.Deformity in foot and ankle occurred during the bone transport in 2 cases and were cured by draft therapy with adding ring fixator.The functions of donor sites were not found malfunctional.Conclusion Flow-through anterolateral thigh flap transfer combined with bone transport have high success rate of limb salvage,good quality of os teogenesis,and satisfactory function recovery.It is an optimal method to repair the segamental mutilating injury of the lower extremity.

ABSTRACT

OBJECTIVES: Depression is a common mental illness and a major cause of suicide. Although serum lipids have been associated with depression and suicide, there has been much debate. In this study, we investigated the relationship between depression, suicide, and serum lipids in patients admitted with depressed mood. METHODS: A total of 134 subjects were divided into 86 non-suicide patients and 48 suicide attempters. The serum lipid levels and sub-scores of the Korean Symptom Checklist-95 (KSCL95) were compared. We also investigated the relationship between serum lipids and sub-scores of KSCL95 and investigated whether serum lipids were risk factors for suicide attempts. RESULTS: There was no difference in serum lipids between the two groups. Among the sub-items of KSCL95, obsession was higher in non-suicide group. Triglyceride showed positive correlations with anxiety, phobic anxiety, agoraphobia, schizophrenia, and self-regulation problem. High triglyceride was a risk factor for suicide attempts. CONCLUSIONS: Triglyceride is associated with depression, anxiety, and self - regulation, and high serum triglyceride levels may be a risk factor for suicide attempts.

ABSTRACT

PURPOSE: Thumb reconstruction plays most important role in hand injuries because total loss of a thumb constitutes about 40% disability in the hand. The reconstruction can be accomplished by pollicization, free toe-to-thumb transfer, wrap around procedure and lengthening extraction. However, we sometimes need consecutive or double free flaps in the reconstruction of mutilating hand injuries. METHODS: We reconstructed a mutilating hand injury in a 54-years old man. Because of severe crushing injury of right thumb and index fingers, we reconstructed a thumb with pollicization using nearly amputated middle finger. Although it survived completely, the adjacent soft tissues which had been covered by fillet flap from the space past was necrosed on 1 month. We debrided the necrotic tissues and covered it with anteromedial thigh perforator free flap consecutively because he had an anatomical variation in branches of lateral femoral circumflex artery. RESULTS: He had an uneventful postoperative course without any complication such as infection, dehiscence and flap necrosis. Three months later, he had undergone tenolysis and defatting procedure of flap site. He recovered the some amount of grip function and was happy with the result. CONCLUSIONS: In severe hand trauma including thumb amputation, thumb reconstruction using pollicization and perforator free flap could be an alternative option. It provides minimal donor site morbidity and an acceptable functional result.

ABSTRACT

Psoriatic arthritis is an uncommon disease in the community and probably occurs in no more than 5 percent of the general psoriatic population. The authors experienced a patient who had the findings of mutilating type of psoriatic arthritis with severe joint deformities. A 30 year-old man was admitted to our hospital due to multiple joint pain and deformities with wheel chair bound state. We report a case of mutilating type in the psoriatic arthritis with brief review of literatures.
