ABSTRACT

ObjectiveTo understand the vaccination situation of non-national immunization program (non-NIP) vaccines in Changning District, and to provide evidence for further strengthening the management of vaccination services and guiding community health service centers to construct vaccination clinics for children and adults. MethodsNumber of non-NIP vaccine doses in community health service centers, Changning District, was collected from Shanghai Immunization Information System from 2016 to 2020. Descriptive epidemiological analysis was conducted. ResultsIn 2020, the overall number of non-NIP vaccine doses, proportion of non-NIP vaccine doses and average number of non-NIP vaccine doses in the community health service centers, Changning District, reached up to 115 361 doses, 63.41% and 1 664.54 per10 000 persons, respectively. Furthermore, the proportion of non-NIP vaccine doses showed a significantly upward trend (χtrend2=131.110, P<0.01). The number of non-NIP vaccine doses increased mostly in adults aged >18 years. Human papillomavirus vaccines (HPV) and influenza vaccines (InfV) had the highest increase in vaccine doses. Additionally, non-NIP vaccination peaked from September to December in adults. ConclusionAlong with the number of non-NIP vaccine doses increase, it is necessary to strengthen the management of non-NIP vaccination and explore effective measures for construction of adult vaccination clinics to meet the public demand for vaccination.

ABSTRACT

Abstract Objective To evaluate the clinical and epidemiological profile of bacterial meningitis and meningococcal disease in pediatric patients admitted to a Brazilian Secondary Public Hospital. Methods A descriptive observational study was conducted. Microbiologically proven bacterial meningitis or meningococcal disease diagnosed from 2008 to 2018 were included. Results A total of 90 patients were diagnosed with proven bacterial meningitis. There were 64 confirmed cases of meningococcal disease. The prevalence was higher in boys (n=38), median age 30 months (1-185). The main clinical manifestations were: meningococcal meningitis (n=27), meningococcemia without meningitis (n=14), association of meningococcemia with meningitis (n=13), and fever without a known source in infants (n=7).Admissions to intensive care unit were necessary for 45 patients. Three deaths were notified. Serogroup C was the most prevalent (n=32) followed by serogroup B (n=12).Pneumococcal meningitis was identified in 21 cases; out of the total, 10 were younger than two years. The identified serotypes were: 18C, 6B, 15A, 28, 7F, 12F, 15C, 19A and 14. Pneumococcal conjugate 10-valent vaccine covered four of the nine identified serotypes.Haemophilus influenzae meningitis serotype IIa was identified in three patients, median age 4 months (4-7). All of them needed intensive care. No deaths were notified. Conclusion Morbidity and mortality rates from bacterial meningitis and meningococcal disease remain high, requiring hospitalization and leading to sequelae. Our study observed a reduced incidence of bacterial disease over the last decade, possibly reflecting the impact of vaccination.

ABSTRACT

National Immunization Program (NIP) in korea is regulated by government for effective prevention of infectious diseases since 1954. But the program is only performed in public health center, so many people had various complaints such as inconvenience to use public center, high cost when vaccinated in private medical organization and lack of speciality and registration etc. In this reason, it was very difficult to raise the vaccination rate to 95 percent, infectious disease preventable rate. With this background national assembly and government try to expand the NIP coverage both public health center and private medical organization with increasing budget from 2009. So in this paper an overview and preparation of Korean pediatrician according to the expanding plan of NIP are described.

ABSTRACT

Nip2 mRNA expression of hippocampus was clarified in the postnatal, adult and aging rat. We observed the change of Nip2 mRNA expression in the ischemic rat hippocampus at 3, 6, 24 and 72 hours after reperfusion. And we investigated the relation between Nip2 and apoptosis after ischemic insults. The Rats were killed 2, 4, 7 days after birth and normal adult rats and aged rat were killed. Male F344 rats were subjected to transient middle cerebral artery occlusion. Reperfusion was achieved by withdrawing the filament after 90 min minutes, and rats were sacrificed 3, 6, 24, 72 hours after reperfusion. Hippocampal sections were stained for TUNEL using ApopTag kit following the protocol provided by the manufacturer, and stained with 2, 3, 5 -triphenyl tetrazolium chloride and cresyl violet We used quantitative reverse transcription and polymerase chain reaction to characterise changes in the mRNA expression of Nip2 in the rat models of transient focal ischemia and postnatal, adult and aged rat. Nip2 mRNA expression were increased in the rat of postnatal development and aging more than these of adult. After reperfusion, marked increase of Nip2 mRNA was observed after 3 and 6 hours. After that time mRNA expression of Nip2 was decreased gradually. The TUNEL staining detected DNA fragmentation in neurons of the entorhinal cortex, forcep major corpus callosum and secondary visual cortex at 24 hours. And TTC staining results showed the whitish infact changes of hippocampal CA1 region and lateral habenular nucleus. We hypothesize that the overexpression of Nip2 is concerned with sensitivity to the ischemic insult at postnatal period and aging period. And, early apoptotic events after cerebral ischemic insults relate to Nip2 mRNA overexpression.
