ABSTRACT

Aims: the current research aimed to investigate LncRNA-MIAT in patients with nonHodgkin lymphoma (NHL) and to assess its correlation with clinicopathological features and treatment protocols of NHLs among Egyptian patients with Occult hepatitis C virus (HCV) infection (OCI). Patients & Methods: This study was conducted on 20 patients with NHL and 30 healthy subjects as the control group. All subjects were screened for HCV-RNA in both plasma and PBMCs. RT-PCR determined lncRNA-MIAT. Results: lncRNA-MIAT relative expression level was upregulated in NHL groups (2.73±0.86) compared to controls (1.06±0.07), P ˂0.001*. Among NHL, patients with OCI (3.2±0.63) had significantly higher levels of lncRNA-MIAT compared to HCV (2.6±1.08) and non-HCV (2.4±0.4), P ˂0.001*. Additionally, the relative expression levels of lncRNA-MIAT were significantly positively correlated with laboratory and clinicopathological features of NHL. Interestingly, concerning the treatment of DLBCLNHL, there were significantly higher levels of lncRNA-MIAT in no treatment subgroup (n=10, 3.31±0.95) compared to successfully treated subgroups [CHOP (n=7, 1.58±0.34) and R-CHOP (n=3, 11.16±0.21), P ˂0.001* Conclusions: lncRNA-MIAT level was upregulated in NHL patients, particularly patients with OCI. Thus, circulatory lncRNA-MIAT may serve as a promising non-invasive diagnostic marker for NHL associated with OCI

ABSTRACT

Objective • To investigate the current status of self-perceived burden of the patients with non-Hodgkin's lymphoma (NHL) and analyze the related factors. Methods • A total of 169 NHL patients who were hospitalized in Ruijin Hospital, Shanghai Jiao Tong University School of Medicine from Jan. to Oct., 2019, and their caregivers were enrolled by convenience sampling method. The general information questionnaire, Social Support Rating Scale (SSRS), Self-rating Depression Scale (SDS), Zarit Caregiver Burden Interview (ZBI) and Self-perceived Burden Scale (SPBS) were used to investigate, and the factors related to the score of SPBS were analyzed. Results • The average score of SPBS was 28.95±11.27, while the score of economic burden was the highest (3.13±1.37). Multiple regression analysis suggested that the family income per month per person (P=0.001), Karnofsky performance status (KPS) (P=0.027), SDS score (P=0.006) and ZBI score (P=0.000) were associated with the SPBS score in the NHL patients. Conclusion • The overall selfperceived burden in the patients with NHL is mild-to-moderate, but the economic burden is not neglectable. Low monthly income, poor ability of self-care, depressive symptoms and heavy burden of caregivers can aggravate the self-perceived burden of the patients with NHL.

ABSTRACT

Primary lymphoma of female genital tract (PLFGT) is uncommon, and its etiology is unclear. It can present with pain, distention, vaginal bleeding, or discharge or be found as an incidental finding at a routine pelvic examination or surgery for other indications. PLFGT can be diagnosed with imaging, with pathomorphology being the key to diagnosing this disease. Further diagnosis and typing are needed with flow cytometry and immunohistochemistry. PLFGT should be differentiated from secondary genital lymphoma and other primary malignant tumors of the reproductive system. Preoperative diagnosis can be challenging due to its rarity. In addition, there is a lack of experience in treating PLFGT. Based on the pathological classification, staging, and non-Hodgkin lymphoma (NHL) guidelines, an individualized approach should be determined. R-CHOP has been found to significantly improve the survival of patients with NHL. However, the efficacy and safety of chemotherapy as the standard treatment for PLFGT still needs to be confirmed. Prognosis was evaluated according to the international prognostic index (IPI).

