ABSTRACT

STUDY DESIGN: Prospective study. OBJECTIVES: This study was conducted to investigate changes in perceptions of treatment using narcotic analgesics and quality of life in chronic back pain patients. SUMMARY OF LITERATURE REVIEW: Negative perceptions of narcotic analgesics as pain killers have been established as factors affecting compliance and adherence. MATERIALS AND METHODS: A total of 140 patients who had chronic back pain for over 3 months were examined using clinical scales such as the Korean version of the Oswestry Disability Index (KODI), the Short Form-12 (SF-12), and a visual analog scale (VAS). The survey regarding narcotic analgesics classified patients as having positive perceptions if they reported absolutely not wanting to use them or being unlikely to use them at the primary interview and after 4 weeks of treatment. RESULTS: Ninety-four patients (68%) reported negative perceptions of narcotic analgesics at the primary interview. Sixty-one of those patients (64%) changed their perceptions, reporting positive perceptions after 4 weeks of treatment, as indicated by the ODI (p=0.01), SF-12 (p=0.01), and VAS (p=0.01) scores. A change from positive to negative perceptions after 4 weeks of treatment was observed in 13 patients (28%) who experienced adverse effects of narcotics treatment (p=0.01). Among the 33 patients (23%) whose negative perceptions did not change, dissatisfaction with previous treatment was found to be a contributing factor in 22 (66%). CONCLUSIONS: Clinical improvements after treatment using narcotic analgesics in chronic back pain patients resulted in a significant positive impact on perceptions about narcotic analgesics. Narcotic analgesics could be an alternative treatment choice in chronic back pain patients because of improvements in their quality of life.

ABSTRACT

OBJECTIVE:To evaluate the application of narcotic analgesics in the inpatients of our hospital, and to provide ref-erence for rational drug use in clinic. METHODS:The application of narcotic analgesics in the inpatients of our hospital during 2013-2016 was analyzed statistically in respects of consumption sum,DDDs,DDC and B/A. RESULTS:There were 13 kinds of narcotic analgesics in the inpatients of our hospital. Top 3 narcotic analgesics in the list of consumption sum were Fentany transder-mal patches (8.4 mg),Oxycodone hydrochloride prolonged-release tablets (20 mg) and Fentany transdermal patches (4.2 mg) in our hospital during 2013-2016. Top 2 drugs in the list of DDDs were Fentany transdermal patches(4.2 mg)and Fentany transder-mal patches (8.4 mg). DDC of Oxycodone hydrochloride prolonged-release tablets (10,20 mg) ranged 350-370 yuan, taking up first or second place. B/A of Oxycodone hydrochloride prolonged-release tablets(10,20 mg)was<0.50,those of Bucinnazine hy-drochloride injection and Compound platycodon tablets were >1.50;those of other drugs mostly ranged 0.50-1.50. CONCLU-SIONS:The consumption trend of narcotic analgesics varies rarely in our hospital during 2013-2016,and the clinical use of narcot-ic analgesics is basically rational. Morphine sustained-release tablet and Fentany transdermal patch have become the first choice for pain treatment.

ABSTRACT

Objective To evaluate the current situation and trend of narcotic analgesics used in 29 hospitals of Nanjing from 2010 to 2012 and provide references for reasonable clinical drug utility .Methods Consumption of narcotic analgesics in 29 hospitals of Nanjing was analyzed retrospectively in terms of sales value , DDDs, DDC, etc.The DDDs, sales value and variety of narcotic anal-gesics were analyzed statistically according to the level , type and district of hospitals .Results There were a total of 13 kinds of nar-cotic analgesics in the 29 hospitals and the sales value increased yearly .During the 3 years, the DDDs of fentanyl citrate injection ,fent-anyl transdermal system and remifentanil hydrochloride were ranked in the forefront and the DDDs rank of controlled -release morphine and controlled-release oxycodone hydrochloride was stable .The sales value and DDDs in Drum Tower District were greater than other districts and increased yearly .Conclusion The application of narcotics analgesic in 29 hospitals of Nanjing was rational and complied with the relevant national regulations .

ABSTRACT

Objective To analyze analgesics administration from July 2005 to July 2006 in emergency rooms of the First, the Second and Fourth Affiliated Hospitals of China Medical University in order to provide a reference for clinically rational administration. Method The data of 2313 prescriptions with analgesics administered during one-year period were analyzed in many respects including the overview of the prescriptions, the frequency of anal-gesics administration, the system of defined daily doses (DDDs) and drug utilization index (DUI) of narcotic anal-gesics were analyzed, and a survey of 200 patients managed with some of those analgesics was done by using ques-tionaire as they were admitted to and discharged from the emergency room. The pain intensity was evaluated by a 10-point numerical rating scale (NRS). The respondents, excluding the mute or deaf, police custody, victims of domestic violence, mental disorder and age under 14,rated the levels of satisfaction with medication for pain relief. The data of frequency and percentage of the administration of analgeics were analyzed, and the scores of NRS were evaluated with the Paired-samples t -test. Results Most of analgesics were in the form of parenteral route usage, of which anisodamine and bucinnazine were employed in large proportion, and a small number of them was in the form of tablets. Trauma was the commonest cause of pain. Of the narcotic analgesics, meperidine was the most com-monly used analgesics, and its DDDs and DUI were much lower than that of WHO limits. Of the 200 patients, 71.5% patients rated a considerably high satisfaction with scores of (7.47 ±2.21) and (5.00 ± 3. 16) by NRS before and after medication, respectively ( ( -value 23.38,P < 0.01) .The patients presenting pain intensity with a scores of 4 or greater accounted for 57.5% . Conclusions The patients suffering from pain could lie rationally treated in the emergency rooms of those three hospitals. Narcotic analgesics should be cautiously employed, and there is room for improvement in pain management practice in emergency room.
