ABSTRACT

Introdução: A maioria dos defeitos nasais que se apresentam para reconstrução são secundários à excisão tumoral. O Objetivo é analisar a eficácia da técnica reconstrutora utilizada para a cobertura do defeito após exérese tumoral de acordo com a subunidade anatômica nasal acometida. Método: Estudo retrospectivo dos prontuários de 118 pacientes que durante o período de agosto de 2012 a março de 2017 foram submetidos à ressecção dos tumores nasais no Instituto Mário Penna em Belo Horizonte/MG. Resultados: A incidência foi maior em mulheres (56%) e brancos (54,3%) com idade média de 71,3 anos. Foram ressecados 125 tumores e realizadas 122 reconstruções nasais. O carcinoma basocelular (90,4%) foi o tumor de pele não melanoma mais prevalente, sendo o subtipo histológico sólido (33,6%) o mais frequente. As técnicas para reconstrução dos defeitos que acometiam apenas uma subunidade nasal foram em sua maioria utilizando o retalho bilobado (26,5%). Nas reconstruções nasais complexas, o retalho miocutâneo bilobado (45,8%) com extensão para a região glabelar (englobando os músculos prócero, corrugador e nasal) foi o mais utilizado, principalmente em defeitos no terço inferior do nariz. Cerca de 78 pacientes apresentaram acompanhamento oncológico superior a um ano, sendo avaliados 82 tumores no total. Sete (8,5%) tumores recidivaram mesmo após ressecção completa e, entre os seis pacientes com margens comprometidas, apenas um (1,2%) recidivou. Conclusão: As técnicas reconstrutoras utilizadas foram eficazes para o tratamento do câncer de pele nasal e cobertura dos defeitos após ressecção, apresentando baixos índices de complicação e recidiva.


Introduction: A majority of the nods present for reconstruction are secondary to tumor excision. The objective is to analyze the efficacy of the reconstructive technique used to cover the defect after tumor exeresis according to the affected nasal anatomical subunit. Method: Retrospective study of the medical records of 118 patients submitted to resection of the nasal tumors at the Mário Penna Institute in Belo Horizonte/ MG from August 2012 to March 2017. Results: Incidence was higher in women (56%) and whites (54.3%) average age of 71.3 years. A total of 125 tumors were resected, and 122 nose reconstructions were performed. Basal cell carcinoma (90.4%) was the most prevalent nonmelanoma skin tumor, the most frequent solid histological subtype (33.6%). The techniques for reconstruction of defects that affect only one nasal subunit were mostly using the bilobed flap (26.5%). In complex nose reconstructions, the bilobed myocutaneous flap (45.8%) with extension to a glabella region (encompassing the procerus, corrugator and nasal muscles) was the most used, mainly in defects in the lower third of the nose. About 78 patients had cancer follow-up of more than one year, and 82 total sin tumors were evaluated. Seven (8.5%) Tumors retreated even after complete resection, and, among the six patients with compromised margins, only one (1.2%) relapsed. Conclusion: The reconstructive techniques used were effective for treating nasal skin cancer and coverage of defects after resection, with low rates of complication and recurrence.

Introdução: O carcinoma basocelular é o tipo mais frequente de lesão tumoral de pele acometendo frequentemente a região nasal. Os defeitos resultantes exigem o emprego de técnicas de reconstrução nasal. A primeira cirurgia plástica descrita é a reconstrução nasal segundo o método indiano. Ao longo dos séculos sofreu diversas modificações pelos maiores nomes da especialidade, culminando no retalho frontal paramediano. Relato do Caso: Paciente do sexo masculino, 62 anos, apresentou-se no serviço de Cirurgia Plástica do Hospital Federal de Ipanema com lesão ulcerada, descamativa, ocupando toda a região lateral do nariz com três anos de evolução, sugestiva ao exame físico de tumor maligno de pele. Foi realizada exérese da lesão cutânea e imediata reconstrução com retalho frontal paramediano e posterior cirurgia de refinamento. Conclusão: Devido à sua vascularização pela artéria supratroclear e textura semelhante ao tecido nasal, o retalho frontal paramediano constitui-se como o gold standard para a correção de grandes defeitos nasais.


Introduction: Basal cell carcinoma is the most frequent type of tumoral lesion of the skin, often affecting the nasal region. The resulting defects require the use of nasal reconstruction techniques. The first plastic surgery described is the nasal reconstruction, according to the Indian method. Over the centuries, it has been the object of several modifications by the biggest names in the specialty, culminating in the paramedian frontal flap. Case Report: A 62-year-old male patient went to the Plastic Surgery Service of the Federal Hospital of Ipanema, presenting an ulcerated, scaly lesion, occupying the entire lateral region of the nose with three years of evolution, suggesting at physical examination malignant skin tumor. It was performed cutaneous lesion excision and immediate reconstruction with a paramedian frontal flap and further refinement surgery. Conclusion: Due to its vascularization by the supratrochlear artery and texture similar to the nasal tissue, the paramedian frontal flap is the gold standard for the correction of significant nasal defects.

RESUMEN El melanoma mucoso maligno es un tumor neuroectodérmico de menos del 5% de las neoplasias nasosinusales. Es un tumor de mal pronóstico clásicamente su promedio de supervivencia a cinco años es cercano al 28% y 17,5% a diez años, siendo aún peor cuando existe metástasis en cuello. Se presentan a continuación dos casos de melanoma mucoso en cavidad nasal y senos paranasales, el primero mujer 58 años con melanoma septal llevada a septotomía abierta más quimioterapia y radioterapia adyuvante, con recurrencia en cuello submaxilar derecho tratada con vaciamiento supraomohioideo actualmente sin recurrencia; el segundo hombre de 40 años con antecedente de resección melanoma septal quien presenta nuevo melanoma región etmoidal izquierda luego de su primera cirugía y es llevado a resección endoscópica transnasal sin tratamiento adyuvante por decisión del paciente.


