ABSTRACT
Objective: To observe the value of real-time three-dimensional echocardiography (RT-3DE) for evaluating left ventricular global and regional systolic function in children with left ventricular noncompaction (LVNC). Methods: According to left ventricular ejection fraction (LVEF) detected with conventional echocardiography, 34 children with LVNC were divided into LVEF normal group (group A, LVEF≥50%, n=25) and LVEF reduced group (group B, LVEF0.05), while rEF of apical septal, inferior and lateral segments, as well as med inferoseptal, inferior and inferolateral segments were significantly different between group A and C (all P0.05), while rESV of basal inferolateral and anterolateral segments, med anterior, anteroseptal, inferior, inferolateral and anterolateral segments, apical anterior, septal and lateral segments, as well as rEF in all 17 segments of myocardium in left ventricular were significantly different between group B and C (all P<0.05). Conclusion: RT-3DE can be used to evaluate the regional and global systolic function, which is helpful to early diagnosis, treatment and prognosis of LVNC children.

Objective To investigate the value of real-time three-dimensional echocardiography(RT-3DE) timing-excursion parametric index and 17 segment time-volume curves index in patients with leftventricular noncompaction(LVNC). Methods Ten patients with LVNC (proven by MRI) ,twenty subjects with normal LV function were examined by Philips iE33 with X3-1 probe. Results Parameter index (including Tmsv 16-SD,Tmsv 12-SD,Tmsv 6-SD,Tmsv 16-Dif,Tmsv 12-Dif,Tmsv 6-Dif,Tmsv 16-SD%,Tmsv 12-SD%, Tmsv 6-SD%, Tmsv 16-Dif%, Tmsv 12-Dif%, Tmsv 6-Dif%) of 17 segment time to minimal systolic volume was significantly higher in patients with LVNC than that in subjects with normal LV function( all P＜0.05). Average and minimum value of excursion was significantly lower in patients with LVNC than that in subjects with normal LV function ( P＜0.05). Conclusions RT-3DE with timeexcursion parameters and 17 segment of time- volume curve parameters can rapidly and accurately evaluate left ventricular systolic synchrony in patients with LVNC.

Se presenta el caso de una paciente de 36 años de edad que consulta por presentar palpitaciones y disnea de esfuerzo, diagnosticándose una insuficiencia cardíaca debida a una miocardiopatía no compactada y severo deterioro de la función sistólica ventricular izquierda. Ante la falta de mejoría a pesar del tratamiento médico máximo tolerado, se decide la colocación de un cardiodesfibrilador implantable con resincronizador. Al año de la colocación del dispositivo, la paciente se encuentra asintomática con franca mejoría de los parámetros ecocardiográficos.
We present a case of a 36 years-old female patient who presents palpitations and dyspnea, with heart failure due to non-compacted cardiomyopathy and severe left ventricular systolic deterioration. Due to the absence of improvement instead of full medical tolerated treatment, we decided to install cardiac resynchronization therapy device with an implantable cardioverter defibrillator device. One year after placement of the device, the patient is asymptomatic with clear improvement of echocardiographic parameters.
Nós relatamos o caso de uma paciente de 36 anos de idade, com palpitações e dispnéia, e o diagnóstico de insuficiência cardíaca por cardiomiopatia não-compactada e comprometimento grave da função sistólica do ventrículo esquerdo. Decidiu-se a colocação de um desfibrilador cardioversor implantável com terapia de ressincronização. Um ano após a colocação do aparelho, o paciente fica assintomática, com melhora dos parâmetros ecocardiográficos.
