ABSTRACT

ABSTRACT

BACKGROUND: Mammograms are one of the most effective preventive means for the early detection of breast cancer. OBJECTIVE: To describe the features of patients and results of mammograms performed at a public breast imaging service of the Santiago Metropolitan Area. MATERIAL AND METHODS: We reviewed the reports of mammograms performed on 174,017 women and 18 men, between 2008 and 2018 in an Imaging Center. The BI-RADS classification was used in the reports. RESULTS: Forty-six percent of mammograms (75,781) were reported as BI-RADS 2. The high proportion of BI-RADS 4 reports (674 reports) was seen in patients aged 40 to 49 years, corresponding to 30% of reports in this age range. Among patients aged 50 to 59 years, there were 779 BI-RADS 4 reports (35%). BI-RADS 5 reports were more common among patients aged 50 to 59 years (50 reports, 30%) and among patients aged 70 years or older (83 reports, 28%). CONCLUSIONS: The presence of a significant number of women between 40 and 49 years of age with a BI-RADS 4 mammography result stands out; being an opportunity to develop new clinical research and public health strategies within the framework of the Universal Health Care policy for breast cancer in Chile.

ABSTRACT

Objective@#To evaluate the efficacy, toxicity and cosmetic outcomes of hypofractionated simultaneous integrated boost intensity-modulated radiotherapy (IMRT-SIB) after breast conservative surgery (BCS) for early breast cancer patients.@*Methods@#A total of 76 patients with stage TisT1-2N0M0 breast cancer treated with BCS were enrolled in the analysis. The patients who underwent breast radiotherapy without regional lymph node irridiation and hypo fractionated IMRT/VMAT were used. All patients received whole breast IMRT/VMAT with tumor bed SIB. The doses delivered to the whole breast was 42.4 Gy in 16 fractions, and the dose delivered to tumor bed for SIB was 49.6 Gy in 16 fractions. Cosmetic evaluation was based on the Harvard system. Acute and late toxicities were scored according to CACAT version 3.0. Survival and recurrence rates were calculated by Kaplan-Meier method. The univariate and multivariate analysis were conducted with logistic regression.@*Results@#The median follow-up was 29 months (range 16-40 months). The follow-up rate was 100%. The 1-, 2-and 3-year overall survival rates were 100%. No recurrence or metastasis was observed in this study. The incidence of grade 1 acute skin toxicity was 68.4%, grade 2 was 7.9%. The late skin toxicity of grade 1 was 13.1%, grade 2 was 2.6%.In all, 82.4% of patients had excellent and good cosmetic outcome. The Mean dose of the tumor bed was predictive factor for grade 2 dermatitis.@*Conclusion@#The efficacy, cosmetic effect, the acute and late treatment-related toxicity of hypofractionated IMRT/VMAT-SIB in patients with early breast cancer following BCS might be acceptable. A longer follow-up is needed to define the efficacy on outcomes.@*Trial registration@#Chinese clinical trial registry, ChiCTR1800016287

ABSTRACT

Solid papillary breast carcinoma is a rare type of breast carcinoma. We present four cases which highlight the radiological and histological findings of solid papillary carcinoma. Mammogram supplemented with ultrasound played an important role in detecting solid papillary carcinomas which usually presents as intraductal lesions or intracystic mass with Doppler signal. Excision biopsy is a better option than core biopsy as the latter modality may not be representative and the diagnosis of solid papillary breast carcinoma may be missed.

ABSTRACT

Objective To explore the relationship between MR enhancement signs with MVD and VEGF expression in early breast carcer or benign lesion, improve the correct diagnosis rate of breast mass. Methods The MR enhancement signs(early enhmcement rate, eurve type of enhancement)of 31 patients with breast cancer and 19 benign lesions were analyzed retrospectively. Results Mean early enhancement rate of 31 neoplasm(20 invasive duet csncer,6 invasive lobule cancer,2 mucous cancer,3 situ lobule caner)were(68.4±14.7)%,enhancement curve type of which manifested as 18 type Ⅰ curve,11 typeⅡcurve and 2 type Ⅲ curve.Mean early enhancement rate of 19 benign mass(11 fibre adenoma,6 cystic hyperplasia,1 angioma, lobular tumor,1 infecmd cyst)were(47.6 ±15.2)%,of which 2 were typeⅠcurve,10 type Ⅱ cnrve and 7 type Ⅲ curve. In early cancer focus, mean MVD and VEGF expression index were 48.2±5.6 and 1.8 ±0.3,while in benign mass they were 30.8 ±6.1,0.9 ±0.3 respectivdy.Condusion Early enhancement rate,MVD and VEGF expression index in malignant focus were higher than those in benign mass, type Ⅰ curves were predominate in malignant lesions,by which early breast cancer could be diagnosed in time.
