ABSTRACT

Background:The skin is the largest organ of the body, comprising of epidermis, dermis and hypodermis. Thus, a wide range of diseases can develop from the skin ranging from infectious diseases to malignancy, some of which may present as non-healing ulcers. Skin biopsy forms the fundamental basis for differentiation of similar looking lesions, thus helping the pathologists to make a definitive diagnosis and more so to the clinician for better management of patients. The objective was to study the histopathological spectrum of chronic non healing ulcers of skin for proper management and treatment. Methods:This was a hospital-based study which was conducted in SKIMS,Soura, a tertiary care hospital of Kashmir valley for a period of 1year extending from January 2018 to December 2018. All the patients who presented with the complaint of non-healing ulcer for more than 4 to 6 weeks were subjected to skin biopsy and histopathological examination. Results:A total of 260 biopsies were examined. Out of 260 patients 146 were males and 114 were females. Ninety out of 260cases (34.61%) and 170 (65.39%) were diagnosed as malignant and benign ulcers respectively.Diabetic ulcer was the second most common cause ofnon-healing ulcers followed by bacterial infections and tuberculosis. Squamous cell carcinoma was the most common neoplastic pathology. Conclusions: It was concluded from the study that non-healing skin ulcers can be encountered at any age in daily medical practice

ABSTRACT

ABSTRACT Introduction: Population aging is a demographic concern worldwide and causes the increase of chronic noncommunicable diseases, such as diabetes mellitus, with complications associated with peripheral nerve damage such as neuropathic ulcer. Because of all this, it is the responsibility of the nursing staff to find a solution to the needs of these patients, by using the theories of Sister Callista Roy and Dorothea E. Orem, in order to improve the state of health and make them adapt to the chronicity of the condition, to continue with their daily live with the highest possible degree of independence and the lowest degree of disability. Objective: To analyze the coinciding points between Roy and Orem's theories in the care of the elderly with neuropathic ulcer. Methods: Systematic bibliographic review for carrying out a critical reflexive analysis of the content of original and review articles. The search strategy adopted was the use of keywords or descriptors, connected through the Boolean operator AND, in the SciELO, Ebsco, LILACS and MEDLINE/Pubmed databases, during the month of January 2017. Conclusions: The purpose of nursing is to promote ways of adaptation that support psychophysical integrity, group identity for self-concept, the function of role and interdependence to achieve the overall health of the elderly with neuropathic ulcer(AU)

ABSTRACT Introduction: Self-care activities are learned as the individual matures, and are affected by cultural beliefs, habits, customs within the family and the society. Age, stages of development and health status can affect the individual's ability to perform self-care activities. Objective: To describe Dorothea E. Orem's self-care deficit theory's adaptation to people with diabetes mellitus complicated by neuropathic ulcer. Methods: Systematic bibliographic review for carrying out a reflexive critical analysis of the content of original and review articles. The search strategy adopted was the use of keywords or descriptors, connected through the Boolean operator AND. The search was carried out in the databases SciELO (Scientific Electronic Library Online), Ebsco (Elton Bryson Stephens Company), LILACS (Latin American Literature and Caribeem Ciências da Saúde), and MEDLINE/Pubmed (Medical Literature Analysis and Retrieval System Online), during the month of January 2017. Conclusions: The relevance is supported regarding the self-care of diabetic people with complicated neuropathic ulcer in the community. The lack is shown of specific studies of this type from the nursing perspective. The achievement represented by raising awareness about self-care as required by the person affected with diabetes mellitus with complicated neuropathic ulcer for improving or maintaining the state of health, through the possibilities offered to the community nurse(AU)

ABSTRACT

Objective To identify high?risk groups of Charcot foot( CN) in the people with diabetic foot neuropathic ulcerations( NU) . Methods Twenty cases patients with CN who were diagnosed in General Hospital of the Chinese People Air Force from June 2008 to June 2013 and 58 patients with diabetic neuropathic ulcer who were hospitalized from January 2010 to December 2011 and followed up until June 2014 without foot deform?ity were retrospectively analyzed. All patient's general condition, examination and laboratory results, diabetic chronic complications, complication, diabetes distribution of foot ulcers, and plain features. Results There were no statistically significant differences in terms of patients' average age, sex ratio, proportion of smokers, BMI, HbA1c,blood lipid, dorsalis pedis artery diameter and diabetic nephropathy (Ⅲ?Ⅳperiod) ,chronic kidney dis?ease stage 3 above, proliferation diabetic retinal pathological changes, the prevalence of coronary heart disease between the two groups(P>0. 05). Compared with NU group, patients with single high proportion(40. 00%(8/20) vs. 10. 34%(6/58)),Short duration of diabetes((12. 37±5. 64) years vs. (14. 27±8. 04) years),Feet long numbness(6(5,9) years vs. 4(2,20) years),low rate of hardening of the arteries narrow(ABI<0. 9)( 0 ( 0/20) vs. 39. 66%( 13/58) ) ,high recurrent diabetic foot ulcer prevalence( 70. 00%( 14/20) vs. 25. 86%( 15/58)),more patients with diabetes mellitus autonomic neuropathy(75. 00%(15/20) vs. 39. 66%(23/58)),less combined with hypertension ( 25. 00%( 5/20 ) vs. 58. 62%( 34/58 ) ) , the differences were significant ( t orχ2=6. 981,2. 259,4. 068,3. 887,12. 405,7. 436,6. 724;P<0. 05) . Diabetic foot wound distribution on mesopodi?um of CN group and NU group was 36. 84%(7/19),6. 90%(4/58) respectively, the difference was significant (χ2=11. 443,P=0. 003) . Diabetic foot amputation rate( Wanger 4,5 grade) of CN group and NU group was 44. 44%(4/9),6. 90%(2/29) respectively, the difference was significant(χ2=4. 732,P=0. 020). Conclusion The characteristics of high?risk groups of diabetics Charcot foot in the people with diabetic foot neuropathic ulcerations are middle aged,no foot of ischemia, combine the diabetic autonomic neuropathy and the feet always with recurrent ulcers.