ABSTRACT

Introduction: Epstein–Barr virus (EBV) might be an aetiological agent involved in the pathogenesis of certain Non-Hodgkin’s Lymphomas (NHLs). EBV infection has been diagnosed by serologic testing within the tumour biopsies of patients with NHL. However, the association between EBV and NHL is inconsistent with a preference for certain anatomic sites, histologic subtypes and immunosuppressed patients. The objective of this study was to characterise the B-cell NHLs of the oral cavity and maxillofacial region using histological and immunophenotypical techniques and to determine its association with EBV infection. Materials and Methods: This was a descriptive cross-sectional study that included 14 cases of B-cell NHLs of the oral cavity and maxillofacial region. The haematopoietic and lymphoid tissue tumours classification of WHO was used to categorize the cases. In-situ hybridisation for EBV–encoded RNA was performed to confirm the EBV infection. Results: The average age of the patients included in the study was found to be 48.8 ± 23 years with a higher female to male ratio (1.3:1). Our study suggested that diffuse large B-cell lymphomas (DLBCLs) and Burkitt’s lymphomas (BLs) constitute the predominant subtypes of lymphomas affecting the oral cavity and maxillofacial regions. Conclusion: The findings from our study support the view that at least a relatively smaller proportion of B-cell NHLs that occur in the oral cavity and maxillofacial region do not have a pathogenic association with EBV.

ABSTRACT

Non-Hodgkin lymphoma (NHL) is an adverse group of neoplasms that can occur at nodal and extranodal sites with varied prognosis. NHL involving soft tissue as a primary site accounts to 0.1% of cases and may mimic sarcoma; therefore, it is necessary to differentiate them as described by travis and co-authors, similarly. We describe a case of 72-year-old male patient with a mass in the right thigh. Magnetic resonance image reported a benign soft tissue tumour; fine-needle aspiration cytology suggested a soft tissue sarcoma and computed tomography imaging reported no other sites of involvement. PET (Positron emission tomography) scan done after first cycle of chemotherapy revealed enlarged right-side femoral and external iliac group of lymph nodes. On histological examination and immunohistochemistry, it was diagnosed as primary diffuse large B-cell lymphoma involving skeletal muscle. The case is presented to highlight the diagnostic difficulties due to its presentation on a rare site.

ABSTRACT

Primary caecal lymphoma or the colonic lymphoma is a rare tumor of the gastrointestinal (GIT) tract and comprises only 0.2-1.2% of all colonic malignancies, both in adults and pediatric age group. The most common variety of colonic or caecal lymphoma is a on-Hodgkin’s Lymphoma (NHL) which arises from the lymphoid elements of the intestine. GIT is the most frequently involved site, accounting for 50-60% of all extra nodal lymphomas, and most of them are NHL. In adults, the stomach is the most common location of GIT lymphomas, followed by the small intestine, but the most common GI site of NHL in children is the terminal ileum and the ileo-caecal region. Diagnosis is difficult since lymphoma presents with vague abdominal pain with loss of weight and appetite. It may present as lump abdomen with complications such as intestinal obstruction, bleeding, perforation and peritonitis and intussusseption. Histologically it is B or T cell type with small or large cell variation, but frequently encountered is diffuse large B-cell cell lymphoma (DLBCL). DLBCL has low incidence but favorable outcome in young adults, lesions localized to one area or organ and children below 5 years of age, but has an aggressive course in children between 10-15 years of age and also in adults above 55 years of age. DLBCL or GIT Lymphomas in general have male preponderance. We present a rare case of caecal lymphoma (DLBCL) involving appendix and right ovary in a 12 year old girl who presented with vague symptoms of abdominal pain and abdominal mass. Clinically and radiologically, provisional diagnosis of Ileo-caecal tuberculosis with possibility of adhesions leading to a mass lesion was considered. Histopaththological examination (HPE) revealed the diagnosis and prognosis of the case.

