ABSTRACT

Objective To evaluate the effect of helical tomotherapy (HT) megavoltage CT (MVCT) under different acquisition pitch and registration conditions upon the accuracy of radiotherapy for used nasopharyngeal carcinoma,aiming to provide reference for image-guided radiotherapy (IGRT) for nasopharyngeal carcinoma.Methods MVCT scans were performed on an anthropomorphic head& neck phantom which was simulated the positioning errors in the x,y and z direction.The obtained images were registered with the planning CT images.Acquisition pitch was set as the coarse,normal and fine modes.Registration conditions were set as bone registration,bone and soft tissue registration and full-image registration.Registration accuracy was determined by comparing the measured value with the preset value.The accuracy of radiotherapy under different acquisition pitch and registration conditions was statistically compared.Results The 3D errors of bone,bone and soft tissue and full-image registration under different acquisition pitch (coarse,normal and fine) were (1.51±0.47,1.54±0.35,1.81±0.53) mm and (1.41± 0.37,1.53±0.36,1.56±0.39) mm and (1.51±0.27,1.57±0.32,1.73±0.33) mm,respectively.The bone registration yielded the highest accuracy (P<0.05).When the registration condition was set as bone registration,the accuracy of three acquisition pitch did not significantly differ (all P>0.05).The fine mode of acquisition pitch possessed the highest stability,whereas required the longest time.Conclusion The acquisition pitch and registration conditions should be selected during HT MVCT based on the clinical requirement of each patient with nasopharyngeal carcinoma.

ABSTRACT

Objective To compare the neck skin dose between fixed-field dynamic intensity-modulated radiation therapy (dlMRT),volumetric modulated arc therapy (VMAT),and helical tomotherapy (HT) in the treatment of early-stage nasopharyngeal carcinoma.Methods A total of 16 early-stage nasopharyngeal carcinoma patients undergoing radiotherapy were enrolled as subjects.The neck skin was delineated by contraction of the outer edge of neck by 3 mm.Dose planning was made by the traditional method (TP group)and a new method (NP group),in which the neck skin was considered as the organ at risk.Dmean and V5-V70 for the neck skin were recorded.The paired t-test was used to analyze the differences between two plans in each radiotherapy method.An analysis of variance was used to compare the same plan between the three radiotherapy methods.Results The HT group had significantly higher Dmean and V5-V70 for the neck skin than the dIMRT group and the VMAT group (P=0.00,0.00,0.00,0.00,0.00,0.00,0.00,0.02).Using dIMRT,the D and V10-V60 for the neck skin were reduced by 7%,8%,22%,25%,38%,59%,and 85% in the NP group than in the TP group (P=0.00,0.00,0.00,0.00,0.00,0.00,0.00).Using VMAT,the D and V20-V40 for the neck skin were reduced by 4%,19%,29%,and 34% in the NP group than in the TP group (P=0.02,0.01,0.02,0.01).Using HT,the V30-V60 for the neck skin were reduced by 20%,29%,50%,and 67% in the NP group than in the TP group (P=0.00,0.00,0.00,0.00,0.03).Conclusions In the treatment of early-stage nasopharyngeal carcinoma,HT causes a higher radiation dose to the neck skin than dIMRT and VMAT,while dIMRT and VMAT have similar neck skin doses.The neck skin dose can be significantly reduced with the neck skin as the organ at risk.

ABSTRACT

Objective To study dosimetric characteristics of helical tomotherapy (HT) by comparing its treatment plans with linear accelerator-based step-and-shoot intensity modulated radiation therapy (IMRT) for nasopharyngeal carcinoma(NPC). Methods Targets on CT images of 10 NPC patients were delineated and transferred to HT and IMRT treatment plan systems. The prescription dose was 70 Gy/33 f for pGTV and GTVnd,60 Gy/33 f for FTV1 ,and 54 Gy/33 f for PTV2. The limit dose of organ at risk was parotid V35 <50% ,brain-stem<54 Gy,spinal cord <45 Gy and lens <9 Gy. Data of the two groups were compared by paired t-test. Results The dose distribution, conformality and homogeneity were good in both groups.But the homogeneity index(HI) and Dmean of PTV1 in HT group were better than IMRT group( P < 0.01 ).The Dmean of PTV1 in HT group(63.84 Gy)was lower than IMRT group(70.30 Gy). The Dmean, V35 and V30 of parotids,and the Dmax of larynx-esophagus were lower in HT group than IMRT group. Conclusions Helical tomotherapy treatment plan has a better homogeneity, steeper dose gradient, and a better protection for organs at risk.
