ABSTRACT

Objective: The present study aims to investigate the factors related to self-rated ikigai (purpose in life) among older residents participating in hillside residential community-based activities in Nagasaki City.Methods: A self-administered anonymous questionnaire survey was carried out with older residents participating in two hillside residential community-based activities in Nagasaki City, Japan. The questionnaire included questions on sociodemographic information (age, sex, family structure, education, and self-rated economic satisfaction), self-rated health, mental health status measured using Geriatric Depression Scale-15 (GDS-15), and self-rated ikigai score that was estimated using a visual analog scale.Results: A total of 32 older residents (7 males, 25 females) participated in the questionnaire survey. Although self-rated ikigai score was not associated with sociodemographic factors, there were associations between the score, self-rated health (P=0.001), and mental health (GDS-15) (P=0.015). Statistically significant correlations between self-rated ikigai score and social participation (ρ=0.426, P=0.017), self-rated health (ρ=−0.485, P=0.007), and mental health (GDS-15) (ρ=−0.523, P=0.007) were observed.Conclusion: Increasing social participation may increase individual ikigai, preventing poor self-rated health and low mental health status in older people. Maintaining their social participation in the community might be effective for the health promotion of older residents in hillside residential areas of Nagasaki City.

ABSTRACT

Objective:The Nagasaki Prefectural Centre of Medicine and Welfare for Children was established in 1954 as a center for children with physical disabilities in Nagasaki prefecture. Medical education consultation in the local area of Nagasaki prefecture was started in the following year.Methods:We examined the number of patients and the area selected for this project over the recent 36 years. Documents from the center's library were examined, and orthopedic physicians who had previously participated in the project were interviewed.Results:The number of patients and visiting areas in the project continuously decreased. This patient tendency was caused by the declining birth rate in Japan and the satisfaction from human services in local areas. The decrease in the number of local areas visited was affected by the financial administration of our center and the development of a traffic network in the mainland of Nagasaki prefecture. Health visitors now working in the island continue to require visits to pediatric specialists from the mainland twice a year. The number of visits to four selected isolated islands has been maintained till date.Conclusion:Medical support from the mainland is necessary for isolated islands because of the absence of physicians, especially those for children with physical disabilities, and visiting the hospital in the mainland by airplane is difficult for these children.

La presión científica alimentada por los diversos intereses geopolíticos de la primera mitad del siglo XX, fue sin duda el motor de impulso necesario para la investigación y el desarrollo de la tecnología nuclear. Una vez superada la crisis desencadenada por la Segunda Guerra Mundial, el conocimiento obtenido sobre los generadores atómicos aplicados a las necesidades civiles, no demoro en consolidar la energía nuclear como una opción futurista a la previsible escasez mundial de combustible. Es por eso, que el átomo se presento como una carta maestra para la seguridad, energía, economía, desarrollo y el cuidado ambiental. Sin embargo, pese a los controles para limitar su desarrollo como elemento militar, esta “panacea” no carece de generar dudas sobre su costo/beneficio. La historia relata algunos acontecimientos relacionados con dichos generadores, que han causado daños irreparables de manera inmediata y a largo plazo en el medio ambiente y en las poblaciones aledañas a aquellas estructuras. Research and development of nuclear technology were done by a science under pressure of the geopolitical interests in the first half of the 20th century. Once the crisis triggered by the World War II passed, the knowledge gained about atomic generators applied to civilian needs, was able to quickly consolidate nuclear energy as a futuristic option for the global fuel shortage. At the same time, the atom was presented as a trump card for security, energetic, economic, and environmental development. However, despite the controls to limit its expansion as a military weapon, this “panacea” does not lack of doubts its costs versus its benefits. History can tell some events related to these generators, which have caused irreparable damage in their surrounding environment and nearby towns immediately and in a long term.
