ABSTRACT

Objective To study the effects and side effects of different dose of rHu-EPO on the treatment of brain injury in premature infants. Methods The infants who had suffered fetal distress and first one minute of Apgar was under 7score after birth and were sent to NICU within 24h were studied. We totally collected 90 infants and divided into three groups randomly, including large dose group, small dose group and control group. The large and small group were injected hypodermicly with rHu-EPO 1000U/kg, 500U/kg, three times per week for three weeks, and the control group were given general treatment without rHu-EPO at same time. Before treatment, one week and three weeks after treatment, we collected concentration of NSE, S100B and skull ultrasound to assess the effects. Neonatal behavioral neurological assessment(NBNA) were performed twice before and at weeks of correct gestational age. To survey the side effects, we collected general information such as the incidence rate of ROP and hemangioma, AST/ALT/PLT/Urea /Cr and so on. Results After one-week treatment, the concentration of NSE and S100B were no significant difference(P > 0. 05) in the small dose group, but were statistically significant in the large dose group(P < 0. 05). After three-week treatment, the comparison of NSE、S100B in both groups was statistically significant(P < 0. 05). The head ultrasound comparison was of statistically significant in both group(P < 0. 05), and so as NBNA and head MRI. The frequency of blood transfusion was statistically significant in both group(P < 0. 05) compared with control group. Routine blood test including liver and kidney function showed that there was no significant difference before and after treatment(P > 0. 05). Conclusion The neuroprotective effect of rHu-EPO on brain injury in preterm infants is connected with its dose and period of treatment, it need high dose or long time to express neuroprotective function.

ABSTRACT

Objective To explore the impact of family single room practices in NICU on maternal anxiety and breastfeeding status and neuropsychological development in preterm infants. Methods One-hundred NICU-born preterm infants were divided into the experimental group (n= 48) and the control group (n=52) according to random number table. Neonates in the experimental group were guaranteed parental care in the family ward for at least 4 hours a day during hospitalization in NICU, while the control group used a traditional video-based approach for daily scheduled visits. Finally, the results of exclusive breastfeeding, nosocomial infection, maternal anxiety, parental satisfaction, and full-term NBNA score after correction of gestational age were collected and recorded. Results The rates of exclusive breastfeeding were 82.50%(33/40),42.50%(17/40)during hospitalization and six months old in the experimental group, and 17.07%(7/41),12.20%(5/41)in the control group. The difference between the two groups was statistically significant during hospitalization and at 6 months of age (χ2=32.25, 9.40,P <0.01). The score of maternal anxiety was 33.00(31.00, 41.75)in the experimental group, and 43.00(34.50, 46.00)in the control group. The difference between the two groups was statistically significant (Z=-2.97, P =0.003).The NBNA score in the experimental group was 38.50(38.00, 40.00), and 37.00(36.00, 39.00)in the control group. The difference between the two groups was statistically significant(Z=-2.86, P<0.01). Conclusions The development of family ward is conducive to raising the rate of exclusive breastfeeding, reducing the incidence of nosocomial infection, relieving the anxiety of mother-infant separation and enhancing the satisfaction of parents, which can promote the behavioral and neuropsychological development of preterm infants.

ABSTRACT

Objective To investigate the difference of nerve behavior among the different types of fullterm small for gestational age infants(SGA) infants and the reasons.Methods The neonatal behavioral neurological assessment(NBNA) scores were given for 148 full-term SGA infants aged 7,14 and 28 days born in Affiliated Hospital of Hebei University.The comparason analysis was conducted among three types including symmetic,asymmetric and the mixture of them.Results Significant differences (P < 0.05) were found between the NBNA scores for full-term SGA infants and normal infants.The NABA scores of full-term SGA infants were lower than those of normal infants.The NBNA scores for three types of full-term SGA infants followed the order of asymmetric > symmetric > mixture of them,significant difference was found between any two types of three types(P < 0.05).Conclusion Full-term SGA infants show the poorer quality of nerve behavior compared with the nomal infants.The neurodevelopment levels for three types of full-term SGA infants are asymmetric,symmetric,mixture of them in the order.

ABSTRACT

@#ObjectiveTo discuss the effect of neonatal behavioral neurological assessment (NBNA) and developmental screening test (DST) on newborns with hypoxic ischemic encephalopathy (HIE). MethodsNBNA was performed for 60 newborns with HIE within 12-14 days and DST was also performed at 6th month after birth.ResultsMild and severe degree HIE newborns with low NBNA scores had unfavorable prognosis. Conclusions NBNA and DST can provide evidence for early interference for newborn's HIE.
