ABSTRACT

Abstract: Cutaneous metastases from internal malignant neoplasms are a rare event and a late clinical finding that is associated with disseminated disease and a poor prognosis. Skin metastases from colon tumors occur in only 4% of cases of metastatic colorectal cancer. They are most often located on the abdominal skin. We report a case of 54-year-old male patient with a cutaneous metastatic focus on the lower abdomen as the initial presenting symptom of an underlying colon cancer.

ABSTRACT

AIM:Toinvestigatetheinteractionofpolymorphismsofintercellularadhesionmolecule-1(ICAM-1) gene K469E and monocyte chemoattractant protein-1 (MCP-1) gene -2518A/G in the invasion and metastasis of gas-tric carcinoma .METHODS:Based on TNM classification , 4 500 patients with confirmed gastric carcinoma from the First Affiliated Hospital of Xinxiang Medical University in China from December 2009 to November 2014 were divided into stageⅠ group, stageⅡgroup, stageⅢgroup, stageⅣgroup, and stage 0 group, with 900 cases in each group.No significant difference among the 5 groups in age, gender, ethnicity, birthplace and living habit was observed .The genetic polymor-phisms of ICAM-1 gene K469E and MCP-1 gene -2518A/G were analyzed by the technique of polymorphism-polymerase chain reaction ( PCR) in peripheral blood leukocytes of above-mentioned cases .RESULTS:Statistical tests showed signi-ficant differences in the frequencies of K469E (EE) and -2518A/G (GG) among each group (P<0.01).The risk of the invasion and metastasis of gastric carcinoma significantly increased in subjects with K 469E ( EE) genotype and in those with -2518A/G (GG) genotype.Combined analysis of the polymorphisms showed that distribution frequency of K 469E (EE)/-2518A/G ( GG) in stage Ⅰ group, stage Ⅱ group, stage Ⅲ group, stage Ⅳ group and stage 0 group was 39.22%, 53.22%, 59.22, 65.44%and 12.11%, respectively (P<0.01).The people who carried with K469E (EE)/-2518A/G ( GG) had a high risk of the invasion and metastasis of gastric carcinoma , and statistical analysis suggested a positive interaction in a super-multiplicative model between K469E (EE) and -2518A/G (GG) in increasing the risk of the invasion and metastasis of gastric carcinoma .CONCLUSION: ICAM-1 gene K469E ( EE) and MCP-1 gene-2518A/G ( GG) are the risk factors in the invasion and metastasis of gastric carcinoma , and significant interactions be-tween genetic polymorphisms of K 469E and -2518A/G added the risk of the invasion and metastasis of gastric carcinoma .

Bonetumorisonekindofmalignanttumorwhichthreatslifeamongchildrenandadoles-cent.Osteosarcoma (OS ) and Ewing sarcoma (ES ) have been paid much attention by medical community because of their complex occurrence,development,metastasis mechanism and poor prognosis.The research showed that activation of protooncogene and mutation of suppressor gene,absence of apoptosis signal and hypox-ia-inducible factor (HIF)all participated in the occurrence,development and metastasis of OS and ES.It is suggested that clarified pathomechanism of bone tumors could provide more effective therapeutic strategies and lower the mortality of patients.

ABSTRACT

As multidrug resistance related genes and cell factors are discovered one after another,people have a more thorough understanding to the oncological mechanisms of cancer metastasis.Many evidences of the inherent link between the two tips have been found.These researches provide new ideas to further clarify the regulation mechanisms between them.It is revealed that the inherent link maybe occur on the gene expression and transcription,and with complicated regulative factors.Indepth study of the relations between cancer multidrug resistance and metastasis will help to guide the clinical treatment of tumor.

ABSTRACT

Objective To investigate the clinical characteristics, treatment and prognostic factors of postoperative recurrent-metastatic gastrointestinal stromal tumors (GIST). Methods The clinical data of 56 patients with postoperative recurrent-metastatic GIST between January 1997 and December 2007 were analyzed retrospectively , and the prognostic factors were evalutated. Results Initial recurrence-metastasis was diagnosed after a median of 17.3 months from primary resection. Of the 56 cases, 19 cases received resection, 8 cases received imatinib targeted therapy,14 cases were treated by imatinib targeted therapy and surgery, the 3 year survival rates were 65% ,66% ,89% respectively. The rate of recurrence-metastasis after reoperation is 76%. Univariate analysis revealed that surgery, targeted therapy and age were related to the survival rates, multivariate analysis demonstrated that surgery and targeted therapy were the independent prognostic factors for survival. Conclusions For recurrent-metastatic GIST, a multimodal approach including surgery and targeted therapy improves survival.

Micrometastasis of carcinoma is one of the most significant pieces of evidence for molecular staging.Detection of micrometastasis of non-small-cell lung cancer plays a significant role in staging precisely,guiding treatment and predicting the prognosis of patients.This review is focused on the latest developments of markers such as CK19,LUNX,CEA mRNA,oncogene and anti-oncogene.The sites about micrometastasis of non-small-cell lung cancer are usually reported in lymph node and bone marrow,rarely in pleural cavity and increasingly in peripheral blood.