ABSTRACT

Objective To investigate the expression of B-cell activating factor ( BAFF ) and its specific receptor BAFF-R in patients with B-cell non-Hodgkin′s lymphoma ( B-NHL) and to analyze the cor-relations between BAFF and the development of B-NHL.Methods RTQ-PCR and Western blot assay were used to measure the expression of BAFF and its specific receptor BAFF-R in patients with B-NHL.Fluores-cence immunocytochemical staining was used to determine the localization of BAFF and BAFF-R in Raji cells, a B-NHL cell line. The expression of BAFF in tumor tissues from patients with B-NHL of different his-tologic subtypes was measured by immunohistochemistry.WST proliferation and TUNEL assays were used to evaluate the effects of BAFF and BAFF-R on the proliferation, survival rate and apoptosis of Raji cells.Lin-ear correlations between the concentrations of lactate dehydrogenase ( LDH) and the expression of BAFF and BAFF at mRNA and protein levels in patients with B-NHL were analyzed. Results BAFF and its specific receptor BAFF-R were expressed in Raji cells and played an important role in the survival and proliferation of B-NHL cell line. The expression of BAFF in tumor cells from patients with B-NHL varied with the different histologic subtypes of B-NHL.Patients with small B-cell malignant lymphoma, large B-cell lymphoma ( LBCL) , mucosa-associated lymphoid tissue lymphoma ( MALT lymphoma) and follicular lymphoma showed higher levels of BAFF, while those with mantle cell lymphoma showed lower levels of BAFF.Compared with the healthy subjects, patients with B-NHL showed significantly increased expression of BAFF at mRNA and protein levels. The levels of LDH were closely related to the expression of BAFF at mRNA and protein lev-els.Conclusion BAFF and its specific receptor BAFF-R might play an important role in the growth and survival of malignant B cells.

ABSTRACT

Aims: Non-Hodgkin’s lymphoma (NHL) is a heterogeneous group of hematological malignancies. Several angiogenic factors are important in NHL. The objective of this study was to determine plasma levels of various proangiogenic [vascular endothelial growth factor (VEGF), platelet derived growth factor (PDGF), tumor necrosis factor (TNF- α), transforming growth factor (TGF-β), interleukin (IL-6), IL-8] and antiangiogenic [IL-4, IL-12, interferon gamma (IFN-)] factors in NHL patients and implication of CHOP (cyclophosphamide, hydroxydaunorubicin, oncovin, prednisone) therapy on these mediators. Study Design: This study was conducted on 100NHL patients recruited from the Oncology Hospital, Menofia University, Egypt. Fifty patients had different doses of CHOP chemotherapy and 50 patients were without treatment. Another 119 healthy blood donors were served as healthy controls. Methodology: Enzyme-linked immunosorbent assay (ELISA) was used to detect the concentrations of these mediators in plasma of NHL patients and normal controls. Results: Several proangiogenic (VEGF, PDGF, IL-8, and TNF-α) (P<.001, P<.01, P<.01, and P<.001) and antiangiogenic (IL-4,IL-12) (P=.05,P<.01) mediators were decreased in NHL patients. In contrast, IL-6, and IFN-) were increased (P<.001). After CHOP treatment, VEGF, and PDGF were significantly increased (P<.001) as compared to NHL without therapy Fig.1. Reduction in IL-8 (P<.01) and TNF-α (P<.001) in untreated NHL patients was continued after CHOP treatment. IL-6 was the only elevated cytokine in NHL patients with (P<.001) or without (P<.001) treatment compared to healthy individuals. Concerning the antiangiogenic mediators, treatment resulted in reduction in the secretion level of IFN-) (P<.001) and IL-12 (P<.001). Although CHOP treatment required more than 8 doses to be able to down regulate proangiogenic (VEGF,PDGF, IL-8,TNF-α and IL-6) and elevate antiangiogenic (IL-4,IL-12,IFN-) mediators, changes were statistically insignificant. Conclusion: Collectively, our study stressed on the importance of having an angiogenic profile of NHL patients under treatment which could be used to monitor the efficacy of cancer therapy particularly in therapy with antiangiogenic drugs. Further studies are required to confirm these results.