ABSTRACT Sinonasal mucosal melanoma is a neuroectodermic tumor account less 5% of nasosinusal tumors. It has a poor prognosis with a five-year average survival near 28% and 17.5% to 10 years; if neck metastasis is even worst. Here we present two clinical cases. First one a woman 58 years old with septal melanoma who receives an open septectomy with chemotherapy and radiotherapy with submaxillary recurrence treated with supraomohyoid neck dissection. The second case, a man 40 years old with previous septal melanoma surgery who has an ethmoid sinus recurrence and receives transnasal endoscopic surgery without adjuvant therapy by patient desire.

ABSTRACT

Purpose To study the clinicopathologic features and differential diagnosis of primary mucosal melanoma of the nasal cavity (PMMNC).Methods 17 cases of PMMNC diagnosed from January 2003 to September 2016 were studied by clinical pathological analysis and immunohistochemical staining,and relevant literatures were reviewed.Results 73% of the PMMNC was characterized by unilateral nasal congestion and intermittent epistaxis and 61% of the PMMNC occurred in the nasal septum and nasal side wall.Microscopically,the organizational structure and morphology were complex and diverse,which had several cell types including epithelioid cell type (6cases,35.3%),spindle cell type (3 cases,17.6%) and snall cell type (5 cases,29.4%),the other 3 cases (17.6%)were mixed cell type.Mitotic activity and tumor necrosis were more likely to be seen in PMMNC,among other clinicopathological features with a small amount of fibrous stroma and melanoma and rich blood vessels.The immunohistochemical study showed that the positive rate of S-100 and HMB-45 were both 93.8%(15 cases) and those of Melan-A and vimentin were both 87.5% (14 cases),while CK and EMA were both negative (16 cases).Conclusion PMMNC is a rare disease and the phenotype of S-100,HMB-45,Melan-A and vimentin are useful for diagnosis of PMMNC.

Leiomyoma is a benign tumor arising from smooth muscle cells. They are commonly found in the uterus, skin, and gastrointestinal tract. However, it is rare in the head and neck area. Especially, only 3% of leiomyomas in the head and neck area occur in the nasal cavity. Rhinologists need to know leiomyoma as a differential diagnosis of the tumors in the nasal cavity. Herein we present two cases of angioleiomyoma in the nasal cavity and review the literature.

ABSTRACT

Introdução: Os miopericitomas representam cerca de 1 % dos tumores vasculares, sendo relativamente comum na região de cabeça e pescoço, 25% dos casos, e raro nas cavidades nasais e paranasais. Objetivo: Descrever um caso de miopericitoma em fossa nasal. Relato do Caso: Apresentamos um caso de paciente adulta, do sexo feminino, com queixas de obstrução nasal, dor em região de fossas nasais e epistaxe eventual em fossa nasal direita, apresentando massa angiomatosa e facilmente sangrante, não pulsátil, ocupando toda fossa nasal direita. Comentários Finais: Os mopericitomas são tumores vasculares incomuns, raramente localizados na cavidade nasal e nos seios paranasais. Devem ser incluídos no diagnóstico diferencial das massas bem delimitadas, vasculares e de crescimento lento à tomografia computadorizada.


Introduction: The myopericytomas represent about 1% of the vascular tumors, is relatively common in the region of head and neck, 25% of the cases, and uncommon in the nasal and paranasal cavities. Objective: To describe one case of myopericytoma in nasal cavity. Case Report: We present a case of an adult patient, of the female sex, with complaints of nasal obstruction, pain in the nasal cavities region and eventual epistaxis in the right nasal cavity, which present an angiomatous and easily bleeding, non-pulsatile mass occupying all the right nasal cavity. Final Considerations: The myopericytomas are uncommon vascular tumors, rarely located in the nasal cavity and in the paranasal sinuses. They must be included in the differential diagnosis of the well delimited, vascular and slow growth masses upon computed tomography.

ABSTRACT

Leiomioma de cavidade nasal e seios paranasais é raro. Ele constitui menos de 1 por cento de todos os leiomiomas do corpo humano. Isto se deve à escassez de células musculares no nariz. Estas neoplasias podem ser classificadas em três grupos: leiomioma, angiomioma e leiomioma epitelióide. Somente 15 casos de angiomioma foram encontrados na literatura. O tratamento de escolha é a excisão cirúrgica. Um novo caso e a revisão da literatura são apresentados.


Liomyomas of the nasal cavity and paranasal sinuses are rare. They make up less than 1 percent of all leiomyomas in the human body. This is due to the paucity of smooth muscle in the nose. They are classified in three groups: leiomyoma, angiomyoma and epithelioid leiomyoma. Only 15 cases of vascular leiomyomas have been found in the literature. The treatment of choice is surgical excision. Hereby we present a new case and review the literature.

ABSTRACT

Objective:To further understand the advantage of radiofrequency treatment of hemangioma ofnasal cavity under nasal endoscopy. Method:Fifteen cases with hemangioma of nasal cavity were treated withradiofrequency under nasal endoscopy. Result:The hemangioma of fifteen cases could be removed completely. Thecomplication was absent. None of them recured in six months to four years following up. Conclusion:This methodhas many advantages such as clear operation visual field,less hemorrhage and postoperative pain,no facial scar.The radiofrequency under endoscopy is valuable in treatment for hemangioma of nasal cavity.