ABSTRACT

Objective To investigate the FDG PET-CT image characteristics of bone metastases from lung cancer and the clinical value of 18F-fluorodeoxyglucose(FDG)PET-CT for diagnosis to bone metastases from lung cancer.Methods 32 cases of bone metastasis from lung cancer examined by FDG PET-CT had been studied.All of them were proved by pathology after operation or biopsy.Their FDG PET-CT diagnosis were analyzed by comparing with CT diagnosis. Results All of 115 lesions of 32 cases showed intense uptake of 18F-FDG. SUV mean was 7.47±3.48. 115 lesions located in 42 pieces of bones. Most importantly.16 lesions of occult bone metastases of 12 cases were found.The most related bone were ribs(26/115,22.61%),thoracic vertebral(21/115,18.26%),iliac bone(18/115,15.65%)and lumbar vertebral(16/115,13.91%).96 lesions(83.48%)showed lytic type on CT images,1 lesion(0.87%)showed sclerotic type,and 2 lesions(1.74%)showed mixture type.Conclusion The FDG PET-CT can early detect bone metastases from lung cancer.show their exact location,and have superiority in the screen of bone metastases from lung cancer.

ABSTRACT

Objective To study the characteristics of dynamic susceptibility-contrast(DSC)MR perfusion curves,color images and perfusion values in pre-operative brain metastasis.Methods Twenty- eight brain metastases underwent DSC MR perfusion imaging by using a first-pass T_2~* echo-planar sequence. The patients' data were transferred to on-line workstation.Time-signal intensity curves,color perfusion maps and rCBV,rMTT values in both tumor parenchyma and peri-tumor edema were analyzed,and independent t- test was used and P0.05).Conclusion Different originated brain metastases have nearly same characteristics in DSC MR perfusion imaging.

ABSTRACT

Objective To recognize the CT manifestations of benign metastasizing leiomyoma (BML).Methods Four cases of BML proved by pathology were retrospectively analysed.Among these, 2 cases were pulmonary BML,1 ease was leiomyomatosis peritonealis diseminata(LPD),and 1 case was combined with pulmonary BML and LPD.All of them were female,and had a history of hysterectomy,the average age were 45.The CT findings of them were analyzed.Results The CT of pulmonary BML in three cases showed multiple rounded,well-defined discrete nodules,0.2—2.0cm in diameter,random distribution,and no calcification in the nodules.No hilum or mediastinum lymphadenopathy,and no pleural effusion.There were no enhancement after intravenous contrast examination.The CT findings of LPD in two cases were many masses around the vena cava,abdominal aorta and/or iliac artery.The masses were 4— 5cm in diameter with smooth margin and homogeneous density.No lymph node or ascites were found.One case who got intravenous contrast enhanced CT scan showed significant enhancement of the lesions;another case who got PET CT study showed no significant tracer concentration.Conclusion When multiple metastasizing nodular lesions with sharp margin and homogeneous density were found in the lungs or abdomen,they should be considered the possibility of BML in women with hysterectomy history.

ABSTRACT

Objective To evaluate diagnostic value of diffusion tensor imaging(DTI)in ringlike- enhanced lesions.Methods Nine abscesses,12 glioblastomas,10 metastases confirmed clinically or pathologically underwent conventional MRI and DTI.Average diffusion coefficient(ADC)value,fractional anisotropy(FA)value and maps were calculated in the central portion and peripheral edema of the lesions. Results On DTI,the abscesses displayed as hyperintense signal with hypointense or isointense signal of edema;but glioblastomas and metastases all showed as hypointense signal with isointense or hypointense signal of edema.On ADC map,the abscesses showed as hypointense signal,the mean ADC value was (0.66?0.07)x10~(-3)mm~2/s,The mean ADC value were(2.50?0.11)x10~(-3)mm~2/s and(2.37?0.52)x10~(-3)mm~2/s for the glioblastomas and metastases,respectively,all demonstrated as hyperintense signal with slightly hyperintense signal of edema.The difference between abscess and necrotic tumors was statistically significant(F=108.80,P

ABSTRACT

Objective To investigate the value of superparamagnetic iron oxide particles(SPIO) in MR imaging for differentiation of metastatic lymph nodes from normal or reactively hyperplastic lymph nodes. Methods For the time course study,10 ?mol Fe of SPIO per extremity was administered to 6 animals, and images were obtained before and 1 h to 48 h after administration. Reactively hyperplastic lymph node model was established in 6 animals by injection of egg yolk emulsion, and tumor bearing lymph node model was established in another 6 rabbits. MR images of the popliteal lymph nodes were obtained before and 12 h after subcutaneous administration of SPIO. Imaging results were analyzed and correlated with pathological findings. Results In unenhanced images, the signal intensity of tumor free and metastatic lymph nodes did not differ significantly. After administration of SPIO, a significant signal intensity loss occurred in the tumor free lymph node, and the maximal loss occurred at 12 h, and the signal intensity reduced by 49%,28%,59%, and 89% on T 1WI, T 2WI, PDWI, and T  2WI, respectively at 48 h( P =0.000). However, the signal intensity of the metastatic lymph nodes remained unchanged. Conclusion MR imaging after subcutaneous administration of SPIO help to differentiate metastatic from normal or reactively hyperplastic lymph nodes.