ABSTRACT

Background & objectives: The prerequisite of radioimmunotherapy is stable binding of a radionuclide to monoclonal antibodies, which are specific to the tumour-associated antigen. Most B-cell lymphomas express CD20 antigen on the surface of the tumour cells, making it a suitable target for therapeutic radioactive monoclonal antibodies. In the present study, the immunoconjugate of biosimilar Rituximab (Reditux™) and macrocyclic chelator, p-SCN-Bz-DOTA, was prepared and radiolabelled with Lutetium-177 followed by quality control procedures. Methods: Rituximab(BioSim) was desalted with sodium bicarbonate (0.1M, pH 9.0) and incubated with DOTA-SCN (1:50). The effectiveness of the conjugation was evaluated by determining the number of chelators per antibody molecule. This conjugate was radiolabelled with Lutetium-177 and purified using PD10 column. The quality control parameters like pH, clarity, radiochemical purity, in vitro stability and sterility were studied. Immunoreactivity of 177Lu-DOTA-Rituximab (BioSim) was assessed using RAMOS cells. The radioimmunoconjugate (RIC) after stringent quality assurance was injected in three patients and the biodistribution profile was analysed. Results: An average of 4.25 ± 1.04 p-SCN-Bz-DOTA molecules could be randomly conjugated to a single molecule of Rituximab (BioSim). The radiochemical purity of the labelled antibody was >95 per cent with preserved affinity for CD20 antigen. The final preparation was stable up to about 120 h when tested under different conditions. A favourable biodistribution profile was observed with liver showing the maximum uptake of the RIC. Interpretation & conclusions: A favourable radiochemical purity, stability and biodistribution of the radiolabelled immunoconjugate indicate that clinical trials for evaluation of toxicity and efficacy of 177Lu-DOTA-antiCD20 antibody-Rituximab (BioSim) in patients of relapsed and refractory non Hodgkin’s lymphoma can be considered.

ABSTRACT

Objective: We investigated the clinical significance and prognostic value of peripheral blood absolute lymphocyte count (ALC) in the diagnosis of diffuse large B-cell lymphoma. Methods:The medical records of 143 patients with diffuse large B-cell lymphoma were reviewed. These patients were diagnosed and underwent surgery in Tianjin Medical University Cancer Institute and Hospital, China, between February 2002 and April 2008. The correlation between different ALC levels and clinical features was statisti-cally analyzed. Results:Patients with low ALC (<1.0×109/L) at diagnosis showed B symptoms, an international prognostic index score of ≥2, advanced clinical stage (Ann Arbor III/IV), high lactate dehydrogenase level, and high β2-microglobulin level (P<0.05). Pa-tients with high ALC (≥1.0 × 109/L) responded well to treatment (P=0.003). In multivariate analysis, Ann Arbor stage and ALC level were independent prognostic factors of patients with diffuse large B-cell lymphoma. Patients with low ALC often exhibited advanced clinical stage (Ann ArborⅢ/Ⅳ). Conclusions:ALC level obtained at diagnosis is a novel, powerful prognostic factor of diffuse large B-cell lymphoma.

ABSTRACT

Primary isolated bone marrow disease as a presenting feature of lymphoma is very rare. We describe the case of a Chinese with isolated bone marrow small B-cell lymphoma as a first manifestation. A 55-year old woman was admitted to our hospital with fever. Her peripheral blood smear and laboratory findings were suggestive of bicytopenia. Bone marrow specimen showed diffusely distributed small-sized lymphocytes. Combined with immunophenotypic and chromosomal analysis, a diagnosis of primary bone marrow B-cell non-Hodgkin's lymphoma was made. The patient was treated with R-CHOP (rituximab and cyclophosphamide, epirubicin, vindesine, and prednisone) regimen for six cycles. She had complete remission and is still alive without relapse. We concluded that primary bone marrow mature small B-cell lymphoma is a rare but distinctive subtype of lymphoma. The prognosis for this entity is poor but rituximab-based treatment is promising for improving its outcomes.

ABSTRACT

BACKGROUND: Reduced bone mineral density (BMD) is a significant sequelae in children receiving chemotherapy for acute lymphoblastic leukemia (ALL) and non-Hodgkin lymphoma (NHL). Reduced BMD is associated with an increased risk for fractures. Pamidronate, a second-generation bisphosphonate, has been used to treat osteoporosis in children. This study evaluated the safety and efficacy of pamidronate in children with low BMD during and after chemotherapy for ALL and NHL. METHODS: Between April 2007 and October 2011, 24 children with ALL and NHL were treated with pamidronate. The indication was a decreased BMD Z-score less than -2.0 or bone pain with a BMD Z-score less than 0. Pamidronate was infused at 1 mg/kg/day for 3 days at 1-4 month intervals (pamidronate group, cases). The BMD Z-scores of the cases were compared with those of 10 untreated patients (control group). Lumbar spine BMDs were measured every 6 cycles using dual energy X-ray absorptiometry and Z-scores were calculated. Bone turnover parameters (25-hydroxyvitamin D, alkaline phosphatase, parathyroid hormone, osteocalcin, and type I collagen c-terminal telopeptide) were analyzed. RESULTS: The median cycle of pamidronate treatment was 12. Increases in BMD Z-scores were significantly higher in the pamidronate group than in the control group (P<0.001). BMD (mg/cm2) increased in all pamidronate-treated cases. Twenty patients who complained of bone pain reported pain relief after therapy. The treatment was well tolerated. CONCLUSION: Pamidronate appears to be safe and effective for the treatment of children with low BMD during and after chemotherapy for ALL and NHL.

ABSTRACT

Cancer chemotherapy engenders a gamut of unpredictable and perilous toxicities, which can cause substantial mortality, morbidity, and results in escalated healthcare costs due to increased requirement for ameliorative treat-ments and toxicity-related hospitalization. The main objective of this article is to report a range of adverse effects of CHOP regimen in patients with intermediate grade Non-Hodgkin’s Lymphoma (NHL). This randomized prospective study was conducted on de novo intermediate grade NHL patients, aged >20≤60 years, receiving CHOP chemotherapy regimen. Out of 30 patients who satisfied the inclusion criteria, only 25 patients completed 6 cycles of chemotherapy and were evaluated for various toxicities. Evaluation of all the 25 subjects showed that, in comparison with the baseline, most patients exhibited toxic damages. The hepatopancreatic, dermatological, vascular and nephrological damages, eventually resulted in weight loss, hepatomegaly, alopecia, hypotension, as well as abnormal levels of serum liver markers, serum creatinine and blood glucose. But, the necessity to interrupt the CHOP treatment did not arise in any of the patients. Thus, in this study combination chemotherapy with CHOP protocol was well tolerated by majority of the subjects.

Aim To construct and express anti-CD20Fab-LDP,generate anti-CD20Fab-LDM and identify its biological activity. Methods PCR and overlapping PCR were used to construct anti-CD20Fab-LDP.DNA sequence was analyzed by the Terminus of Dideoxy Nucleotide. The product was purified by affinity chromatography and analyzed by Western blot and its antigen-binding activity was examined by FACS.Specific killing activity in vitro of anti-CD20Fab-LDM was analyzed by MTT.Results The data of DNA sequence showed that anti-CD20Fab-LDP was correct. The fusion protein was recovered in high yield(up to 4 mg·L~(-1))after proteinG purification. The fusion protein could bind to Raji cells(CD20+),and similar affinity data were obtained with anti-CD20Fab.Anti-CD20Fab-LDP showed potent cytotoxicity to Raji cells with IC_(50) values of 0.9×10~(-10) mol·L~(-1).Conclusions Anti-CD20Fab-LDP with high level expression was successfully obtained and could bind to Raji cells cells.Anti-CD20Fab-LDM showed specific killing activity to Raji cells in vitro.

ABSTRACT

Gingival hyperplasia is a frequent complication in transplant patients who receive cyclosporine or calcium channel blockers. We studied an unusual case involving a renal transplant recipient with post-transplant non-Hodgkin's lymphoma that manifested as gingival hyperplasia. We initially consider that it was a side effect of cyclosporine and nifedipine. The lesion did not respond to dose reductions or the withdrawal of cyclosporine and nifedipine, and the gingival hyperplasia progressed in a localized fashion, becoming ulcerated and bleeding easily. Histological examination revealed the presence of malignant lymphoma.

ABSTRACT

HIV-infected individuals have a high risk of developing non-Hodgkin lymphoma (NHL). NHL is the second most common malignant disorder associated with HIV infection, the first being Koposi's sarcoma. In the late 1990s, however, NHL became more common than Kaposi's sarcoma in some areas. Historically, AIDS-definding lymphomas have included systemic B-cell NHL (ie, immunoblastic and Burkitt's lymphoma) and primary brain lymphoma. We report a case of small bowel NHL in AIDS patient. A 58-year-old man presented with chronic fatigue and progressive abdominal pain. Gastroduodenal endoscopy shows multiple polypoid lesion on duodenum and diagnosed as Burkitt's lymphoma by histologic finding, immunohistochemical study associated with HIV positive. This is the first report of small bowel NHL from AIDS patients in Korea.
